    Abstract [en]

    Self-management programs require a range of indicators to evaluate their outcomes. The Health Education Impact Questionnaire (heiQ) was developed to meet this need. The heiQ contains 40 items with 4 response categories, representing eight scales. We developed a Swedish version of the heiQ that was tested by cognitive interviews (n=15) and psychometrically (n=177) using classical test theory (CTT) and Rasch measurement theory (RMT). The Swedish heiQ was easily understood by interviewees and met CTT criteria, with supported scaling assumptions (corrected item-total correlations, ≥0.37) and reliability (ordinal alpha ≥0.78). General support was demonstrated for the measurement properties of the eight heiQ scales by acceptable RMT fit. However, there were signs of malfunctioning response categories for four items in two scales, and of suboptimal item coverage of the measurement continua. The Swedish heiQ appears comparable to other available language versions. Further efforts may be needed to optimize response categories and measurement precision.

    Abstract [en]

    BACKGROUND: As frail older people might have difficulties in expressing themselves, their needs are often interpreted by others, for example, by significant others, whose information health care staff often have to rely on. This, in turn, can put health care staff in ethically difficult situations, where they have to choose between alternative courses of action. One aspect that might be especially difficult to express is that of existential loneliness. We have only sparse knowledge about whether, and in what way, the views of frail older persons and their significant others concerning existential loneliness are in concordance.

    OBJECTIVE: To contrast frail older (>75) persons' experiences with their significant others' perceptions of existential loneliness.

    METHODS: A case study design was chosen for this study. Individual interviews with frail older persons (n = 15) and interviews with their significant others (n = 19), as well as field notes, served as a basis for the study. A thematic analysis was used to interpret data. Ethical considerations: This study was conducted in accordance with the principles of research ethics.

    FINDINGS: The findings showed three themes: (1) Meaningless waiting in contrast to lack of activities, (2) Longing for a deeper connectedness in contrast to not participating in a social environment and (3) Restricted freedom in contrast to given up on life.

    DISCUSSION: Knowledge about the tensions between older persons' and their significant others' views of existential loneliness could be of use as a basis for ethical reflections on the care of older people and in the encounter with their significant others.

    CONCLUSION: It is of importance that health care professionals listen to both the frail older person and their significant other(s) and be aware of whose voice that the care given is based on, in order to provide care that is beneficial and not to the detriment of the older person.

    Bakgrund: Kommunikation med hjälp av tolk är ett behov som ständigt ökar inom hälso- och sjukvården. Sjuksköterskan ska kunna föra dialog med patienten baserad på respekt, lyhördhet och empati. För att kunna arbeta efter dessa riktlinjer krävs tolk om patienten har bristande språkkunskaper i svenska. Att undersöka patienters upplevelse av att kommunicera med hjälp av tolk i möten med sjuksköterskor kan uppmärksamma tolksamtalens styrkor och brister och på så vis förbättra patientens säkerhet, integritet och delaktighet. Syfte: Syftet var att belysa patienters upplevelser av att kommunicera med hjälp av tolk i möten med sjuksköterskor. Metod: En allmän litteraturstudie som baserades på vetenskapliga kvalitativa artiklar och en artikel med mixad metod. Resultat: Tre huvudkategorier med tre subkategorier vardera framträdde. 1. Tolken ger empowerment: Tolken skapar förutsättningar för att uttrycka sig, Att känna sig stärkt och Att bli bemött med respekt. 2. Tolken – ett nödvändigt ont: Att tvingas förlita sig på någon annan, Att känna sig som en börda och Att hamna vid sidan av. 3. Tolken hindrar: Ett hinder mot fri kommunikation, Ett hot mot integriteten och Språkhinder som består. Diskussion: Tolken är betydelsefull för patientens delaktighet. Patienter upplever ibland ett missnöje över tolkens bristande kompetens. Det är viktigt för patienten att få avgöra huruvida professionell tolk eller närstående ska användas som tolk i mötet med sjuksköterskan. Resultatet diskuteras delvis utifrån modellen personcentrerad vård.

    Abstract [en]

    In recent years, the study of body image shifted from focusing on the negative aspects to a more extensive view of body image. The present study seeks to validate a measure of positive body image, the Body Appreciation Scale-2 (BAS-2; Tylka & Wood-Barcalow, 2015a) in Denmark, Portugal, and Sweden. Participants (N = 1012) were adolescents and young adults aged from 12 to 19. Confirmatory factor analyses confirmed the one-dimensional factor structure of the scale. Multi-group confirmatory factor analyses indicated that the scale was invariant across sex and country. Further results showed that BAS-2 was positively correlated with self-esteem, psychological well-being, and intuitive eating. It was negatively correlated with BMI among boys and girls in Portugal but not in Denmark and Sweden. Additionally, boys had higher body appreciation than girls. Results indicated that the BAS-2 has good psychometric properties in the three languages.

    Abstract [en]

    Objectives

    To derive improved understanding of the implicit meanings of challenges in daily life from the perspective of persons with type 2 diabetes.

    Methods

    A meta-synthesis was conducted with an interpretive and constructivist approach. Four databases were searched for articles published between 2007 and 2011, producing 37 articles for analysis. Van Deurzen's life world theory was applied as an analytic grid.

    Results

    Challenges in daily life with type 2 diabetes could be understood as living in a tension between opposing forces, implying a struggle with inevitable paradoxes: living in the present and for the future, trusting oneself while relying on others, and being normal while feeling changed and different.

    Discussion

    This synthesis adds knowledge to previous understanding of living with type 2 diabetes, revealing the complexity of daily life when struggling with a lifelong illness. Person-centred care could be used to understand what challenges diabetes may cause in family and working life and the ambivalent feelings the illness can lead to. Future research is needed to implement and evaluate a person-centred care in practice. Since new qualitative research is continuously added to this topic, metasyntheses should be undertaken regularly.

    Abstract [en]

    BACKGROUND: The 10-m walk test (10MWT) is a widely used measure of gait speed in Parkinson's disease (PD). However, it is unclear if different standardizations of its conduct impact test results.

    AIM OF THE STUDY: We examined the clinical significance of two aspects of the standardization of the 10MWT in mild PD: static vs. dynamic start, and a single vs. repeated trials. Implications for fall prediction were also explored.

