    Abstract [en]

    AIM: This paper is a report of a study that explores and describes the meaning of the phenomenon of health-promoting self-care as experienced by people living with rheumatic diseases.

    BACKGROUND: People with rheumatic diseases estimate health status as low and health belief and health status influence self-care behaviours. Several self-care behaviours are used in the efforts to mitigate the diseases.

    METHOD: The study had a descriptive phenomenological approach based on a reflective life-world perspective. Data were gathered in 2007 by unstructured open-ended interviews with 12 individuals living with rheumatic diseases.

    FINDINGS: The meaning of health-promoting self-care as experienced by people living with rheumatic diseases was that self-care takes place against a background of continual hope and belief to influence health in positive ways. Self-care was a way of life and implied being ready to understand and respond to signals from the body. Three inter-related constituents elucidated their experiences: dialogue, power struggle and choice. Self-care was experienced as dialogues with the body and with the immediate environment. In order to respond to signals from the body, power struggles were required to be entered into when fighting the diseases. Choices were required to be made and things that were beneficial for the body were prioritized.

    CONCLUSION: In this study, the meaning of health-promoting self-care as experienced by people living with rheumatic diseases was that self-care was a way of life. This meant to be ready to understand and respond to signals from the body. Self-care required dialogues, power struggles and choices.

    Every six baby that is born gets colic. It means that the child cries for at least three hours three days a week. For the child’s parents this is one big strain and feelings like despair, helplessness and isolation are common. The nurse’s task is to support the parents through respectful and empathic actions in order to relieve sufferings and increase their feelings of confidence. The aim of this study was to describe how the nurse could give physical and mental support to parents with colic infants. The study is based on seven articles, which have been checked according to different review models depending on whether the article was qualitative or quantitative. The result was presented in two central categories, physical and mental support. In the two central categories subcategories were formed. Physical support could be given by parents being introduced to using a cry diary, structuring their daily life in order to reduce their stress and using baby massage. Mental support could be given by the nurse listening to the parents and showing respect for their situation. To give advice and to teach the parents different treatment strategies could also be of help here. The conclusion is that the general nurse is given different support tactics on how to give support to parents with a colic infant. The nurse decides when, where, what and how the support shall be given to the parents.

    Behandlingsmetoder för dental erosion: En kvantitativ litteraturstudie2018Independent thesis Basic level (degree of Bachelor), 10 credits / 15 HE creditsStudent thesis

    Syftet med litteraturstudien var att kartlägga behandlingsmetoder för dental erosion. Den metod som användes var en litteratursökning vilken gjordes under mars månad 2018, i databasen PuBMed. Litteraturstudiens resultat sammanställdes utifrån 19 artiklar. Ämnen och metoder för behandling av dentala erosioner var Kaseinfosfopeptid - amorft kalciumfosfat (CPP - ACP), fluor, Polyvinylmetyletermalein syra (PVM/MA) syra, Er:YAG och CO2 laser, samt Euclea natalensis växt. Olika variabler såsom emaljförlust, dentinförlust, ythårdhetsförändring, remineraliseringsgrad och storlek på dentinkanaler mättes för att värdera effekten av utläkning vid dental erosion. I fyra av fem studier som testade kaseinfosfopeptid-amorft kalciumfosfat (CPP-ACP) i form av tuggummi eller tandkräm sågs en signifikant effekt på dental erosion. Fluor och fluor i olika kombinationer och formula såsom gel/lack, tandkräm och munskölj hade också en positiv effekt på dental erosion. PVM/MA syran gav en god effekt på dental erosion då alla öppnade dentinkanaler var stängda efter användning. Däremot visade behandling med Er:YAG och CO2 laser samt Euclea natalensis ingen stor effekt. Slutsatsen är att användning av CPP-ACP,  fluor och fluor i kombination med andra  ämnen  samt PVM/MA syra har goda effekter vid behandling av dental erosion

    ECG warning system is established for real time monitoring of a patient's electrocardiogram (ECG) and automatic detection of certainement cardiac diseases, namely myocardial infarction, hyperkalemia and atrioventricular block. A distinctive research work related on the assimilation of modern technologies: software, computer and information technologies. ECG early warning system's algorithm is developed in accordance to measuring the average of the ECG signatures and Interprets the data with simulated healthy curve. The prototype system INITIALLY classifies the data and Evaluated it with natural healthy simulated curve. Meanwhile the system discards f healthy curve exists otherwise the system stores the distinguished abnormalities in the curve then transfers Warning to the doctor and patient. Cardiac patients can be assisted by this warning system by detecting abnormalities at the very early stage. Consequently, doctors Provide better tools to identify cardiac diseases resulting things more accurate medical advice

    Bakgrund: Varje år dör ca 95 000 människor i Sverige och 60 % av dem beräknas behöva palliativ vård. De flesta lider av undernäring i form av anorexi och kakexi eller biverkningar av strålning och kemoterapi som är en gemensam orsak till aptitförlust eller aptitlöshet vilket orsakas av olika symtom. Dessa problem kan orsaka ett lidande. Genom personcentrerad vård och kompetenta sjuksköterskor med specialistutbildning kan patientens livskvalitet höjas. Syfte: Studiens syfte var att beskriva hur palliativa patienter upplever sin aptitförlust. Metod: Allmän litteraturstudie baserad på kvalitativa artiklar. Resultat: Studien visade att patienter med aptitförlust har en stor önskan om att bli hörda och att sjuksköterskan ska finnas där för att lyssna på dem. Upplevelser som önskan att överleva, skuldkänslor, förlust av kontroll på olika nivåer och relationer som är konfliktladdade påverkade patienters aptit negativt. Upplevelser och aptit har ett stort inflytande på livskvaliteten. Slutsats: Att arbeta efter personcentrerad vård kopplad till KASAM kan vara en lösning för att kunna öka patientens livskvalitet i samband med ökat matintag. Webbaserad utbildning som DUNÄT kan vara en stor hjälp till att öka förståelse för nutritionens vikt. Kollegial utbildning kan vara till hjälp för att öka medvetenhet om möjliga problem. En noggrann omvårdnadsanamnes gör det möjligt att rätt åtgärd kan inledas. Medvetenheten om att patienten är en unik person med unika upplevelser kan göra stor skillnad för alla inblandade.

    Bakgrund: Övervikt under uppväxtåren har ökat markant de senaste åren, 20-25% av Sveriges tioåringar är idag överviktiga. För att häva denna utveckling krävs att bland annat sjuksköterskor utövar förebyggande arbete. Syfte: Syftet med litteraturstudien var att belysa hur sjuksköterskor kan förebygga övervikt/fetma hos barn och ungdomar. Metod: Studien är en systematisk literaturstudie och bygger på tio vetenskapliga artiklar samt en avhandling. Dessa har granskats och analyserats. Kategorisering och materialet gjordes med hjälp av Kims domäner med anvisningar för en modell för deduktiv metoanalys.Resultat: Sjuksköterskors förebyggande arbete med barn i tidig ålder krävs för att häva utvecklingen av övervikt/fetma. En modell skulle kunna vara vägledande för sjuksköterskor för att ge information och undervisning samt att motivera och engagera hela familjen. Diskussion: Brist på engagemang och motivation hos barn och föräldrar hämmar preventionsarbetet. Sjuksköterskornas bemötande bör vara individanpassat. En modell som hjälp för sjuksköterskors preventionsarbete har framtagits i studien utifrån Kims domäner.

