    Abstract [en]

    AIMS AND OBJECTIVES: To describe lesbian, gay and bisexual parents' experiences of nurses' attitudes in child health care.

    BACKGROUND: Lesbian, gay and bisexual people are often reluctant to disclose their gender identity for fear of discrimination. This fear may lead to avoidance of healthcare for themselves or their children and may negatively affect families' health and well-being.

    DESIGN: A qualitative inductive design was employed.

    METHODS: Semi-structured interviews were conducted with 14 lesbian, gay or bisexual parents (11 mothers and 3 fathers) with child health care experiences in southern Sweden. Interviews were analysed using qualitative content analysis.

    RESULTS: Two themes were identified. One, a 'sense of marginalization', included lesbian, gay and bisexual parents' experiences of heteronormative attitudes among child health care nurses which led them to feel alienated and questioned as parents. Another, 'being respected for who you are', included experiences of being respected and included at child health care appointments.

    CONCLUSIONS: Findings paint a complex picture of lesbian, gay and bisexual parents' interactions with child health care nurses in that they experienced both positive and negative attitudes. Knowledge gaps about lesbian, gay and bisexual families within the child health care field must be filled.

    RELEVANCE TO CLINICAL PRACTICE: Child health care nurses should work with the entire family to provide the best care for the child; however, discrimination in health care is common and often caused by a lack of knowledge. The number of children living with same-sex parents has increased more than ten-fold since the end of the 1990s. It is therefore important to explore lesbian, gay and bisexual parents' experiences with child health care nurses' attitudes to improve quality of care.

    Abstract [en]

    PURPOSE: Linking ready-made questionnaires to codes within the International Classification of Functioning, Disability and Health, Children and Youth Version with the intention of using the information statistically for studying mental health problems can pose several challenges. Many of the constructs measured are latent, and therefore, difficult to describe in single codes. The aim of this study was to describe and discuss challenges encountered in this coding process.

    MATERIALS AND METHODS: A questionnaire from a Swedish research programme was linked to the International Classification of Functioning, Disability and Health, Children and Youth Version and the agreement was assessed.

    RESULTS: Including the original aim of the questionnaire into the coding process was found to be very important for managing the coding of the latent constructs of the items. Items from the International Classification of Functioning, Disability and Health, Children and Youth Version chapters with narrow definitions for example mental functions, were more easily translated to meaningful concepts to code, while broadly defined chapters, such as interactions and relationships, were more difficult.

    CONCLUSION: This study stresses the importance of a clear, predefined coding scheme as well as the importance of not relying too heavily on common linking rules, especially in cases when it is not possible to use multiple codes for a single item. Implications for rehabilitation The International Classification of Functioning, Disability and Health, Children and Youth Version, is a useful tool for merging assessment data from several sources when documenting adolescents' mental functioning in different life domains. Measures of mental health are often based on latent constructs, often revealed in the description of the rationale/aim of a measure. The latent construct should be the primary focus in linking information. By mapping latent constructs to the International Classification of Functioning, Disability and Health, Children and Youth Version, users of the classification can capture a broad range of areas relevant to everyday functioning in adolescents with mental health problems. The subjective experience of participation, i.e., the level of subjective involvement, is not possible to code into the International Classification of Functioning, Disability and Health, Children and Youth Version. However, when linking mental health constructs to the International Classification of Functioning, Disability and Health, Children and Youth Version codes, the two dimensions of participation (the being there, and the level of involvement) need to be separated in the linking process. This can be performed by assigning codes focusing on being there as separate from items focusing on the subjective experience of involvement while being there.

    Including vulnerable groups of students such as students with learning disabilities in mainstream school research, require ethical considerations and questionnaire adaptation. These students are often excluded, due to low understanding or methodologies generating inadequate data. Students with disability need be studied as a separate group and provided accessible questionnaires. This pilot study aims at developing and evaluating student self-reported measures, rating aspects of student experiences of school-based Physical Education (PE). Instrument design, reliability and validity were examined in Swedish secondary school students (n = 47) including students, aged 13, with intellectual disability (n = 5) and without impairment and test–retested on 28 of these students. Psychometric results from the small pilot-study sample were confirmed in analyses based on replies from the first wave of data collection in the main study (n = 450). Results show adequate internal consistency, factor structure and relations between measures. In conclusion, reliability and validity were satisfactory in scales to measure self-efficacy in general, in PE, and aptitude to participate. Adapting proxy ratings for functioning into self-reports indicated problems. Adequacy of adjustments made were confirmed and a dichotomous scale for typical/atypical function is suggested for further analyses.