    METHODS: 151 people with PD (mean age and PD duration, 68 and 4 years, respectively) completed the 10MWT in comfortable gait speed with static and dynamic start (two trials each), and gait speed (m/s) was recorded. Participants then registered all prospective falls for 6 months.

    RESULTS: Absolute mean differences between outcomes from the various test conditions ranged between 0.016 and 0.040 m/s (effect sizes, 0.06-0.14) with high levels of agreement (intra-class correlation coefficients, 0.932-0.987) and small standard errors of measurement (0.032-0.076 m/s). Receiver operating characteristic curves showed similar discriminate abilities for prediction of future falls across conditions (areas under curves, 0.70-0.73). Cut-off points were estimated at 1.1-1.2 m/s.

    CONCLUSIONS: Different 10MWT standardizations yield very similar results, suggesting that there is no practical need for an acceleration distance or repeated trials when conducting this test in mild PD.

    Bakgrund: Vården har under många år arbetat utifrån ett paternalistiskt synsätt, där sjukvårdspersonal varit den aktiva beslutsfattaren och patienten varit den passiva. Detta har inneburit att patientens autonomi, perspektiv och preferenser inte tagits tillvara. Det paternalistiska synsättet har gradvis gått över till en mer patientcentrerad vård, där patienten blir en del av vårdteamet. För att öka patientdelaktigheten och självbestämmande inom dagens vård, rekommenderas sjukvårdspersonalen att arbeta efter SDM-modellen, en modell som ska hjälpa både patient och sjukvårdspersonal att tillsammans komma fram till de olika beslut som ska tas i olika vårdsituationer. Syfte:  Att beskriva patienters upplevelser av Shared decision making inom den somatiska vården. Metod: En allmän litteraturöversikt med beskrivande sammanställning, baserad på sju stycken kvalitativa vetenskapliga artiklar.

    Resultat: Patienternas upplevelse av SDM-modellen delades in i tre huvudkategorier med två underkategorier. Resultatet visar vikten av att som patient få information på ett språk som är begripligt för att kunna agera beslutsfattare. Men även att som patient känna tillit till sjukvårdspersonal samt att känna sig respekterad och uppleva trygghet.

    Diskussion: Genom att sjukvårdspersonal arbetar med personcentrerad vård sätts fokus på patienten och ökar inte endast patientens delaktighet utan gör också att patientens autonomi respekteras. Det är sjukvårdspersonalens ansvar att delge patienter rätt information samt bjuda in till samtal för att öka patienters delaktighet i vårdsammanhanget.

    Health related resources among older persons that receive preventive home visits: implications for further recommendations on health promotion2019Conference paper (Other academic)
    Abstract [en]

    In the sustainable development goals for 2030 the emphasis is on health promotion to enable healthy lives and wellbeing for all. Health promotion from an individual perspective focus on enabling the person to take control and improve health, by focusing on positive health related resources. Health promotion can be applied to older persons during a preventive home visit (PHV). Most of the studies made on PHVs to older persons have focused on risk factors. Suggestions for improvement of the PHV would be to add recommendations that promote health related resources, not only look at risk factors. In order to develop health promotion for older persons there is a need for research about positive health related resources. Therefore this study aim for exploring health related resources that could be found among older persons that have received a PHV.

    Methods: A cross-sectional study design was used including older persons age ≥75 years old, living in their own homes, and that had received PHV. Data were collected during a period of nine months, in two municipalities in the south of Sweden. Questions covered wellbeing, nutrition, physical health, functional ability, housing and economy. Binary logistic regression was used to analyse data.

    Results: In total 625 older persons were included in the study, 55.4 % were women, mean age was 80.6 (standard deviation 2.2 years). Preliminary results indicate that following factors are significantly associated with good general health (adjusting for age and gender): being able to do things that make one feel valuable, satisfied with existence, physically active several times per week, having no physical problems to participate in social activities, not feeling sad, not having reduced energy and not having reduced endurance.

    Conclusion: This results indicate that it can be of importance to pay attention to some specific health related resources when giving health promotion advices during the PHV.

    Abstract [en]

    School nurses are one of the first health-care professionals to meet unaccompanied refugee children and adolescents and therefore have an important impact on health outcomes. The aim of this study was to describe school nurses' experiences working with unaccompanied refugee children and adolescents. Semistructured interviews were conducted with school nurses (n = 14) who worked with unaccompanied refugee children and adolescents. The interviews were analyzed via qualitative content analysis. The results fell into three themes: (a) knowledge of trauma-informed care, (b) knowledge of intercultural nursing, and (c) importance of self-awareness. School nurses require the development of tailor-made skills that focus on crisis, trauma, and cultural awareness to meet the complex needs associated with working with unaccompanied refugee children and adolescents.

    Background & aims

    Eating difficulties, having a poor nutritional status, and low activity of daily living are all prevalent issues in the geriatric population. The aim of this study was to explore associations between patients' eating difficulties, their nutritional status and their activity of daily living in patients aged 60 years or older who were admitted to an acute geriatric unit.

    Methods

    A cross-sectional study was conducted between March and September 2016 at the geriatric department of North Denmark Regional Hospital, Hjørring. The inclusion criteria were: ≥ 60years old and hospitalized for a minimum of 24 h. The patients' eating difficulties were assessed using the Minimal Eating Observational Form (MEOF-II), including observations related to ingestion, deglutition and energy/appetite. Eating difficulties were determined on the basis of one or more components of the MEOF-II. Poor nutritional status was defined as an age-specific low body mass index (BMI), <20 kg/m2 if < 70 years, or <22 kg/m2 if ≥ 70 years. Activity of daily living was assessed using the Barthel-100 Index and defined as low (<50) or high (≥50).

    Results

    A total of 297 geriatric patients were included; the mean age was 83.0 (7.7) years and 56.2% of the patients were female. The prevalence of eating difficulties was 55%. Geriatric patients with eating difficulties had a risk increase of 155% of having poor nutritional status (p = 0.003). Geriatric patients with eating difficulties had a risk increase of 60% of having low activity of daily living (p < 0.001).