    Bakrund: Personer som lider av långvarig smärta är återkommande inom hälso- och sjukvården. Långvarig smärta beräknas kosta det svenska samhället cirka 80 miljarder kronor per år. Att behandla långvarig smärta kan vara svårt eftersom smärta ofta innefattar psykologiska faktorer så som känslor, tankar och/eller beteende. Hälso- och sjukvårdspersonalens bemötande är en viktig del i omvårdnaden av patienter med långvarig smärta. Syfte: Syftet var att belysa patienters upplevelser av hälso- och sjukvårdspersonalens bemötande vid långvarig smärta. Metod: En allmän litteraturstudie med elva inkluderade originalartiklar. Datainsamling gjordes i databaserna Cinahl Complete och PubMed. Granskningen utgick från en granskningsmall för kvalitativa och kvantitativa artiklar. Analysen utfördes i fem steg utifrån en modell av Friberg (2012b). Resultat: Patienter med långvarig smärta ansåg att det var viktigt att betraktas som en unika personer med individuella behov. Att bli behandlad med respekt och få sin smärtupplevelse bekräftad var viktigt i bemötandet. När hälso- och sjukvårdspersonalen visade intresse och gav adekvat information ökade patientens delaktighet. I resultatet presenteras negativa men även positiva upplevelser av hälso- och sjukvårdspersonalens bemötande. Diskussion: Studiens kvalitet bedömdes utifrån Shenton (2004) trovärdighetsbegrepp. De tre centrala fynden kommunikation, tilltro och kunskap diskuterades i relation till etik, bemötande samt samhällsperspektiv.

    Abstract [en]

    Background: There are about 30 000-40 000 people in Sweden who have schizophrenia. The disease often starts at a young age with symptoms such as delusions, hallucinations, incoherent speech and movements and inertia. The whole family is affected as a child undergoes the major transformations the disease involves. The lack of understanding from the surrounding environment contributes to great disappointment and concern among parents. Aim: The purpose of this study has been to describe the experience of being a parent of a child with schizophrenia. Method: A systematic literature review has been carried out, based on twelve scientific articles. Results: Results state that parents feel guilty about not have given their child the proper attention as they identified the first symptoms of schizophrenia. There is also a major concern due to the changes a child undergoes, which makes the parents feel very uncertain and insecure about the future. The information and help offered as the child was given its diagnosis was by parents considered as inadequate. The sharing of experience and reflections within specific support group, consisting of people in similar situations, gave parents a sense of security. Discussion: Contact to health services should be established as early as possible at disease onset. As a nurse it is important to primarily take care of the individual, but in dealing with diseases like schizophrenia the family is also very important to take in to account. Conclusion: If measures, such as better and more regular flow of information was introduced earlier on in the process the parents everyday life would be greatly improved and thus, lead to a better functioning family situation.

    Abstract [sv]

    Diabetes typ 2 är en kronisk sjukdom som ständigt ökar. Syftet med denna litteraturstudie var att beskriva patienters upplevelser av att leva med diabetes typ 2. Nio vetenskapliga artiklar analyserades. Analysen resulterade i fem huvudkategorier; upplevelser av att ändra livsstil, coping - ett sätt att hantera sin diabetes, upplevelser av en förändrad självkänsla, upplevelser av vårdpersonalens bemötande och framtidssyn. Resultat som framkom i studien var svårigheter med att ändra livsstil beträffande kostvanor, hantering av medicinering och fysisk aktivitet. Att hantera diabetes med hjälp av coping var vanligt förekommande. Vårdpersonalens bemötande upplevdes positivt när det handlade om kontinuitet och negativt när det handlade om tillrättavisningar och oengagerad vårdpersonal. Genom ökade kunskaper hos vårdpersonal om hur patienter upplever sin diabetes kan också omvårdnaden utvecklas och förbättras för denna patientkategori.

    Diabetes type 2 is a chronic disease that increases. The aim with this literature study was to describe patients' experiences of living with diabetes type 2. Nine scientific articles were analyzed. The analysis resulted in five central categories; experiences of changing lifestyle, coping - a way to handle your diabetes, experiences of a changed self-esteem, experiences of nursing staffs refuting and future sights.

    Results that emerged in the study were difficulties with changing lifestyle concerning eating habits, handling of medicine and physical activity. To handle diabetes with the aid of coping were common occurring. Nursing staff refuting was experienced positively when it was about continuity and negative when it was about admonitions and uncommitted nursing staff.

    Through increased knowledge from nursing staff about how patients experience their diabetes nursing could be developed and be improved for this patient category.

    OBJECTIVE: To investigate long-term sick leave among primary care patients with musculoskeletal disorders and the predictive value of health-status and sense-of-coherence measures. METHODS: Patients aged 17 to 64 years who, during seven weeks, attended one of six primary care centers because of non-traumatic musculoskeletal pain and who completed the SF-36 health questionnaire and the sense of coherence (SOC) scale at baseline and after one year. RESULTS: Of 189 patients, 36 (19%) were sicklisted for at least three months before and/or after their visit; the most common diagnoses were non-specific soft-tissue or multiple joint, low back, and shoulder pain. The long-term sicklisted patients had significantly worse baseline SF-36 and SOC scores than the non-sicklisted patients; moderate improvement in the SF-36 bodily pain but no improvement in the physical functioning scores occurred. The duration of sick leave at baseline and the SF-36 bodily pain score were significant predictors of continuos one-year work disability. CONCLUSION: Long-term sick leave was common among primary care patients with musculoskeletal pain. The physical functioning and return-to-work outcomes after one year were poor. The SF-36 bodily pain scale might be helpful in identifying at risk patients.

    Abstract [en]

    How can discharge planning (DP) for patients who require care in the home following a period in hospital be understood and developed through the lenses of complexity theory? With the help of complexity theory and practice-based narrative research this study discloses the formal routines and complex dynamic practices that are associated with DP. A study of the literature established that there was an almost total absence of complexity-theoretical perspectives on interpreting and developing DP.                 

    The researchers collected narratives about the DP processes using qualitative interviews with the nurses responsible for this in a hospital ward: these were audio-recorded and transcribed verbatim. They also participated in and documented meetings where these nurses, as a group, discussed DP.                 

    The findings show that nurses have to continuously deal with uncertainty, surprises and the unknown. They have to make sense and take charge of dynamic complex events and new knowledge, and manage complex relations and information. The researchers argue that looking upon practice from the lenses of complexity theory, and therefore accepting the complexity of practice, could facilitate the development of nurses' skills in order to guarantee good quality in DP.

