    Abstract [en]

    This study highlights school nurses' view of schoolchildren's health and their attitude to document it in the school health records. A strategic sample of 12 school nurses was interviewed. The interviews were semistructured and analysed with qualitative content analysis. The findings showed that the school nurses' viewed schoolchildren as physical healthy although they called attention to growing problems related to a changed lifestyle. Psychosocial ill-health was however increasing and the most common reason for visiting the school nurse was psychosomatic expressions. According to the nurses' descriptions, health was related to the individual, the school and the family situation. The family situation was mentioned as one of the most important factors of schoolchildren's health. The nurses described no problem to document schoolchildren's physical health. Ethical consideration, tradition, lack of time and the structure of the record were however factors that were said to hinder the documentation of the psychosocial health. In order to promote, protect and recover schoolchildren's health, more research is needed about how beliefs, experience, ethical consideration and resources influence the school nurse's daily work with schoolchildren's health.

    Abstract [sv]

    Inledning: Baljväxter har flera miljö- och näringsmässiga fördelar och spås att bli framtidens föda. I Sverige finns det en värdefull kulturskatt som består av svenska baljväxter från förr som har samlats in av Programmet för odlad mångfald, POM. För att nå ut till konsumenter kan en sensorisk beskrivning vara en framkomlig väg som samtidigt ger ett mervärde för produkten och befrämjar en ökad konsumtion av baljväxter.

    Syfte: Syftet med detta examensarbete, inom ämnet mat och måltidskunskap, är att ta fram sensoriskt beskrivande ord för sex svenska baljväxter från förr. Examensarbetet ska sedan beskriva de utvalda svenska baljväxterna med hjälp av de framtagna sensoriskt beskrivande orden för att kunna presentera eventuella sensoriska skillnader.

    Material och metod: Sex olika sorters baljväxter ingick i undersökningen som bestod av en kvalitativ gruppdiskussion där sensoriskt beskrivande ord togs fram och ett andra steg en kvantitativ intensitetsbedömning av de olika sorterna. Båda undersökningarna gjordes av en expertpanel utvalda för sin kunskap om baljväxter.

    Resultat: De sensoriska ord som beskriver baljväxter är sötma, syrliga, beska, kastanj, nötig, smörig, frisk och fyllig smak samt med en varierande eftersmak. Munkänslan beskrivs med orden mjölig, krispig, fast och len. Intensitetsbedömningarna visade på små smakskillnader mellan baljväxterna men trots detta fanns särskiljande attribut för de sex olika baljväxtsorterna.

    Slutsats: De beskrivande sensoriska ord som detta examensarbete har utmynnat i kommer att hjälpa konsumenten att hitta rätt och öka mervärdet på produkterna. Samtidigt bevaras den svenska kulturskatten och konsumenten kan utveckla sin kulturella identitet tillsammans med alla de fördelaktiga egenskaper baljväxter besitter.

    Abstract [en]

    The aim of this study was to examine consumers and producers view on beverages and it’s meaning for Skåne (a region in southern Sweden) and how to create a strong culinary region using beverages. It gets more common to connect food and tourism, where food and beverages can be used to attract visitors. The research methods used were qualitative and quantitative and the data was collected by survey and semi-structured interviews. The respondents weren’t aware of current beverage projects in the region. Most of the respondents expressed that products from Skåne brings added value, the informants shared the same view. The informants also expressed the importance of collaboration and solidarity, the problems of communication and visibility by the customers and finally the importance to experience the beverages as more than just a ‘drink’. There are many advantages by marketing beverages and region together, and it can be positive for both parts.

    Inledning: Det är inte många områden som berör så många människor som livsmedel och livsmedelskonsumtion. Vi alla konsumerar olika livsmedel på ett eller annat sätt även om det är med mycket skiftande perspektiv. Det man konsumerar är heller inte statistiskt, utan förändras vid olika situationer som livet ger. Konsumtionen uppfyller olika behov hos olika individer och mycket påverkar ens val. Vägen från bonden till konsument blir allt längre som gör att konsumenter vill ha mer information om slutprodukterna för att kunna värdera produkten med sina referensramar.

