    The enabling role of institutional entrepreneurs in the adoption of IPSAS within atransitional economy: the case of Estonia2017Conference paper (Other academic)
    Abstract [en]

    In the light of neo-institutional theory, and adopting the case-study research methodology, this paper explains the adoption of IPSASs for modernising public sector accounting in Estonia. This study reveals that the smooth – yet gradual – adoption of IPSASs-compliant accounting systems was enabled by the involvement of powerful actors (i.e. institutional entrepreneurs) with foreign and/or business backgrounds. In addition, the change in public sector accounting in Estonia was facilitated by the developments in its international positioning, characterised by the transition from a Soviet-communist to a market economy and subsequent EU membership, and by the absence of past accounting practices which could potentially hinder the change.

    Abstract [en]

    Purpose - The purpose of this paper is to explore the relationship between trust and contract in the context of externalized local public services provision. Design/methodology/approach - A multi-theoretical framework is used to analyse different combinations of control mechanisms (i.e. trust and contract) with reference to three cases of externalized water service provision in Estonia consisting of inter-organizational relationships between local governments and water companies with different ownership structures (public, private and mixed public-private). Findings - The relationship between trust and contract, which can either be substitutes or complements, or eventually erode each other, is contingent upon the capacity of interacting individuals (and related organizations) to keep interests aligned in water services provision. Originality/value - The relationship between trust and contract is analysed by considering the interactions between key actors within the underlying governance setting.

    Background: Breast cancer is themost common cancer among women in Sweden. Thetreatment of breastcancer may lead to women being forcedto surgery remove her breast. Women’s breasthas always been a symbol of the very feminine. Aim: The aim was to highlight theaffected women's experiences after a mastectomy. Method: The study was a literature review based on qualitative, scientificarticles. Results: The results showed thatmany women experiencechanges in their body.Through the loss ofthe breast, they lost femininity, sexuality and identity.They experienced theirbodies’ mutilated, causing grief. Their relationshipfor their partner was affected, positively or negatively depending on if thewomen considered themselves to besexually accepted or not. The scarsymbolizes both the loss, fear of new cancer and victory to having beencured. Conclusion: It is important that thenurse has the healthpromotion forethought when meeting with women who have undergone a mastectomy.To help themovercome their experiencesand encourage themto achieve a highfeeling of life connection.

    Abstract [en]

    Background: Asthma is a chronic inflammatory airway disease which is common in all ages. Over 300 million people worldwide are estimated to have the disease.

    Aim: The aim of this study was to investigate whether the oral health differs between children with and without asthma.

    Method: The method used was a literature review and the literature searches were made in the medical database PubMed. The results were based on a compilation of 13 clinical scientific studies and 11 of these compared children with and without asthma. The remaining articles studied either how the oral health was affected by the combination treatment with specific drugs or compared the oral health of children with different duration and medication for asthma.

    Results: Some of the included studies found that children with asthma had an increased cariesprevalence compared with children without asthma, while other studies showed that there was no difference between the groups. Regarding the plaque, bleeding and caries-related bacteria, the results were also conflicting. Medication with β2-agonist combined with corticosteroids contributed to a reduced salivary flow and an increased level of caries-related bacteria among children with asthma.

    Conclusion: There are conflicting results regarding whether the oral health differs between children with and without asthma. Combination treatment with β2-agonists and corticosteroids contributed to a reduced salivary flow and an increased level of caries-related bacteria.