    Conclusions

    Eating difficulties were highly prevalent in geriatric patients and were associated with poor nutritional status and reduced activity of daily living. The identification of eating difficulties may be important for nutritional interventions and maintenance or improvement of activities of daily living in the geriatric population

    Walking difficulties is the main contributor to fear of falling in people with Parkinson’s disease2017Conference paper (Refereed)
    Abstract [en]

    Fear of falling is more common and pronounced in people with Parkinson’s disease than in controls. We conducted a series of studies that used multivariable regression analyses to identify explanatory factors of fear of falling in people with Parkinson’s disease. Three independent samples were used. The first study (n=154) was a postal survey study that used well-established self-rating scales. The second study replicated the first one by using a clinical sample (n=104) and added unexplored motor aspects (e.g., gait speed, functional balance performance) as well as global cognition. The third clinical study (n=241) included independent variables that focused on personal (e.g. general self-efficacy) and environmental factors as well as Parkinson-related disabilities. In all three studies, walking difficulties was the strongest explanatory (60–68%) factor. The results imply that walking difficulties in daily life should be the primary target in order to reduce fear of falling in people with Parkinson’s disease.

    Women immigrating with their children are a vulnerable group often exposed to ill health and a bad environment. In Europe, women from the Middle East have a complex situation, but immigrant women from low socioeconomic status backgrounds are at highest risk, also with regard to the perinatal period in life. In Sweden all women, independent of their immigrant status are offered free high-quality perinatal care performed by midwives free of charge and nearly 100% use this care, though differently than the nonmigrant population. Often care is sought late in pregnancy, and problems with regard to communication affects the situation as well as care-seeking patterns. However, experiences from families’ originating from the Middle East show their high trust in midwives in Sweden. This trust must be handled with care since it is a great challenge for the health-care organization in Sweden as well as in other countries to host large migrating population, reaching out and communicating in a satisfactory way.

    Abstract [en]

    Deployed family members meet different responses from within their families in context of deployments. In a dialogical participatory action research study involving Swedish soldiers and their families, 110 adults and 25 children narrated about their experiences of being or living with a deployed family member. In use of systemic and narrative ideas in combination with Appreciative Inquiry, the study focused on how soldiers and their families manage socially and relationally before, during and after deployments. Areas in focus were emerging interplay and communication; support needed, developed and used; solutions; recommendations to other families and different family members. Dialogical interviews were conducted with the whole participating family and/or some of the family members. Twenty-nine of the families (n=59) were followed before, during and after deployment. Twelve families were also interviewed later on – three years after the last interview closely after the deployment. With ethical considerations, the interviews were recorded, transcribed verbatim and thematic analyzed by the researcher. Result of the study: Narratives in different perspectives (soldier, veteran, home-staying partner, child, parent, sibling and others) lived and told, exposing different responses to and understanding of the phases of the on-going deployment cycle including for Swedish soldiers two periods of leave at home.

    Abstract [en]

    This study examines emergent responses in Swedish families when a family member working in the Swedish Armed Forces get deployed in an international military operation. A descriptive design encompassing eight years of dialogical participatory action research was used. During this period, 110 adults and 25 children participated in individual and/or group dialogical interviews. Qualitative content analysis was applied and emergent suggestions were subsequently validated in dialogues with participants. This eventually resulted in six ideal types of responses: Partnership; Supporting and mobilizing; Normalizing – ordinary life; Adjusting and negotiating; Emotional reacting; Suffering. The types, recognized by participating family members, including the soldiers, emerge as a matrix for the narrations of experiences and connected responses to the deployment. The Swedish families reject the notion that the general concept of military families captured ‘who we are’. We discuss the meaning of this in the context of organization (the Swedish Armed Forces) and society (family, government/politics).

    Existentiell ensamhet bland äldre personer: volontärers uppfattningar2018Report (Other academic)
    Abstract [sv]

    Äldre personer med många sjukdomar och komplexa vård- och omsorgsbehov tillhören sårbar grupp i samhället. Ohälsan och olika utmanande situationer i livet som denäldre personen ställs inför kan leda till att den äldre personen blir mer medveten omsin existens och framkalla upplevelser av existentiell ensamhet. Ändå vet vi förhållandevislite om hur existentiell ensamhet upplevs och hur den kan upptäckas. Volontärerär en grupp som möter äldre personer och som skull kunna sitta inne med kunskapom existentiell ensamhet bland äldre. Syftet var därför att ta reda på hur volontäreruppfattar och beskriver äldre personers upplevelser av existentiell ensamhet. Studieningår i ett större forskningsprojekt, den så kallade LONE-studien där, förutom volontärer,även äldre personer, deras närstående och personal ingår. Den aktuella undersökningenhade en kvalitativ induktiv ansats och 25 volontärer har intervjuats i sexfokusgruppsamtal. Under intervjuerna har en temaguide använts och intervjuerna haranalyserats med kvalitativ innehållsanalys. Volontärernas beskrivningar av hur de uppfattaratt äldre personer upplever existentiell ensamhet resulterade i fem sub-teman:att känna sig betydelselös, avsaknad av tillhörighet och förankring, farhågor inför detokända, förlust av sitt tidigare jag samt en önskan om att skona och längtan efter attdela. Dessa fem sub-teman bildade tillsammans huvudtemat: Vilsenhet, längtan eftermening och efter någon som vågar träda in i min värld. Vår slutsats är att volontärersitter inne på viktig kunskap om upplevelser av existentiell ensamhet och om de existentiellafunderingar som äldre har och att volontärer har en viktig roll i vård- och omsorgsarbetegenom att tillgodose äldre personers behov av att möta medmänsklighetoch kunna dela sina tankar och upplevelser.

    BACKGROUND: A growing body of studies indicate benefits of physiotherapy for patients in palliative care, for symptom relief and wellbeing. Though physiotherapists are increasingly acknowledged as important members of palliative care teams, they are still an underutilized source and not fully recognized. The aim of this study was to explore the variety of activities described by physiotherapists in addressing the needs and problems of patients and their families in specialized palliative care settings.

    METHODS: Using a free-listing approach, ten physiotherapists working in eight specialized palliative care settings in Sweden described as precisely and in as much detail as possible different activities in which patients and their families were included (directly or indirectly) during 10 days. The statements were entered into NVivo and analysed using qualitative content analysis. Statements containing more than one activity were categorized per activity.

    RESULTS: In total, 264 statements, containing 504 varied activities, were coded into seven categories: Counteracting a declining physical function; Informing, guiding and educating; Observing, assessing and evaluating; Attending to signs and symptoms; Listening, talking with and understanding; Caring for basic needs; and Organizing, planning and coordinating. In practice, however, the activities were intrinsically interwoven. The activities showed how physiotherapists aimed, through care for the body, to address patients' physical, psychological, social and existential needs, counteracting the decline in a patient's physical function and wellbeing. The activities also revealed a great variation, in relation not only to what they did, but also to their holistic and inseparable nature with regard to why, how, when, where, with whom and for whom the activities were carried out, which points towards a well-adopted person-centred palliative care approach.