    Evaluation of models for health supportive family conversation intended to turn a negative weight development in preschool children2015Konferensbidrag (Övrigt vetenskapligt)
    Abstract [en]

    Introduction: Obesity in children is a growing health problem in the world. In Sweden, 14-15 percent of 4-year-old children are overweight and 3 percent are obese. Prevention of overweight and obesity is most effective if started as early as possible. Studies have shown the need for involving the family since parents’/guardians’ attitudes and beliefs may be crucial for the development of the child's eating habits and have thus a key role in the preventive work regarding a healthy lifestyle for the children. In Sweden, the Child Health Service (CHS) comprises a comprehensive universal nurse-led CHS program with health surveillance directed to the children and their parents, including parental support. The CHS have the opportunity to pay attention to early development of overweight and implement measures that can contribute to a healthy lifestyle during childhood as well as adulthood. The need for evidence based methods/models to be used in combating child overweight and obesity is obvious. However, there is a lack of evidence based models to prevent the prevailing childhood obesity used by clinical active nurses.

    Purpose: The overall purpose is to evaluate models for health supportive family conversations to families with preschool children with the intent to turn a negative weight development.

    Method: The study has a longitudinal design with health supportive family conversations being evaluated both qualitatively and quantitatively. A strategic sample will be applied on CHS centers from different socioeconomic areas in the southern part of Sweden. The models that will be used are the Calgary Family Assessment Model (CFAM) and The Calgary Family Intervention Model (CFIM). The models have its their theoretical foundation in e.g. theories of systems, communication, and change. The study will have a naturalistic approach with a pre-posttest design in which nurses in CHS are trained in the model for health supportive family conversations. Supervision will be provided during the intervention. Children’s IsoBMI and The Lifestyle Behavior Checklist will be used as pre- and post-test and at the 12 month follow-up. Evaluation interviews will be held with the families and the nurses separately.

    Preliminary results: A pilot interview showed that the health supportive family conversations led to increased knowledge and understanding about the family lifestyles related to the child obesity. Involvement of the family lead to a consciousness within the family about ingredients in sweet drinks and food. The fact that the CHS nurse was the one who conducted the conversations were of great importance, since the nurse was well known to the family.

    Skolsköterskors utmaningar med att dokumentera skolbarns psykosociala hälsa – en kvalitativ studie

    Syfte: Syftet med studien var att undersöka skolsköterskors utmaningar relaterat till dokumentation av skolbarns psykosociala hälsa

    Metod: Sex fokusgruppsdiskussioner genomfördes. I diskussionerna ingick frågor om situationer som var särskilt utmanande att dokumentera samt vad som begränsar och/eller underlättar dokumentationen av den psykosociala hälsan. Kvalitativ innehållsanalys användes för att tolka data.  

    Resultat: Analysen resulterade i ett övergripande tema: Att göra sin plikt och rädsla för att göra fel samt tre underteman: osäkerhet relaterat till den egna förmågan; rädsla för framtida konsekvenser och strategier för att hantera dokumentationen. 

    Slutsats: Skolsköterskor som förlitar sig på sin intuition och som använder en strukturerad dokumentationsmodell ökar möjligheterna till en tillförlitlig dokumentation. För att ytterligare utveckla sina yrkeskunskaper kan regelbunden yrkesmässig handledning vara av stor betydelse. Detta kan i sin tur bidra till ökad forskning och utveckling till förmån för skolbarns psykosociala hälsa.

    Abstract [en]

    The aim of this study was to explore school nurses’ experience of challenges related to documenting schoolchildren’s psychosocial health in Sweden. Six focus group discussions were carried out. Areas for discussions included questions about situations, especially challenging to document as well as what constrains and/or facilitates documenting psychosocial health problem issues. Qualitative content analysis was used for interpreting the data. The analysis resulted in one overarching theme: having to do one’s duty and being afraid of doing wrong; and three subthemes: uncertainty related to one’s own ability, concerns related to future consequences, and strategies to handle the documentation. School nurses relying on their intuition and using a structured documentation model may increase the opportunities for a reliable documentation. To further develop their professional skills with regular, clinical supervision can be of great importance. This in turn may increase contributions to research and development for the benefit of schoolchildren’s psychosocial health.

    Självskattad psykisk ohälsa ökar bland ungdomar och det finns tydliga skillnader mellan flickor och pojkar. Fler flickor rapporterar symtom på ohälsa och dessa skillnader har ökat över tid. Det är fler flickor som rap- porterar att de känner sig stressade och färre flickor uppger att de trivs med livet jämfört med pojkar. Flickor är medvetna om kraven på deras utseende och kravet på att lyckas i livet. De är också medvetna om de könsskillnader som finns både i skolan och media och som påverkar dem. Samhörighet med kamrater och familj är betydelsefullt liksom trygghet i form av tillit till kamrater och att känna sig trygg där de vistas enligt tonårsflickorna själva.