    Abstract [sv]

    Att studera kvinnliga sexualförbrytare är problematiskt då gruppen är så liten. Studien är en litteraturstudie för att klargöra vad som är sammanställt kring denna lilla grupp. Uppsatsens frågeställningar handlar om huruvida det existerar någon generell bild av vem den sexualbrottsdömda kvinnan är, om det finns olika distinkta typer av sexualbrottsdömda och om svenska förhållanden är möjliga att översätta till internationella. Därför innehåller uppsatsen också en genomgång av de fall som utretts vid kriminalvårdens riksmottagning för långtidsdömda. För att behålla objektiviteten i fråga om internationell typologi presenterar författaren en rutnätsmodell med hjälp av vilken fall av den här typen kan spaltas upp förtydligas och presenteras utan typologi. Mycket av den existerande forskningen har fokuserat på att dela upp populationen i olika typologier. Tyvärr verkar det inte existera någon koncensus kring viken typologi man använder eller för den skull vad man använder typologin för. Således blir tidigare forskning svår att överskåda och använda vid exempelvis utveckling av behandlingsansatser. Då ämnet i sig varit och är färgat av tabun kring kvinnlig kriminalitet och sexualitet tar arbetet upp genusaspekter som en viktig del. Tabun som omgärdar ämnet ökar också sannolikheten att det förekommer stora mörkertal kring hur stor omfattningen av denna kriminalitet egentligen är. Det är uppsatsens slutsats att de undersökta svenska fallen passar in i den internationella bilden. Fortfarande kvarstår mycket forskning för att belysa denna del av kriminologin men uppsatsen kan sägas vara ett litet steg i rätt riktning.

    Bakgrund: Missbruk och beroende av alkohol och narkotikapreparat är ett utbrett problem och en stor riskfaktor för såväl fysisk som psykisk ohälsa. Människor med missbruk eller beroendeproblematik riskerar att bli stigmatiserade då det förekommer fördomar mot denna patientgrupp. Enligt Hälso- och sjukvårdslagen ska vården vara lika för alla samt ges med respekt för allas lika värde. Trots detta förekommer det ojämlikheter i vården för vissa utsatta grupper vilket kan leda till ökat lidande och sämre hälsa och välbefinnande. Således finns det ett behov av att belysa uppfattningarna om patientgruppen för att kunna förbättra behandlingssituationer och på så sätt främja en bättre vård. Syfte: Syftet var att belysa vårdpersonalens uppfattningar av patienter med substansbrukssyndrom som kan påverka mötet i den somatiska vården. Metod: En litteraturöversikt som baseras på elva empiriska studier. Resultat: Det framkom att negativa uppfattningar om patientgruppen är vanligt förekommande, men även att empati visades för patientgruppen då vårdpersonal såg personen bakom sjukdomen. I resultatet tas det också upp vad dessa uppfattningar fick för konsekvenser för patienterna. Slutsats: En ökad medvetenhet kring substansbrukssyndrom och vårdpersonalens uppfattningar kan leda till ett mer förstående förhållningssätt inom sjukvården och således ett ökat välbefinnande hos patienten. För att säkerställa god vård för patientgruppen är det emellertid väsentligt att stödja, handleda och utbilda vårdpersonal. Vidare forskning om huruvida svensk vårdpersonal uppfattar patientgruppen hade varit att föredra då det finns en kunskapslucka inom området.

    Abstract [en]

    This article elucidates Somali women's experiences of cooking and meals after immigration to Sweden. Six Somali women participated in repeated focus group interviews. Content analysis of the interviews resulted in four themes: change in routines and content of the daily meals, changed experiences related to cooking and shopping for groceries, the social dimensions in food-related occupations, and change of identity and roles. According to the women, variety of factors related to their life in Sweden had led to changes in their food occupations and meals: environmental changes, societal factors and the fact that the women secured employment. Although their new focus on employment led to altered responsibility and time for the cooking, food-related occupations remained important for the creation of identity and the maintenance of the family. This study may inform the development of strategies to restrict the negative impacts of immigration on Somali women's health. Future research will increase understandings of the relationships between food-related occupations and women's roles, identity and health.

    Abstract [en]

    Background: Immigrants are considered globally to be a vulnerable subpopulation. Vulnerable population groups have a higher prevalence of obesity than the general population. Despite increased immigration of people from Somalia to Sweden in recent years, little research has been undertaken about obesity and obesity-related health risks among Somali immigrants. The present study aimed to investigate the prevalence of overweight and obesity, as well as possible relationships between weight status and socio-demographic characteristics, dietary habits and physical activity (PA) among Somali immigrants in Sweden.

