    Hälsopromotion i skolan: utvärdering av DISA - ett program för att förebygga depressiva symtom hos ungdomar2016Doctoral thesis, comprehensive summary (Other academic)
    Abstract [en]

    Background: DISA (Depressive Symptoms In Swedish Adolescents) is a cognitive behavioral intervention aimed at preventing stress and depressive symptoms in adolescents. It is frequently used in Swedish schools for students aged 13-15 years. DISA is commonly offered to females, but at some schools, the intervention is also offered to males. In this study, the application of the intervention for both females and males is evaluated.

    Aim: The overarching aim of this thesis was to explore the experience, significance, and effectiveness of the school-based intervention DISA. The specific aims were to investigate the effectiveness of depressive symptoms and self-reporting health, and costs and cost-effectiveness as well as to elucidate the experiences of adolescents and tutors.

    Methods: The effectiveness of DISA was investigated using method triangulation. The thesis is based on two quantitative and two qualitative studies. The quantitative studies were quasi-experimental trials with an intervention group (I, IV), and a control group (IV), with follow-up measurements obtained at 3 and 12 months after baseline. The qualitative studies were based on focus group interviews with adolescents (II) and tutors (III). The school-based cognitive behavioral prevention program DISA was presented by school health staff and teachers once per week for ten weeks. Students in grade 8 (median age: 14) participated in the intervention in study I (n=62, 52% females) and study IV (n=462, 79% females), and 486 students (46% females) were allocated to the control group (IV). Focus group interviews were conducted with 89 adolescents (II) and 22 tutors (III). The interviews were analyzed with qualitative content analysis.

    Findings: The intervention group decreased their self-reported depressive symptoms and improved their self-rated health more than the control group (p<0.05) at the 12-month follow-up. The majority of the adolescents rated the cognitive behavioral program as a positive experience, and the attendance rate was high. The incremental cost-effectiveness ratio was approximately USD 6,300 per quality-adjusted life year (QALY). The focus group interviews with the adolescents revealed that the students found that they developed intrapersonal strategies through DISA, such as directed thinking, improved self-confidence, stress management, and positive activities. They also gained an increased interpersonal awareness regarding trusting the group and considering others. However, structural constraints of the DISA program, such as negative framing and an emphasis on performance, were also noted. The focus group interviews with the tutors identified an overall theme of striking a balance between strictly following the manual and meeting student needs.

    Conclusions: The DISA program appears to be a feasible, cost-effective school-based cognitive behavioral program with high levels of student adherence and satisfaction, as well as positive mental health benefits. However, a desire for a more health-promoting approach was expressed.

    Sleep and media habits in school children and adolescents2015In: Journal of Sleep Disorders & Therapy, ISSN 2167-0277Article in journal (Other academic)

    The aim was to investigate the effects of sleep, television use and texting and computer habits on overweight, enjoyment of school and feelings of tiredness at school in school-age children and adolescents. This cross-sectional study was conducted in Sweden on school children aged 6-16. A questionnaire was distributed to the children (n=3011 in survey I; n=204 in survey II). Children who slept less than the median length of time reported enjoying school to a lesser degree. Fewer hours of sleep were found to be associated with having a bedroom television using the television or computer more than 2 hours a day, being tired at school and having difficulties in sleeping and waking up. Overweight and obesity were found in 15.8% of the study population; obesity alone was found in 3.1%. Relationships between lifestyle factors and overweight were studied using multivariate logistic regression analysis. Having a bedroom television and using the television more than 2 hours per day were found to be associated with overweight but using the computer more than 2 hours a day was not. About 61% of the students aged 16 reported checking Facebook or social media at least once a day and 27% reported doing so for more than 10 times a day. One fourth of the students aged 16 had a habit of sending or receiving text messages at night at least once a week. Texting at night and frequent checking of Facebook and social media sites were related to sleep problems.

    Abstract [sv]

    Ett möte med experter på barns sömnproblem genomfördes i februari 2014 varvid detta kunskapsdokument togs fram. Kunskapssammanställningen och rekommendationerna har tagits fram gemensamt av experterna och bygger på konsensus bland deltagarna. Läkemedelsverkets ansvar var att facilitera mötet samt att tillsammans med experterna sammanställa kunskapsdokumentet. Som stöd för kunskapsdokumentet finns bakgrundsdokument som respektive författare ansvarar för. I bakgrundsdokumenten finns, förutom detaljerad information om varje ämnesområde, även referenser. Socialstyrelsen medverkade vid expertmötet, liksom i framtagandet av kunskapsdokumentet. Inför mötet har SBU (2013) identifierat kunskapsluckor inom området, vilka publicerats i SBU:s databas över vetenskapliga kunskapsluckor, se www.sbu.se. En vetenskaplig kunskapslucka innebär att systematiska litteraturöversikter saknas eller att de visar på osäker effekt. Man fann att detta gäller "Melatonin till barn som har flera funktionsnedsättningar och sömnproblem", "Melatonin som behandling av sömnproblem hos barn med adhd" och " Melatonin för i övrigt friska barn med sömnproblem". SBU konstaterade tidigare (2010) att "Effekten av alimemazin vid behandling av sömnbesvär" också är envetenskaplig  kunskapslucka.