    Detta kapitel kommer att handla om tonårsflickors hälsa och hur tonårs- flickor själva upplever sin vardag, vilket kommer att belysas med resultat från en forskningsstudie, samt diskuteras utifrån ett genusperspektiv och Känsla av sammanhang (KASAM).

    The aim of the study was to investigate Swedish school nurses’ perceptions of schoolchildren’s health. The study is based on two national surveys in which school nurses responded to questions about schoolchildren’s health in 2015 (n = 181) and 2005 (n = 129). A statistical comparison showed that physical and mental health of schoolchildren in high-risk areas was perceived as worse than that of children in low-risk areas. There were no differences regarding the number of spontaneous visits or consulting reasons related to gender in high- and low-risk areas. A national framework for data collection and reporting of schoolchildren’s health may be important for school nurses to demonstrate the need for increased support in, for example, high-risk areas. By paying attention to factors that can affect schoolchildren’s health, school nurses can also direct health promotion to areas where it is needed. Increased cooperation among school health professionals may also prevent illness and improve health.

    Abstract [en]

    Two recent studies have challenged the well-established belief that offending behaviors are inversely related to the personality trait of conscientiousness. Therefore, the aim of this study was to explore prisoners’ levels of traits according to the Five-Factor Model (FFM) of personality compared to control groups, with a focus on conscientiousness. Two separate samples of inmates in Swedish high-security prisons were investigated in three studies. Inmates and non-inmates completed a Swedish-language translation of Goldberg’s (1999) International Personality Item Pool questionnaire (IPIP-NEO, Bäckström, 2007). Male inmates (n = 46) in Studies 1 and 2 scored higher on conscientiousness than non-inmates (norm data based on approximately 800 males, and a students’ sample), which conflicts with previous results. Study 3 further explored the conscientiousness differences on the facet level. Male and female inmates (n = 131) scored higher on order and self-discipline (even after an adjustment for social desirability) than students (n = 136). In conjunction with previous findings, these differences are interpreted as being either temporal or enduring adjustments to the prison environment. It is suggested that researchers and clinical teams should cautiously interpret the FFM factor of conscientiousness (and its facets) when planning the further treatment of inmates.

    In the Nordic countries, substantial policy and intervention efforts have been made to increase adolescents' consumption of fruit and vegetables and to reduce their intake of sweets and soft drinks. In recent years, social inequalities in food habits have been attracted particular governmental interest and several initiatives addressing the socioeconomic gradient in food habits have been highlighted. However, few internationally published studies have evaluated how trends in adolescents' food habits develop in the context of Nordic nutrition policy, or have compared differences between the Nordic countries.

    Methods

    The study was based on Danish, Finnish, Norwegian and Swedish cross-sectional data from the international Health Behaviour in School-Aged Children (HBSC) study, collected via three nationally representative and comparable questionnaire surveys in 2001/2002, 2005/2006 and 2009/2010. Food habits were identified by students' consumption of fruit, vegetables, sweets and sugar sweetened soft drink. Socioeconomic status (SES) was measured with the Family Affluence Scale (FAS). Multilevel logistic regression was used to analyze the data.

    Results

    Trends in fruit consumption developed differently across countries, characterized by an increase in Denmark and Norway and more stable trends in Sweden and Finland. Vegetable consumption increased particularly in Denmark and to a lesser extent in Norway, whereas Sweden and Finland displayed stable trends. Decreased trends were observed for sweet and soft drink consumption and were similar in Norway, Sweden and Finland. Sweet consumption decreased across all survey years, whereas soft drink consumption decreased between 2001/2002–2005/2006 and was stable thereafter. Denmark displayed an increase between 2001/2002–2005/2006 followed by a similar decrease between 2005/2006–2009/2010 for both sweet and soft drink consumption. Socioeconomic inequalities in fruit and vegetable consumption were observed in all countries, with no cross-country differences, and no changes over time. Small but not significant cross-country variation was identified for SES inequalities in sweet consumption. Reduced SES inequalities were observed in Sweden between 2005/2006 and 2009/2010. SES was not associated with soft drink consumption in this study population, with the exception of Denmark for the survey year 2009/2010.