    CONCLUSIONS: The study provides hands-on descriptions of how person-centred palliative care is integrated in physiotherapists' everyday activities. Physiotherapists in specialized palliative care help patients and families to bridge the gap between their real and ideal everyday life with the aim to maximize security, autonomy and wellbeing. The concrete examples included can be used in understanding the contribution of physiotherapists to the palliative care team and inform future research interventions and outcomes.

    Abstract [en]

    BACKGROUND: Breast cancer (BC) is the most common type of cancer in women worldwide. Post-treatment, patients suffer from side effects and have various rehabilitation needs, which means that individualization is fundamental for optimal rehabilitation. This systematic review (SR) of SRs aims to evaluate the current evidence on rehabilitation interventions in female patients following BC treatment.

    METHODS: Full-text SRs published in English from 2009 were searched in Embase, PubMed, Cinahl Complete, PsycINFO, AMED, SCOPUS, and Cochrane Library.

    INCLUSION CRITERIA: SRs of randomized or non-randomized controlled trials investigating the effects of rehabilitation interventions in women following BC treatment. All outcomes were considered. Methodological quality was evaluated using the AMSTAR 2 tool and interrater agreement was evaluated. Out of 1269 citations retrieved, 37 SRs were included.

    RESULTS: Five rehabilitation areas were identified: exercise and physical activity (PA), complementary and alternative medicine (CAM), yoga, lymphoedema treatment, and psychosocial interventions. The most solid evidence was found in exercise/PA and yoga. Exercise interventions improved outcomes such as shoulder mobility, lymphoedema, pain, fatigue and quality of life (QoL). Effects of yoga were shown on QoL, anxiety, depression, sleep disturbance, fatigue and gastrointestinal symptoms. The effect of CAM was shown on nausea, pain, fatigue, anger and anxiety but these results need to be interpreted with caution because of low methodological quality in included studies in the SRs. Among the lymphoedema treatments, positive effects were seen for resistance training on volume reduction and muscle strength and psychosocial interventions such as cognitive behavioural therapy had positive effects on QoL, anxiety, depression and mood disturbance.

    CONCLUSIONS: This SR of SRs show solid positive effects of exercise/PA and yoga for women following BC treatment, and provides extended knowledge of the effects of CAM, yoga, lymphoedema treatment and psychosocial interventions. It is evident that more than one intervention could have positive effects on a specific symptom and that the effects depend not only on intervention type but also on how and when the intervention is provided. The results can be used as a foundation for individualized rehabilitation and aid health care professionals in meeting patients' individual needs and preferences.

    The number of patients undergoing appearance-altering procedures is increasing, whether they do so as treatment for an existing condition or as an elected cosmetic procedure. Consequently, many patients experience difficulties in coming to terms with changes to their looks, or may have unrealistic expectations of the outcomes and impacts of the treatment. Despite clear evidence that psychological factors play a significant part in many aspects of cosmetic surgery, patients often have very limited or even non-existent access to psychological input as part of their treatment. Therefore, a European Union-funded consortium initiated a survey of 66 healthcare professionals (HCPs) working in the medical aesthetics sector from Bulgaria, Italy, Norway and Romania with the aim of ascertaining their views on the need to enhance psychosocial care, the need for appropriate training and the willingness of HCPs in this sector to undertake this training if available. The results show that there is a pressing need to develop high quality, evidence-based, relevant vocational educational training for those specialising in the care of patients undergoing appearance-related surgery and associated treatments.

    OBJECTIVE: The focus of this study was to determine if there is any significant difference in academic achievement for girls and boys with a cleft compared to the general population of Swedish students at graduation from compulsory school. DESIGN: A retrospective population-based study using data obtained from the Swedish Medical Birth Register that was linked to the Swedish School-Grade Register. PARTICIPANTS: Two hundred seventy girls and 241 boys with cleft palate (CP), 222 girls and 429 boys with cleft lip (CL), and 299 girls and 531 boys with cleft lip and palate (CLP) were compared with the compulsory school population comprising 609,397 girls and 640,007 boys. MAIN OUTCOME MEASURES: (1) Odds of receiving the lowest grade and reduced odds in receiving high grades in Mathematics, English, and Swedish. (2) grade point average (GPA). RESULTS: In all 3 subject grades, for boys with cleft there was no difference when compared to the male population. Girls with cleft were similar to their peers with a few exceptions. Girls with CLP had lower Math grades, and girls with CP had lower Math, English, and Swedish grades. Girls with CP and CLP achieved a significantly lower GPA in comparison to the female population and boys with CP and CL achieved lower GPAs in comparison to the male population. CONCLUSIONS: This study indicates that educational outcomes for girls with cleft are more negatively affected than for boys with cleft.

    Abstract [en]

    This study aims to assess whether notification of an abnormal Pap smear result via a phone call, delivered by a trained healthcare provider, has an effect on women's HRQoL, coping and awareness of HPV. For this intervention study, women were consecutively recruited from a women's health clinic in Sweden. Women in the intervention group (n = 113) were notified of their Pap smear result via a phone call by a trained healthcare provider, while those in the comparison group (n = 122) were notified via a standard letter. A questionnaire was used to collect data. The results found no significant differences between the groups for HRQoL. However, 42.5% of women in the intervention group versus 48.3% in the comparison group reported anxiety. Women in the intervention group were more satisfied with the manner in which they were notified of their abnormal result than those in the comparison group (92.0% vs. 67.2%; p < 0.001), more aware of HPV (71% vs. 50%; p = 0.001), and called healthcare services less often (10.6% vs. 18.0%; p = 0.113), no significance. In conclusion, notification of an abnormal Pap smear result via a phone call does not increase women's HRQoL or reduce their anxiety.

