The break remains: elderly people's experiences of a hip fracture 1 year after discharge2010In: Disability and Rehabilitation, ISSN 0963-8288, E-ISSN 1464-5165, Vol. 32, no 2, p. 103-113Article in journal (Refereed)
    Abstract [en]

Purpose. To explore experienced long-term consequences of a hip fracture and conceptions of what influences hip fracture recovery among community-living elderly people 1 year after discharge. Method. Fifteen subjects (13 females and 2 males), aged 66-94, were interviewed. The phenomenographic method was used for analysis. Results. Experiences of insecurity and restricted life dominated the interviews. The descriptive categories within experienced consequences of a hip fracture were: (1) isolated life with more restricted activity and fewer social contacts, with the two sub-categories (a) more insecure and afraid and (b) more limited ability to move, (2) disappointed and sad that identity and life have changed and (3) satisfied with the situation or feeling even better than before the fracture. The categories within conceptions of what influences hip fracture recovery were: (4) own mind and actions influence recovery, (5) treatment and actions from others influence recovery and (6) you cannot influence recovery. Conclusion. The findings accentuate that the negative consequences of a hip fracture are substantial and long-lasting. As it strikes mostly elderly people, who may have experienced earlier losses and growing disabilities, a hip fracture could add to the risk of losing important life values. Furthermore, the findings indicate that all health care professionals who meet the patients need to consider the patients' own experiences and possible fear and not merely focus on the physical injury and disabilities.

A life-breaking event: early experiences of the consequences of a hip fracture for elderly people2008In: Clinical Rehabilitation, ISSN 0269-2155, E-ISSN 1477-0873, Vol. 22, no 9, p. 801-811Article in journal (Refereed)
    Abstract [en]

Objective: To explore and describe the consequences of an acute hip fracture as experienced by home-dwelling elderly people shortly after discharge from hospital.Design: Semi-structured interviews using the phenomenographic method.Subjects and setting: Eighteen subjects were interviewed in their own homes one month after discharge. Results: The interviewees described experiences of changes in their relation to the body, themselves, to others and to their whole life situation. These experiences were described as being limited in movement, having lost confidence in the body, becoming humble and grateful, respecting oneself and one's own needs, becoming more dependent on others, gaining more human contact and being treated in a friendly way by others, being secluded and trapped at home, feeling old, closer to death and having lost their zest for life, and taking one day at a time and being uncertain about the future. Conclusion: Experienced consequences of a hip fracture were multidimensional and involved dramatic changes in the interviewees' life situation, including existential thoughts and reappraisal of the years of life that remained. The results indicate that the fracture seemed not only to break the bone but also to cause social and existential cracks, as experienced in the early phase after the injury.

Validering av turbidimetrisk metod för koncentrationsbestämning av albumin i cerebrospinalvätska2018Independent thesis Basic level (degree of Bachelor), 10 credits / 15 HE creditsStudent thesis
    Abstract [sv]

    Abstract [sv]

Sömnvanor bland patienter på sjukhus2013Independent thesis Basic level (degree of Bachelor), 10 credits / 15 HE creditsStudent thesis
    Abstract [en]

Background: Sleep is the most important human recovery process and research supports assumptions that sleep is an important component in the healing process. Lack of sleep can be the onset of certain diseases such as diabetes, hypertension and cardiovascular disease. Sleep is an integral part of basic nursing. It is important for nurses in hospitals to form a correct understanding of the patients’ sleep since they among other things are responsible for administering ad lib hypnotic medication. Aim: To examine the sleep habits of inpatients in two hospitals in southern Sweden, and to compare these with the patients’ sleeping habits in the home a month before hospitalization. Method: Statistical analysis of a part of a Point Prevalence Study among inpatients in southern Sweden. Results: Patients in hospitals reported that they had slept poorly and had lower sleep quality than at home. Forty percent experienced sleep during hospitalization as poor or very poor. The use of hypnotics was significantly higher in-hospital than at home, and the increase was gender related. Conclusion: The study supports previous findings with respect to sleep disturbances, but also with respect to how nurses relate to sleep and sleep disturbances in patients. A thorough self-reflection is stipulated as a way to pass the symbolic barrier to the implementation of existing knowledge.