    Bakgrund: Livsmedel som produceras och säljs i affären ska betraktas som att de är självklara att konsumera säkert. Men på grund av tidigare matskandaler i Europa där spårbarheten har varit knapp samt en ökad medvetenhet hos konsumenterna ifrågasätts även säkerheten. Det förutsätts att den mat som produceras är säker att förtära.

    Syfte: Syftet med arbetet var att beskriva hur olika livsmedelsaktörer ser på intern- och extern spårbarhet. EG förordningen syftar till att livsmedelsföretagen måste använda sig av extern spårbarhet genom att alltid kontrollera ett steg framåt och ett steg bakåt i livsmedelskedjan. Förordningen kräver dock inte intern spårbarhet.

    Metod: Med en fenomenografisk ansats utfördes fyra stycken kvalitativa semistruktureradeintervjuer på plats och över telefon för att få fram djupgående information om livsmedelsaktörernas syn på spårbarhet.

    Resultat: Resultatet indikerade på att livsmedelsföretagens syn på spårbarhet och intern spårbarhet var lika men att tillvägagångssättet var olika beroende på vilken del i livsmedelskedjan man tillhörde och verksamhetens utformning. Förordningen var inget märkvärdigt för företagen utan det var en naturlig del i företaget för att producera säkra livsmedel. Däremot fanns det utomstående krav att hålla sig ständigt uppdaterad, kund- och konsumentkrav, media och andra klimatförhållanden som var mycket mer omfattande än förordningen som företagen arbetade efter för att inte förlora viktiga kunder och konsumenter.

    Diskussion: De utomstående kraven gentemot förordningen visade resultatet av att företagen gör mer än vad förordningen kräver. Vissa delar i livsmedelskedjan, i detta fall detaljhandeln/”retailers” ställde större krav på sina leverantörer som påverkade även andra företag som exempelvis levererar helhetslösningar och tjänster till dessa leverantörer till detaljhandeln.

    Slutsats: I ett ständigt förändringsbart fenomen som spårbarhet håller företagen sig uppdaterade och arbetar hela tiden för nöjda kunder och konsumenter, som gör att de agerar mer än vad förordningen förespråkar. Tillvägagångssättet vid reklamationer, returflöden och tillbakadraganden visade att aktörerna var högst angelägna om kund- och konsumentförtroenden.

    Det kommer ständigt nya studier med olika hälsoråd angående kosten och fysiskaktivitet om hur vi ska undvika fetma, högt blodtryck samt hjärt- och kärlsjukdomar.Intresset av proteinets effekt på uppbyggnad av muskelmassa vid fysisk aktivitet harökat. Detta har medfört ett stort urval av olika drycker med proteintillskott påmarknaden, främst ur animaliskt ursprung.Syftet med detta examensarbete var att ta fram en vegetabilisk proteinberikadhavredryck som förväntas hjälpa kroppen vid återhämtning. Arbetet innebar att utvecklaett recept baserat på de Nordiska Näringsrekommendationer 2004 (NNR) med fokus påprotein, kolhydrater och fett. Delmålet var även att utesluta allergener. Dryckenbaserades på ett medelvärde från de fysiskt aktiva och äldre människors näringsbehov,detta för att bredda den tilltänkta dryckens målgrupp.Fyra smaksatta drycker framställdes med olika energiinnehåll utifrån två olikahavredrycker, en naturell och en kalciumberikad. För att utvärdera konsumenternasuppfattning av de olika dryckernas sammansättning utfördes ett konsumenttest.Resultatet av testet visade att det fanns en statistiskt signifikant skillnad kringkonsistens, smak och utseende. Flertalet respondenter var positiva till dryckernassensoriska egenskaper.

    The aim of this study was to explore the perceptions of elder abuse held by older persons in Sweden. Six focus group interviews were conducted. Through qualitative content analysis, the text revealed four themes: causes of elder abuse, conceptions of elder abuse, consequences of elder abuse, and coping with elder abuse. Changing society and families, as well as individual determinants, were believed to be causes of elder abuse. The main conception of elder abuse was that of robbery or assault, and the main consequence was fear. Coping with abuse included individual strategies and improvements in society. Issues of age discrimination were intrinsic in the findings. Gender differences were evident in the group dynamics. Nurses and other health care personnel need to be aware of what older persons believe to be the cause of elder abuse and what they consider abusive. Awareness of the results of this study can help in refining interview and assessment techniques and in designing training manuals.