    This study examined age differences in phasic heart rate in response to neutral, negative and positive pictures. Heart rate changes and subjective ratings were analyzed in 22 middle-aged (40-55 years) and 30 older (56-78 years) participants. The effects of valence on the HR pattern across time were similar to that obtained by Bradley and co-workers. Conversely to previous studies, we did not report any age-related reduction in cardiac reactivity. Instead, when viewing positive pictures, the triphasic wave form appeared in the group of older adults, but for younger participants, it was replaced by a sustained deceleration. These results were interpreted in the light of the socioemotional selectivity theory.

    Background: Myocardial infarction is the most frequent cause of death in Sweden. Previous research has indicated large gender differences as regards the type of care received by the patient. Aim: The aim of the literature review was to describe women’s experiences after myocardial infarction. Methods: A literature review based on eleven qualitative articles was performed. Findings: The articles were categorized into three main groups; physiological and psychological experiences, experiences of changed relationships, and women’s coping in daily life. Conclusions: Drawing on previous research done in this field, we conclude that it is of crucial importance that all healthcare professionals – irrespective of profession – which encounter female heart attack sufferers, are increasing their knowledge base and their emphatic skills to adequately be care for this patient group. It is especially essential for nurses to be aware of the gender differences and to give the affected woman, and her whole family, necessary and individually tailored, support and rehabilitation.

    Bakgrund: Hjärtinfarkt är den vanligaste dödsorsaken i Sverige. Det är stora skillnader i vården mellan män och kvinnor som drabbats av hjärtinfarkt. Syftet: Litteraturstudiens syfte var att beskriva kvinnors upplevelser efter hjärtinfarkt. Metod: En litteraturstudie genomfördes där endast kvalitativa artiklar granskades. Resultat: Elva artiklar granskades och tre huvudkategorier bildades; fysiska och psykiska upplevelser, upplevelser av förändrade relationer samt kvinnors hantering av sin vardag. Slutsats: Inom vården bör all personal använda ett holistiskt synsätt för att öka kunskapen och förståelsen för kvinnor som drabbats av hjärtinfarkt. Det är viktigt att sjuksköterskorna uppmärksammar de könsskillnader som finns och att både kvinnorna och deras familjer får individuellt anpassat stöd och rehabilitering. Sjuksköterskan bör även vara uppmärksam på vilka kvinnor som är i behov av stöd och en längre kontakt med vården och lägga fokus på dessa kvinnor.

    During the fall of 2005 and the spring of 2006, there was a nosocomial infection outbreak caused by a multiresistant Escherichia coli producing Extended-Spectrum Beta-Lactamase (ESBL) in a unit at the Central Hospital in Kristianstad. The bacteria were believed to be transmitted through patient contact with the hands of staff members. Since five of the cases of ESBL-producing E. coli were found in wounds of patients from the unit, wounds in other units needed to analyzed. In Sweden, patients with wounds are treated in hospitals as well as in municipal health care settings. The aim of this study was to investigate, improve and evaluate hygiene routines among the hospitals’ and municipal health care facilities’ staff members. In addition, a screening for the microbial flora, including multi antibiotic resistance, in patients’ wounds in these facilities was carried out.

    The study used a cross-sectional pre- and postintervention design. The data collection was designed as naturalistic observational study with participant observations and interviews. The study was performed at two central general hospitals, three general hospitals and six counties municipal health care settings in Sweden. The coverage area were 370 000 inhabitants. The data from the investigation in 2006 were analyzed and presented in individually written reports for each unit. The national regulations on basic hygiene were implemented in 2007. A poster was distributed to all participating units and displayed for patients and visitors. The message of the poster stated: “[We respect patient hygiene safety by: wearing short sleeves; long hair wearing up; have short unpainted nails; not wearing jewellery on our hands or arms; disinfect our hands before and after every patient and care procedure. Please remind us if we forget!]“. In 2006, 214 units, 2541 staff members in a patient care situation and 218 staff members were observed when treating wounds. In 2008, 221 units, 2339 staff members in a patient care situation and 142 staff members were observed when treating wounds.

    The working clothes provided by the employer significantly increased for municipal staff members (from 39% to 62%) as did the use of plastic aprons also (from 53% to 75%). Staff members with either short hair or long hair wearing it up was significantly decreased for municipality staff members (from 94% to 88%). The correct absence of jewellery increased significantly (from 53% to 69%), as did the correct absence of nail polish, build up nails, and infected cuticles (from 74% to 77%)). The use of gloves increased significantly for all staff members (from 83% to 89%). The staff members were observed during a wound treatment situation, and the correct absence of jewellery increased for hospital staff members (from 75% to 89%). The use of gloves decreased (from 91% to 80%). The use of plastic apron significantly increased (from 27% to 52%). In the microbiology screening study, a total of 394 patients with 438 wounds were included. The most frequent microbe was Gram-positive cocci, which was found in 458 (60%) of the 760 isolates and in 353 (81%) of the wounds. Two ESBL-producing isolates were found, one Escherichia coli and one Enterobacter aerogenes.

    The findings suggest that the combined interventions and the method used with multidisciplinary teams as well as multiple strategies for interventions might have a positive effect on focusing on hygiene routines. Knowledge of the local bacteria flora and their antibiotic resistance as well as the type of wounds, is of importance when deci ing what antimicrobial treatment to suggest, and the monitoring needs to be continuing.

    Abstract [en]

    Introduction: For fast and effective antibiotic therapy of serious infections like sepsis, it is crucial with rapid information about antibiotic susceptibility, especially in a time when the number of infections caused by multi resistant bacteria has escalated in the world.Methods: Here, we have used a semi-quantitative MALDI-TOF-MS based method for antibiotic resistance detection, MBT-ASTRA™, which is based on the comparison of growth rate of the bacteria cultivated with and without antibiotics. We demonstrate a new protocol where several parameters have been optimized and automated leading to reduced hands-on time and improved capacity to simultaneously analyse multiple clinical samples and antibiotics.Results: Ninety minutes of incubation at 37 °C with agitation was sufficient to differentiate the susceptible and resistant strains of E. coli and K. pneumoniae, for the antibiotics cefotaxime, meropenem and ciprofloxacin. In total, 841 positive blood culture analyses of 14 reference strains were performed. The overall sensitivity was 99%, specificity 99% and the accuracy 97%. The assay gave no errors for cefotaxime (n = 263) or meropenem (n = 289) for sensitive and resistant strains, whilst ciprofloxacin (n = 289) gave six (0.7%) major errors (false resistance) and four (0.5%) very major errors (false susceptibility). The intermediate strains showed a larger variety compared to the E-test MIC values.Conclusions: The hands-on time and the analysis time to detect antibiotic resistance of clinical blood samples can be substantially reduced and the sample capacity can be increased by using automation and this improved protocol.

    Mulitresistent staphylococcus aureus (MRSA) is a bacteria that is resistent against penicillin, this makes it difficulte to treat. MRSA causes woundinfections and other seriouse conditions. If MRSA establishes in an hospital environment it often spreads fast. Common ways of spreading MRSA is through direct or indirect contact. The nurse should work according to guidelines, apply hygienic routines and prevent the spread of infections. Aim: The aim of the literaturestudie was to illustrate factors that affects nurses care of patients carrying MRSA. Method: A literature study was carried out. Results: the study found four categories: knowledge, attitudes, guidelines and risk. They showed that nurses had to little knowledge about MRSA and the preventive routines that ought to be followed. If the nurse had knowledge she also had a positive attitude and was able to give same care to the patient carrying MRSA as to the ones that not carrying it. Conclusion: Knowledge, attitudes and guidelines about MRSA needs to be improved. This gives a better care of MRSA infected patients and decreases the infections risks for both medical personel and patients.

