    Optimization of several parameters in order to reduce time in antibiotic susceptibility testing in a clinical laboratory2017Konferansepaper (Annet vitenskapelig)
    Abstract [en]

    Background - When sepsis or bacteraemia is suspected, patient blood samples are cultivated in blood culture bottles and then further incubated for identification of the organism and antimicrobial susceptibility testing. These methods are slow, identifying causative pathogens in a couple of hours, and antibiotic susceptibility results within 18-36 hours.

    Here we present optimization of several parameters in order to evaluate if the MBT ASTRA™ method can be a rapid tool, used for routine antibiotics susceptibility testing, in a clinical laboratory.

    Methods – MALDI-TOF MS measurements were performed with a Microflex LT/SH bench-top mass spectrometer (Bruker) with standard settings. The resulting spectra were uploaded in the MBT-ASTRA™ software, which normalizes the peaks and determines the AUC and RG values for each setup.

    Results - The bacterial preparation steps generated a new protocol, which reduced time with 30-60 minutes.

    The antibiotics susceptibility test was optimized for 90 minutes incubation time. 200 µl McFarland 0.5 bacterial suspension in broth were incubated in broth at 37°C, with and without 32 µg/ml Cefotaxime, 16 µg/ml Meropenem and 4 µg/ml Ciprofloxacin.

    The suspensions were transferred to 0.45 µm pore size filter membraned 96 well plate. They were centrifuged; washed; fixated and eluted; put on a MALDI-target, and covered by matrix solution. All could be automated with robot, which reduced time with 60 minutes.

    Conclusion – Rapid susceptibility testing becomes more requested with the increase of resistance bacteria causing infections. Our study can be a valuable tool for clinical laboratories striving for reduction in time handling of antibiotic susceptibility testing.

    Youth - the future manpower: studies on unemployment, quality of life and work attitudes in young people2005Doktoravhandling, med artikler (Annet vitenskapelig)
    Abstract [en]

    The aim was to gain knowledge of experiences of unemployment in young adults with special regard to quality of life (QOL) and to highlight work attitudes and related factors among adolescents. Three studies were performed in Kristianstad municipality, Sweden. In Study 1, a questionnaire was sent in 1998 to 264 unemployed young adults (md=22 years, range 20?25) and to a control group of 528 randomly selected young adults who were studying or working. In Study 2, interviews with 16 unemployed young adults(md=22 years, range 20?24) were performed. In Study 3, 606 students in upper secondary school (md=18, range 18?22) were invited to participate in a questionnaire study. In Study 1, the prevalence of three or more mental health symptoms was 41% in males and 60% in females. Unemployed reported lower mental health than controls. In unemployed females, without very good parental support and with low self-esteem, 76% had three or more mental health problems. Unemployed had lower QOL than controls. However, this picture seemed diversified as 35% reported that QOL had become worse, 41% reported unchanged QOL and 24% reported that QOL had become better since unemployment started. In Study 2, the variation in experiences of unemployment was established in more depth by the qualitative research approach. Perception and action were constituents of QOL among unemployed young adults and QOL was regarded as an outcome of these two dimensions. Perception and action varied greatly, and four groups with substantial differences in QOL were identified: fighters, optimists, exposed and unworried subjects. Study 3 focused on work attitudes. Two separate dimensions of work attitudes were found: work ethics (WE), including attitudes to using the social welfare system, and general work attitudes (GWA), including the general attitude towards work and life as a context. Being female, having very positive work experiences, having good QOL, high SOC and good subjective health were positively related to WE. Being female, studying on a practical programme, being content with the school period, having high SOC and very good parental support were positively related to GWA. This thesis shows that unemployment is experienced in various ways, and these experiences are not only negative. Individual factors such as QOL, SOC, health and parental support are of importance to both unemployment experiences and attitudes to work. Consequently, when the purpose is to increase young adults? possibilities to participate in working life, it can be argued that strategies where the focus is on the individual perspective instead of the collective approach are relevant.

    Abstract [en]

    BACKGROUND: Person-centered care (PCC) is a key component to effective illness management and high-quality care but conceptually underdeveloped in outpatient care settings for long-term disorders such as rheumatoid arthritis (RA), and there is a lack of instruments measuring patient perceived PCC.

    AIM: To conceptualize and operationalize outpatient PCC as a basis for measuring patient-perceived PCC in nurse-led outpatient rheumatology clinics.

    DESIGN: A conceptual outpatient PCC framework was developed based on theory and patient experiences.The resulting framework was operationalizedinto items that were tested regarding their accordancewith the theoretical framework and their measurement properties according to Rasch measurement theory (RMT) using the RUMM2030 software. 