    Conclusion

    Different trends resulted in increased country differences in food habits during the time of observations. In survey year 2009/2010, Danish students reported a higher intake of fruit and vegetable consumption than their counterparts in the other Nordic countries. Finnish students reported the lowest frequency of sweets and soft drink consumption. Despite the positive dietary trends documented in the present study, the majority of Nordic adolescents are far from meeting national dietary recommendations. Our findings underline the need for more comprehensive initiatives targeting young people's food habits as well as a more deliberate and focused action to close gaps in social inequalities that affect food choices.

    Abstract [en]

    Aim: Reviewing the scientific literature 2006-2011 on infant massage.

    Background: The study is a follow-up of a literature review of studies on infant massage published 1995-2005, in which it was shown that infant massage has a favorable effect on weight gain, health, sleep and behavioral development in infants and on parental-infant interaction.

    Methods: A computer-aided search utilizing the Cochrane Central Register of Controlled Trials, Pubmed and CINAHL was conducted in January 2012, the MeSH term «infant massage», limited to clinical controlled trials (CCT) for infants aged 0-23 months, being employed.

    Findings: A total of 29 articles were found, concerning 1670 children altogether who satisfied the inclusion criteria, 18 of the studies concerning preterm infants. Results in five different areas are taken up: weight gain (n=8), analgesia (n=5), health (n=5), behavioral development (n=7) and depression prevention in mothers (4).

    Conclusions: No harmful effects of infant massage were reported. It was found to have positive effects on children's growth, health and behavioral development, and to have beneficial effects on the mothers' psychological well-being when they gave their children massage. It is concluded that parental education in infant massage is a cost-effective and health promoting measure.

This cross-sectional study was conducted in Sweden on school children aged 6-16. A questionnaire was distributed to the children (n=3011 in survey I; n=204 in survey II). Children who slept less than the median length of time reported enjoying school to a lesser degree. Fewer hours of sleep were found to be associated with having a bedroom television using the television or computer more than 2 hours a day, being tired at school and having difficulties in sleeping and waking up. Overweight and obesity were found in 15.8% of the study population; obesity alone was found in 3.1%. Relationships between lifestyle factors and overweight were studied using multivariate logistic regression analysis. Having a bedroom television and using the television more than 2 hours per day were found to be associated with overweight but using the computer more than 2 hours a day was not. About 61% of the students aged 16 reported checking Facebook or social media at least once a day and 27% reported doing so for more than 10 times a day. One fourth of the students aged 16 had a habit of sending or receiving text messages at night at least once a week. Texting at night and frequent checking of Facebook and social media sites were related to sleep problems.

    Abstract [sv]

    Ett möte med experter på barns sömnproblem genomfördes i februari 2014 varvid detta kunskapsdokument togs fram. Kunskapssammanställningen och rekommendationerna har tagits fram gemensamt av experterna och bygger på konsensus bland deltagarna. Läkemedelsverkets ansvar var att facilitera mötet samt att tillsammans med experterna sammanställa kunskapsdokumentet. Som stöd för kunskapsdokumentet finns bakgrundsdokument som respektive författare ansvarar för. I bakgrundsdokumenten finns, förutom detaljerad information om varje ämnesområde, även referenser. Socialstyrelsen medverkade vid expertmötet, liksom i framtagandet av kunskapsdokumentet. Inför mötet har SBU (2013) identifierat kunskapsluckor inom området, vilka publicerats i SBU:s databas över vetenskapliga kunskapsluckor, se www.sbu.se. En vetenskaplig kunskapslucka innebär att systematiska litteraturöversikter saknas eller att de visar på osäker effekt. Man fann att detta gäller "Melatonin till barn som har flera funktionsnedsättningar och sömnproblem", "Melatonin som behandling av sömnproblem hos barn med adhd" och " Melatonin för i övrigt friska barn med sömnproblem". SBU konstaterade tidigare (2010) att "Effekten av alimemazin vid behandling av sömnbesvär" också är envetenskaplig  kunskapslucka.

    Abstract [en]

    Background: Supporting positive mental health development in adolescents is a major public health concern worldwide. Although several school-based programs aimed at preventing depression have been launched, it is crucial to evaluate these programs and to obtain feedback from participating adolescents. This study aimed to explore adolescents’ experiences with a School-based cognitive-behavioral depression prevention program. Methods: Eighty-nine adolescents aged 13–15 years were divided into 12 focus groups. The focus group interviews were analyzed using qualitative content analysis. Results: Three categories and eight subcategories were found to be related to the experience of the school-based program. The first category, intrapersonal strategies, consisted of the subcategories of directed thinking, improved self-confidence, stress management, and positive activities. The second category, interpersonal awareness, consisted of the subcategories of trusting the group and considering others. The third category, structural constraints, consisted of the subcategories of negative framing and emphasis on performance. Conclusions: The school-based mental health program was perceived as beneficial and meaningful on both individual and group levels, but students expressed a desire for a more health-promoting approach.