    Huvudsyftet var att utveckla recept och tillaga hållbara och vegetabiliska lunchalternativ till en blandkostmeny. De nya måltiderna skulle vara välsmakande och näringsriktiga enligt Nordiska näringsrekommendationer (2012). Med hjälp av creative design togs tio vegetabiliska luncher fram och näringsberäknades med hjälp av Dietist Net. Det utfördes ett gillandetest och en gruppdiskussion med 19 gymnasieelever. Erhållen feedback användes i förbättringsarbetet av måltiderna. Slutsatsen blev att genom att planera menyn efter måltider och med grönsaker som ungdomarna redan tycker om så skulle de lättare kunna vänja sig vid den vegetabiliska maten. Det fanns heller inga svårigheter att ta fram recepten, däremot vad det svårt att nå upp till önskad mängd av vitamin D, zink och järn i måltiderna, det fanns även svårigheter att hålla sig till de Nordiska näringsrekommendationerna (2012) för innehållet av salt i måltiderna. När det gällde de ekonomiska kraven fanns det heller inga direkta skillnader i kostnad vid jämförelse av de vegetabiliska menyerna med blandkostmenyerna, då vegetabiliska råvaror generellt sett är billigare än animaliska råvaror. Oatlys berikade havredrycken samt iMat var viktiga för att tillgodose måltiderna med tillräckliga mängder av B12 och vitamin D, de var dock även de mest kostsamma ingredienserna. Ekonomiskt kan det justeras enkelt beroende på vilka mål skolorna har med den vegetabiliska kosten. Genom att varva de vegetabiliska måltiderna med blandkostmåltider eller använda sig av mejeriprodukter i recepten istället går det att få ner priset på måltiderna.

    BACKGROUND: Knowledge about how elderly persons perceive the impact of chronic heart failure (CHF) on daily life is important when planning nursing care. For this purpose, disease specific instruments are needed. However, few instruments have been developed or tested specifically on elderly persons. AIM: To validate a Swedish version of the Minnesota Living with Heart Failure Questionnaire (LHFQ) on elderly persons with CHF, and use it to describe the impact of CHF on daily life in the same population. METHODS: The sample comprised of 357 persons, aged between 65 and 99, diagnosed with CHF. A questionnaire including background data, the LHFQ and the SF-12 was used. RESULTS: A factor analysis resulted in four dimensions: physical, emotional, treatment and pleasure. LHFQ showed convergent validity and ability to discriminate between known groups. Cronbach's alpha for the total scale was 0.94. Impairments in the physical dimension were most common, especially fatigue (88%) and shortness of breath (87%). CONCLUSIONS: The LHFQ showed satisfying psychometric properties in an elderly Swedish population with CHF and can, with minor alterations, be recommended for research and clinical use. The impact of chronic heart failure on daily life was mostly physical, but other impairments were also common.

    Background

    A main goal in nursing care of persons with chronic heart failure (HF) is to strengthen their health related quality of life (HRQoL). This presupposes knowledge about the relation between heart failure and HRQoL. Existing studies have shown incongruent results about whether HRQoL is affected differently depending on age or sex of elderly persons with chronic HF.

    Aim

    This study aimed to investigate if age, sex, disease severity, comorbidity and living conditions predict health related quality of life among persons 65 years or older with chronic HF.

    Methods

    The study included a sample of 357 persons. HRQoL was measured by the Minnesota Living with Heart Failure Questionnaire and the Short Form-12 Health Survey Questionnaire. Multiple regression analyses were performed to analyse the relation between the predictors and HRQoL.

    Results

    The main finding was that self-rated disease severity was strongly associated with HRQoL, but also age, sex, diabetes and respiratory diseases was associated with some of the dimensions of HRQoL.

    Conclusions

    Interventions aimed at delaying the progress of the disease, assist persons' to cope with the disease and maintain the domains of HRQoL that are still feasible could be important to improve HRQoL in elderly persons with chronic HF.

    Abstract [en]

    The quality of meals is included in the constant improvement of each preschool. The study is carried out on behalf of the City of Malmö and Environmental Management and focuses on this quality work. By compiling a checklist of meal quality and by identifying what the meal quality means for persons working with the meal (managers, cooks, educators and parents), it is our wish that this will be a help in their quality work.