    This study aims to assess (a) women's awareness of the human papillomavirus (HPV), (b) women's health-related quality of life (HRQoL) and levels of anxiety and depression symptoms, and (c) to compare the outcomes between women who are aware of the sexually transmitted nature of the HPV infection and women who are not. Swedish women who have been notified of an abnormal Pap smear result completed a questionnaire. This questionnaire consisted of sociodemographic characteristics, items on awareness of HPV, and how to cope with the Pap smear result and the instruments: the Functional Assessment of Chronic Illness Therapy - Cervical Dysplasia and the Hospital Anxiety and Depression Scale (HADS). Data were analyzed using descriptive statistics, Student's t-test, χ-tests, Fisher's exact test, the Mann-Whitney U-test, and Fisher-Freeman-Halton exact test. In total, 122 women participated. The women reported a median (quartile 1-quartile 3) score of 87.6 on the Functional Assessment of Chronic Illness Therapy - Cervical Dysplasia (81.8-107.0), compatible with a good HRQoL. The median (quartile 1-quartile 3) scores on HADS-anxiety and HADS-depression were 7.0 (4.0-10.0) and 3.0 (1.0-5.3), respectively; however, 48.4% of the women reported anxiety (compared with 20% in a normal population). There were no statistically significant differences in the median scores in any of the scales, including the prevalence of distress between the subgroups. Women with abnormal Pap smear results have a good HRQoL; they can become anxious, but not depressed. Awareness of HPV as a sexually transmitted infection is low, but being aware does not impact on women's HRQoL or on anxiety and depression.

    OBJECTIVES: The aim of this study was to investigate public health nurses' experiences of using interpreters when meeting with Arabic-speaking first-time mothers.

    DESIGN AND SAMPLE: An inductive qualitative design was used. Individual interviews were conducted with Swedish public health nurses (n = 11) with experience of working in child health care with interpreters when meeting with Arabic-speaking first-time mothers.

    MEASURES: Data were analyzed using qualitative content analysis.

    RESULTS: The analysis of the interviews resulted in one overarching theme: Having to accept and learn to incorporate interpreters when meeting with Arabic-speaking first-time mothers. Three subthemes were identified: (a) enabling an understanding of the situation of the mothers; (b) contributing to a trustful relationship; and (c) creating disturbing elements in the dialogue.

    CONCLUSIONS: Using interpreters optimized the conversation between the public health nurse and the Arab-speaking first-time mother; however, the complexity of using interpreters was also apparent.

    Abstract [en]

    In late stage Parkinson’s disease (PD) (i.e., Hoehn and Yahr (HY) stages IV-V), both motor and nonmotor symptoms (NMS) are pronounced, and the patients become increasingly dependent on help in their daily life. Consequently, there is an increasing demand on health-care and social care resources for these patients and support for their informal caregivers. The aim of this study was to assess satisfaction with care in late stage PD patients and to identify factors associated with satisfaction with care. Moreover, to assess their informal caregivers’ satisfaction with support and to identify factors associated with caregivers’ satisfaction with support. Factors potentially associated with satisfaction with care/support were assessed in 107 late stage PD patients and their informal caregivers () and entered into multivariable logistic regression analyses. Fifty-eight (59%) of the patients and 45 (59%) of the informal caregivers reported satisfaction with their overall care/support. Patients satisfied with their care reported higher independence in activities of daily living (ADL) (Katz ADL index; ), less depressive symptoms (Geriatric Depression Scale, GDS-30; ), and higher individual quality of life (QoL) (Schedule for the Evaluation of Individual Quality of Life Questionnaire, SEIQoL-Q; ). Multivariable logistic regression analyses identified depressive symptoms () and independence in ADL () as independently associated with satisfaction with care. For informal caregivers, the analyses identified patients’ HY stage () and caregivers’ QoL (Alzheimer’s Carers Quality of Life Inventory, ACQLI; ) as independently associated with satisfaction with caregiver support. The results indicate that an effective both pharmacological and nonpharmacological PD therapy is important, to adequately treat motor and NMS (e.g., depressive symptoms) in order to improve depressive symptoms and patient independence in ADL. This may benefit not only the patients, but also their informal caregivers.

    BACKGROUND: It is unclear to which degree Levodopa (L-dopa) remains effective also in the late stage of Parkinson's disease (PD) and to which degree motor fluctuations and dyskinesias remain a problem.

    OBJECTIVE: To assess responsiveness of motor symptomatology to L-dopa in a group of patients with late stage PD. Moreover, to investigate the extent to which motor fluctuations and dyskinesias occur.

    METHODS: Thirty PD patients in Hoehn and Yahr (HY) stages IV and V in "on" were included. L-dopa responsiveness was assessed with a standardized L-dopa test in the defined "off" and defined "on" states. Motor function was assessed by the Unified PD Rating Scale (UPDRS) III and timed tests. Motor fluctuations and dyskinesias were assessed by the UPDRS IV. The participants were further monitored for 10 days with a mobile movement-analyses-system, the Parkinson's Kinetigraph (PKG). The median (q1-q3) L-dopa equivalent daily dose (LEDD) was 799 (536-973) mg.

    RESULTS: The UPDRS III score improved with ≥15% in 15 (50%) and with ≥30% in six (20%) participants during the L-dopa test. The median (q1-q3) UPDRS III score in "off" was 46 (37-53) and in "on" 36 (28-46). Twenty-one (70%) of the participants reported either predictable or unpredictable "off" fluctuations (items 36-37). The prevalence of dyskinesias (item 32, duration of dyskinesias ≥1) was 47%. The PKG indicated that dyskinesias primarily were mild and that a majority had a pronounced "off" symptomatology, spending a large proportion of the day either asleep or very inactive.

    CONCLUSIONS: Half of a group of patients with late stage PD had an L-dopa response of ≥15% on the UPDRS III. According to the UPDRS IV, a majority of the patients had motor fluctuations and about half had dyskinesias, although the PKG results suggested that these were not very severe.

    Abstract [en]

    Objective: To assess responsiveness to Levodopa (L-dopa) in patients with late stage Parkinson’s disease (PD). Moreover, to investigate if the L-dopa effect is stable or whether motor fluctuations and dyskinesias are present.

    Background: A majority of PD patients treated with L-dopa develop motor complications, in many cases within a few years of treatment. It is unclear to which degree L-dopa remains effective also in the last stages and whether motor fluctuations and dyskinesias remain a problem.