    Bakgrund: Compassion fatigue är ett nytt begrepp och är en respons på stress som kan påverka sjuksköterskors hälsa och skapa svårigheter att ge en god vård.

    Syfte: Syftet var att beskriva sjuksköterskors erfarenheter av compassion fatigue.

    Metod: Denna litteraturöversikt grundades på tio vetenskapliga artiklar med kvalitativ ansats. Artiklarna granskades, analyserades och sammanställdes till ett nytt resultat och en ny helhet.

    Resultat: Resultatet visade att sjuksköterskorna involverade sig för mycket och kände stark ansvarskänsla för patienter. En påfrestande arbetsmiljö i kombination med svårigheter att hålla professionella gränser i patientkontakten, kan vara orsaken till att compassion fatigue uppstår. Compassion fatigue påverkade arbetslivet genom att sjuksköterskorna använde distansering till patienter och anhöriga för att undvika känslomässigt svåra situationer. Sjuksköterskor hade behov av organisatoriska förändringar för att inte drabbas av tillståndet. Compassion fatigue kan förebyggas och motverkas på individnivå genom att ha en hälsosam balans mellan yrkesliv och privatliv. För att minska risken att drabbas av compassion fatigue, är det viktigt med reflektion om sina tankar och känslor för att kunna bearbeta dem.

    Diskussion: Sjuksköterskor som känner stark ansvarskänsla och involverar sig för mycket i patientkontakten har en ökad risk att drabbas av tillståndet. Distansering är en osund strategi som sjuksköterskor använder för att bevara sin hälsa, men som sänker kvaliteten i omvårdnaden. Nyexaminerade sjuksköterskor löper större risk att drabbas av compassion fatigue. Genom att införa information och öka kunskapen om tillståndet redan på utbildningsnivå kan det skapa en medvetenhet och förbereda sjuksköterskestudenter inför det kommande arbetslivet.

    Background: It is well known that unemployment is a great problem both to the exposed individual and to the whole society. Unemployment is reported as more common among young people compared to the general level of unemployment. Inequity in health status and lifesatisfaction is related to unemployment. The purpose of this population-based study was to describe QOL among unemployed young people compared to those who are not unemployed, and to analyse variables related to QOL for the respective groups.

    Methods: The sample consisted of 264 young unemployed individuals and 528 working or studying individuals as a reference group. They all received a questionnaire about civil status, educational level, immigration, employment status, self-reported health, self-esteem, social support, social network, spare time, dwelling, economy and personal characteristics. The response rate was 72%. The significance of differences between proportions was tested by Fisher's exact test or by χ2 test. Multivariate analysis was carried out by means of a logistic regression model.

    Results: Our results balance the predominant picture of youth unemployment as a principally negative experience. Although the unemployed reported lower levels of QOL than the reference group, a majority of unemployed young adults reported good QOL, and 24% even experienced higher QOL after being unemployed. Positive QOL related not only to good health, but also to high self-esteem, satisfaction with spare time and broad latitude for decision-making.

    Conclusion: Even if QOL is good among a majority of unemployed young adults, inequalities in QOL were demonstrated. To create more equity in health, individuals who report reduced subjective health, especially anxiety need extra attention and support. Efforts should aim at empowering unemployed young adults by identifying their concerns and resources, and by creating individual programmes in relation not only to education and work, but also to personal development.

    BACKGROUND: Working life is an important arena in most people's lives, and the working line concept is important for the development of welfare in a society. For young people, the period before permanent establishment in working life has become longer during the last two decades. Knowledge about attitudes towards work can help us to understand young people's transition to the labour market. Adolescents are the future workforce, so it seems especially important to notice their attitudes towards work, including attitudes towards the welfare system. The aim of this study was to describe and analyse upper secondary school students' work attitudes, and to explore factors related to these attitudes. METHODS: The sample consisted of 606 upper secondary school students. They all received a questionnaire including questions about quality of life (QOL), sense of coherence (SOC), subjective health and attitudes towards work. The response rate was 91%. A factor analysis established two dimensions of work attitudes. Multivariate analyses were carried out by means of logistic regression models. RESULTS: Work ethics (WE) and general work attitudes (GWA) were found to be two separate dimensions of attitudes towards work. Concerning WE the picture was similar regardless of gender or study programme. Males in theoretical programmes appeared to have more unfavourable GWA than others. Multivariate analyses revealed that good QOL, high SOC and good health were significantly related to positive WE, and high SOC was positively related to GWA. Being female was positively connected to WE and GWA, while studying on a practical programme was positively related to GWA only. Among those who received good parental support, GWA seemed more favourable. CONCLUSION: Assuming that attitudes towards work are important to the working line concept, this study points out positive factors of importance for the future welfare of the society. Individual factors such as female gender, good QOL, high SOC and good health as well as support from both parents, positive experience of school and work contacts related positively to attitudes towards work. Further planning and supportive work have to take these factors into account.

    Chronic obstructive pulmonary disease (COPD) is a chronic lung disease. The disease is the fourth leading cause of death and more than 5 percent of the adult population worldwide is affected.

    The aim of this study was to investigate the association between periodontitis and COPD.

    Search for material for the literature review was done in the medical database PubMed where the keywords were: "Chronic obstructive pulmonary disease" and "Periodontal health or periodontal disease". The result is based on twelve studies that were extracted and analyzed. The studies have investigated pocket depth, attachment loss, plaque index, bleeding on probing and bone loss. Ten studies compared a COPD group with a healthy control group, one study compared two COPD groups and one study examined a COPD group and a group with other lung diseases.

    Most studies found that people with COPD had a higher incidence of pocket depth, attachment loss, plaque index, bleeding on probing and bone loss. The conclusion of the literature review is that there is an association between periodontitis and COPD and that smoking is a link between the two diseases.

    Background: Chronic Obstructive Pulmonary Disease (COPD) is the fourth leading cause of death in the world. COPD is a progressive disease that could lead to chronic hypoxemia, which requires treatment as domiciliary Long-Term Oxygen Therapy (LTOT). There is a need for increased knowledge about self-care strategies used by individuals living with COPD and LTOT. Objective: The aim was to explore experiences and self-care strategies in patients living with both COPD and LTOT. Sample: The sample consisted of five men and five women diagnosed with COPD being prescribed LTOT for more than one year. Method: Ten interviews were undertaken and analyzed for both manifest and latent content. Results: Living with COPD and LTOT was associated with experiences of guilt although there were doubts about what had caused the lung disease. Both the lung disease and the oxygen therapy had a negative impact on their self-image. Anxiety was expressed when thoughts about the remaining time occurred. There was a constant balance between diminishing abilities and increasing restrictions related to the lung disease and the therapy. In order to compensate for arising imbalance, self-care strategies had been initiated aimed at preserving the present state of health, enabling and facilitating physical activity and promoting a positive attitude. Conclusion: The current study suggests that individuals living with COPD and LTOT are encouraged to adopt self-care strategies directed towards maintaining stability with regard to the lung disease, the oxygen therapy, physical capability and emotional reactions.