     

    To inspire the composition various organizations working methods and ways to manage, evaluate and make use of different models and tools to improve the dining quality, has been studied.

     

    In the literature, the work has received confirmation of theories that emerged during the analysis work of what a meal is and how quality can be measured. The results show that food quality is described as a complex concept consisting of different parts, where the food, atmosphere, service, together form the meal quality. The semi-structured interviews and the inquiry form, have given an indication of the preschool work with quality in the meal activities and it shows that on a wide range and large differences. When various occupational groups were asked, what ”meal quality” means, the responses are consistent to the food, that it was good, appetizing, organic or where locally produced.

     

    It turned out that the cooperation between managers and other employees worked but that there were no directives "from above" which regulate the work concerning meals.

    Teamwork between the kitchen and preschool teachers, which is so important in quality work, was very insignificant and they were looking for ways to solve this.

    The criteria for the Check list that includes headings as, "Food and quality of raw materials", "Dining" situation", “Work environment and collaboration" and "Education and development" are based on the results we received from interviews, literature studies and field trips.

    Contemporary society is increasingly impacted by automation; however, few studies have considered the potential consequences of automation on ecosystems and their management (hereafter the automation of urbangreen infrastructure or UGI). This Perspective Essay takes up this discussion by asking how a digital approach to UGI planning and management mediates the configuration and development of UGI and to whose benefit? This is done through a review of key issues and trends in digital approaches to UGI planning and management. We first conceptualize automation from a social, ecological, and technological interactions perspective and use this lens to present an overview of the risks and opportunities of UGI automation with respect to selected case studies. Results of this analysis are used to develop a conceptual framework for the assessment of the material and governance implications of automated UGIs. We find that, within any given perspective, the automation of UGI entails a complex dialectic between efficiency, human agency and empowerment.     Contemporary society is increasingly impacted by automation; however, few studies have considered the potential consequences of automation on ecosystems and their management (hereafter the automation of urban green infrastructure or UGI). This Perspective Essay takes up this discussion by asking how a digital approach to UGI planning and management mediates the configuration and development of UGI and to whose benefit? This is done through a review of key issues and trends in digital approaches to UGI planning and management. We first conceptualize automation from a social, ecological, and technological interactions perspective and use this lens to present an overview of the risks and opportunities of UGI automation with respect to selected case studies. Results of this analysis are used to develop a conceptual framework for the assessment of the material and governance implications of automated UGIs. We find that, within any given perspective, the automation of UGI entails a complex dialectic between efficiency, human agency and empowerment. Further, risks and opportunities associated with UGI automation are not fixed but are dynamic properties of changing contextual tensions concerning power, actors, rules of the game and discourse at multiple scales. We conclude the paper by outlining a research agenda on how to consider different digital advances within a social-ecological-technological approach.

    Bönor har visats ha många hälsofrämjande egenskaper, bland annat en fördelaktig fördelning av makronutrienter och vatten, ett lågt kaloriinnehåll, låg fetthalt, långsamma kolhydrater och viktiga mineraler. Samtidigt vill Livsmedelsverket rekommendera en fyrdubbling av intaget av baljväxter hos det svenska folket. Genom att delvis ersätta animaliskt protein med protein från baljväxter menar Livsmedelsverket att intaget av mättat fett och överflödig energi kan minskas, och därmed minskar också risken att drabbas av övervikt och hjärt- och kärlsjukdomar.Syftet med studien är att undersöka vilken attityd en grupp grundskoleelever har till att äta baljväxter, med särskilt fokus på bönor och linser, och att studera hur mycket bönor och linser de äter i dagsläget. Studien vill även pröva ifall eleverna kan få en mer positiv attityd till bönor och linser om de exponeras för olika måltidstillbehör som är baserade på bönor under en tid.Enkäter delades ut före och efter experimentet för att mäta skillnader i attityd mot baljväxter bland skolungdomarna, med frågor om deras attityd till baljväxter samt vegetarisk mat.Resultatet visade en positiv utveckling av attityden till baljväxter. Studiens sammanställda resultat påvisade en förbättrad attityd till baljväxter hos eleverna. Att öka konsumtionen av baljväxter kräver nytänkande kring skapandet av attraktiva rätter eller måltidstillbehör, och arbetet ger flera exempel på hur man kan gå tillväga i framtida försök.