    Abstract [en]

    Aim: The aim of this study was to test the psychometric properties of the Salutogenic Health Indicator Scale (SHIS) in an adolescent population. Methods: The investigation was performed among Swedish students aged 13–15 years (n = 817; 58% girls). The SHIS was assessed for respondent acceptability, and its psychometric properties were evaluated according to classical test theory (regarding unidimensionality, targeting, reliability, and external construct validity). Results: The adolescents found it easy to complete the questionnaire, which was completed in an average of 4 minutes. Exploratory factor analysis, which is based on polychoric correlations, identified one factor, supporting the instrument’s unidimensionality. Floor/ceiling effects were ⩽ 3.3%. Reliability estimates yielded a Cronbach’s alpha value of 0.93; the test–retest reliability (n = 50; 2-week interval) coefficients were 0.89 for the total SHIS score and 0.52–0.79 for item scores. Spearman correlations with other variables were based on a priori expectations (self-rated general health, 0.595; depressive symptoms, −0.773; anxiety, −0.577; and sleep problems, 0.519). Conclusions: Our observations support both the acceptability and the psychometric properties of the SHIS as a brief, unidimensional assessment tool for salutogenic health in adolescents. Further studies using modern test theory are needed to better understand the measurement properties of the SHIS, including the functioning of its response categories and its comparability between adolescents and adults.

    Skolan är en värdefull arena för hälsofrämjande arbete enligt såväl gällande skollag som vägledningsdokument för elevhälsan. På många skolor har livskunskap införts på schemat som ett led i det hälsofrämjande arbetet. De senaste decenniernas rapporter om ökande självrapporterad psykisk ohälsa har även lett till lansering av ett flertal manualbaserade (standardiserade) program med syfte att minska utåtagerande beteende, eller för att förebygga stress, depressiva symtom och ångest. Din Inre Styrka Aktiveras (DISA) är ett skolbaserat program som används frekvent på svenska högstadieskolor, framför allt för flickor, men även för pojkar. En utvärdering av DISA är genomförd i form av en doktorsavhandling (Garmy, 2016). I detta kapitel presenteras en sammanfattningav denna undersökning.

    AIM: The aim of this study was to investigate the feasibility and cost-utility of a school-based cognitive-behavioral (CB) depression prevention program.

    METHODS: A quasi-experimental trial with an intervention group and a control group, with follow-up measurements obtained at three and 12 months after baseline, was conducted. The setting was six Swedish municipalities. The participants were students in grade 8 (median age: 14). A total of 462 students (79% girls) were allocated to the school-based CB prevention program, and 486 students (46% girls) were allocated to the control group. The school-based CB prevention program, Depression in Swedish Adolescents (DISA), was presented by school health service staff and teachers once per week for 10 weeks.

    RESULTS: The main outcome measures were self-reported depressive symptoms and self-rated health; the secondary outcome measures were adherence and cost-utility. The intervention group decreased their self-reported depressive symptoms (as measured by the Center for Epidemiological Studies Depression Scale) and improved their self-rated health (as measured by the visual analog scale) at the 12-month follow-up more than the control group ( p < .05).

    CONCLUSIONS: Given the challenges of conducting a study in a complex, everyday school setting with baseline differences between the intervention and control group, it is difficult to make accurate interpretations of the effectiveness of the intervention. However, with these limitations in mind, the results indicate that the DISA program is a feasible school-based prevention program.

    Utvärdering av ett samverkansprojekt mellan Cancerreahbiliteringsmottagningen och Försäkringskassan i Kristianstad2016Report (Other academic)

    Cancerrehabilitering ska vara en integrerad del av svensk cancervård och alla patienter med cancer och deras närstående ska få möjlighet till rehabiliteringsinsatser utifrån sina individuella behov. Försäkringskassans uppdrag är att besluta om och betala ut en stor del av de förmåner som ingår i socialförsäkringen. För att förbättra bemötandet av sjukskrivna personer med cancer, utsågs två handläggare på Försäkringskassan att inrikta sig på sjukskrivna personer med cancer. Dessa två handläggare fick extra utbildning i ämnet, och ett samarbete inleddes mellan Försäkringskassan och cancerrehabiliteringsmottagningen på Centralsjukhuset i Kristianstad. Det övergripandesyftet med studien var att undersöka patienters och medarbetares erfarenheter och upplevelser av Försäkringskassans och Cancerrehabiliteringsmottagningens insatser. Utvärderingens form har teoretiskt inspirerats av metoder som beskriver vikten av att forskningspersoner görs delaktiga i forskning och utvärdering och därmed bidrar till kunskapsutveckling. Datainsamlingen bestod av enkäter och fokusgruppsintervjuer och både kvantitativa och kvalitativa analyser har använts. Slutsatsen är att patienterna är övervägande nöjda med bemötandet från såväl handläggarna på Försäkringskassan och på Cancerrehabiliteringsmottagningen, men att mer insatser krävs för att nå alla patienter.