    Methods: The study included 30 patients with PD in Hoehn and Yahr (HY) stages IV and V in “on” and/or having a substantial need of help with ADL (<50% Schwab and England Scale) and having been diagnosed with PD for =7 years. Nineteen were men, median age 83 and median PD duration 12 years. Nineteen were in HY stage IV and 11 in V. L-dopa responsiveness was assessed with an L-dopa test and motor evaluation according to a standardized protocol in the defined “off” and defined “on” state. Motor performance was assessed by the Unified PD Rating Scale (UPDRS) III and timed tests for gait and hand-arm. The participants were further evaluated with a mobile movement-analyses-system, the Parkinson’s Kinetigraph (PKG) for 10 days.

    Results: The number of participants with an improvement in UPDRS III during L-dopa test of =15% were 15 (50%) and of =30% were 6 (20%). The median UPDRS III score in “off” was 46 (q1-q3, 37-53) and in “on” 36 (28-46), with a median improvement of 15.5% (8-27%), p<0.001. There was a statistically significant effect in almost all variables; UPDRS III total score; speech (item 18); resting tremor (item 20); tremor (items 20, 21); rigidity (item 22); bradykinesia (items 23-26, 31); gait (item 29); axial signs (items 18, 19, 22, 27-30); dyskinesias (Clinical Dyskinesia Rating Scale); hand-arm movement; gait test and blood pressure. According to the UPDRS IV, 67% experienced predictable off-fluctuations, while 30% had unpredictable off-fluctuations. The prevalence of dyskinesias according to item 32 (duration of dyskinesias =1) of the UPDRS IV was 47%. According to the PKG registrations, some patients were experiencing significant motor fluctuations, though very few had significant dyskinesias.

    Conclusions: Half of a group of patients with late stage PD (HY IV-V in “on”) had a significant L-dopa response (=15% on the UPDRS III). According to the UPDRS IV, a majority of the patients still had motor fluctuations and about half had dyskinesias.

    BACKGROUND: Non-motor symptoms (NMS) are common in late stage Parkinson's disease (PD), as the frequency and severity of most of these symptoms increase with advancing disease.

    OBJECTIVE: To assess effect of dopaminergic therapy on NMS in late stage PD and to investigate relationships between dopaminergic effect on NMS and on motor function.

    METHOD: Thirty PD patients in Hoehn and Yahr (HY) stages IV and V in "on" were included. Dopaminergic effect on non-motor symptomatology was assessed by the modified version of the Non-Motor Symptoms Scale (NMSS) in the "off" and the "on" state during a standardized L-dopa test, in parallel also assessing motor function.

    RESULTS: NMS were common and many of the symptoms occurred in >80% of the individuals. The highest NMSS scores were seen within the NMSS domains 3: mood/apathy and 7: urinary in both the "off" and the "on" state. There was a statistically significant (p <  0.001) improvement in the modified NMSS total score (median) from 79 in "off" to 64 in "on". There were statistically significant differences between the "off" and the "on" state for domains 2: sleep/fatigue, 3: mood/apathy, 5: attention/memory, 6: gastrointestinal and 7: urinary. The differences in the NMSS score between the "off" and the "on" state were in general larger for motor responders than for motor non-responders. In motor non-responders, differences of the NMSS score between the "off" and the "on" state were found for the total score, domain 3: mood/apathy and its item 11-flat moods.

    CONCLUSION: There is an effect of dopaminergic medication on NMS in late stage PD, to some extent also for those with a non-significant response on motor function during L-dopa test. It is therefore of importance to optimize dopaminergic therapy in order to give the most effective symptomatic treatment possible.

    AIM: To investigate self-reported needs for care, support and treatment among persons who frequently visit psychiatric emergency rooms (PERs).

    DESIGN: A cross-sectional design. Qualitative and quantitative data were collected using an interview-based manual. Qualitative data were analysed using content analysis, whereas quantitative data were analysed using descriptive, non-parametric statistical tests.

    RESULTS: Persons who frequently visit PERs self-reported unmet needs for care, support and treatment in life domains such as health, socialisation, daytime activities, and emotional and financial security.

    CONCLUSION: To meet the needs of persons who frequently visit PERs, close cooperation between concerned welfare actors should be implemented.

    PURPOSE: Doctoral student well-being is an important matter that shapes the well-being of academics throughout their careers. Given that well-being has been found to be closely related to employee productivity and efficiency, strategies associated with maintaining well-being during PhD studies might be crucial for higher education, its outcomes and-just as importantly-for a balanced life of PhD students.  Method: Based on 17 studies, this literature review critically assesses the literature on doctoral student well-being.  Results: Theoretical models, concepts of well-being, and methods applied are discussed, as are the results of the articles. The reviewed studies are then discussed based on a SWOT analysis addressing the strengths and weaknesses of the reviewed research as well as the identified opportunities and threats, which can be used as a basis for future research. Based on the review findings and the SWOT analysis, a multidimensional view of the well-being of doctoral students is proposed.  Conclusions: The study proposes a more student-centred approach to meeting doctoral students' needs, and the enhancement of doctoral student well-being in order, as a long-term goal, to improve academics' well-being and productivity.Doctoral student well-being is an important matter that shapes the well-being of academics throughout their careers.     AIMS AND OBJECTIVES: The aim of this study was to explore municipal healthcare employees' experiences of relationships with care recipients and colleagues. The specific research questions were when do the relationships enhance well-being, and what prerequisites are needed for such relationships to occur?.

    BACKGROUND: Employees in health and social care for older people often depict their work in negative terms, and they often take a high number of sick leaves. Despite the heavy workload, other employees express well-being at work and highlight social relationships as one reason for this. However, a greater understanding of how these relationships can act as resources for workplace well-being is needed.

    DESIGN: The design of the study was qualitative and exploratory.

    METHODS: Qualitative interview studies were conducted with twenty-three healthcare employees in municipal healthcare. Thematic analysis was used to analyse the data.

    RESULTS: Two themes were identified as resources for promoting relationships between employees and care recipients or colleagues: (i) Being personal - a close interpersonal relationship to a care recipient - and (ii) Colleague belongingness - a sense of togetherness within the working group. Spending quality time together, providing long-term care and providing additional care were antecedents for a close interpersonal relationship with care recipients. Trust, mutual responsibility and cooperation were antecedents for a sense of togetherness within the working group.