    Bakgrund: Inom vården är möten med vuxna patienter som känner obehag inför blodprovstagning eller rädsla vid situationer där stick är inblandat vanligt förekommande. Studier visar att det rör sig om cirka tio procent av den vuxna befolkningen som känner stickrädsla. Sjuksköterskan har därvid ett stort ansvar att hjälpa patienterna genom de ibland påfrestande situationer som kan uppstå på grund av stickrädslan. Syfte: Syftet var att beskriva vad sjuksköterskan kan göra för att underlätta för den vuxna patienten med stickrädsla vid sticksessioner. Metod: Studien är en litteraturstudie där resultatet baserats på kvantitativa och kvalitativa originalartiklar. Artiklarna har granskats, analyserats och sammanställts. Resultat: Resultatet visar att det finns olika sätt att underlätta för den stickrädda vuxna patienten. Det är möjligt genom att smärtlindra patienten, genom att miljön och den medicinska utrustningen är sådan att den kan minska stressen för den stickrädda vuxna patienten, samt genom kommunikation, där lyhördhet och respekt ingår, med patienten. Slutsats: Vårdpersonal som möter vuxna stickrädda patienter i samband med sticksessioner behöver ha kunskap om stickrädsla hos vuxna och redskap för att underlätta för dem och ge dem ett gott bemötande.

    Syftet med studien var att kartlägga kostvanor och munhygienvanor samt uppfattning av den egna munhälsan hos skiftarbetare. Studien har utförts som en kvantitativ tvärsnittsstudie med användning av enkät. Undersökningsgruppen bestod av sammanlagt 100 skiftarbetare från industri- och vård- och omsorgssektorn i Skåne. Det insamlade materialet analyserades i statistikprogrammet Statistical Package for the Social Sciences (SPSS) version 23.0. Resultatet av studien visade att skiftarbetarnas kostvanor påverkades negativt av skiftarbete genom ökat småätande, intag av sötad dryck och oregelbundna måltider på grund av varierande arbetstider.  När det gäller munhygienvanor påverkades inte tandborstningen på samma sätt, däremot rengöringen mellan tänderna utförs inte regelmässigt. Studiens slutsats visade att skiftarbetarna hade goda munhygienvanor när det gäller tandborstning och fluoranvändning dock med mindre regelbunden rengöring mellan tänderna. Kostvanorna var bristfälliga med ökat småätande och sötade drycker mellan måltiderna.

    Chronic rhinosinusitis is one of the most common health care challenges, with significant direct medical costs and severe impact on lower airway disease and general health outcomes. The diagnosis of chronic rhinosinusitis (CRS) currently is based on clinical signs, nasal endoscopy and CT scanning, and therapeutic recommendations are focussing on 2 classes of drugs, corticosteroids and antibiotics. A better understanding of the pathogenesis and the factors amplifying mucosal inflammation therefore seems to be crucial for the development of new diagnostic and therapeutic tools. In an effort to extend knowledge in this area, the WP 2.7.2 of the GA(2)LEN network of excellence currently collects data and samples of 1000 CRS patients and 250 control subjects. The main objective of this project is to characterize patients with upper airway disease on the basis of clinical parameters, infectious agents, inflammatory mechanisms and remodeling processes. This collaborative research will result in better knowledge on patient phenotypes, pathomechanisms, and subtypes in chronic rhinosinusitis. This review summarizes the state of the art on chronic rhinosinusitis and nasal polyposis in different aspects of the disease. It defines potential gaps in the current research, and points to future research perspectives and targets.

    The purpose was to investigate the uptake and release of fluoride (F) from F-impregnated chewing sticks (miswaks). In the first series, 3-cm-long pieces were impregnated in 1, 2, 3 and 4% NaF solutions for 3 h, 1 day and 3 days (10 pieces/test). There was a dose-response effect with respect to both impregnation time and the concentration of the F solution. In the second and third series, totally 40 miswak pieces were impregnated in 3% NaF for 1 day and 3 days; the outer layer (bark) was separated from the inner spongy part (pulp) and analyzed separately. F was released from both parts, but somewhat more was released from the bark than from the pulp; a plateau was reached at around 30 min. In vivo, 9 healthy subjects used three products for 2 min in a crossover design: (1) a miswak impregnated in 3% NaF for 1 day, (2) a miswak impregnated in 3% NaF for 3 days, and (3) 1 g of F toothpaste (containing 1,450 ppm F as NaF) on a toothbrush. The highest F concentration at the approximal area was obtained after using the miswak impregnated in 3% NaF for 3 days compared with the other products (p < 0.05 or p < 0.01). To conclude, NaF-impregnated miswaks produced a rapid release of F in vitro as well as in vivo and may be an interesting vehicle for home care use for caries prevention in countries where they are used regularly.

    Abstract [en]

    A multi-residue pressurised liquid extraction (PLE) methodology has been established for the determination of the four ionophores: lasalocid, monensin, salinomycin and narasin in solid environmental matrices. The PLE methodology is combined with solid phase extraction as clean-up using liquid chromatography coupled to tandem mass spectrometry applying electrospray ionisation for detection. The samples were freeze-dried prior to extraction. The absolute recoveries for soil and sediment ranged from 71 to 123% (relative standard deviation (RSDs) below 16%) and in the range 94–133% (RSDs 9–35%) for poultry manure. The final method allowed for the detection of four ionophores down to a few hundred ng kg−1 in natural solid matrices with limit of quantifications (LOQs) being 0.96, 0.87, 0.98, and 0.64 μg kg−1 in soil for lasalocid, monensin, salinomycin, and narasin, respectively. Corresponding LOQs in sediment were 1.28, 1.34, 1.39, and 0.78 μg kg−1 for the respective ionophores, while in manure the LOQs were 0.98, 1.01, 1.45, and 1.01 μg kg−1.

    Abstract [en]

    BACKGROUND: Measurement of person-centred care (PCC) outcomes is underdeveloped owing to the complexity of the concept and lack of conceptual clarity. A framework conceptualizing outpatient PCC in rheumatology nurse-led clinics has therefore been suggested and operationalized into the PCC instrument for outpatient care in rheumatology (PCCoc/rheum).

    OBJECTIVE: The aim of the present study was to test the extent to which the PCCoc/rheum represents the underpinning conceptual outpatient PCC framework, and to assess its measurement properties as applied in nurse-led outpatient rheumatology clinics.

    METHODS: The 24-item PCCoc/rheum was administered to 343 persons with rheumatoid arthritis from six nurse-led outpatient rheumatology clinics. Its measurement properties were tested by Rasch measurement theory.