    Background: Supporting positive mental health development in adolescents is an important school health concern, but there is a need to investigate the suitability of the interventions used.

    Aim: The aim is to investigate the experiences of school health professionals in conducting a universal school-based programme aimed at preventing depressive symptoms in adolescents.

    Methods: Twenty-two school health professionals participated in four focus groups. The interviews were analysed using qualitative content analysis.

    Findings: The overall theme identified in the study consists of ‘striking a balance between strictly following the manual and meeting the students’ needs’. Three subthemes emerged: ‘doing good and sowing seeds for the future’, working with insufficient tools, and ‘personal development as a professional and as an individual’.

    Conclusions: School health professionals conducting the programme found it valuable in a school setting, but considered support from the school administration essential.

    Abstract [en]

    The aim of this study was to examine sleep habits (i.e., bedtimes and rising times) and their association with nighttime text messaging in 15- to 17-year-old adolescents. This cross-sectional study analyzed data from a web-based survey of adolescent students attending secondary schools in southern Sweden ( N = 278, 50% female). Less than 8 hr of time in bed during school nights was significantly associated with more sleep difficulties, wake time variability on school days and weekends, daytime tiredness, and less enjoyment at school (all ps < .05). Sending and/or receiving text messages (Short Message Service [SMS]) at night was significantly associated with later bedtimes, shorter time in bed, daytime tiredness during school, and irregular sleep habits ( p < .05). These findings highlight the importance of regular, consistent sleep habits and the problems associated with sleeping with a cell phone in the bedroom.

    Context: Previous studies have supported the psychometric properties of the 22-item Zarit Burden Interview (ZBI-22) scale among family caregivers of people with various disorders, including Parkinson´s disease (PD). However, its short-forms have not been psychometrically tested among PD family caregivers, and available psychometric analyses have not accounted for the ordinal nature of item-level data.

    Objectives: To assess the psychometric properties of the ZBI-22 and its short forms among family caregivers of people with PD, while taking account for the ordinal nature of data.

    Methods: Cross-sectional postal survey ZBI-22 data from 66 family caregiver members (59% women; mean age, 69.6 years) of a local Swedish PD society branch were analysed according to classical test theory methods based on polychoric/polyserial correlations.

    Results: Missing item responses were ≤5%. Corrected item-total correlations were ≥0.42 and floor-/ceiling effects were <20%, besides for the briefest (4- and 1-item) short-forms (20% and 40% floor effects, respectively). Reliability was good for all scales (ordinal alpha, 0.89-0.95).  External construct validity was in general accordance with a priori expectations. Short-forms demonstrated good criterion-related validity (rs 0.87-0.99) and discriminative ability (AUC, 0.91-0.98) relative to the full ZBI-22.

    Conclusion: This study provides support for the reliability and validity of the ZBI-22 and its various short forms for use among PD family caregivers. In studies where caregiver burden is a central outcome, either ZBI-22 or -12 is suggested for use; other short-forms can be used when caregiver burden is of less central focus or for clinical screening.

    Abstract [en]

    The value of course evaluations has been debated since they frequently fail to capture the complexity of education and learning. Group Concept Mapping (GCM), a participant-centred mixed-method was explored as a tool for evaluation and development in nursing education and to better understand students' learning experiences, using data from a GCM-based evaluation of a research training assignment integrating clinical practice and research data collection within a Swedish university nursing program. Student nurses (n = 47) participated in a one-day GCM exercise. Focus group brainstorming regarding experiences from the assignment that the students considered important and instructive yielded 98 statements that were individually sorted based on their student-perceived relationships, and rated regarding their importance/instructiveness and need for development. Quantitative analysis of sort data produced a 2-dimensional map representing their conceptual relationships, and eight conceptual areas. Average cluster ratings were plotted relative to each other and provided a decision aid for development and planning by identifying areas (i.e., "Research methodology", "Patients' perspectives", and "Interviewer role") considered highly important/instructive and in high need for development. These experiences illustrate the use and potential of GCM as an interactive participant-centred approach to evaluation, planning and development in nursing and other higher health science educations.