    CONCLUSIONS: The findings provide an empirical base to raise awareness of relationships with care recipients and colleagues as health aspects. Relationships among employees in healthcare are vital resources that must be considered to create sustainable workplaces, and consequently improve the quality of care.     BACKGROUND: Cleft care provision in the United Kingdom has been centralized over the past 15 years to improve outcomes for children born with cleft lip and palate. However, to date, there have been no investigations to examine how well these multidisciplinary teams are performing. METHODS: In this pilot study, a cross-sectional questionnaire surveyed members of all health care specialties working to provide cleft care in 11 services across the United Kingdom. Team members were asked to complete the Team Work Assessment (TWA) to investigate perceptions of team working in cleft services. The TWA comprises 55 items measuring seven constructs: team foundation, function, performance and skills, team climate and atmosphere, team leadership, and team identity; individual constructs were also aggregated to provide an overall TWA score. Items were measured using five-point Likert-type scales and were converted into percentage agreement for analysis. RESULTS: Responses were received from members of every cleft team. Ninety-nine of 138 cleft team questionnaires (71.7%) were returned and analyzed. The median (interquartile range) percentage of maximum possible score across teams was 75.5% (70.8, 88.2) for the sum of all items. Team performance and team identity were viewed most positively, with 82.0% (75.0, 88.2) and 88.4% (82.2, 91.4), respectively. Team foundation and leadership were viewed least positively with 79.0% (72.6, 84.6) and 76.6% (70.6, 85.4), respectively. CONCLUSIONS: Cleft team members perceive that their teams work well, but there are variations in response according to construct.

    AIMS AND OBJECTIVES: The aim of this study was to describe how EL was eased, as narrated by frail older people.

    BACKGROUND: Existential loneliness (EL) is an unavoidable part of the human condition. It is a complex phenomenon that has been described as disconnection from life. If EL is acknowledged in the care of older people, the experience of EL can be reduced.

    DESIGN: In this qualitative study, we used an exploratory and descriptive design.

    METHODS: The study was based on 22 narrative interviews with frail older people, 76 to 101 years old, who were receiving long-term care and services. We analysed the data using conventional content analysis.

    RESULTS: Being acknowledged by others, that is, being the focus of others' concern, eased the experience of EL, as did encountering intimacy and having meaningful exchanges of thoughts and feelings. Further, EL was pushed into the background and eased when participants could bracket negative thoughts and feelings, that is, when they could adjust and accept the present situation, view life in the rear-view mirror, be in contact with spiritual dimensions and withdraw and distract themselves.

    CONCLUSION: Existential loneliness can be eased by experiencing meaningful togetherness with others and oneself when something else comes to the forefront, pushing EL to the background. Frail older peoples' opportunities to ease EL may be facilitated by health care staff (HCS) providing person-centred care and create possibilities for solitary time and meaningful togetherness.

    IMPLICATIONS FOR PRACTICE: If frail older people's ongoing processes of adjusting and accepting their situation are understood and confirmed by people in their environment, for example, by nurses, family and friends, the experience of living a meaningful life can be supported, which, in turn, can ease EL.

    Abstract [en]

    This study explores the impact of living with Parkinson’s disease (PD). Nineteen persons (11 women) aged 55–84 diagnosed with PD 3–27 years ago participated. Data were collected through semistructured interviews, which were recorded, transcribed verbatim, and analysed by qualitative content analysis. Four categories represented the impact of living with PD: “Changed prerequisites for managing day-to-day demands,” “Loss of identity and dignity,” “Compromised social participation,” and “The use of practical and psychological strategies.” There was a shift from an internal to an external locus of control in managing, control, competence, relatedness, and autonomy. According to self-determination theory, a shift towards extrinsically motivated behaviours may occur when these basic needs are thwarted, leading to compensatory strategies or needs substitutes with negative consequences on health and well-being. We suggest a needs-based approach as an important starting point to better understand the consequences of living with PD and to explore the means for people with PD to acquire an improved quality of life on their own terms. In conclusion, our findings suggest for a shift in focus, from a biomedical to a needs-based approach to understand the impact of living with PD and facilitate more person-centred care and person-centred outcome measurement.

    Abstract [en]

    Background

    There has been a lack of a standardized protocol for collection of patient reported outcomes (PRO) and detection of and indication for psychological treatment in cleft care. The objectives of this paper was to translate Cleft Hearing Appearance and Speech Questionnaire (CHASQ) into eight European languages, to investigate whether levels of PRO in patients with cleft lip and/or cleft palate (CL/P) were comparable across countries and to investigate clinician experience of the instrument.

    Methods

    The PRO measure—CHASQ—was translated into Bulgarian, Estonian, Greek, Latvian, Macedonian, Romanian, Serbian and Swedish and implemented with patients in the respective countries. A focus group discussion was conducted to investigate health care professional experience on the use of CHASQ in their clinics. Data was analysed in accordance with the principles of thematic analysis.

    Results

    Analysis showed statistically significant differences between countries and groups of diagnosis in CHASQ scores. CHASQ helped clinicians gain patient information and informed on treatment decisions, broadened the clinicians’ role as caregivers and was perceived as short and easy to use. Limited time and resources in clinics were limitations in implementing the instrument.

    Conclusions

    Translation and utilization of CHASQ facilitated international comparison and cooperation. Linguistically, valid replicas of CHASQ are now available in many European languages. Results from this study show that CHASQ may be used for collection of PRO on patient satisfaction and to spark conversation between clinicians, patients and families.

    Background

    Earlier research has investigated psychosocial and educational issues in populations of patients with cleft identifying several areas of concern. The objective of this study was to investigate current beliefs and knowledge about psychosocial and educational issues in parents and health care professionals (HCP) of children with cleft lip and/or palate (CL/P).

    Method

    Parents were interviewed concerning the view of society on people with visible differences, their child’s social life, educational progress, and information regarding psychosocial care. Interviews with HCPs concerned characteristics related with educational achievement, behavior, and social relationships. Fifteen parents of children 9 to 13 years of age with CL/P and 10 HCPs and were interviewed. Data from interviews was analyzed with thematic analysis.

    Results

    Eight children were reported to have emotional issues related to their cleft. Eleven parents, however, did not perceive that their child was treated differently in society. HCPs expressed concerns regarding for example coping with being different, low self-esteem, shyness, disadvantage on first impression, and acceptance of themselves. A majority of the HCPs did not think patient cognition and behavioral or physical development were specifically affected.

    Conclusions

    The results revealed that parent experience and views were diverse—from no specific problems related to the cleft, to both emotional and educational issues. The beliefs and level of knowledge in HCPs also varied. All HCPs, however, wished for more information and training regarding psychosocial issues.