Notes taken during 15 sessions were analyzed by means of open coding. The analysis revealed the importance of being aware of the parallel process that occurs among the client, the primary nurse, and the unit staff.

    The aim of this study was to explore the way in which traumatic childhood experiences influenced the daily life of women with Borderline Personality Disorder (BPD). An explorative design comprising in-depth interviews and a qualitative content analysis was used. The findings revealed one main theme—Longing for Reconciliation—comprising two themes: (1) Living with a sense of shame and guilt and (2) Struggling to be released from a sense of being trapped. Longing for reconciliation influenced the women's daily life as well as their search for the meaning of traumatic childhood experiences. New meaning can emerge through change, forgiveness, and reconciliation, thus helping women with BPD overcome their struggle and gain the insight and strength to look into their secret knowledge. Meaning can emerge and help them to develop an empathic understanding of themselves in the context of past and present relationships.

    A health-care organization simultaneously belongs to two different institutional value patterns: a professional and an administrative value pattern. At the administrative level, medical problem-solving processes are generally perceived as the efficient application of familiar chains of activities to well-defined problems; and a low task uncertainty is therefore assumed at the work-floor level. This assumption is further reinforced through clinical pathways and other administrative guidelines. However, studies have shown that in clinical practice such administrative guidelines are often considered inadequate and difficult to implement mainly because physicians generally perceive task uncertainty to be high and that the guidelines do not cover the scope of encountered deviations. The current administrative level guidelines impose uniform structural features that meet the requirement for low task uncertainty. Within these structural constraints, physicians must organize medical problem-solving processes to meet any task uncertainty that may be encountered. Medical problem-solving processes with low task uncertainty need to be organized independently of processes with high task uncertainty. Each process must be evaluated according to different performance standards and needs to have autonomous administrative guideline models. Although clinical pathways seem appropriate when there is low task uncertainty, other kinds of guidelines are required when the task uncertainty is high.

    Due to an ageing population the numbers of patients with hip fractures are increasing. They often suffer from concomitant diseases and are therefore prone to be affected by complications such as pressure ulcers. The prevention of pressure ulcers among patients with a hip fracture is crucial. The aim of this study was to improve the quality of care and patient safety in patients with a hip fracture. A new evidence based clinical pathway was introduced to prevent hospital acquired pressure ulcers. Furthermore, the purpose was to bring attention to pressure ulcer prevention and to facilitate changes in clinical practice to improve quality of care and patient safety. A total of 478 patients with a hip fracture were consecutively included between April 2003 and March 2004. The new evidence based clinical pathway was introduced on October 1st 2003. The results from the first 210 patients in the control group and the last 210 patients in the intervention group are presented in this article. In the intervention group, hospital acquired pressure ulcers decreased by 50% (p < 0.007). It is possible to reduce the development of hospital acquired pressure ulcers among elderly patients with a hip fracture even though it is not possible to eliminate the effect of factors such as increased age and the patients’ medical status which are often the two main risk factors.

    This presentation reports from a focused ethnographic study on how teachers of Swedish Home Economics and their students in collaboration with each other and objects make sense of food. The study was based on a social constructionist perspective in combination with theories on space and culture. Food in Home Economics offers a challenging complexity. On the one hand Swedish Home Economics is a school subject with a long history, framed by certain conditions such as the syllabus and issues of time. Here, teachers and students meet according to plan with the expressed intention to teach and learn about food. On the other hand we as humans use food to make sense of the world. Home Economics can thus be conceived of as a space where a rich array of interests, beliefs, traditions and norms relating to food coexist and are being transformed. This study was conducted through what can be described as a focused ethnography where attention was directed towards a particular setting and by giving emphasis to particular behaviour in particular settings rather than attempting to portray a whole cultural system. Data gathering was initially broad and explorative with observations to find ways to proceed, which was followed by focus group interviews with students and teachers. Formal data gathering thus began in early 2007 with a short fieldwork and concluded with focus group interviews with teachers in the spring of 2009. This work resulted in three different data sets that was analysed both separately and in conjunction with each other with the help of different analytical theories. The main finding was that food in Swedish Home Economics could be described as contested: teachers and students made sense of food in opposing ways.