    Abstract [en]

    Sample size is a major factor in statistical null hypothesis testing, which is the basis for many approaches to testing Rasch model fit. Few sample size recommendations for testing fit to the Rasch model concern the Rasch Unidimensional Measurement Models (RUMM) software, which features chi-square and ANOVA/F-ratio based fit statistics, including Bonferroni and algebraic sample size adjustments. This paper explores the occurrence of Type I errors with RUMM fit statistics, and the effects of algebraic sample size adjustments. Data with simulated Rasch model fitting 25-item dichotomous scales and sample sizes ranging from N=50 to N=2500 were analysed with and without algebraically adjusted sample sizes. Results suggest the occurrence of Type I errors with N≥500, and that Bonferroni correction as well as downward algebraic sample size adjustment are useful to avoid such errors, whereas upward adjustment of smaller samples falsely signal misfit. Our observations suggest that sample sizes around N=250 to N=500 may provide a good balance for the statistical interpretation of RUMM fit statistics studied here with respect to Type I errors and under the assumption of Rasch model fit within the examined framed of reference (i.e., about 25 item parameters well targeted to the sample).

    PURPOSE: SCOPA-SLEEP is a rating scale for night-time sleep and daytime sleepiness (DS) proposed for use among people with Parkinson's disease (PD) as well as others. We translated it into Swedish and assessed its psychometric properties in PD and age-matched healthy controls.

    METHODS: Following translation according to the dual-panel approach, the Swedish SCOPA-SLEEP was field-tested regarding comprehensibility, relevance and respondent burden (n = 20). It was then psychometrically tested according to classical test theory (data completeness, scaling assumptions, targeting, reliability and construct validity) using data from 149 people with PD and 53 age-matched healthy controls from the prospective Swedish BioFINDER study.

    RESULTS: SCOPA-SLEEP took a mean of 3.5 min to complete and was considered easy to use and relevant. Missing item responses were <8 %, corrected item-total correlations were ≥0.47 (except for one DS item among controls), factor analyses suggested one dimension per scale, floor/ceiling effects were ≤17 %, reliability was ≥0.85 except for the DS scale among controls (0.65) and construct validity was supported.

    CONCLUSIONS: Observations concur with previous evaluations, thus providing initial support for the Swedish SCOPA-SLEEP among people with PD. Further studies are needed to establish its generic properties and to understand its measurement properties in better detail.

    The 12-item Short Form Health Survey (SF-12) is a generic health rating scale developed to reproduce the Physical and Mental Component Summary scores (PCS and MCS, respectively) of a longer survey, the SF-36. The standard PCS/MCS scoring algorithm has been criticized because its expected dimensionality often lacks empirical support, scoring is based on the assumption that physical and mental health are uncorrelated, and because scores on physical health items influence MCS scores, and vice versa. In this paper, we review the standard PCS/MCS scoring algorithm for the SF-12 and consider alternative scoring procedures: the RAND-12 Health Status Inventory (HSI) and raw sum scores. We corroborate that the SF-12 reproduces SF-36 scores but also inherits its problems. In simulations, good physical health scores reduce mental health scores, and vice versa. This may explain results of clinical studies in which, for example, poor physical health scores result in good MCS scores despite compromised mental health. When applied to empirical data from people with Parkinson's disease (PD) and stroke, standard SF-12 scores suggest a weak correlation between physical and mental health (rs .16), whereas RAND-12 HSI and raw sum scores show a much stronger correlation (rs .67-.68). Furthermore, standard PCS scores yield a different statistical conclusion regarding the association between physical health and age than do RAND-12 HSI and raw sum scores. We recommend that the standard SF-12 scoring algorithm be abandoned in favor of alternatives that provide more valid representations of physical and mental health, of which raw sum scores appear the simplest.

    Detta kapitel handlar om arbetet med att utforma riktlinjer för hur  man möter och ger stöd till föräldrar som fått ett barn med funktionsnedsätt- ning (Downs syndrom). Tidsmässigt rör det om den första tiden för föräldrarna, om vad som sägs till dem och vad som görs inom förloss- nings- och barnsjukvården. Hur reagerar föräldrarna i den för dem nya, oväntade, okända och sannolikt skrämmande situationen? Vad vill för- äldrarna ha och vad behöver de ha och inte minst vad ges de för initialt omhändertagande? De nya nationella medicinska riktlinjerna har för första gången fått ett eget avsnitt som handlar om just detta första om- händertagande. Avsnittet har sin utgångspunkt i både beprövad erfaren- het och empirisk forskning. Vårdprogrammet presenterades 21 mars 2013.

    Detta arbete kan också ses som ett exempel på hur man kan ”fånga upp” ett vårdproblem från empirin, processa det genom  forskningsprocessen och hur man sedan implementera resultatet åter till empirin.

    Abstract [en]

    The purpose of the study was to to describe which factors of health related quality of life were associated with a high sense of coherence in a 5 year perspective. The sense of coherence, SOC-scale, EORTC QLQ C-30 and PR 25 questionnaires were applied to a sample of 222 men at baseline and over 5 years following radical prostatectomy. Result showed significant association to SOC in psychological aspects (emotional and cognitive functioning, p=<0.00-0.04 respectively 0.04) and for hormone treatment related symptoms (i.e. manhood p=<0.05). High SOC was associated with quality of life (index) in the early post treatment period and to aspects of general functioning (role-, emotional- respectively cognitive) in year 3 and 5. High sense of coherence was interpreted to be a health resource for experiencing life quality connected to different aspects at different time points of the data collection. The result showed stability in SOC and QoL scoring over 5 years.