    RESULTS: Ninety-two per cent of individuals (n = 316) answered the PCCoc/rheum. Items successfully operationalized a quantitative continuum from lower to higher degrees of perceived PCC. Model fit was generally good, including lack of differential item functioning (DIF), and the PCCoc/rheum was able to separate individuals with a reliability of 0.88. The four response categories worked as intended, with the exception of one item. Item ordering provided general empirical support of a priori expectations, with the exception of three items that were omitted owing to multidimensionality, dysfunctional response categories and unexpected ordering. The 21-item PCCoc/rheum showed good accordance with the conceptual framework, improved fit, functioning response categories and no DIF, and its reliability was 0.86.

    CONCLUSION: We found general support for the appropriateness of the PCCoc/rheum as an outcome measure of patient-perceived PCC in nurse-led outpatient rheumatology clinics. While in need of further testing, the 21-item PCCoc/rheum has the potential to evaluate outpatient PCC from a patient perspective.

    BACKGROUND: Person-centred care (PCC) is considered a key component of effective illness management and high-quality care. However, the PCC concept is underdeveloped in outpatient care. In rheumatology, PCC is considered an unmet need and its further development and evaluation is of high priority. The aim of the present study was to conceptualize and operationalize PCC, in order to develop an instrument for measuring patient-perceived PCC in nurse-led outpatient rheumatology clinics.

    METHODS: A conceptual outpatient PCC framework was developed, based on the experiences of people with rheumatoid arthritis (RA), person-centredness principles and existing PCC frameworks. The resulting framework was operationalized into the PCC instrument for outpatient care in rheumatology (PCCoc/rheum), which was tested for acceptability and content validity among 50 individuals with RA attending a nurse-led outpatient clinic.

    RESULTS: The conceptual framework focuses on the meeting between the person with RA and the nurse, and comprises five interrelated domains: social environment, personalization, shared decision-making, empowerment and communication. Operationalization of the domains into a pool of items generated a preliminary PCCoc/rheum version, which was completed in a mean (standard deviation) of 5.3 (2.5) min. Respondents found items easy to understand (77%) and relevant (93%). The Content Validity Index of the PCCoc/rheum was 0.94 (item level range, 0.87-1.0). About 80% of respondents considered some items redundant. Based on these results, the PCCoc/rheum was revised into a 24-item questionnaire.

    CONCLUSIONS: A conceptual outpatient PCC framework and a 24-item questionnaire intended to measure PCC in nurse-led outpatient rheumatology clinics were developed. The extent to which the questionnaire represents a measurement instrument remains to be tested.

    BACKGROUND: Person-centered care (PCC) is a key component to effective illness management and high-quality care but conceptually underdeveloped in outpatient care settings for long-term disorders such as rheumatoid arthritis (RA), and there is a lack of instruments measuring patient perceived PCC.

    AIM: To conceptualize and operationalize outpatient PCC as a basis for measuring patient-perceived PCC in nurse-led outpatient rheumatology clinics.

    DESIGN: A conceptual outpatient PCC framework was developed based on theory and patient experiences.The resulting framework was operationalizedinto items that were tested regarding their accordancewith the theoretical framework and their measurement properties according to Rasch measurement theory (RMT) using the RUMM2030 software. 

    RESULTS: The conceptual framework comprised five intermingled domains (personalization, shared decision-making, empowerment, communication, and social environment) that were operationalized into a 24-item PCC instrument for outpatient care in rheumatology (PCCoc/rheum). RMT analyses found items to successfully map out a quantitative continuum from lower to higher PCC levels, with generally acceptable model fit and a reliability of 0.88. Three items exhibiting multidimensionality, dysfunctional response categories, and/or unexpected hierarchical ordering were omitted. The revised PCCoc/rheum showed good accordance with the conceptual framework, improved fit, functioning response categories, and no DIF; reliability was 0.86.

    SUMMARY AND IMPLICATIONS: We found support for the PCCoc/rheum as a conceptually based outcome measure of patient perceived PCC. Results implied minor theoretical reconsiderations, and related scale revisions improved theoretical and clinical clarity. While in need of further testing, the PCCoc/rheum has great potential as a measure of outpatient PCC from a patient perspective.

    AIM AND OBJECTIVE: To describe and understand the meaning of living with persistent rheumatoid arthritis.

    BACKGROUND: A considerable number of patients with rheumatoid arthritis live with an ongoing active and symptomatic illness despite access to potent antirheumatic treatment. There is, however, a lack of knowledge about the meaning of living with this severe long-term illness, defined as persistent rheumatoid arthritis.

    DESIGN: A descriptive design based on a hermeneutic phenomenological method was used.

    METHODS: Ten adults with persistent rheumatoid arthritis and at least five years disease duration were interviewed. The interviews were analysed according to van Manen's method.

    RESULTS: Living with persistent rheumatoid arthritis revealed four overall themes: an existence dominated by painful symptoms and treatment, radical changes and limitations in one's life, a continual struggle to cope with one's life and to master the illness, and a dependency on those who are close by and the world around. The lifeworld was affected to a varying extent and in various ways by the illness but also by the dependence on its treatment and care that was not experienced as sufficiently meeting needs in terms of security, access to and coordination of care as well as team and rehabilitation services.

    CONCLUSIONS: Persistent rheumatoid arthritis and its treatment entail a radical effect on the person's life and quality of life. Current ordinary rheumatology care does not seem to meet the individual needs of the person with persistent rheumatoid arthritis in an optimal way.

    RELEVANCE TO CLINICAL PRACTICE: A greater knowledge about and understanding of the person who lives with persistent rheumatoid arthritis is important for facilitating the development of care and the relief of suffering. A holistic alternative to conventional clinical practice, such as person-centred care, could be tested as an innovative model of care. Our findings might serve as material for educational and counselling purposes for healthcare professionals.

    Objective To describe how people with rheumatoid arthritis (RA) experience the care provided by Swedish nurse-led rheumatology outpatient clinics. Methods Eighteen adult people with a diagnosis of RA who had had at least three documented contact sessions with a nurse-led clinic were interviewed. The interviews were analysed with qualitative content analysis. Results Care was expressed in three categories: social environment, professional approach and value-adding measures. A social environment including a warm encounter, a familial atmosphere and pleasant premises was desired and contributed to a positive experience of care. The nurses' professional approach was experienced as empathy, knowledge and skill, as well as support. The care was described as person centred and competent, as it was based on the individual's unique experience of his/her disease and needs. The nurses' specialist knowledge of rheumatology and rheumatology care was highly valued. The offered care represented added value for the participants, instilling security, trust, hope and confidence. It was perceived as facilitating daily life and creating positive emotions. The nurse-led clinics were reported to be easily accessible and provided continuity of the care. These features were presented as fundamental guarantees for health care safety. Conclusion The experiences emphasized the need for a holistic approach to care. In this process, the organization of care and the role and skills of the nurse should be focused on the individual's needs and perspectives. The social environment, professional approach and value-adding measures are particularly relevant for optimal care at nurse-led rheumatology outpatient clinics.

    Background Person-centered care (PCC) is increasingly emphasized as a key component of effective illness management and of developing high quality of care. Despite considerable progress of PCC in many areas of care there is currently a gap and a need for means to assess PCC practice in outpatient care. In rheumatology, PCC is considered an unmet need and further development and evaluation of this approach to care is thus of high priority.