    RESULTS: The conceptual framework comprised five intermingled domains (personalization, shared decision-making, empowerment, communication, and social environment) that were operationalized into a 24-item PCC instrument for outpatient care in rheumatology (PCCoc/rheum). RMT analyses found items to successfully map out a quantitative continuum from lower to higher PCC levels, with generally acceptable model fit and a reliability of 0.88. Three items exhibiting multidimensionality, dysfunctional response categories, and/or unexpected hierarchical ordering were omitted. The revised PCCoc/rheum showed good accordance with the conceptual framework, improved fit, functioning response categories, and no DIF; reliability was 0.86.

    SUMMARY AND IMPLICATIONS: We found support for the PCCoc/rheum as a conceptually based outcome measure of patient perceived PCC. Results implied minor theoretical reconsiderations, and related scale revisions improved theoretical and clinical clarity. While in need of further testing, the PCCoc/rheum has great potential as a measure of outpatient PCC from a patient perspective.

    We describe the design background of the mathematics game Fingu for iPad aimed at 4 to 8 year old children. We first describe how Fingu theoretically can support children's development of fundamental arithmetic skills, focusing on conceptual subitizing, the embodiment of numerosity, and finger gnosis. Then we present the results of an exploratory micro-longitudinal study of the game with 11 5- and 6-year old children playing the game for several weeks and being filmed at three occasions. We discuss how their behavior with the game develops over time and can be related to the development of arithmetic skills. Finally we discuss how we will proceed testing the effectiveness of Fingu in a larger controlled study.

    Sedan mer än 200 år har ”Bildning för alla!” varit en pedagogisk utmaning. Devisen är knuten till tankar om likvärdighet, delaktighet och allas rätt till utbildning i ett inkluderande samhälle, men i boken betonas samtidigt de pedagogiska utmaningar som dessa intentioner för med sig. I antologin tar 26 författare från de nordiska länderna och Storbritannien upp aktuella frågor som på olika sätt knyter an till uppgiften om hur utbildningssystemet kan utvecklas i förhållande till elevers olikheter. I boken behandlas teman som varit centrala i forskningen hos professor Jerry Rosenqvist, som nu efter mer än fyra decennier av pedagogisk verksamhet går i pension. Boken tillägnas honom.

    Vårdkedja för ungdomar eller professionella?: en processutvärdering av projektet ”Motverka våld och gäng”2009Rapport (Annet vitenskapelig)

    På uppdrag av Socialdepartementet har SiS under perioden 1 juli 2006 till 31 december 2008 genomfört ett projekt som tog namnet Motverka Våld och Gäng (MVG). I projektet ingick 15 utvalda kommuner, i första hand storstäderna Stockholm, Göteborg och Malmö och deras grannkommuner. I uppdraget ingick att subventionera kommunernas vårdavgifter och att anställa kvalificerade samordnare. Tusentalet ungdomar skrevs in på SiS institutioner från de deltagande kommunerna under projektet som totalt kom att kosta 223 miljoner kronor. Målet med projektet var att de ungdomar som deltog, skulle uppnå positiva och bestående förändringar av sin livssituation, bryta umgänget med kriminella kamrater och förbättra sin skolgång. I uppdraget ingick också att förbättra familjens och nätverkets möjligheter att fungera. Projektet skulle dessutom bidra till att skapa fungerande modeller för samverkan mellan SiS och socialtjänsten. Projektet genomgår två utvärderingar, dels denna processutvärdering, dels en effektutvärdering vars resultat väntas år 2010. Den här redovisade processutvärderingen av MVG-projektet har genomförts av Goran Basic, Joakim Thelander och Malin Åkerström vid Lunds universitet. Materialet består av intervjuer med ungdomar, föräldrar, samordnare, socialtjänstpersonal, institutionspersonal och projektledning. Därutöver har möten observerats och en enkät distribuerats till socialtjänsten.

    Abstract [en]

    The risks and insecurities emphasised in contemporary societies have given rise to diverse forms of policing, such as transnational, intelligence- and operative-based police collaborations. The purpose of this ethnographic study is to analyse the collaborative work among intelligence and operative personnel from different border authorities in Sweden, Finland, Estonia, Lithuania, and Latvia. The focus of this study is how the collaboration activities created an in-group mentality among the participating officers which was contrasted with another category, that of the “norm-dissolving Russian”. This category included concepts such as being a spy, a criminal and a potential military threat, and became a sort of "Other" that reinforced their own in-group bonds. Intelligence and operative personnel present in the analysed collaborative sequences create their professional identities by contrasting themselves with these categories. They build up a kind of group feeling and present a particular moral order that is created and re-created during their collaborative work. The norm-stable and the threatening norm-dissolving moralities are created within the interaction – especially when meeting work groups that differ from their own normative code. Morality is thus created and re-created in the encounter with people that are associated with being the “enemy”, present in the situation both in physical and invisible form.