    The aim of the study was to follow up inter-professional experiences of improvement work one year after a completed CII improvement program aiming at improve health care and social services for people with mental disabilities living in ordinary housing. This study was performed with a qualitative descriptive approach which employed six focus group interviews followed by a thematic analysis. The results revealed four themes; Self-awareness and insights; Behavior and actions in daily practice; Organizational cultures and subcultures; and Organizational practices, using Ken Wilbers' integral theory of four quadrants of realities as a holistic frame in the discussion.

    This study looked at 13 mental health nurses and social workers who were recently trained as case managers (CMs) and the work-related challenges they faced in community mental health services. Data were collected during ten peer supervisions sessions. Participants expressed pride and enthusiasm about their new function as CMs, but they also acknowledged that their new position meant they had to confront existing systems and posed several challenges, including organizational hindrances, economic prerequisites, nominated administrators, role function, and model fidelity. We conclude that the existing organizations seemed to be unprepared to hold and facilitate more person-centered approaches, such as designating CMs. The model fidelity is important, but has to be flexible according to clients' daily state.

    Abstract [en]

    BACKGROUND: Mental ill-health among the general population is increasing in Sweden. Primary Health Care (PHC) and Healthcare Centres (HCC), where district nurses (DNs) work, bear the basic responsibility for treatment of mental ill-health, while severe mental ill-health fall under the responsibility of psychiatric specialist care. The increased prevalence of mental ill-health in the community means that DNs increasingly encounter people with mental health problems - not least as a comorbidity. How well DNs are equipped to deal with mental ill-health is currently unclear.

    AIM: The purpose of this study was to explore district nurses' experience of encountering and dealing with mental ill-health among adult patients at healthcare centres.

    DESIGN: A qualitative explorative approach was used to capture the experiences of the phenomena under study.

    METHODS: Individual interviews were conducted with 10 DNs working at six HCCs. The interviews were transcribed and analysed by qualitative content analysis.

    RESULTS: The result emerged as several subcategories captured by three categories: (i) having competence - a prerequisite for feeling confident; (ii) nursing mental ill-health requires time and commitment; and (iii) working in an organisation without preparedness, encompassed by the synthesising theme; nursing mental ill-health requires specific competence and organisational support.

    CONCLUSION: Working as a DN requires formal and informal competence when encountering patients with complex health needs. The findings revealed that the DNs could feel insecure regarding how to deal with patients with mental ill-health due to lack of knowledge. Assessment of patients with mental ill-health is time- and energy-consuming and calls for improved teamwork at HCCs as well as effective collaboration with psychiatric specialist care and other care givers. The DNs responsibility to fulfil their work considering the increasing number of mental ill-health among people that seeks help at HCCs needs to be acknowledged and met by the PHC organisation.

    Abstract [en]

    BACKGROUND: Apathy, a prevalent and clinically relevant symptom in neurodegenerative disease, is often evaluated by the instrument Apathy Evaluation Scale (AES). However, this instrument has not been translated into Swedish, halting clinical and research efforts. Furthermore, previous studies lack analyses of some basic properties, such as the legitimacy of a total score, or have analysed dimensionality by questionable methods.

    AIM: To translate and psychometrically evaluate a Swedish version of the AES.

    METHOD: The AES was translated, and its psychometric properties were tested in the Swedish BioFINDER study, including cognitively well elderly, and subjects with mild cognitive or parkinsonian symptoms. Psychometric analyses were conducted according to classical test theory (CTT) and aimed to resemble those performed in the English original study by Marin et al. in 1991. Dimensionality was additionally analysed on a matrix of polychoric correlations and parallel analyses.

    RESULTS: Data indicate that the Swedish AES performs satisfactorily regarding data completeness, scaling assumptions, targeting, and reliability. Principal component analyses (with parallel analysis) of polychoric correlation matrices identified a single component. Convergent and discriminative validity correlations accorded with a priori expectations.

    CONCLUSIONS: The study provides initial support that this Swedish AES performs similarly to the English original, and exhibits acceptable psychometric properties according to CTT, including supported unidimensionality, and may be adopted for use in clinical and research settings.