Nervledningsstudie av suralisnerven och dess anatomiska variation2015Independent thesis Basic level (university diploma), 10 credits / 15 HE creditsStudent thesis
    Abstract [sv]

Understanding communicative intentions and semiotic vehicles by children and chimpanzees2013In: Cognitive development, ISSN 0885-2014, E-ISSN 1879-226X, Vol. 28, no 3, p. 312-329Article in journal (Refereed)
    Abstract [en]

Developmental and comparative studies of the ability to understand communicative intentions using object-choice tasks raise questions concerning the semiotic properties of the communicative signals, and the roles of rearing histories, language and familiarity. We adapted a study by Tomasello, Call, and Gluckman (1997), in which a “helper” indicated the location of a hidden reward to children of three ages (18, 24, and 30 months) and to four chimpanzees, by means of one of four cues: Pointing, Marker, Picture and Replica. For the chimpanzees, we controlled for familiarity by using two helpers, one unfamiliar and one highly familiar. Even 18-months performed well on Pointing and Marker, while only the oldest group clearly succeeded with Picture and Replica. Performance did not correlate with scores for the Swedish Early Communicative Development Inventory (SECDI). While there were no positive results for the chimpanzees on the group level, and no effect of familiarity, two chimpanzees succeeded on Pointing and Marker. Results support proposals of a species difference in understanding communicative intentions, but also highlight the need to distinguish these from the complexity of semiotic vehicles and to consider both factors.

Steroid hormone determination in water using an environmentally friendly membrane based extraction technique2008In: Journal of Chromatography A, ISSN 0021-9673, E-ISSN 1873-3778, Vol. 1192, no 1, p. 1-8Article in journal (Refereed)
    Abstract [en]

In this study, a method was developed for determination of steroid hormones (17beta-estradiol, estrone, 17alpha-ethynylestradiol) in tap and sewage water samples from Sweden. Sample preparation and analysis were performed by a hollow-fibre microporous membrane liquid-liquid extraction (HF-MMLLE) set-up combined with gas chromatography-mass spectrometry (GC-MS). In this approach, only the organic liquid in the lumen (10microL) of the hollow-fibre membrane was utilised for depleting extraction. Several parameters were studied, including: type of organic solvent, sample pH, salt and humic acid content. The optimised method allowed the determination of the analyte at the low ngL(-1) level in tap and sewage water. A linear plot gave correlation coefficients better than 0.995 and resulted in a method limit of detection of 1.6, 3 and 10ngL(-1) for 17beta-estradiol, estrone, and 17alpha-ethynylestradiol, respectively, in sewage water. Enrichment factors were over 1400 after derivatisation. The repeatabilities at 50 and 600ngL(-1) were better than 10% and 6%, respectively.

Occurrence and removal of pharmaceuticals in a municipal sewage treatment system in the south of Sweden2009In: Science of the Total Environment, ISSN 0048-9697, E-ISSN 1879-1026, Vol. 407, no 8, p. 2760-2770Article in journal (Refereed)
    Abstract [en]

The occurrence and removal rate of seven pharmaceuticals (ibuprofen, naproxen, diclofenac, fluoxetine, ofloxacin, norfloxacin, ciprofloxacin), two metabolites (norfluoxetine, clofibric acid), one degradation product (4-isobutylacetophenone) and 3 estrogens (17alpha-ethinylestradiol, 17beta-estradiol, estrone) were studied in the inlet and outlet of a tertiary sewage treatment plant (STP) in Sweden as well as between different treatment steps in the STP which includes a conventional activated sludge step. Pharmaceuticals in raw household and raw hospital sewage streams leading to the STP were as well investigated. Hydraulic retention times (HRT) of each treatment step was considered for sampling and for the calculation of the removal rates. These rates were above 90%, except for diclofenac, clofibric acid, estrone and ofloxacin. However, only diclofenac and naproxen showed significant effluent loads (>145 mg/d/1000 inh). Diclofenac was not eliminated during the treatment and in fact even higher concentrations were found at the effluent than in the inlet of the STP. 17alpha-Ethinylestradiol was not detected in any of the samples. Results indicate that a STP such as the one in Kristianstad, Sweden, with a tertiary treatment is sufficient to remove significantly most of the investigated pharmaceuticals. The chemical treatment improved the removal of several pharmaceuticals especially the antibiotics, which showed step removal rates between 55 and 70%. The expected concentration levels of the pharmaceuticals in the surface water (dilution 1 to 10) close to the outlet of the STP are below the no-observed effect-concentration (NOEC). However, despite that this would imply no important effects in the aquatic environment one cannot rule out negative consequences nearby the STP because most of the NOEC values are derived from acute toxicity data. This may underestimate the real impact of pharmaceuticals in the aquatic ecosystem.