    Objectives To develop an instrument for measuring person-centered care from the perspective of the person with rheumatoid arthritis (RA) in nurse-led outpatient clinics.

    Methods A conceptual framework of PCC in the outpatient context and focusing on the meeting between the person with RA and the nurse and on the patient as an active care partner was undertaken. Based on this framework, qualitative interviews (1,2) and a literature review, a 35-item questionnaire was proposed and qualitatively tested regarding acceptability and content validity among 50 persons with RA attending a nurse-led outpatient clinic. Two versions of the questionnaire were tested: one using four response categories (0 = Totally disagree; 3 = Completely agree), and one using two response categories (0 = Disagree; 1 = Agree). Content validity was estimated by calculating Content Validity Index of the individual items (I-CVI) and of the overall instrument (S-CVI).

    Results Respondents found the items easy to understand (77%) and relevant (93%). Seventy-three percent of the respondents preferred the questionnaire version with four response categories. This version took a mean (SD) of 5.3 (2.5) minutes to complete. I-CVI values ranged from 0.87 to 1.00 and S-CVI was 0.94. About 80% of the respondents considered some items to be redundant. This resulted in a reduced 24-item draft questionnaire that yield a total score between 0–72.

    Conclusions A preliminary 24-item patient-reported PCC questionnaire was developed. Psychometric testing is needed for validation of this tool before implementation.

    The study's introduction is given as an overall picture of the fact that Swedish youth generally estimate their health as high but the psychosocial health is estimated to be lower, which includes being satisfied with themselves. Being content with yourself and your body is covered by the body image perception. Social media takes more time and it's used to a greater extent as a communication medium. Living habits, the environment and the conditions of early life have a bearing on the individual's health.The aim of this study was to investigate the body image of young people and the use of social media and to investigate the relationship between them. Method: A quantitative cross-sectional study was performed. The survey was used as a data collection method among Scanian students in grades 1-3 in high school, where 249 students answered questionnaires. The result showed that 33 percent of girls had a higher body image than boys (14 percent) and that 38 percent of boys had a lower body image than girls (18 percent) (p = 0.001). The way girls and boys used social media differs. Girls use social media to a greater extent than boys. The way girls and boys used social media differed. Girls had more different roles on social media than boys, and boys played more online games than girls. Conclusion: Since the boys in this study had lower body image perception and played more online games than girls, it is appropriate from a gender and gender perspective to study boys' body image perception in relation to online gaming. Therefore, further research with a focus on boys' body image should be conducted on the basis of both a qualitative and quantitative method aspect, with the aim of trying to understand which factors underlie their body image perception.

    Hypertension related to lifestyle factors is an increasing problem in Sweden and according to current guidelines should changes in lifestyle associated with these factors be highly prioritized in the treatment of hypertension. District nurse's work with patients with the diagnosis of hypertension includes checking blood pressure and giving advice in lifestyle changes that can reduce the blood pressure. The purpose of this study was to describe the conversation with the patient with newly discovered essential hypertension as it was experienced by the district nurse. The study has a qualitative approach and was conducted with qualitative, semi-structured interviews with seven district nurses. The analysis of the collected data material was done with a content analysis including both the manifest and latent content and resulted in three main categories; alternate between different functions, create confidence in the relationship with the patient and adapt the conversation to the patients needs. The results showed that the district nurses in the conversation with the patient with hypertension were striving to create a trusting relationship and in this relationship provide lifestyle-related healthcare to the patient. In the conversation the district nurse alternated between various functions such as acting as a catalyst and express messages. She tried to ensure the patient's needs. Sometimes dilemmas arose when the district nurse had to prioritise between topics which were important for lifestyle changes, but could be considered as offensive to the patient. Thereby she prevented negative effects that could impact on the relationship. The conclusion of the result was that the conversation and thus the care of patients with hypertension differed depending on which district nurse the patient came to.

    Syftet med studien var att belysa undersköterskors uppfattning om oral hälsa ochmunvård som utförs på inneliggande cancerpatienter.En kvalitativ studie har tillämpats med en intervjuguide som underlag. Åtta informanterintervjuades i anslutning till deras arbetsplats och intervjuerna varade 10-20 minuter.För att analysera intervjumaterialet användes en kvalitativ innehållsanalys.Resultatet redovisas utifrån temat ”oral hälsa hos cancerpatienter viktigt men lågtprioriterat”. Att hjälpa patienterna med den dagliga munhygienen, upplevde inteinformanterna som något obehagligt utan såg det som en självklar del i sin yrkesroll.Informanterna upplevde även att den orala hälsan inte prioriterade och att det var svårtatt utföra munvård när patienterna nekade hjälp.Studiens slutsats visar att informanterna uppfattar den orala hälsan som en viktig del förpatientens välbefinnande men att munvård inte är det första som prioriteras hoscancerpatienter.

    The aim of this study was to determine whether the initial mechanical efficiency (ratio of work output to initial metabolic cost) of isolated cardiac muscle is over 60%, as has been reported previously, or whether it is approximately 30%, as suggested by an estimate based on the well-established net mechanical efficiency (ratio of work output to total, suprabasal energy cost) of 15%. Determination of initial efficiency required separation of the enthalpy output (i.e. heat + work) into initial and recovery components. The former corresponds to energy produced by reactions that use high-energy phosphates and the latter to energy produced in the regeneration of high-energy phosphates. The two components were separated mathematically. Experiments were performed in vitro (30 degrees C) using preparations dissected from rat left ventricular papillary muscles (N=13). Muscle work output and heat production were measured during a series of 40 contractions using a contraction protocol designed to mimic in vivo papillary muscle activity. Net mechanical efficiency was 13.3+/-0.7%. The total enthalpy output was 2.16 times greater than the initial enthalpy output, so that initial mechanical efficiency was 28.1+/-1.2%.

    Background: An increasing number of severe ill patients receive care at home. Area manager district nurse in municipal home care alone is responsible for both nursing and medical interventions regarding the patient. A high workload increases the risk of making mistakes when district nurse have the responsibility for many patients. Objective: The aim of this study was to describe district nurses' experiences of working in municipal home care with nursing responsibilities for patients in regular housing. Methods: Qualitative methods with inductive approach used in the study. Through the strategic selection 14 district nurses from four municipalities had been interviewed. Results: The results revealed three categories: "Job satisfaction", "Supporting Resources" and” Experiences of difficult situations". District nurses experience of job satisfaction could be related to patient contact, use of their skills, public trust, variation of work, being at work and cooperation with others in care. District nurses skills, relationship with the patient and family, team work and group work, collaboration with other health care units, management support, work environment and facilities and administrative work is supportive resource in the district nurse work. District nurses in the study had individually different workloads and prerequisites for supporting resource in their work because they experienced different amounts of job satisfaction and perceptions of difficult situations such as ethical dilemmas, unpredictability, inadequacy, frustration and irritation. Conclusion: Functional supportive resource in the district nurse work can increase district nurses experience of job satisfaction. Along with resources and continuous professional development are prerequisites for experiencing job satisfaction, which is needed to fulfill the district nurses obligation to provide good care to patients. Further research is needed in the area since the municipal home care is in constant development in all communities and it is unclear how best to pursue it.