    Tensions in the field of health care: knowledge networks and evidence-based practice: an action research approach2011Doctoral thesis, monograph (Other academic)
    Abstract [en]

    Empirically, this thesis has focused on nine research and development (R&D) networks set up to promote a professional approach to care and strengthen the collaboration between health care sectors in a Swedish health care setting. The research project was embedded in an action research approach intended to encourage network development by means of a dialogical process. The specific research question was: What are the actors‟ perceptions of knowledge networks and how might we account for the networks‟ evolution, role and ways of working? Bourdieu‟s concepts reproduction and symbolic violence were used as analytical tools and were chosen as a way of answering and explaining the empirical story line. Data was collected by use of a multi-method approach consisting of 39 interviews, observations, document review and reflexive notes. The intention was to elicit data that supported both network development and the theoretical explanation to come. It appeared that the networks concerned had several advantages, such as being a forum for internal dialogue and exchange of experiences. In addition, two main patterns emerged: Firstly, most of the participants within the networks were advocates of a linear top-down model of implementation of evidence-based knowledge into practice. Secondly, they experienced inertia in the transfer process. From the collaborative process undertaken it emerged that their linear top-down model of knowledge transfer seemed to be firmly rooted. Theoretically, the thesis contributes to an understanding of why the process of knowledge transfer was considered by the participants within the networks to be a sluggish process. The thesis also contributes to an explanation of why they adhered to the macro-discourse of evidence-based medicine at the expense of involving practitioners outside the networks in horizontal patterns of exchange. It is argued that the networks had a symbolic value and were also a product of and reproduced the evidence-based discourse and the prevailing structures within their field. This contrasted with the role of networks as arenas for generation of local knowledge in the network literature. A major challenge facing health care sectors is that of how to support practitioners in the incorporation of new practices resulting in actual changes.

    Bakgrund: Depression är en vanligt förekommande sjukdom som behandlas främst med psykoterapi och antidepressiva men det behövs fler resurser och komplementära omvårdnadsåtgärder. Yoga har blivit allt mer aktuellt inom vården. Det huvudsakliga fokuset inom yoga är att lära sig vara i nuet genom meditation, andning och rörelse. Sjuksköterskan har det yttersta ansvaret för omvårdnad och ska hålla sig uppdaterad med den senaste forskningen för omvårdnad. Syfte: Syftet var att undersöka effekten av yoga som en omvårdnadsåtgärd vid depression och depressiva symtom. Metod: Kvantitativ litteraturstudie. Resultat: Yoga har visat sig ha en positiv effekt på depression och depressiva symtom efter 4 veckor till 6 månaders intervention. Diskussion: I diskussionen presenteras tre huvudfynd: Yoga har en positiv effekt vid depression och depressiva symtom, yoga har en positiv effekt för kvinnor vid depression och depressiva symtom, yoga kan användas som en komplementär omvårdnadsåtgärd vid depression och depressiva symtom.

    Abstract [en]

    Objective: To evaluate a set of psychometric properties (i.e., data completeness, targeting andexternal construct validity) of the Parkinson’s disease Activities of Daily Living Scale (PADLS) in people with Parkinson’s disease (PD). Specific attention was paid to the association between PADLS and PD severity, according to the Hoehn & Yahr (H&Y) staging.

    Methods: The sample included 251 persons with PD (mean age 70 [SD 9] years). The data collection comprised a self-administered postal survey, structured interviews and clinical assessments at home visits.

    Results: Data completeness was 99.6% and the mean PADLS score was 2.1. Floor and ceiling effects were 22% and 2%, respectively. PADLS scores were more strongly associated (rs>0.5) with perceived functional independence, dependence in ADL, walking difficulties and self- rated PD severity than with variables such as PD duration and cognitive function (rs<0.5).PADLS scores differed across H&Y stages (Kruskal-Wallis test, p<0.001). Those in H&Y stages IV-V had more ADL disability than those in stage III (Mann-Whitney U-test, p<0.001), whereas there were no significant differences between the other stages.

    Conclusion: The PADLS revealed excellent data completeness, acceptable targeting and external construct validity. It seems to be well suited as a rough estimate of ADL disability inpeople with PD.

    OBJECTIVES: To identify information available in six European countries (England, France, Germany, Netherlands, Portugal, Sweden) that addresses the specific needs of people with young onset dementia (YOD) and their carers, and identify gaps.

    METHODS: Search of websites of organisations with potential interest in dementia. Narrative synthesis and comparative analysis.

    RESULTS: 21 sources of information were identified (Netherlands 6, England 6, France 3, Germany 2, Portugal 2, Sweden 2); 11 were from voluntary sector organisations. Sources dedicated to YOD were limited (4 websites, 4 books); all other YOD information was sub-entries in generic dementia sources, difficult to locate and with limited coverage of relevant topics. Gaps related to implications of living with YOD in Germany, Portugal and Sweden.

    CONCLUSION: Availability of information varies among countries, some having no dedicated source and incomplete coverage of issues of importance to YOD.

    PRACTICAL IMPLICATIONS: Information is an important means of supporting carers; their needs change as the condition progresses. A comprehensive resource collating key information is needed so that the issues that differentiate the specific needs of people living with YOD from those of people with dementia in older age are available and easily located.

    Bakgrund: Idag genomförs drygt 50 hjärttransplantationer i Sverige per år. Som sjuksköterska är det viktigt att förstå patientens känslor och upplevelser under väntan för att kunna främja hälsa och lindra lidande. Indikationerna för en hjärttransplantation är en hjärtsvikt i funktionsklass tre eller fyra.