    Aim: The aim of this study was to test the psychometric properties of the Salutogenic Health Indicator Scale (SHIS) in an adolescent population. Methods: The investigation was performed among Swedish students aged 13–15 years (n = 817; 58% girls). The SHIS was assessed for respondent acceptability, and its psychometric properties were evaluated according to classical test theory (regarding unidimensionality, targeting, reliability, and external construct validity). Results: The adolescents found it easy to complete the questionnaire, which was completed in an average of 4 minutes. Exploratory factor analysis, which is based on polychoric correlations, identified one factor, supporting the instrument’s unidimensionality. Floor/ceiling effects were ⩽ 3.3%. Reliability estimates yielded a Cronbach’s alpha value of 0.93; the test–retest reliability (n = 50; 2-week interval) coefficients were 0.89 for the total SHIS score and 0.52–0.79 for item scores. Spearman correlations with other variables were based on a priori expectations (self-rated general health, 0.595; depressive symptoms, −0.773; anxiety, −0.577; and sleep problems, 0.519). Conclusions: Our observations support both the acceptability and the psychometric properties of the SHIS as a brief, unidimensional assessment tool for salutogenic health in adolescents. Further studies using modern test theory are needed to better understand the measurement properties of the SHIS, including the functioning of its response categories and its comparability between adolescents and adults.

    Background: Supporting positive mental health development in adolescents is an important school health concern, but there is a need to investigate the suitability of the interventions used.

    Aim: The aim is to investigate the experiences of school health professionals in conducting a universal school-based programme aimed at preventing depressive symptoms in adolescents.

    Methods: Twenty-two school health professionals participated in four focus groups. The interviews were analysed using qualitative content analysis.

    Findings: The overall theme identified in the study consists of 'striking a balance between strictly following the manual and meeting the students' needs'. Three subthemes emerged: 'doing good and sowing seeds for the future', working with insufficient tools, and 'personal development as a professional and as an individual'.

    Conclusions: School health professionals conducting the programme found it valuable in a school setting, but considered support from the school administration essential.

    Skolan är en värdefull arena för hälsofrämjande arbete enligt såväl gällande skollag som vägledningsdokument för elevhälsan. På många skolor har livskunskap införts på schemat som ett led i det hälsofrämjande arbetet. De senaste decenniernas rapporter om ökande självrapporterad psykisk ohälsa har även lett till lansering av ett flertal manualbaserade (standardiserade) program med syfte att minska utåtagerande beteende, eller för att förebygga stress, depressiva symtom och ångest. Din Inre Styrka Aktiveras (DISA) är ett skolbaserat program som används frekvent på svenska högstadieskolor, framför allt för flickor, men även för pojkar. En utvärdering av DISA är genomförd i form av en doktorsavhandling (Garmy, 2016). I detta kapitel presenteras en sammanfattningav denna undersökning.

    Abstract [en]

    AIM: The aim of this study was to investigate the feasibility and cost-utility of a school-based cognitive-behavioral (CB) depression prevention program.

    METHODS: A quasi-experimental trial with an intervention group and a control group, with follow-up measurements obtained at three and 12 months after baseline, was conducted. The setting was six Swedish municipalities. The participants were students in grade 8 (median age: 14). A total of 462 students (79% girls) were allocated to the school-based CB prevention program, and 486 students (46% girls) were allocated to the control group. The school-based CB prevention program, Depression in Swedish Adolescents (DISA), was presented by school health service staff and teachers once per week for 10 weeks.

    RESULTS: The main outcome measures were self-reported depressive symptoms and self-rated health; the secondary outcome measures were adherence and cost-utility. The intervention group decreased their self-reported depressive symptoms (as measured by the Center for Epidemiological Studies Depression Scale) and improved their self-rated health (as measured by the visual analog scale) at the 12-month follow-up more than the control group ( p < .05).

    CONCLUSIONS: Given the challenges of conducting a study in a complex, everyday school setting with baseline differences between the intervention and control group, it is difficult to make accurate interpretations of the effectiveness of the intervention. However, with these limitations in mind, the results indicate that the DISA program is a feasible school-based prevention program.