Hollow-fibre supported liquid membrane extraction for determination of fluoxetine and norfluoxetine concentration at ultra trace level in sewage samples2007In: Journal of Separation Science, ISSN 1615-9306, E-ISSN 1615-9314, Vol. 30, no 15, p. 2513-2521Article in journal (Refereed)
    Abstract [en]

In this study, a method was developed for determining the concentration of the pharmaceutical fluoxetine and its metabolite, norfluoxetine, in sewage water samples. Sample preparation was performed by hollow-fibre supported liquid membrane (HF-SLM) extraction with final analysis using liquid chromatography with UV detection. Several parameters were studied including type of organic solvent, sample and acceptor pH, and salt and humic acid content. The optimised method allowed determination of the analyte at the ng/L level in sewage water. A linear plot gave a correlation coefficient better than 0.991 for both analytes and resulted in limits of detection in sewage water of 11 and 12 ng/L, for fluoxetine and norfluoxetine, respectively. The enrichment factor was over 1700 for both analytes in sewage water. The repeatability and reproducibility were better than 8% and 17%, respectively. The developed methodology was used to study daily variations of fluoxetine and norfluoxetine in municipal sewage streams. Norfluoxetine has been detected for the first time in sewage water and a preliminary analysis gave average concentrations of 150 and 225 ng/L for norfluoxetine and fluoxetine, respectively.

Effects of Plantago major L. leaf extracts on oral epithelial cells in a scratch assay2012In: Journal of Ethnopharmacology, ISSN 0378-8741, E-ISSN 1872-7573, Vol. 141, no 3, p. 825-830Article in journal (Refereed)
    Abstract [en]

Aim of study The present study was undertaken to evaluate the effects from different leaf extracts of the traditional medicinal herb Plantago major L. (plantain) on cell proliferation and migration in vitro, as a test for potential wound healing properties. Materials and methods Water and ethanol-based extracts were prepared from Plantago major fresh and dried leaves, and tested in vitro in a scratch assay with oral epithelial cells. Results The scratch assay produced reliable results after 18 h. Most of the tested extracts increased the proliferation/migration of the oral epithelial cells compared to the negative control. A concentration of 1.0 mg/mL (on dry weight basis) appears to be optimal regardless of type of extract, and among the alternatives, 0.1 mg/mL was always better than 10 mg/mL. Ethanol-based extracts with a concentration of 10 mg/mL had very detrimental effects on cell proliferation/migration. At the other two concentrations, ethanol-based extracts had the most beneficial effect, followed by water extracts of fresh leaves, ethanol plus water extracts of dried leaves and, finally, water extracts of dried leaves. This study suggests that both the water extracts and the more polyphenol-rich ethanol-based extracts of Plantago major leaves have medicinal properties. Further research is, however, needed to determine what compounds are responsible for the wound healing effects.

    Abstract [sv]

    Abstract [sv]

    Abstract [sv]

Barn och stress: förskollärares tankar kring stress hos barn i förskolan2014Independent thesis Basic level (university diploma), 10 credits / 15 HE creditsStudent thesis
    Abstract [sv]

    Abstract [en]

During the last decades the consciousness regarding sustainable development has grown. As a part of this growing consciousness some companies choose to implement sustainability work into their corporations. The companies that choose to do this can also report their sustainability work in a Sustainability report. But as well as with financial information there is an uncertainty regarding the reliability in the information, which could be fixed if the companies would let the sustainability report be assured by an independent third party. The assurance of sustainability reports are not as regulated as for the financial information, this is shown in that the assurance is voluntary and that there are different types of assurance providers.

The purpose of this study was to see if certain factors influence companies concerning assurance of sustainability reports and concerning the type of assurance provider chosen. Do to the purpose of the study a quantative method was chosen, in which the data was collected through documents such as sustainability reports and annual reports from 2013.