    Syfte: Att belysa patientens upplevelse under väntan inför en hjärttransplantation.

    Metod: Studien är en allmän litteraturöversikt som grundar sig på tio vetenskapliga artiklar. Sökningen gjordes i välkända omvårdnadsvetenskapliga databaser. Analysen genomfördes i flera steg. En övergripande genomläsning gjordes först för att sedan gå djupare in i varje artikel och hitta huvudkategorier och underkategorier.

    Resultat: En upplevelse av att vara begränsad framkom som ett fynd. Den fysiska begränsningen var ett faktum precis som en psykisk begränsning. Deltagarna kände sig sjuka. Det sociala nätverket påverkades både positivt och negativt genom att vänner drog sig undan men även nya vänskapsband knöts på grund av den nya situationen. Oro fanns för om det nya hjärtat skulle passa och om det skulle dyka upp i tid. Information framkom som något som både kan hjälpa samt stjälpa.

    Diskussion: Huvudfynden var att vänta på något utan att kunna påverka situationen är påfrestande, att information kan både hjälpa och stjälpa samt att få känna gemenskap skänker tröst och lättnad. Studiens resultat kopplas till närhetsetik, KASAM samt Disease och Illness.

    Abstract [en]

    Objectives. The aim of this study was to investigate perceived and ideal body image (BI) and associated factors among female university students in Saudi Arabia. Methods. This cross-sectional study included 663 university female students. Anthropometric measurements including weight, height, BMI, and BI perception (the 9-figure silhouette) were obtained. Descriptive and logistic regression analysis were conducted. Results. An agreement between actual, perceived, and ideal BI was found in 23% of the participants. Behavioral (activity levels), social (presence of obese parents and fathers’ level of education), and economic factors (households’ monthly income, number of cars in the household, and kind of residence) were positively and significantly associated with the desire to be thinner. Similarly, socioeconomic associations (number of sisters and number of cars in the household) correlated positively and significantly with the desire to be heavier. Conclusions. The whole family should rather be considered in interventions related to appearance concerns and BI discrepancies. Furthermore, campaigns targeting improvement of adolescents’ physical self-image should be a major priority of the public health sector.

    Discrepancies between actual, perceived and ideal body image among young female university students in southwestern Saudi Arabia2014Conference paper (Other academic)
    Abstract [en]

    Rationale: Body-shape preference is an important determinant of obesity and underweight, but studies on body image among females in the Arab region are, at most, scanty. The aim of this study was to describe the prevalence of underweight and overweight/obesity among Saudi Arabian female university students, and to investigate discrepancies between their actual, perceived and ideal body shape.

    Methods: This cross sectional study included 663  female students in a university centre in Saudi Arabia. Anthropometric measurements including weight, height, BMI and body image scale (9-figures silhouette) were used. A cross-tabulation was made between “warped self-image” against “subjective goal image”. Regression analysis was used to predict variables related to body image perception.

    Results: The participants had a mean age of 20.4 years (SD 1.5), 16% were underweight and 25% were over- weight/obese.Discrepancy in subjective goal image: 44% wished they were slimmer and 20% that they were heavier than what they perceived themselves. Thus, 36% were happy with their current body shape.Warped self-image: It was 16% that thought they were slimmer and 18% that they were heavier than what they actually were. Thus, in 61% of cases the perceived body image was in agreement with their actual body shape. Cross-tabulation showed that only 23% had an agreemen tbetween their actual, perceived and ideal body shape. Of all variables entered into the regression analysis, only BMI was able to predict how the subject perceives herself in the body image scale (R square = 0.676; p < 0.000). None of the variables entered was significant enough to predict how the subject wishes to look like.

    Conclusion: Discrepancies between actual, perceived and ideal body image is common among the Saudi Arabian female students and can possibly lead to eating disorders and increased  prevalence of underweight oroverweight/obesity.

    Abstract [en]

    Background: Young Onset Dementia (YOD), defined by first symptoms of cognitive or behavioral decline occurring before the age of 65 years, is relatively rare compared to dementia of later onset, but it is associated with diagnostic difficulty and heavy burden on affected individuals and their informal carers. Existing health and social care structures rarely meet the needs of YOD patients. Internet-based interventions are a novel format of delivering health-related education, counseling, and support to this vulnerable yet underserved group. Methods: The RHAPSODY (Research to Assess Policies and Strategies for Dementia in the Young) project is a European initiative to improve care for people with YOD by providing an internet-based information and skill-building program for family carers. The e-learning program focuses on managing problem behaviors, dealing with role change, obtaining support, and looking after oneself. It will be evaluated in a pilot study in three countries using a randomized unblinded design with a wait-list control group. Participants will be informal carers of people with dementia in Alzheimer's disease or behavioral-variant Frontotemporal degeneration with an onset before the age of 65 years. The primary outcome will be caregiving self-efficacy after 6 weeks of program use. As secondary outcomes, caregivers' stress and burden, carer health-related quality of life, caring-related knowledge, patient problem behaviors, and user satisfaction will be assessed. Program utilization will be monitored and a health-economic evaluation will also be performed. Conclusions: The RHAPSODY project will add to the evidence on the potential and limitations of a conveniently accessible, user-friendly, and comprehensive internet-based intervention as an alternative for traditional forms of counseling and support in healthcare, aiming to optimize care and support for people with YOD and their informal caregivers.