    Methods:This quantitative cross-sectional study included 114 respondents. Data were collected by means of a questionnaire covering socio-demographic factors, PA and dietary habits. Weight and height were also measured.

    Results:Of the 114 respondents, 50.9% had a body mass index (BMI) of 25 or above.In bi-variate analysis, there were no statistically significant differences between those with a BMI below 25 and those with a BMI of 25 or more regarding PA or dietary habits. Multiple logistic regression analysis showed that female gender and being married were associated with having a BMI of 25 or above.

    Conclusion: Socio-demographic factors may be more strongly associated with high BMI than PA or dietary habits among the targeted group and should be taken into account as an issue affecting Somali immigrants in Sweden that warrants further research.

    Abstract [en]

    Background: In today's society, information flowing from many different directions. Knowledge communicated to patients and families of different professions, without coordination, which can lead to misunderstandings. For patients with diabetes type 2, it is important to get information that is understandable and relevant to their illness. Through patient education, the patient may then engage in self-care and take control of the disease. Aim: The purpose of this study was to see how patients with newly diagnosed type 2 diabetes understand self-care after visiting the nurse. Method: An empirical, qualitative interviews were carried out and contents were analyzed using qualitative content analysis inspired by Granheim and Lundman. Results: It emerged during the interviews that about half of the participants expected to suffer from diabetes, when there was a family history. Diabetes was natural to get at a certain age, with or without self-care. Most patients had made changes in diet and smoking during the first months after being diagnosed and confirmed that self-care was useful. Many felt concerned about not being able to continue with the changes, but was not worried about the future. Others had continued very confident and knew that the outcome depended on themselfs. Conclusion: With or without early self-care the disease would had come anyway. The first time after beeing diagnosed, change and self-care feelt important. When the sense of "feel good" reappeared, the motivation faded. With frequent visits to the nurse, the patients were motivated to continue.

    Abstract [en]

    Since 2007 the Swedish government has allocated 500 millions SEK per annum, for a project named “Idrottslyftet”, the aim is to enhance the child and youth sport within five years. Despite this, it seems that more young girls choose to stop participating in organized sport clubs. The aim with this study was to find out why girls stop participating in organized sport clubs. The method that was used was focus groups, and 13 focus groups interviews (n=118) was performed in northeast Skåne. Qualitative content analysis was used as the analysis method. The result showed that there were several factors that influenced the girls to stop participating in organized sport clubs. The factors were friends and family and sport clubs. The physical education was also pointed out as a negative factor. The discussion was divided into two health perspectives; pathogenesis and salutogenesis. In pathogenesis the focus was to find out why young girls choose to dropout of organized sport clubs, and the interviewees perspective on the physical education curriculum. In salutogenesis, the discussion focus on, how sport clubs and physical education in schools can prevent dropouts and pursue more physical activity by young females in their spare time and in their future. The conclusions were that sport clubs that perform child and youth sports should perform their activities conforming with the Swedish Idrotten vill ideology and club education, but also a touch of competition education. A sport club that can balance these three factors will probably reach success in different ways. Schools should oversee the curriculum in physical education and make it more variable and stimulating for the students. But also the society and the families should cooperate to pursue a better and more positive attitude towards physical activity among young people.

    Abstract [sv]

    Projektet har till syfte att undersöka den revolutionerande tekniken bakom IP-telefoni. Eftersom tekniken är relativt ung så finns det fortfarande mycket att lära om hur det fungerar och hur den används. Projektet behandlar fördelar och nackdelar samt vilka hot som finns och hur man kan känna sig säker när man använder IP-telefoni till vardags. Det primära objektivet med projektet är att få en helhetsbild och förstå hur tekniken verkligen fungerar.