    We used a 14-item patient questionnaire, Strategic and Clinical Quality Indicators in Postoperative Pain Management, to describe how orthopaedic patients assessed the quality of care they received. The patients highest and lowest assessments were compared to assessments by other groups of surgical patients as reported in studies being published. Our study included 300 inpatients from four different orthopaedic wards in two county councils in Sweden. Patients answered a questionnaire on the day of discharge. The response rate was 74%, and 44% of the respondents were male. Elective surgery was the main reason for admission, and the mean length of stay was 7.3 days. One of the items rated highest was that pain relief was given quickly when requested. A literature review showed that this item also received a high rating in other studies using the same patient questionnaire. The item rated lowest concerned the regular use of a pain assessment instrument, which also received a low rating from patient groups other than the orthopaedics in this study. In conclusion, we found that orthopaedic patients and other groups of surgical patients gave similar assessments concerning the highest and lowest assessments of postoperative pain management. Nevertheless, further improvement is needed.

    Aims and objectives. The aim of the study was to investigate the quality of life and related factors among older people who are in pain and in need of help to manage daily living. Background. To intervene against the low quality of life in nursing care knowledge about factors affecting it is needed and this is especially important for vulnerable people such as those who suffer from pain and who are in need of help to manage daily living. Methods. Five hundred and twenty-six people, aged 75–102 years participated in this study. Results. Those in pain reported a significantly higher degree of all complaints and lower quality of life in all measures compared with those not in pain. Overall quality of life was associated with mobility problems, sleeping problems and depressed mood, while health-related quality of life was associated with living in special accommodations, walking problems, mobility problems and fatigue. Conclusions. Those in need of help to manage daily living and in pain seem to be at higher risk of lowered quality of life than those not in pain and the lower quality of life among those in pain is probably caused by the complex of complaints rather than pain per se. Relevance to clinical practice. Daily nursing care should identify and treat the complex of complaints related to pain as well as pain itself, to improve everyday life and quality of life for older people in pain.

    Syftet med studien är att undersöka attityder till inälvsmat. Synen på inälvor som mat är varierande beroende på land och kulturella skillnader och ses i vissa kulturer som delikatess och i andra som skräp. Med inälvsmat menas ätliga organ ifrån slaktdjur. För att få veta om benägenheter att konsumera inälvsmat finns måste de faktorer som avgör personens matval studeras. För att svara på denna fråga utformades en egen teori som beskriver de beståndsdelar som ligger till grund för människors konsumtionsval. Under fokusgrupp diskuterades om inälvsmat är en ätligt eller oätligt råvara, deltagarnas kunskaper inom tillagning och om det finns några kulturella skillnader i synen på inälvor. Deltagarna som medverkade i studien fick även under observation tillaga rätter bestående av inälvor och avslutningsvis diskutera om dess attityd till inälvor förändrats. Resultat från studien visar att deltagarnas kulturella bakgrund speglar vilken attityd som existerar till inälvsmat. De medverkande personerna hävdade att de saknade kunskap om tillagning av inälvor och det förekom också en stor variation i vad deltagarna bedömde vara ätlig respektive oätlig inälvsmat. Dessa faktorer kopplas till den framtagna teorin med syfte att uppnå förståelse för människors konsumtionsval av inälvor. I studien ses ett samband mellan konsumtionsval, kulturell bakgrund och attityder till inälvsmat.

    Objective. Postoperative urinary retention and bladder distension are frequent complications of surgery. The aim of this study was to determine the incidence of perioperative bladder distension in a surgical setting and to identify predisposing factors among patients undergoing common general and orthopaedic procedures. Material and methods. This was a prospective observational study of 147 adult patients admitted to orthopaedic and surgical departments. Bladder volumes were measured with an ultrasound scanner on three occasions: after emptying the bladder before being transported to the operating theatre, and then immediately before and after surgery. Results. Thirty-three patients (22%) developed bladder distension (500 ml), eight preoperatively and 25 postoperatively. A total of 21 patients (14%) had a bladder volume 300 ml immediately before surgery. Orthopaedic patients were more likely to develop preoperative bladder distension than surgical patients and had significantly higher postvoid residual volumes. In the binary logistic regression analysis age, gender and time of anaesthesia could not predict bladder distension. Patients undergoing orthopaedic surgical procedures, however, were prone to bladder distension (odds ratio 6.87, 95% confidence interval 1.76 to 26.79, p=0.006). Conclusions. This study shows that orthopaedic surgical patients are more prone to bladder distension perioperatively. The conventional method of encouraging patients to void at the ward before being transported to the operating theatre does not necessarily mean an empty bladder at the start of the operation.