The data that was collected was then analyzed and tested in relations to the formed hypothesis regarding the corporate factors; profitability, size, use of GRI and industry. Corporate factors in companies that had assured their sustainability reports were; low profitability, a high number of employees, high number in sales and high number in total assets. The same factors were found in the companies that had chosen an auditor as assurance provider. Regarding the industries, it was not companies in social-and environmental industries that assured their sustainability reports but those industries did choose to hire an auditor as the assuranceprovider.

Since similar studies regarding companies on Stockholmsbörsen are scarce, this study may contribute to further knowledge concerning assurance of sustainability reports for these companies.

    Abstract [en]

Explanatory pictures and models are frequently used in teaching and learning situations. However, it seems to be simply assumed that they are always beneficial. In this article results from an investigation with 16 Swedish pupils aged 7–9 year are presented based on an analysis that has examined how well this assumption holds up. Concepts from multi-modal theory have been used to investigate how young learners deal with illustrations and text from an early reader booklet about composting domestic refuse. The analysis suggests that expectations that illustrations facilitate the meaning-making of young pupils may be exaggerated. Although the booklet claimed to provide interactive support between image and text most of the examples show pupils ignoring pictures or misinterpreting vital information about composting in both the verbal and non-verbal material. The illustrations did not compensate for the most crucial deficiencies in the written text.

    Abstract [en]

Teaching and learning situations nowadays typically build on richly illustrated material or multimodal presentations. Under these circumstances, the transparency of images and models used for explaining various phenomena becomes central. The present study deals with 20 Swedish children, 9–12 years old, discussing an illustration meant to show the cause of the different appearances of the Moon in the sky. The students’ task was to place eight numbered moon phases in the lunar orbit in the image. The illustration in question was chosen (a) because it was of a kind frequently used to explain the lunar phases and (b) because the phenomenon is known to be difficult to understand for students of all ages. The analysis leans on historical and sociocultural approaches as well as on multimodal semiotics. The results show that a majority of students were able to make sense of the most central features of the illustration but that very few spontaneously reasoned in a way that could be interpreted as the intended meaning-making of the cause of the lunar phases. The results also indicate that the simultaneous adoption of two perspectives necessary for understanding the phenomenon was a stumbling block for most of the students.

What lies behind graphicacy?: relating students' results on a test of graphically represented quantitative information to formal academic achievement2006In: Journal of Research in Science Teaching, ISSN 0022-4308, E-ISSN 1098-2736, Vol. 43, no 1, p. 43-62Article in journal (Refereed)
    Abstract [en]

Based on studies carried out on qualitative data an instrument was constructed for investigating how larger numbers of students handle graphics. This test, consisting of 18 pages, each with its own graphic display(s) and a set of tasks, was distributed to 363 students, 15–16 years of age, from five different schools. The format of the questions varied, as did the format of the graphics. As students' performance was expected to be multidimensional, confirmatory factor analysis was carried out with a structural equation modeling technique. In addition to the identification of a general graphicacy-test factor (Gen) and an end-of-test effect (End′), a narrative dimension (Narr′) was vaguely indicated. This model was then related to a six-factor model of students' formal academic achievement measured by their leaving certificates from compulsory education. The strongest correlation obtained was between the general graphicacy-test dimension (Gen) and a mathematic/science factor (MathSc′) in the grades model. In addition, substantial relationships were detected between the Gen factor and both an overall school achievement factor (SchAch) and a language factor (Lang′) in the grades model.

    Abstract [en]

The veterans in Sweden is a group that is steadily growing, one of the reasons is the new employment agreement that requires the soldiers and officers to commit to Foreign Service. The Swedish Armed Forces have received critique from the government in their investigations concerning the Swedish veteran politics. Since then the Swedish government has introduced a new law that gives the Swedish Armed Forces an utmost responsibility concerning the veteran’s physical and mental health. The purpose of this study is to qualitatively investigate “The Peace berets”, a non-government organization which have had, and is still having a cooperation with the Swedish Armed Forces as a result of the investigation conducted by the Swedish government. We investigate their social work with the veterans and their perspective on the cooperation between them and the public sector.