    Abstract [en]

    The aim of this cross-sectional study was to investigate the prevalence of overweight and obesity in children and adolescents (6-16 years), and relationships between being overweight and sleep, experiencing of fatigue, enjoyment of school, and time spent in watching television and in sitting at the computer. Trained school nurses measured the weight and height of 2891 children aged 6, 7, 10, 14, and 16, and distributed a questionnaire to them regarding television and computer habits, sleep, and enjoyment of school. Overweight, obesity included, was present in 16.1% of the study population. Relationships between lifestyle factors and overweight were studied using multivariate logistic regression analysis. Having a bedroom television and spending more than 2 h a day watching television were found to be associated with overweight (OR 1.26 and 1.55 respectively). No association was found between overweight and time spent at the computer, short sleep duration, enjoyment of school, tiredness at school, or difficulties in sleeping and waking up. It is recommended that the school health service discuss with pupils their media habits so as to promote their maintaining a healthy lifestyle.

    Abstract [en]

    PURPOSE: This study investigated sleep, television, computer habits, and obesity in school-age children.

    DESIGN AND METHODS: This was a cross-sectional self-report survey of 1260 children in grade 4 (mean age, 10.1) living in southern Sweden (49.1% boys). The heights and weights of 1097 (87.1%) of the children were recorded. Descriptive statistics, bivariate analyses, and multiple logistic regression were employed.

    RESULTS: The median length of self-reported sleep on weeknights was 9.5h. Approximately 40% of the children reported receiving <9h of sleep. The median bedtime was 9PM (21:00). On weekends, the median bedtime was 1 h later, and they delayed getting up by 1.5h. The median time spent watching TV and using a computer was 1 h each. The prevalence of being overweight (including obesity) was 18%. Insufficient sleep (<9h) was associated with being overweight, watching TV, or using a computer for two or more hours each day, difficulty falling asleep, and being tired at school.

    CONCLUSIONS: School-age children who receive less sleep are more likely to be overweight and report excessive television and computer use. A strong and urgent need exists to highlight the importance of healthy sleep and media habits. It is challenging for pediatric nurses and school nurses to teach children and their families about healthy sleep and media habits.

    Att förebygga depressiva symtom hos ungdomar: en pilotstudie2014Konferensbidrag (Övrigt vetenskapligt)

    Bakgrund: Att främja välmående och psykisk hälsa hos barn och ungdomar är en viktig uppgift för samhället. Psykisk ohälsa, t ex depression och ångest, innebär en försämrad livskvalitet som kan försvåra barnets kognitiva, sociala och känslomässiga funktioner. Att främja en positiv utveckling och förebygga barns och ungas psykiska ohälsa innebär både ett minskat individuellt lidande och minskade samhällskostnader. Förutsättningen är dock att förebyggande åtgärder är effektiva ur såväl hälsomässiga som ekonomiska perspektiv.

    Syfte: Att utvärdera det skolbaserade programmet Disa som syftar till att förebygga depressiva symptom hos ungdomar.

    Metod:

    Studien, som har en kvasiexperimentell design, inkluderade pre-test, post-test och ett-års-uppföljning samt en illustrativ beräkning av interventionens implementeringskostnader. I studien deltog 62 elever i årskurs 8, samt 7 Disa-ledare.

     

    Resultat: En majoritet av eleverna och samtliga ledare var nöjda med interventionen. Eleverna, både flickorna och pojkarna, skattade sina depressiva symtom signifikant lägre efter kursen, och för flickorna bestod denna effekt vid ett-års-uppföljningen. Implementeringskostnaderna för de första två åren var cirka 2000 kr per elev.

    Slutsats: Positiva resultat av det skolbaserade programmet Disa påvisades, särskilt för flickor. Flera elever tyckte att de fått ett nytt sätt att tänka, och att stämningen förbättrades i klassen.

    The aim of this pilot study was to evaluate the implementation of a universal school-based cognitive behavioral program whose target is to prevent depressive symptoms in adolescents. The study had a quasi-experimental design with pretest, posttest, and a 1-year follow-up and provides an illustrative calculation for the implementation costs of the intervention. Sixty-two students (aged 14) and seven tutors participated. A majority of the students and all of the tutors were satisfied with the intervention. The students, both females and males, rated their depressed symptoms as significantly lower after the course; and for the females, this was maintained 1-year postintervention. The implementation costs for the initial 2 years were about US$300 per student. Positive effects of a universal school-based cognitive behavioral intervention aiming at preventing depressive symptoms in adolescents were found, especially among females.

    Supporting positive mental health development in adolescents with a group cognitive intervention2015Konferensbidrag (Övrigt vetenskapligt)
    Abstract [en]

    Background: Supporting positive mental health development in adolescents is an important school health concern, but there is a need to investigate the suitability of the interventions used.

    Aim: The aim is to investigate the experiences of school health professionals in conducting a universal school-based programme aimed at preventing depressive symptoms in adolescents.

    Methods: Twenty-two school health professionals participated in four focus groups. The interviews were analysed using qualitative content analysis.