    Managing the transformation –: perspectives from human evolution and human behavioral ecology2015Konferansepaper (Annet vitenskapelig)
    Abstract [en]

    Transforming the current society towards sustainability is a formidable task, requiring changes at many levels of society. Reductions in our use of natural resources and environmental impacts of human societies are necessary, while maintaining a progress in satisfying human well-being in a growing world population. Considerable efforts in developing low impact economy and technology will be needed to change societies towards more sustainable social-ecological systems. However, perhaps the most challenging aspect of this transformation is to manage the very roots of the problem: the human mind. Implicit in many, if not all, of the well-known causes of environmental degradation (e.g., externalities of businesses and individual behavior, tragedy of the unmanaged commons, conspicuous consumption) are a human mind originally evolved to maximize individual reproductive success within short-sighted perspectives and small social groups. We are therefore ill equipped to take responsibility for long-term global environmental problems. We argue that an understanding of human evolution and the functioning of the brain as an adaptive unit underlying human behavior will be necessary in order to create societal reorganization and incentives that successfully deal with the challenges of the Anthropocene. Cooperation and altruistic behavior are certainly part of the human repertoire but only if social contexts are arranged to support these behaviors. We believe that evolutionary approaches to human behavior can no longer be left out of the discussion on the environmental crisis, and in environmental policy, and that managing the transformation will also require applying evolutionary science to human behavior.

    This study explores married Vietnamese adolescents' perceptions and experiences related to transition into motherhood and their encounter with health care service. In-depth interviews were conducted with 22 women younger than 20 who were either pregnant or had newly delivered. It emerged from the narratives that young women experienced ambivalence in the transition to motherhood in that they felt too young but also happy to be able to please their husband and the extended family. Patterns were shown indicating that the participants experienced lacking power with regard to decisions in relation to pregnancy, delivery, and contraceptive usage. Feelings of being patronized and ignored in the encounter with health care providers were seen in the narratives. Findings might be used for reproductive health care providers, social workers, and educators in their contact with young mothers to empower them to make their own decisions with regard to marriage, childbearing, and contraception.

    Introduction. Breast cancer follow-up programmes consume large resources and despite the indications that several alternative approaches could be used effectively, there is no coherent discussion about costs and/or cost-effectiveness of follow-up programmes. Patient and methods. In a prospective trial there were 264 breast cancer patients, stage I and II, randomised to two different follow-up programmes- PG (physician group) and NG (nurse group). The trial period was 5 years. The women in the two intervention groups did not differ in anxiety and depression, their satisfaction with care, their experienced accessibility to the medical centre or their medical outcome as measured by recurrence or death. The analyses were done from different lists representing costs at three hospitals in Sweden according to the principles of a cost minimization study. Results: The cost per person year of follow-up differed between the groups, with (sic)630 per person year in PG compared to (sic)495 per person year in NG. Thus, specialist nurse intervention with check-ups on demand was 20% less expensive than routine follow-up visits to the physician. The main difference in cost between the groups was explained by the numbers of visits to the physician in the respective study arms. There were 21% more primary contacts in PG than NG. Discussion. The difference in cost per year and patient by study arm is modest, but transforms to nearly (sic)900 per patient and 5-year period, offering a substantial opportunity for reallocating resources since breast cancer is the most prevalent tumour worldwide.