The study is based on qualitative interviews with four voluntary workers in the organization. In our analysis we have applied a theoretical perspective from the new institutional theory and the collaboration theory, that enlighten our data collected from the interviews. The result shows, as mentioned above, that the organization has a strong collaboration with the Swedish Armed forces, but not with the social workers which could depend on several facts. One of the reasons could be a result of the reigning confusion concerning the Law concerning the Swedish Armed Forces staff at international military interventions (SFS 2010:449).  As a result of the confusion it is uncertain if the Swedish Armed Forces have an utmost responsibility concerning the veterans, which is confirmed by the interviewed persons. As represented in the result, they share different perspectives on whether the utmost responsibility belongs to the Swedish Armed Forces or the commune/ county council where the veteran is domiciled.

Everyday ethical problems in dementia care: a teleological model.2006In: Nursing Ethics, ISSN 0969-7330, E-ISSN 1477-0989, Vol. 13, no 4, p. 340-359Article in journal (Refereed)
    Abstract [en]

In this article, a teleological model for analysis of everyday ethical situations in dementia care is used to analyse and clarify perennial ethical problems in nursing home care for persons with dementia. This is done with the aim of describing how such a model could be useful in a concrete care context. The model was developed by Sandman and is based on four aspects: the goal; ethical side-constraints to what can be done to realize such a goal; structural constraints; and nurses' ethical competency. The model contains the following main steps: identifying and describing the normative situation; identifying and describing the different possible alternatives; assessing and evaluating the different alternatives; and deciding on, implementing and evaluating the chosen alternative. Three ethically difficult situations from dementia care were used for the application of the model. The model proved useful for the analysis of nurses' everyday ethical dilemmas and will be further explored to evaluate how well it can serve as a tool to identify and handle problems that arise in nursing care.

    Abstract [en]

The usage of computers and Information and Communication technologies (ICT) has changed the way people communicate and share information in many areas, and education is the one of the sectors where ICT plays a major role.

Over the past few years educational sector introduced many new technologies to support teaching and learning methods. To maintain competitiveness at international level, the investment in ICT in schools has grown constantly.

Due to the growing influence of ICT on educational sector the view of people has changed drastically on teaching and learning. Previous research points out that ICT develop students’ different abilities, such as increased learning, abilities to process and handle huge amount of data, as well as intellectual growth. It is also helps students to achieve better academic results by performing better in some of the academic courses. ICT also erases previous limits and allows doing things that were not possible to do before. Some of the examples are flexibility in work and studies, distance learning. According to my study many teachers and students, believe that the investment in ICT is very important because it helps to increase the number of IT- skilled citizens as well as makes school more flexible thus, improves the quality of learning. This work investigates also the possibilities that can be explored with ICT in education, such as importance of investment in ICT in schools, importance of ICT in education and learning in general. My study has shown that, both teachers and students sometimes will be involved in their work and studies even at leisure time. My study has found that ICT improves and facilitates the ways of teaching and learning, as well as learning improves. ICT with these portable technologies make students life more flexible. My interviews indicate that flexibility is good, though there are some risks or disadvantages with ICT in education, including increased plagiarism, as well as superficial learning may be increased. Information search with help of ICT has become easier, according to earlier research, though less than half of all respondents in my study doubt this. The study demonstrates that it is important to invest in education and guidance of information search. Wastage of time is also common problem in school and sometimes this happens because students are not able to use the technology efficiently.

Upplevda psykosociala arbetsförhållanden bland uthyrd personal2006Independent thesis Basic level (degree of Bachelor)Student thesis
    Abstract [sv]

    Abstract [sv]

    Abstract [sv]

    Abstract [en]

Due to criticism towards Disney concerning reproducing gender in a stereotypical way in their animated movies, we decided to investigate this since Disney has affected both our own generation and many to come. The investigation focused on gender, particularly on the depiction of women. It also investigated gender roles from a perspective of time by choosing 14 movies; at least one from each decade, starting from the first movie, Snow white and the seven dwarfs from 1937, to the last one, Tangled from 2010. The investigation discovered that women were underrepresented and often depicted in a stereotypical way, meaning that they were always beautiful and almost always rescued by brave princes. Female power was always depicted as negative. However, changes in how the movies depict gender over time have been found, for example, women have become more active and have started to participate in fight- and career related issues, even rescuing the men at times. When it comes to the princes, they turned out have more personality in the later movies and they did not always have to be handsome to gain true love. However, these changes are mostly beneficial for men since the women still have to fulfill the ideal beauty and can still not fulfill their dreams on their own.