    Findings: The overall theme identified in the study consists of ‘striking a balance between strictly following the manual and meeting the students’ needs’. Three subthemes emerged: ‘doing good and sowing seeds for the future’, working with insufficient tools, and ‘personal development as a professional and as an individual’.

    Conclusions: School health professionals conducting the programme found it valuable in a school setting, but considered support from the school administration essential.

    PURPOSE: We describe the frequency and variations in bullying among a representative national sample of school-age children and examine whether sociodemographic characteristics are associated with bullying.

    DESIGN AND METHODS: This study is based on a cross-sectional school-based survey-the Icelandic contribution to the international research network Health Behaviour in School-aged Children (HBSC). The study population included all students in Iceland in grades 6, 8 and 10 (mean ages: 11, 13 and 15years, respectively) (participation rate: 84%; n=11,018). The students completed an anonymous standardized questionnaire administered in the classroom.

    RESULTS: The self-reported frequency of being victimized by bullying at least 2-3 times every month was 5.5%. A younger age, speaking a foreign language at home, not living with one's parents, and living in a rural area, were all associated with higher frequencies of being bullied.

    CONCLUSIONS: Despite efforts to reduce bullying in school, experiences of being victimized through bullying are still too common among Icelandic school-age children. Stakeholders and school health administrators should consider sociodemographic antecedents when planning interventions to reduce bullying at school.

    The aim of this study was to examine sleep habits (i.e., bedtimes and rising times) and their association with nighttime text messaging in 15- to 17-year-old adolescents. This cross-sectional study analyzed data from a web-based survey of adolescent students attending secondary schools in southern Sweden ( N = 278, 50% female). Less than 8 hr of time in bed during school nights was significantly associated with more sleep difficulties, wake time variability on school days and weekends, daytime tiredness, and less enjoyment at school (all ps < .05). Sending and/or receiving text messages (Short Message Service [SMS]) at night was significantly associated with later bedtimes, shorter time in bed, daytime tiredness during school, and irregular sleep habits ( p < .05). These findings highlight the importance of regular, consistent sleep habits and the problems associated with sleeping with a cell phone in the bedroom.

    Abstract [en]

    BACKGROUND: Research on the relationships between adolescent and parental disordered eating (DE) and emotion dysregulation is scarce. Thus, the aim of this study was to explore whether mothers' and fathers' own DE, as measured by SCOFF questionnaire, and emotion dysregulation, as measured by the difficulties in emotion regulation scale (DERS), were associated with their daughters' or sons' DE and emotion dysregulation. Furthermore, the importance of shared family meals and possible parent-related predictors of adolescent DE were explored.

    METHOD: The total sample comprised 1,265 adolescents (M age  = 16.19, SD = 1.21; age range 13.5-19 years, 54.5% female) whose parents had received a self-report questionnaire via mail. Of these, 235 adolescents (18.6% of the total sample) whose parents completed the questionnaire were used in the analyses. Parents' responses were matched and compared with those of their child.

    RESULTS: Adolescent girls showed greater levels of DE overall than did their parents. Furthermore, DE was associated with emotion dysregulation among both adolescents and parents. Adolescent and parental emotion dysregulation was associated, although there were gender differences in the specifics of this relationship. The frequency of shared dinner meals was the only variable that was associated to DE and emotion dysregulation among adolescents, while parental eating disorder was the only variable that enhanced the probability of adolescent DE.

    CONCLUSION: The present study contributes to the literature by demonstrating that there are significant associations between parents and their adolescent children in terms of DE, emotion dysregulation, and shared family meals. Future studies should break down these relationships among mothers, fathers, girls, and boys to further clarify the specific associational, and possibly predictive, directions.

    The fundamental cause of weight-related problems, from obesity to anorexia, is an imbalance between calories consumed and calories expended. The "nutritional status" concept embraces more than food-intake. It elucidates the dynamics between supply, demand and factors that affect metabolism, energy balance and energy expenditure. In recent years, major changes in the spatial-temporal structures of everyday life that could be possible contributors to weight-related issues of adolescents have emerged. A modern life style of low activity, irregular meal times, late-night food intake, stress and sleep deprivation possibly leads to a disturbed regulation of food intake which further can generate physical and/or psychological illnesses. Traditional studies of eating behavior use food diaries focusing on the average intake of energy and nutrients (e.g. Bellisle et al., 2003). Such methods are demanding for the respondent and require details about consumed amounts. A "Meal Matrix" (Lennernas & Andersson, 1999) has been used in studies of several different Swedish cohorts (e.g. Wissing et al., 2000). The Meal Matrix consists of seven food categories and eight different meal "types". Categorization is based on visible properties (food types) but at the same time reflecting invisible properties (nutrients). In the present study the Meal Matrix was developed further to be used as part of a questionnaire in a study of eating behaviors in 1281 adolescents between 12 and 17 years old in a southern Swedish municipality. Meal patterns in adolescents were assessed in relation to time of day for intake, sleep and physical activity. The aim of the study was to 1) test a self-report concept for food based classification of eating behaviors in adolescents, 2) to evaluate the nutritional quality of food and meals among them and 3) to get information about the rhythm of eating and slee-ping in relation to time of day and the biological clock.