    Background: The Locomotor Capabilities Index (LCI) is a validated measure of lower-limb amputees' ability to perform activities with prosthesis. We have developed the LCI Swedish version and evaluated its validity and reliability. Methods: Cross-cultural adaptation to Swedish included forward/backward translations and field testing. The Swedish LCI was then administered to 144 amputees (55 women), mean age 74 (4093) years, attending post-rehabilitation prosthetic training. Construct validity was assessed by examining the relationship between the LCI and Timed "Up-and-Go" (TUG) test and between the LCI and EQ-5D health utility index in 2 subgroups of 40 and 20 amputees, respectively. Discriminative validity was assessed by comparing scores in different age groups and in unilateral and bilateral amputees. Test-retest reliability (1-2 weeks) was evaluated in 20 amputees (14 unilateral). Results: The Swedish LCI showed good construct convergent validity, with high correlation with the TUG (r = -0.75) and the EQ-5D (r = 0.84), and discriminative validity, with significantly worse mean scores for older than younger and for bilateral than unilateral amputees (p < 0.01), and high internal consistency (Cronbach alpha 0.95). In test-retest reliability the intraclass correlation coefficient was 0.91 (95% CI 0.79-0.96) but for the unilateral amputees was 0.83 (95% CI 0.56-0.94). Ceiling effect occurred in 23%. Conclusion: The Swedish version of the LCI demonstrated good validity and internal consistency in adult amputees. Test-retest reliability in a small subsample appears to be acceptable. The high ceiling effect of the LCI may imply that it would be most useful in assessing amputees with low to moderate functional abilities.

    Abstract [en]

    I describe methods used by home carers to cope with troublesome clients and analyze how the organizational context affects their use. Empirical data consist of participant observation and field interviews with 14 home carers in three municipalities in southern Sweden. Home carers might try to avoid working with troublesome clients, prepare themselves mentally before visiting them, try to withhold them psychologically, try to understand them and explain their behavior, try to change their behavior, and vent experiences with colleagues. Although some of these methods are practiced frontstage, in interactions with clients, others take place backstage, only involving staff. Their use is influenced both by properties of frontstage work, for instance, the intimate nature of many tasks and the experienced need to keep up a "good" relationship with clients, and by the organization of backstage work, for instance, the organization of staff meetings and supervisors' understanding of coping processes.

    Student nurses are faced with a demand for moral responsibility during their clinical education. In order to gain an understanding of how they reflect on the phenomenon of moral responsibility in nursing practice, repeated focus group interviews were conducted. Fourteen student nurses in their final year of the three year baccalaureate programme in nursing were interviewed during their final placement. The interpretive content analysis disclosed the manifestations of moral responsibility as being available for the patient, shielding the patient and being confronted with the voice of conscience. The findings highlighted the importance of space given to ethical reflection on a regular basis as a joint activity. This included by both student nurses and professionals.

    Abstract [en]

    This article reports a study exploring the meaning of the complex phenomenon of moral responsibility in nursing practice. Each of three focus groups with a total of 14 student nurses were conducted twice to gather their views on moral responsibility in nursing practice. The data were analysed by qualitative thematic content analysis. Moral responsibility was interpreted as a relational way of being, which involved guidance by one's inner compass composed of ideals, values and knowledge that translate into a striving to do good. It was concluded that, if student nurses are to continue striving to do good in a way that respects themselves and other people, it is important that they do not feel forced to compromise their values. Instead they should be given space and encouragement in their endeavours to do good in a relational way that advances nursing as a moral practice.

    Aim. This paper is a report of a study to interpret nurses' experiences of moral strength in practice. Background. Moral strength is said to be beneficial in providing nursing care for patients, thereby contributing to high qualitative care. However, few studies focus on the meaning of nurses' moral strength. Methods. This study included eight Registered Nurses working in different aspects of health care in southern Sweden. Individual interviews were conducted in 2006 and 2007. We recorded, transcribed verbatim, and interpreted the interviews by a method grounded in hermeneutics. Findings. Three themes were interpreted on three different levels: the action level as 'having courage to act on one's convictions', the relational level as 'being attentive and recognizing vulnerability', and the existential level as 'facing the unpredictable'. Overall, moral strength was understood as a driving force to be someone special in the care of patients, i.e. someone who makes a difference. Conclusion. The value of nurses' moral strength in patient care should be recognized. Attention must be given to aspects outside the individual, e.g. professional and institutional processes that influence the work environment. Clinical team supervision can help make such processes visible and supportive.