Chain of care development in Sweden: results of a national study2003In: International journal of integrated care, ISSN 1568-4156, Vol. 3, p. e01-Article in journal (Refereed)
    Abstract [en]

Chains of Care are today an important counterbalance to the ever-increasing fragmentation of Swedish health care, and the ongoing development work has high priority. Improved quality of care is the most important reason for developing Chains of Care. Despite support in the form of goals and activity plans, seven out of ten county councils are uncertain whether they have been quite successful in the development work. Strong departmentalisation of responsibilities between different medical professions and departments, types of responsibilities and power still remaining in the vertical organisation structure, together with limited participation from the local authorities, are some of the most commonly mentioned reasons for the lack of success. Even though there is hesitation regarding the development work up to today, all county councils will continue developing Chains of Care. The main reason is, as was the case with Chain of Care development up to today, to improve quality of care. Although one of the main purposes is to make health care more patient-focused, patients in general seem to have limited impact on the development work. Therefore, the challenge is to design Chains of Care, which regards patients as partners instead of objects.

Competition and integration in Swedish health care2010In: Health Policy, ISSN 0168-8510, E-ISSN 1872-6054, Vol. 96, no 2, p. 91-97Article in journal (Refereed)
    Abstract [en]

Despite of an insignificant track record of quasi-market models in Sweden, new models of this kind have recently been introduced in health care; commonly referred to as "choice of care". This time citizens act as purchasers; choosing the primary care centre or family physician they want to be treated by, which, in turn, generates a capitation payment to the chosen unit. Policy makers believe that such systems will be self-remedial, that is, as a result of competition the strong providers survive while unprofitable ones will be eliminated. Because of negative consequences of the fragmented health care delivery, policy makers at the same time also promote different forms of integrated health care arrangements. One example is "local health care", which could be described as an upgraded community-oriented primary care, supported by adaptable hospital services, fitting the needs of a local population. This article reviews if it is possible to combine this kind of integrated care system with a competition driven model of governance, or if they are incompatible. The findings indicate that some choice of care schemes could hamper the development of integration in local health care. However, geographical monopolies like local health care, enclosed in a noncompetitive context, lack the stimulus of competition that possibly improves performance. Thus, it could be argued that if choice of care and local health care should be combined, patients ought to choose between integrated health care arrangements and not among individual health professionals. Competition-exposed integration: an impossible composition?2013In: What healthcare can we afford?: Better, quicker, lower cost health services, 2013, p. 106-Conference paper (Other academic)
    Abstract [en]

    Context

Swedish health care, like many other health care systems, is in a constant development mode to meet never-ending demands for improved efficiency and quality. Competitive and integrative policies are for example concurrently introduced in Swedish primary care; citizens‘ choice of primary care is launched while primary care is expected to integrate its activities with other providers for the creation of =local health care‘. Competition has though a tendency fragment the provision of services. The aim of this study is therefore to explore whether or not these two strategies are compatible in practice.

    Methods

Group interviews were conducted at four locations in Sweden. The groups included persons aged between 20 and 45 years, 46 and 64 years and 65 years or over. The interviewees were living either in a big town or in a small community. Altogether, 21 randomly selected individuals participated in the group interviews. A deductive approach was chosen: six question topics were formulated with guidance from a theoretical framework about choice of care. The group interviews were thus semistructured without any predetermined codes. Each group interview took between 1 and 1.5 h to complete. Moreover, the conversations were recorded and transcribed as verbatim reports. As a consequence of the deductive approach, directed content analysis was chosen for the analysis of the group conversations.

    Results

Choice of care is executed from the perspectives of being a prospective or current patient, which, in practice, imply choices are performed passive and active respectively. If the later group perceive interpersonal continuity, accessibility and demeanour of health professionals as favourable, they remain faithful to their actively chosen provider. The only condition that seems to trigger this group of patients to reconsider their choices is if they been the subject of bad manners. Those executing passive choices are less faithful to their original choice. When these former prospective patients, often younger persons, are in need of primary care they often disregard their choice if waiting times are shorter at other providers. This group generally prefer accessible service and seldom consider where it is provided. The group of passive choices also include citizens accepting suggestions presented by the authorities, founded on the conviction that ―they know what is best for me.