    Previous research has shown that about 30% of adolescent girls and 15% of adolescent boys suffer from disordered eating (DE) which can be defined as problematic eating below criteria for eating disorders according to DSM-V (Hautala et al., 2008; Herpertz-Dahlman et al., 2008). Even sub-clinical unhealthy weight-control behaviors have predicted outcomes related to obesity and eating disorders five years later (Neumark-Sztainer et al., 2006). However, two issues question the validity of DE. First, in contrast to eating disorders, under- or overweight/obesity are not necessary parts of DE. Second, some symptoms and correlates of DE are similar to those of depression. E.g., parent-adolescent relationships seem to play an important role in explaining both DE (Hautala et al., 2011; Berge et al., 2010) and internalizing problems (Soenens et al., 2012). Thus, this study examined associations between DE and a wide range of internalizing and externalizing problems, parent-adolescent relationship characteristics, and food intake and sleep habits in a general population of adolescents. Comparing results with and without controlling for depression reveals whether DE is a specific problem or merely a depressive symptom. This study also explored whether DE and the other variables under study are associated independently of weight status (underweight, overweight/obesity, and normal weight), specific to under- or overweight, or spurious if taking weight status into account.

    The study is based on the first wave of an on-going longitudinal study, and all measures are child-reported (N=1,281). Adolescents attending grades 7 to 10 in a Southern Swedish municipality (age 12.5 to 19.3, M = 15.2, SD = 1.2) filled out questionnaires in class.  DE was measured using the SCOFF, a five-item screening scale validated for use in general populations (e.g. Muro-Sans et al., 2008; Noma et al., 2006).

    The results of univariate ANOVAs indicate that associations with DE were largely independent of weight status. Moreover, most associations with disordered eating were spurious when controlling for depression. However, some associations remained. Above and beyond depression effects, adolescents with DE reported lower self-esteem, stronger feelings of being over-controlled by their parents and active withholding of information towards them, consumption of fewer meals during the week, and higher levels of daytime sleepiness. Boys with ED slept more hours during the week and ate more fruits and vegetables than boys without ED. In conclusion, despite an overlap between depressive symptoms and disordered eating, this study provides ample evidence that sleep, nutrition habits, self-esteem, and parental control issues distinguish eating disordered adolescents from those suffering from general depressive symptoms.

    Detta kapitel handlar om arbetet med att utforma riktlinjer för hur  man möter och ger stöd till föräldrar som fått ett barn med funktionsnedsätt- ning (Downs syndrom). Tidsmässigt rör det om den första tiden för föräldrarna, om vad som sägs till dem och vad som görs inom förloss- nings- och barnsjukvården. Hur reagerar föräldrarna i den för dem nya, oväntade, okända och sannolikt skrämmande situationen? Vad vill för- äldrarna ha och vad behöver de ha och inte minst vad ges de för initialt omhändertagande? De nya nationella medicinska riktlinjerna har för första gången fått ett eget avsnitt som handlar om just detta första om- händertagande. Avsnittet har sin utgångspunkt i både beprövad erfaren- het och empirisk forskning. Vårdprogrammet presenterades 21 mars 2013.

    Detta arbete kan också ses som ett exempel på hur man kan ”fånga upp” ett vårdproblem från empirin, processa det genom  forskningsprocessen och hur man sedan implementera resultatet åter till empirin.

    Abstract [en]

    Parents of children with a mild intellectual disability experience more distress and require more support than other parents. The aim was to investigate the perceived family needs of parents of children with an MID and to investigate the relationship between parents’ perceived self-efficacy in their parental role and in collaborating with professionals as well as with their perceived needs for support. Interviews were based on questionnaires to the parents of 38 children. The results revealed that parents perceived need for information, respite, and venues in which to meet other parents in similar situations. The informational needs were related to parental self-efficacy and obtaining support. A lower need for information was related to higher perceived control over services. In conclusion, it appears that professionals need to work to strengthen parents’ ability to ask for support and to express the needs. Well-informed parents will develop stronger parental self-efficacy and perceived control over services.