    Vidare behandlas telefonens historia då det är viktigt att se var tekniken har sina rötter och hur den utvecklats sedan dess. Som en röd tråd från telefonens teknologi och det publika telefonnätets rötter görs en jämförelse mellan de två teknologierna för att se skillanderna och förmodade fördelar och nackdelar med de båda systemen. Olika sorters protokoll som har ett nära förhållande till IP-telefoni så som SIP, RTP, UDP och IP behandlas. För att få en sanningsenlig bild över hur vardagliga användare upplever IP-telefoni så har en enkätundersökning angående vanor och möjliga problem gjorts hos de anställda på Region Skåne.

    Abstract [en]

    The coverage areas of cellular networks are logically subdivided into service areas. Each service area has a local anchor node which “hides” the mobility inside the area and the entire network has a global anchor node which “hides” the mobility between areas.

    The concept of unique local anchor nodes per service area was invented to simplify routing but has been found to complicate expansion. The rapidly growing demand for cellular access has therefore prompted for alternative solutions with pools of local anchor nodes per service area. Such pools are now deployed by several operators all over the world.

    Users in pooled service areas are mapped to specific pool members according to a load distribution policy, but the mapping can change as a result of node failures or operator interventions. Such changes take a certain time to implement and cause additional load on the anchor nodes. We study these processes in detail and derive closed form expressions which allow operators to control the trade-off between rapid changes and acceptable loads.

    Finally we show that the key assumptions of our model are in agreement with measured data and demonstrate how the model can be applied to investigate the effects of different network settings (timers) under different user behaviour (traffic and mobility).

    Contrary to current solutions to this problem, which typically are slow and/or inaccurate, our results enable fast and accurate analysis of different scenarios thereby enabling operators to maximise utilisation of the existing investments and at the same time avoid potentially dangerous situations of overload.

    ECG warning system is established for real time monitoring of a patient's electrocardiogram (ECG) and automatic detection of certainement cardiac diseases, namely myocardial infarction, hyperkalemia and atrioventricular block. A distinctive research work related on the assimilation of modern technologies: software, computer and information technologies. ECG early warning system's algorithm is developed in accordance to measuring the average of the ECG signatures and Interprets the data with simulated healthy curve. The prototype system INITIALLY classifies the data and Evaluated it with natural healthy simulated curve. Meanwhile the system discards f healthy curve exists otherwise the system stores the distinguished abnormalities in the curve then transfers Warning to the doctor and patient. Cardiac patients can be assisted by this warning system by detecting abnormalities at the very early stage. Consequently, doctors Provide better tools to identify cardiac diseases resulting things more accurate medical advice

    Abstract [en]

    Previous research shows that many academics experience a high level of workload and time pressure at work, which could lead to a work life conflict. Time pressure and well-being are current topics, which is evidenced in the daily press.  Previous research indicates a knowledge gap regarding research about work life balance, and its effect on the well-being of individuals. Thus, the purpose of this study is to explore how accountants experience time pressure, from a well-being perspective.

    In order to fullfil the purpose of the study, a qualitative method was chosen. Time pressure has been explored from three perspectives: time deadline pressure, time budget pressure, and workload. The term well-being includes life satisfaction, job satisfaction, and work life balance. This study is based on empirical material gathered from eight accountants settled in southern Sweden. To increase reliability of the study, empirical data have been collected through a method triangulation consisting of interviews, observations and documentary research.

    The result of this study shows that occurrence of time pressure in the accounting profession is dependent on documentation requirements, constant interruptions and the expectations of the client. Despite time pressure and workload, the accountants are pleased with their profession. However, situations occur when time pressure is prevailing, which negatively impacts accountants’ well-being. The extent of time pressure and workload that accountants feel comfortable with depends on personal characteristics.

    The findings show that personal characteristics are crucial regarding how the accountant experience time pressure. Depending on personal characteristics, if time pressure is prevailing it instantly leads to an imbalance between work and life, which results in decreased well-being for accountants.