    AIM:

    To explore existential loneliness among older people in different healthcare contexts from the perspective of healthcare professionals.

    BACKGROUND:

    Professionals meet and care for older people in most care contexts and need to be prepared to address physical, psychological, social and existential needs. Addressing existential loneliness can be both challenging and meaningful for professionals and is often not prioritised in times of austerity.

    DESIGN:

    A multiple case study design was used.

    METHODS:

    Focus group interviews were conducted with healthcare professionals (n = 52) in home, residential, hospital and palliative care settings. The analysis was performed in two steps: firstly, a within-case analysis of each context was conducted, followed by a cross-case analysis.

    FINDINGS:

    Differences and similarities were observed among the care contexts, including for the origin of existential loneliness. In home care and residential care, the focus was on life, the present and the past, compared to hospital and palliative care, in which existential loneliness mainly related to the forthcoming death. The older person's home, as the place where home care or palliative care was received, helped preserve the older person's identity. In hospital and palliative care, as in institutional care, the place offered security, while in residential care, the place could make older people feel like strangers. Creating relationships was considered an important part of the professionals' role in all four care contexts, although this had different meanings, purposes and conditions.

    CONCLUSIONS:

    The context of care matters and influences how professionals view existential loneliness among older people and the opportunities they have to address existential loneliness.

    IMPLICATIONS FOR PRACTICE:

    Support for professionals must be tailored to their needs, their education levels and the context of care. Professionals need training and appropriate qualifications to address existential loneliness related to existential aspects of ageing and care.

    PURPOSE: Existential loneliness is part of being human that is little understood in health care, but, to provide good care to their older patients, professionals need to be able to meet their existential concerns. The aim of this study was to explore health care professionals' experiences of their encounters with older people they perceive to experience existential loneliness.

    METHOD: We conducted 11 focus groups with 61 health professionals working in home care, nursing home care, palliative care, primary care, hospital care, or pre-hospital care. Our deductive-inductive analytical approach used a theoretical framework based on the work of Emmy van Deurzen in the deductive phase and an interpretative approach in the inductive phase.

    RESULTS: The results show that professionals perceived existential loneliness to appear in various forms associated with barriers in their encounters, such as the older people's bodily limitations, demands and needs perceived as insatiable, personal shield of privacy, or fear and difficulty in encountering existential issues.

    CONCLUSION: Encountering existential loneliness affected the professionals and their feelings in various ways, but they generally found the experience both challenging and meaningful.

    Hur upplever sjuksköterskan mötet med den äldre personen inom kommunal vård ochomsorg? Ett sätt för sjuksköterskan att känna närvaro i mötet är genom att helhjärtatlyssna på den äldre när hen inbjuder till samtal om sina tankar eller om livshistorian.Sjuksköterskan kan genom mötet uppleva en högre mening med sitt arbete och få enmöjlighet att etablera en relation med den äldre. Att vårda äldre personer ställer högakrav på omvårdnaden där en helhetssyn med fokus på personcentrerad omvårdnadstår i centrum och kunskaper i äldre personers psykologiska och sociala åldrande kanhjälpa sjuksköterskan vara närvarande i mötet. Syftet var att beskriva sjuksköterskorsupplevelser av faktorer som bidrar till möjligheten att vara närvarande i mötet medäldre personer inom kommunal vård och omsorg. Nio intervjuer genomfördes medsjuksköterskor inom kommunal vård och omsorg. Intervjuerna analyserades med hjälpav kvalitativ innehållsanalys. Resultatet visade att sjuksköterskorna hade som sin främstaintention att prioritera mötet med vårdtagaren. Faktorer som bidrog till sjuksköterskornasmöjlighet till närvaro i mötet med äldre var relaterade till; (I) mötet i form avatt kunna bygga på en tidigare relation, att ha gemensamma beröringspunkter, att haen ömsesidig öppenhet och vilja att dela varandras glädje- och sorgeämnen (II) vårdtagareni form av att vårdtagaren har förtroende för sjuksköterskan, att vårdtagareninbjuder till samtal och vårdtagarens tillstånd (III) sjuksköterskan i form av kunskaperom vårdtagaren och betydelsen av att vara närvarande i mötet, personlig mognad ochsjälvkännedom, att vara genuint intresserad av vårdtagaren och visa respekt och attkunna hantera sina känslor, samt (IV) organisationen i form av att ha tid, att ha möjlighetatt prioritera mellan olika arbetsuppgifter, hur vårdarbetet fördelas och organiserasoch den kontext som sjuksköterskan befinner sig i. Resultatet visade att sjuksköterskornahade som intention att vara närvarande i mötet. Det var dock inte alltid så lättatt få tid till mötet på grund av tids- och/eller personalbrist, men även sjuksköterskanspersonliga mognad och erfarenhet hade betydelse för att kunna/våga vara närvarandei mötet. En positiv och stödjande arbetsledning samt ett nära samarbete med kollegersågs som viktiga resurser i det dagliga omvårdnadsarbetet.

    BACKGROUND: In many European countries, women have a higher sickness absence rate than men. Women also report higher levels of work-life conflict, which has a negative impact on women's self-perceived health. Interventions studies on work-life balance literature are scarce. This research adds knowledge about work-life balance by examining female employees' experiences of a newly developed intervention program, the BELE program (Balance in Everyday Life Empowerment program), aiming to enhance the work-life balance and wellbeing of female employees.

    METHODS: All participants in the BELE program were included in the sample (n = 55) of the study. The methods used for gathering data were focus group interviews (n = 8) and semi-structured individual interviews (n = 8). A qualitative content analysis was used for the analyses.

    RESULTS: The results showed that a majority of the participants experienced a process of change for enhancement of wellbeing and balance in their everyday life when taking part in the BELE program. In the analyses, three main themes emerged. Reflecting and strengthening inner resource, Trying to change everyday patterns and habits and Experiencing more balance and well-being.

    CONCLUSIONS: The results point out the importance of awareness and reflection on everyday life to promote work-life balance and wellbeing. The BELE program was described as a wakeup call to one's own life and as an enhancer of empowerment processes and equality in the homes. Moreover, the results showed the need for health education in the workplace focusing on both work and private life to enhance balance and wellbeing among female employees'. The findings indicate that it is important to work at individual and group levels in work-life balance interventions and not merely at the organizational level or in the workplace setting.