    An intervention applying a palliative care approach in residential care: influences on nurse assistants' work situation2013Konferansepaper (Annet vitenskapelig)
    Abstract [en]

    Introduction

    Palliative care is an approach characterized by a holistic view of the person, which supports the individual for living with dignity and with the best possible level of wellbeing until the end of life. Those who, in practice, provide care in Swedish residential care facilities for older people (RCFs) are nurse assistants (NAs), a work group with limited education. They also often stand alone with difficult decisions with regards to the care provision, as the registered nurse has the role of consultant rather than leader. In an explorative qualitative study we found that NAs experienced that it was difficult to focus on relationship aspects in their work, as the main discourse in the residential care focused on tasks. Palliative care was described as something that was applied only during a short define phase i.e. last days of the residents’ life. These results were used as a basis for development of an intervention focus on a palliative care approach in RCFs. The intervention consisted of study circles with NAs, and workshops together with their leaders, focusing on improvement work.

    Aim

    To investigate how an intervention focusing on a palliative care approach in RCFs influenced the NAs and their work.

    Method

    Two quantitative studies, based on a questionnaire, including several measurements, that was answered pre-, and post- intervention and one qualitative study based on individual interviews after the intervention. A total of 75 nurse assistants participated in the intervention and answered the questionnaire at baseline and at two follow-ups, in comparison with 110 nurse assistants who served as controls. 

    Results

    The evaluation of the intervention showed that the NAs, after the intervention, had increased their focus on the residents’ situation and to a greater extent stated that they focused on the residents’ life stories and on aspects that brought meaning to their lives. The NAs also stated that they experienced less criticism from their superiors as well as from the residents after the intervention. However, the evaluation also showed that the NAs had a more negative view of the leadership, were more critical to the medical and the nursing care, and that their job satisfaction had decreased after the intervention. Interviews after the intervention showed that they, as a result of the intervention, had not only gained increased insight into their own significance in their encounter with residents and their relatives, but also an increased awareness of the needs of the residents and their relatives. The intervention also contributed to an increased openness in the workgroup. However, the nurse assistants also expressed frustration over barriers, primarily in the form of a lack of resources and limited leadership, standing in the way of the implementation of changes.

    Conclusion

    The results indicate that the NAs, through discussions and reflections over praxis in their ordinary work group, developed an increased awareness about, and focus on relationship aspects. However, in spite of the leaders’ involvement, the intervention was not sufficient for changing the organizational prerequisites for working in line with the palliative care approach. When implementing a palliative care approach in RCFs, more focus on support to the leaders in needed in order to maintain sustainable changes.

    The aim of this thesis was to study nurse assistants’ experience of palliative care and to investigate how an intervention with a focus on a palliative care approach in residential care facilities influenced the nurse assistants and their work situation. The thesis is based on two qualitative and two quantitative studies, focusing on nurse assistants working at residential care facilities for older people. The qualitative studies were based on focus-group interviews before the intervention (I) and individual interviews after the intervention (IV). The quantitative studies (II, III) were based on a questionnaire, including several measurements, that was answered pre-, and post- intervention. The intervention consisted of study circles with nurse assistants, and workshops together with their leaders, focusing on improvement work. A total of 75 nurse assistants participated in the intervention and answered the questionnaire at baseline and at two follow-ups, in comparison with 110 nurse assistants who served as controls. The results show that the nurse assistants experienced that it was difficult to focus on “being”, i.e. on relationship aspects in their work, since the main discourse in residential care focused on “doing”, i.e. on task oriented aspects. Palliative care was described as something that was only applied during a short and defined phase, namely the very last days of the residents’ life. The results also show that nurse assistants experienced difficulties facing emotional and existential issues with regards to both the residents and their relatives (I). The results of study I were, in turn, used as a basis for the development of the intervention. The evaluation of the intervention showed that the nurse assistants, after the intervention, had increased their focus on the residents’ situation and to a greater extent stated that they focused on the residents’ life stories and on aspects that brought meaning to their lives (III). The nurse assistants also stated that they experienced less criticism from their superiors as well as from the residents after the intervention (II). However, the evaluation also showed that the nurse assistants had a more negative view of the leadership (II), were more critical to the medical and the nursing care (III), and that their job satisfaction had decreased (II) after the intervention.     Abstract [en]

    Background: Patient-reported outcome measurements are important for measuring changes in patients’ health over time, evaluating the quality of care given, and improving the quality of care service. The Integrated Palliative care Outcome Scale (IPOS) is one such measure, capturing clinically important physical, psychological, social, and existential aspects of patients’ palliative care needs. When patients are unable to complete the IPOS questionnaire themselves with or without assistance, a staff version is also available.