    Bakgrund: Varje år dör ca 95 000 människor i Sverige och 60 % av dem beräknas behöva palliativ vård. De flesta lider av undernäring i form av anorexi och kakexi eller biverkningar av strålning och kemoterapi som är en gemensam orsak till aptitförlust eller aptitlöshet vilket orsakas av olika symtom. Dessa problem kan orsaka ett lidande. Genom personcentrerad vård och kompetenta sjuksköterskor med specialistutbildning kan patientens livskvalitet höjas. Syfte: Studiens syfte var att beskriva hur palliativa patienter upplever sin aptitförlust. Metod: Allmän litteraturstudie baserad på kvalitativa artiklar. Resultat: Studien visade att patienter med aptitförlust har en stor önskan om att bli hörda och att sjuksköterskan ska finnas där för att lyssna på dem. Upplevelser som önskan att överleva, skuldkänslor, förlust av kontroll på olika nivåer och relationer som är konfliktladdade påverkade patienters aptit negativt. Upplevelser och aptit har ett stort inflytande på livskvaliteten. Slutsats: Att arbeta efter personcentrerad vård kopplad till KASAM kan vara en lösning för att kunna öka patientens livskvalitet i samband med ökat matintag. Webbaserad utbildning som DUNÄT kan vara en stor hjälp till att öka förståelse för nutritionens vikt. Kollegial utbildning kan vara till hjälp för att öka medvetenhet om möjliga problem. En noggrann omvårdnadsanamnes gör det möjligt att rätt åtgärd kan inledas. Medvetenheten om att patienten är en unik person med unika upplevelser kan göra stor skillnad för alla inblandade.

    Abstract [en]

    Background: There are about 30 000-40 000 people in Sweden who have schizophrenia. The disease often starts at a young age with symptoms such as delusions, hallucinations, incoherent speech and movements and inertia. The whole family is affected as a child undergoes the major transformations the disease involves. The lack of understanding from the surrounding environment contributes to great disappointment and concern among parents. Aim: The purpose of this study has been to describe the experience of being a parent of a child with schizophrenia. Method: A systematic literature review has been carried out, based on twelve scientific articles. Results: Results state that parents feel guilty about not have given their child the proper attention as they identified the first symptoms of schizophrenia. There is also a major concern due to the changes a child undergoes, which makes the parents feel very uncertain and insecure about the future. The information and help offered as the child was given its diagnosis was by parents considered as inadequate. The sharing of experience and reflections within specific support group, consisting of people in similar situations, gave parents a sense of security. Discussion: Contact to health services should be established as early as possible at disease onset. As a nurse it is important to primarily take care of the individual, but in dealing with diseases like schizophrenia the family is also very important to take in to account. Conclusion: If measures, such as better and more regular flow of information was introduced earlier on in the process the parents everyday life would be greatly improved and thus, lead to a better functioning family situation.

    Abstract [en]

    Mobile wireless networks constitute an indispensable part of the global Internet, and with TCP the dominating transport protocol on the Internet, it is vital that TCP works equally well over these networks as over wired ones. This paper identifies the performance dependencies by analyzing the responsiveness of TCP NewReno and TCP CUBIC when subject to bandwidth variations related to movements in different directions. The presented evaluation complements previous studies on 4G mobile networks in two important ways: It primarily focuses on the behavior of the TCP congestion control in medium- to high-velocity mobility scenarios, and it not only considers the current 4G mobile networks, but also low latency configurations that move towards the overall potential delays in 5G networks. The paper suggests that while both CUBIC and NewReno give similar goodput in scenarios where the radio channel continuously degrades, CUBIC gives a significantly better goodput in scenarios where the radio channel quality continuously increases. This is due to CUBIC probing more aggressively for additional bandwidth. Important for the design of 5G networks, the obtained results also demonstrate that very low latencies are capable of equalizing the goodput performance of different congestion control algorithms. Only in low latency scenarios that combine both large fluctuations of available bandwidths and a mobility pattern in which the radio channel quality continuously increases can some performance differences be noticed.