    Objectives: The aim of the present study was to evaluate the effects of a novel bone substitute system (Natix®), consisting of porous titanium granules (PTG) and a bovine-derived xenograft (Bio-Oss®), on hard tissue remodelling following their placement into fresh extraction sockets in dogs. Material and methods: Six modalities were tested; Natix® granules with and without a covering double-layered Bio Gide® membrane; Bio-Oss® with and without a covering double-layered Bio Gide® membrane; and a socket left empty with and without a covering double-layered Bio Gide® membrane. Linear measurements, indicative of buccal bone height loss, and an area measurement indicative of buccal bulk bone loss were made. The statistical analysis was based on the Latin Square design with two blocking factors (dog and site). Tukey's post hoc test was used to adjust for multiple comparisons. Results: Histological observation revealed that while bone formed around both the xenograft and the titanium particles, bone was also noted within titanium granules. Of the five modalities of ridge preservation techniques used in this study, no one technique proved to be superior. Conclusion: The titanium granules were observed to have promising osseoconductive properties.

    To receive a diagnosis of pancreatic cancer can lead to a tremendous change in a person’s life. Thoughts regarding death may cause a personal crisis which can have negative influences on the patient’s social, mental and spiritual state. Most people with pancreatic cancer are diagnosed in a late stage of the disease. Therefore, palliative care is often one of the options that are offered to patients. Moreover, to be able to provide patients with nursing care and tend to their needs, it is very important to see them as unique individuals. The purpose with the literature review was to illustrate the experiences of palliative nursing care of patients with pancreatic cancer. A systematic literature review of scientific articles was performed. The study showed that patients’ experiences of need for information, patients’ need for time, support and personal relations, patients’ experiences of hope as well as patients’ need for symptom control were of importance in palliative nursing care.

    Bakgrund: Människor besöker akutmottagningen vid försämrat hälsotillstånd. Patienterna omhändertas genom triagering beroende på patienten tillstånd. På en akutmottagning arbetar olika vårdprofessioner med varandra i team. Ett möte utspelar sig då patienten kommer i kontakt med vårdpersonalen. Patienten kan känna sig i en utsatt position då kontakt etableras med sjukvården. Patienterna uttrycker sina upplevelser i mötet och beskriver sin vistelse relaterat till bemötandet. Syfte: Syftet var att belysa personers upplevelser av att vara patient på akutmottagningen inom den somatiska vården. Metod: Studien är en kvalitativ innehållsanalys bestående av 10 vetenskapliga artiklar. Vi använde oss utav Fribergs (2012) analysmetod. Resultat: Kommunikationens betydelse, väntetiden, att bli sedd, förtroendet för vårdpersonal och patientens delaktighet var faktorer som påverkade patientens upplevelser. Dessa kunde ge positiva eller negativa upplevelser för patienten. Kategorierna är oberoende av varandra och kan vara tillfredsställande även om någon av kategorierna brister. Slutsats: Studier visar att relevanta arbetsmodeller kan implementeras för att påverka patienternas återkommande besök genom förbättrat bemötande och kommunikation.

    Background: When health care staff interact with immigrant patients who have a different native tongue language skills are sometimes not sufficient. Aim: The aim is to describe the employees’ experiences when communicating with patients with a foreign native tongue. Method: A literature review was carried out based on ten scientific articles. These articles were reviewed with a review model used by the Red Cross Collage. Result: Based on the given context the results were compiled into three categories: Language barrier as an obstacle for health care, here attention is drawn to the fact that a ”language barrier”, which occurs between employees and immigrant patients, is an obstacle in health care. Language barrier as a stress factor, emphasizes that the language barrier can create stress for the staff. Language barrier which requires temporary interpreters: a need and an obstacle, temporary interpreters are presented here, namely bilingual employees or patients’ relatives. This is perceived as a necessity and an obstacle when communicating. Conclusion: The Swedish National Board of Health and Welfare emphasizes that the healthcare staff shall give all patients good health and social care no matter the native language. The strategies to combat language barriers should be specific for different professionals. The need to use trained interpreters, in order to assure a satisfactory communication between personnel and patients, is important.  To fight the language barriers actions from researchers, decision-makers and healthcare staff are required.

    Abstract [en]

    Apicidin is a potent histone deacetylase inhibitor (HDACI) that selectively binds to histone deacetylases (HDACs) class I and interferes with the deacetylation process, which results in modification of acetylation level of cellular proteins. The aim of the study was to investigate the potential time and dose dependent cytotoxicity of the test compound, Apicidin, in pancreatic cancer cells Capan-1 and Panc-1 as well as estimate maximal tolerable dose (MTD) of the test agent and determine EC50 using four complementary colorimetric cytotoxicity or viability assays. The cells were treated with increasing concentrations of Apicidin (0-5000nM) for 2, 4 and 6h (short term exposure) or 24, 48 and 72h (long term exposure) before conducting cytotoxic analyses with lactate dehydrogenase assay or viability analyses with sulforhodamine B (SRB), methyl tetrazolium (MTT) and crystal violet (CV) assays. In order to investigate whether Apicidin irreversibly affects the cells already during the short term exposure, the medium containing Apicidin was removed and replaced with fresh culturing medium after 6h of treatment. The cells were then incubated for additional 24, 48 or 72h before carrying out the analysis. The results obtained from cytotoxicity and viability assays indicated, that Apicidin was well tolerated by both cell lines at concentrations below 100nM at any given time point and at all applied concentrations during the short term (6h or less) treatment. Continuous prolonged term exposures (48h or greater) of the cells to Apicidin with concentration exceeding 100nM resulted in significantly increasing cytotoxicity and sustained significant loss of cell viability. Moreover, long term exposure of pancreatic cancer cells Capan-1 and Panc-1 to Apicidin concentrations exceeding 100nM showed an initial anti-proliferative effect before cytotoxicity onset. In summary, MTD was exposure time dependent and estimated to 100nM for long term treatment and to at least 5000nM for treatment not greater than 6h. EC50 concentration of Apicidin was established after long term treatment, however with some variation when comparing the different assays and cell lines. Results from this study may encourage reinvestigating the capacity of potent HDACI Apicidin as an attractive agent for interfering with the deacetylation process catalyzed by HDACs for potential pancreatic cancer intervention.

    The aim of our study was to examine the oral health status using an oral assessment guide, ROAG, among elderly people in a block of service. Another aim was to evaluate the elderlys perception of there own oral health using a questionnaire. The study were undertaken at a ward in Kristianstad, with 39 caretakers. 21 caretakers participated in the study. The assessment of the oral health status was done on the same individuals (n=21) who answered the questions in the questionnaire. The questions were read by the authors. The caretakers answered the questions, and the answers were noted by the authors. Problems with teeth and saliva were most common. Using the assessment guide showed that three caretakers had no problem with their oral health status, while ten had four or more problems. The majority experienced a few problems with their oral health. Mouth dryness were the most common experienced problem.