    Syftet med denna studie var att beskriva äldres upplevelser och erfarenheter av uppsökandeverksamhet i Osby kommun. Förebyggande hälsoinsatser för äldre bedrivs iSverige med olika benämningar. I Osby kommun bedrivs de med benämning uppsökandeverksamhet. Uppsökande verksamhet med förebyggande insatser och råd kanstärka äldres egenvård och skjuta upp funktionsnedsättning samt förbättra den äldrepersonens välbefinnande och livskvalitet. Ur samhällsekonomiskt perspektiv kan uppsökandeverksamhet också minska kostnader för vård och omsorg på sikt.Metoden är en kvalitativ intervjuundersökning med öppna frågor. Studien genomfördesunder juni- november år 2015 och tio äldre personer intervjuades. Av dessa var detfem kvinnor och fem män. Intervjuerna analyserades enligt Graneheim och Lundmanskvalitativa innehållsanalys.I resultatet framkom två kategorier och sju subkategorier. Essenserna som hittadesfrån intervjuerna var: förväntningar, tillit i mötet, attityder till förändring, information,utförarens kompetens och personlighet, vikten av relation och kreativitet. Resultatetvisar tydligt att äldre upplever arbetet med uppsökande verksamhet värdefullt och villse detta arbete fortsätta.Denna studie ger utan tvekan stöd att utföra uppsökande verksamhet och uppsökandeverksamhet kan ses som en redskap för de äldre att skapa bra förutsättningar att bevarahälsan och ger möjlighet till ett aktivt och självständigt liv.

    Background: In order to better understand people in demanding medical situations, an awareness of existential concerns is important. Studies performed over the last twenty years conclude that when dying and death come closer, as in the case with older people who are stricken by infirmity and diseases, existential concerns will come to the fore. However, studies concerning experiences of existential loneliness (EL) are sparse and, in addition, there is no clear definition of EL. EL is described as a complex phenomenon and referred to as a condition of life, an experience, and a process of inner growth. Listening to someone who knows the older person well, as significant others often do, may be one way of learning more about EL. Methods: This study is part of a larger research project on EL, the LONE study, where EL is explored through interviews with frail older people, their significant others and health care professionals. The aim of this study was to explore frail older (> 75) persons' EL, as interpreted by their significant others. The study is qualitative and based on eighteen narrative interviews with nineteen significant others of older persons. The data was analysed using Hsieh and Shannon's conventional content analysis. Results: According to the interpretation of significant others, the older persons experience EL (1) when they are increasingly limited in body and space, (2) when they are in a process of disconnecting, and (3) when they are disconnected from the outside world. Conclusion: The result can be understood as if the frail older person is in a process of letting go of life. This process involves the body, in that the older person is increasingly limited in his/her physical abilities. The older person's long-term relationships are gradually lost, and finally the process entails the older person's increasingly withdrawing into him- or herself and turning off the outside world. The result of this study is consistent with previous research that has shown that EL is a complex phenomenon, but the implications of this research include a deepened understanding of EL. In addition, the study highlights the interpretations of significant others.

    Abstract [en]

    BACKGROUND: Family caregivers' experiences of formal care when caring for persons with dementia through the process of the disease is sparsely investigated.

    AIMS: To investigate family caregivers' experiences of formal care when caring for a person with dementia, through the stages of the disease.

    DESIGN: A qualitative approach with focus group interviews.

    METHODS: Four focus group interviews were conducted in October 2011 with 23 spouses and adult children of persons with dementia and analysed with content analysis.

    RESULTS: The participants' experiences of formal care when caring for a person with dementia were captured in the theme 'Family caregiving requires collaboration with formal care to get support adjusted to needs specific to the stages of dementia'. This can be broken down into the categories 'The dementia diagnosis - entry into formal care as a novice family caregiver', 'Needing expanded collaboration with formal care to continue care at home' and 'Being dependent on a nursing home and trying to maintain involvement'.

    CONCLUSION: Family caregiving requires collaboration with formal care to get support adjusted to the individual's needs, specific to the stages of dementia. Caregivers experience a transition process with three main turning points: the dementia diagnosis; when they realise increased need for formal care to continue caring at home; and when the person with dementia is moved into a nursing home. The interviewed caregivers experience formal care reactive to their needs and this often promoted unhealthy transitions. Formal care needs to be proactive and deliver available care and support early on in the dementia trajectory. Interventions should focus on facilitating a healthy transition for family caregivers through the trajectory of the dementia disease to ensure their well-being.