    Abstract [en]

    Mobile internet has been widely adopted and it is expected to rise to almost 4 billion users by 2020. Despite the research effort dedicated to the enhancement of its performance, there still exists a gap in the understanding of how TCP and its many variants work over LTE. To this end, this paper evaluates the extent to which five common TCP variants, CUBIC, NewReno,Westwood+, Illinois, and CAIA Delay Gradient (CDG), are able to utilise available radio resources under hard conditions, such as during start-up and in mobile scenarios at different speeds. The paper suggests that CUBIC, due to its Hybrid Slow-Start mechanism, enters congestion avoidance prematurely, and thus experiences a prolonged start-up phase. As a result, it is unable to efficiently utilise radio resources during shorter transmissionsessions. Besides, CUBIC, Illinois and NewReno, i.e., the loss-based TCP implementations, offer better throughput, and are able to better utilise available resources during mobility than Westwood+ and CDG - the delay-based variants do.

    Collaboration in public sectors: contradictions, paradoxes and complexities of practice2012Conference paper (Other academic)
    Abstract [en]

    Focus in this paper is collaboration between different parts of a Region in Sweden. The first aim is from reflexive pragmatism perspective study frameworks and processes in a two years project of a collaboration in the Region. The second aim is too understood and explores these processes in terms of theoretical concepts that seriously consider contradictions, paradoxes and complexities of practice. 

    The case in this paper is collaboration in a region where the Regional Council commissioned the Public Health Committee to form sector-integrated health-promoting network together with other units in the region. 

    The method used for get the empirical material is a dialogue between “insider”, in the form of project leader, and “outsider”, in form of researcher. Also interviews with staff and project leaders’ diaries were used as empirical material. So, the focus is on what is said to have happened, not what has actually happened. In order to understand this and try to explain what actually happened, we are addressing a comprehensive theoretical framework. 

    Some theoretical arias take account of collaboration as a contradicting, complex and dynamic practise. The combined characteristic of these theoretical arias need to be that they begin with the complexity of practice, it´s dynamics and constant emergent character. The reflexive pragmatic approach contains a critical perspective and at same time limiting the number of perspectives that will contribute to developing the practice and concepts that managers can use for future effective collaboration. This is in the paper; structure (the bureaucracy form), complexity theories and sensemaking. The concept of boundary objects and trust helped us to complete the picture of collaboration in practice as ambiguous, paradoxical and as mess – the ontological content of collaboration in practice.

    In the political agenda, collaboration is emphasised more than ever due to e.g. the requirement of sustainable societies (economic, ecologically and socially), as expressed in Lisbon Strategy and European Union 2020 strategy which is currently being developed. For this reason, the public sector in Sweden has initiated collaboration between organisations, sectors, departments and various professional roles in order to integrate different factors that are considered of great importance in terms of a sustainable society. A concrete example of the implementation of the Lisbon Strategy on a regional level in Sweden is the development of a regional development programme (RUP), which focuses regionally on local development and how to achieve sustainability in the long term perspective. A project intended to facilitate collaboration in RUP between different sectors on a regional level will take place in Sweden from the year 2008 to 2010. This project is limited to collaboration between sectors that include public health issues and regional

    Abstract [en]

    Why does not the teacher staff act as they are told to according to the school management documents? Why are not the latest curriculum and its contents within public schools carried through? Why do not the rational forms of implementation and management, recommended by management literature, work? (Alvesson & Sveningson 2003); Bartunek 2006; Brunsson 2007; Morrison 2002; Pye 2006; Weick & Sutcliffe 2001; Weisly). Furthermore they are used for managing Swedish public School. Managing Swedish public organizations, as for example public schools, has been discussed and there are controversies as to why actions as management models, laws and other regulations do not give the intended results as expected (Scherp and Scherp 2007). The article, with its empirical material and theoretical issue, is meant to give a comprehension and explanation of why logic and contents of management sometimes fail to correspond to logic and contents in practice. To answer questions about implementation and management this article sets out from what actually happens within organizations, and not what is expected to happen. You cannot explain a rule by another one; a rule needs to be explained by its practice.