    Aim: To translate and culturally adapt the IPOS Patient and Staff versions for use in the Swedish context.

    Method: The process comprised forward and backward translations, cultural adaptation, and expert group reviews. To validate the resulting Swedish version of IPOS, cognitive interviews were conducted with 13 patients and 15 staff from various care contexts.

    Preliminary results: The Swedish expert group changed some words and grammar due to minor discrepancies in the back translation process. The participants in the cognitive interviews responded positively overall to the questionnaire. Deteriorating health and not having Swedish as the native language did not cause problems in completing the questionnaire. After the first round of cognitive interviews, problematic questions and answer options were rephrased, and redundant text was deleted.

    Conclusion: The Swedish IPOS (Patient and Staff versions) has been validated linguistically and culturally, and is now available for clinical use. The next final step in the validation process will be to test the psychometric performance of the Swedish questionnaires.

    Abstract [en]

    Outdoor educators are concerned about a perceived human disconnection from nature. There is awareness of a lack of human affiliation, connection, or identity with nonhuman nature and its impact on attitudes and behaviors. This essay raises the possibility that despite our concern, we may contribute toward this disconnection via language that supports a separation of the natural and the cultural. Our ability to separate ourselves conceptually from the rest of nature may be partially to blame for environmental degradation, therefore challenging the nature-culture dichotomy is both useful and constructive. This essay will present examples of how outdoor educators can attempt to get past this problematic dichotomy and motivate more relational discourse within the practice of outdoor education.

    Relevance’ is one of the most commonly used terms when it comes to reforms in science education. The term is used in manifold ways. It can be understood – among other things – as meeting an interest, fulfilling needs or contributing to intellectual development. Many components of relevant science education go beyond single contents and concepts; many challenges are tied to cross-curricular goals. Specifically, when it comes to the societal and vocational relevance of science education, many demands can only be met when we develop corresponding skills across disciplines and grade levels. This chapter focuses on a set of such cross-curricular goals from a chemistry education perspective, namely, education for sustainability, critical media literacy, innovation competence, vocational orientation and employability. It relates them to the idea of relevant chemistry and science education. Directions for research and curriculum development will be suggested that emerge from taking into account

    Hot och våld som riktas mot kvinnor med psykisk sjukdom är vanligt förkommande i vårt samhälle i dag och medför att kvinnornas psykiska hälsa och självkänsla försämras. För att tillgodose deras behov av vård och stöd måste personal inom det samhällsbaserade vård- och stödsystemet i större utsträckning än vad som görs i dag ställa frågor om utsatthet samt informera om hjälp som finns att tillgå. Dessutom behöver personal utbildning för att bättre förstå bakomliggande orsaker till våld mot dessa kvinnor samt hur de på bästa sätt ska bemötas. För att personal ska kunna tillgodose behov hos våldsutsatta kvinnor med psykisk sjukdom måste verksamhetsledningen oavsett beslutsnivå legitimera personalens arbete med denna grupp kvinnor.

    Background/Objectives

    Persons suffering from severe mental illness (SMI) live – and prefer to live – independently, in either private or public apartment blocks without on-site services. Living in own apartments increase feelings of safety and well-being and function as a robust social status marker. Landlords are important actors in gaining stability and sustainability and it has been found that landlords have a plethora of preconceptions, attitudes, emotions as well as well as ethical dilemmas in offering apartments to this group of persons. Today there is a lack systematic knowledge about the role landlords have come play in providing sustainable housing for these persons. The main aim of this qualitative study was to describe landlords’ experiences of having tenant suffering from SMI

    Methods

    Sixteen landlords in various parts of Sweden participated in open in-depth interviews three years after the government proclaimed a vision zero regarding homelessness among individuals with SMI. Data was subjected to thematic latent content analysis.

    Results

    Landlords experienced being confronted with difficult circumstances such as mismanagement of apartments, sensitivity and provocative behaviors in relation to both tenants with SMI and neighbors. In acute situations landlords tried to collaborate with the community based psychiatric service system but were neglected. As a result and without the knowledge of how to best help they started to provide support to tenants with SMI involving going beyond professional boarders.