    Discussion

Many patients that have made active choices are thus faithful to their choices. This is rare in a consumer-market, which is characterized by high degree of exchangeability of providers; a condition which by and large corresponds with the attitude of those making passive choices. Nevertheless, a majority of patients stay with their choice of provider, often selected among a limited number of options. Moreover, health care providers and patients have long-term relationships, which is typical of a producer-market. In other words, if politicians strive for a competition-exposed primary care, the competition concept ought not to be founded on the theories of a consumer-market. The principles of a producer-market seem instead to be more applicable, which imply that providers will be competitive if they are able to build stable relations with their patients, which, in turn, facilitate for integrative arrangements among health care providers.

    Abstract [en]

It seems impossible to create a comprehensive evaluation model which fully takes into account the multi-dimensional context of integrated health and social care. Clinical integration, as a prerequisite for efficient outcomes of integration, must nonetheless get special attention. For more extensive evaluations, a quality chain matrix, including co-operating acts by different providers, has proven to be useful. Examples of evaluated services in Sweden are given, and the management benefits of the use of evaluation data are highlighted.

Dissolving the Patient Bermuda Triangle2010In: International Journal of Care Coordination, ISSN 2053-4354, Vol. 14, no 4, p. 137-141Article in journal (Refereed)
    Abstract [en]

The differentiation of roles, tasks and responsibilities in health care has gradually increased because of efforts to decentralize, specialize and professionalize our health-care systems. These development approaches can on the one hand be regarded as successful, although there is also a negative flipside. Increased differentiation has concurrently fragmented the delivery of health care, which, in turn, can be divided into structural, clinical and cultural fragmentation. Patients are lost as a result of these conditions of fragmentation. This phenomenon can metaphorically be described as a ‘Patient Bermuda Triangle’. Actions to dissolve the Patient Bermuda Triangles are commonly termed ‘Integrated health care’, a global buzzword that includes integrated care pathway as well as other integrated health-care strategies. Moreover, integrated care is a means to an end: improved patient outcome. To achieve this, it is crucial to have necessary prerequisites in place: both functional and interactional conditions. This procedure seems to be an organic process where the stakeholders go through gradual changes until the optimum level of integration, as well as mutualistic interactions, is established. If these conditions are concealed or impossible to achieve, developmental work should be ended to avoid the evolvement of antagonistic relations between the stakeholders concerned. This state will likely establish a Patient Bermuda Triangle or reinforce an existing one.

    Abstract [en]

In Sweden it is a public sector duty to finance and facilitate the provision of health care. It is thus a “Beveridge” health care system. All residents have accordingly the right to obtain the publicly financed health care. The system is decentralized and includes 21 county councils and 290 municipalities. Furthermore, it rests on a democratic platform: each of these authorities is governed by a parliament, with its representatives elected for a four-year period at every general election.

Is it better to be big? The reconfiguration of 21st century hospitals: Responses to a hospital merger in Sweden2008In: Health Policy, ISSN 0168-8510, E-ISSN 1872-6054, Vol. 87, no 1, p. 92-99Article in journal (Refereed)
    Abstract [en]

Objectives: Swedish hospital mergers seem to stem from a conviction among policy makers that bigger hospitals lead to lower average costs and improved clinical outcomes. The effects of mergers in the form of multisited hospitals have not been systematically evaluated. The purpose of this article is to contribute to this area of knowledge by exploring responses to the merger of Blekinge Hospital.

Methods: The evaluation was guided by the philosophy of triangulation. A questionnaire was sent to 597 randomly selected employees, that is 24% of the health care staff. Four hundred ninety-eight employees answered the questionnaire, giving a response rate of 83%. Furthermore, interviews of different groups of stakeholders were conducted.

Results: A moderate increase of quality was assessed, which, a low proportion of the employees perceived had decisively or largely to do with the merger. The majority perceives economical incentives as the drivers of change, but, at the same time, only 10% of this group believes this target was reached completely or to a large extent.

Conclusions: The employees believe the merger has neither generated economy of scale advantages nor substantial quality improvement. Instead, it seems more rewarding to promote cross-functional collaboration together with clinical specialisation. Needs for both integration and differentiation could thereby be fulfilled.