    Discussion/Conclusion

    The findings give reasons to conclude that community-based psychiatric services need to be more pro-active in their collaboration with landlords. Also education interventions with a focus on how to best help tenants with SMI need to be developed and implemented.

    Samhället ställer ökade krav på uppföljning av hälso- och sjukvårdens kvalitet. Individualiserad vård anges som något av det mest centrala i vården och omvårdnaden av patienten (ICN, 2006) och betonas i olika policy- och kvalitetsdokument både nationellt och internationellt (Nilsson, 2005, OECD, 2004). I närsjukvårdsprofileringen och utvecklingen som redovisats för Nordöstra Skåne betonas att vården skall utgå från patientens behov (Region Skåne, 2003) och innefatta utveckling av nya vård- och behandlingsformer där patienten och närstående står i fokus. Behovet av vård och omvårdnad och dess utförande varierar då en patient blir inlagd på sjukhus och det är viktigt att beakta att patienten och dennes närstående kan känna sig sårbara och utsatta (Hälso- och Sjukvårdsrapporten från 2001). Utvärderingar som belyser ett helhetsperspektiv på vårdtagarna är viktiga och måste innehålla både medicinska- och omvårdnadsdata. I föreliggande rapport är det det omvårdnadsperspektivet som står i centrum.

    FAO has considered insects as food since 2003 [1] and is promoting consumption of insects (entomophagy) in the Western world because of the possibilities for sustainable production it offers. Insects as food are considered to leave smaller ecological footprints than conventional livestock (beef, pigs, and poultry) regarding feed, land and water needs, as well as greenhouse gas and ammonia emissions [2-7]. As an example regarding water, taken from a recent TV documentary, if a family of four people got their protein need from insects one day a week instead of from conventional livestock, over a year they would save the planet about a million Liters of water [8]. This is in agreement with figures above.

    Kvinnlig könsstympning är en företeelse som för många är tabubelagt att prata om och väcker starka känslor. Världshälsoorganisationen beräknar att 100-140 miljoner nu levande flickor och kvinnor har genomgått könsstympning. Kvinnor som på olika sätt har erfarenhet av könsstympning kommer i kontakt med svensk hälso- och sjukvård, socialtjänst och skolväsende, antingen som mödravårdspatienter, som mödrar eller som experter. Hur kan vi samla kunskap för att bemötande och kompetens ska bli bättre? Vilka olika förklaringar finns till att traditionen fortsätter? Vad säger lagen?

    Syftet med denna bok är att beskriva och ibland ge svar på en del av frågeställningarna ovan. Boken har framför allt nordiskt fokus utifrån både hälsa, antropologi och juridik samt erfarenheter kring mötet inom hälso- och sjukvården. Boken vänder sig främst till yrkesverksamma och studenter inom bland annat vård- och omsorg, socialtjänst, skola och polismyndighet och är den första i sitt slag i Norden.

    Andelen äldre personer ökar i det svenska samhället. Med stigande ålder ökar risken för olika sjukdomar och besvär. För att kunna tillgodose en trygg och god vård till denna patientkategori krävs samordnade och välplanerade insatser. Distriktsnämnden för Nordöstra Skånes sjukvårdsdistrikt har beslutat att äldrevårdsteam ska tillskapas för att kunna erbjuda hembesök till äldre vid akuta/halvakuta sjukvårdsbehov. Syftet med detta arbete var att beskriva processen hur projektet äldrevårdsteam enligt "Hässleholmsmodellen" initierades, planerades och startades upp samt som en del i utvärderingen av projektet undersöka vad olika berörda intressenter ansåg var nyttan med äldrevårdsteam utifrån sitt perspektiv. Metoden som användes var intervjuer med yrkesverksamma och intervjuerna har analyserats med kvalitativ innehållsanalys. I intervjuerna framkom att äldrevårdsteamet kan vara en resurs och ett stöd vid bedömning samt utgöra en möjlighet för förbättrad samverkan och delaktighet kring patienten. Genomgående bedömde informanterna att det innebär en stor nytta för den äldre personen att kunna få vård i hemmet och slippa åka iväg till sjukhuset eller vårdcentralen. I några intervjuer betonades värdet av att svårt sjuka och döende människor bereds möjlighet att få vara kvar i sitt hem. Det är av största betydelse att utvärdera projekt på olika sätt och en viktig del är att ta reda på vilken nytta äldrevårdsteamet innebär för den enskilde patienten.
