    Kvinnors upplevelse av att leva med diabetes typ 22013Rapport (Annet vitenskapelig)
    Abstract [sv]

    Bakgrund: Diabetes typ 2 är en kronisksjukdom som ökar på grund av förändrade kostvanor och minskad fysisk aktivitet. Det är en sjukdom som ställer stora krav på individen eftersom det är viktigt med välfungerande egenvård för att må bra. Att drabbas av en kronisk sjukdom upplevs oftast negativt samtidigt som det är viktigt att få ett namn på sjukdo- men. Det finns studier som tyder på att kvinnor verkar må psykiskt sämre och har sämre reglerad diabetes än män. För att kunna stödja kvinnorna i egenvården behövs kunskap om hur de upplever det att leva med diabetes. Syfte: Undersöka kvinnors upplevese av att leva med typ 2 diabetes. Metod: Litteraturstudie som bygger på nio kvalitativa artiklar. Resultat: Fyra teman hittades; besvikelse och svek, kluvenhet, skuld och skam samt oro för framtiden. Diskussion: Det är svårt att vara kvinna och leva med diabetes typ 2. Sjukdomen kräver daglig egenvård och kombinerat med andra roller som kvinnan har i sitt dagliga liv kan kraven upplevas som stressande för en del av kvinnorna. Många kvinnor med diabetes typ två upplevde att de var i behov av bättre stöd och information för att kunna leva med sin sjukdom. Slutsats: Vårdpersonal behöver mer utbildning och ha med ett genusperspektiv för att förstå kvinnornas behov. Diabetesutbildningen behöver utveckas till att omfatta hela familjen, men samtidigt måste den vara individanpassad.

    Abstract [en]

    How can discharge planning (DP) for patients who require care in the home following a period in hospital be understood and developed through the lenses of complexity theory? With the help of complexity theory and practice-based narrative research this study discloses the formal routines and complex dynamic practices that are associated with DP. A study of the literature established that there was an almost total absence of complexity-theoretical perspectives on interpreting and developing DP.                 

    The researchers collected narratives about the DP processes using qualitative interviews with the nurses responsible for this in a hospital ward: these were audio-recorded and transcribed verbatim. They also participated in and documented meetings where these nurses, as a group, discussed DP.                 

    The findings show that nurses have to continuously deal with uncertainty, surprises and the unknown. They have to make sense and take charge of dynamic complex events and new knowledge, and manage complex relations and information. The researchers argue that looking upon practice from the lenses of complexity theory, and therefore accepting the complexity of practice, could facilitate the development of nurses' skills in order to guarantee good quality in DP.

    An intervention applying a palliative care approach in residential care: influences on nurse assistants' work situation2013Konferansepaper (Annet vitenskapelig)
    Abstract [en]

    Introduction

    Palliative care is an approach characterized by a holistic view of the person, which supports the individual for living with dignity and with the best possible level of wellbeing until the end of life. Those who, in practice, provide care in Swedish residential care facilities for older people (RCFs) are nurse assistants (NAs), a work group with limited education. They also often stand alone with difficult decisions with regards to the care provision, as the registered nurse has the role of consultant rather than leader. In an explorative qualitative study we found that NAs experienced that it was difficult to focus on relationship aspects in their work, as the main discourse in the residential care focused on tasks. Palliative care was described as something that was applied only during a short define phase i.e. last days of the residents’ life. These results were used as a basis for development of an intervention focus on a palliative care approach in RCFs. The intervention consisted of study circles with NAs, and workshops together with their leaders, focusing on improvement work.

    Aim

    To investigate how an intervention focusing on a palliative care approach in RCFs influenced the NAs and their work.

    Method

    Two quantitative studies, based on a questionnaire, including several measurements, that was answered pre-, and post- intervention and one qualitative study based on individual interviews after the intervention. A total of 75 nurse assistants participated in the intervention and answered the questionnaire at baseline and at two follow-ups, in comparison with 110 nurse assistants who served as controls. 

    Results

    The evaluation of the intervention showed that the NAs, after the intervention, had increased their focus on the residents’ situation and to a greater extent stated that they focused on the residents’ life stories and on aspects that brought meaning to their lives. The NAs also stated that they experienced less criticism from their superiors as well as from the residents after the intervention. However, the evaluation also showed that the NAs had a more negative view of the leadership, were more critical to the medical and the nursing care, and that their job satisfaction had decreased after the intervention. Interviews after the intervention showed that they, as a result of the intervention, had not only gained increased insight into their own significance in their encounter with residents and their relatives, but also an increased awareness of the needs of the residents and their relatives. The intervention also contributed to an increased openness in the workgroup. However, the nurse assistants also expressed frustration over barriers, primarily in the form of a lack of resources and limited leadership, standing in the way of the implementation of changes.

    Conclusion

    The results indicate that the NAs, through discussions and reflections over praxis in their ordinary work group, developed an increased awareness about, and focus on relationship aspects. However, in spite of the leaders’ involvement, the intervention was not sufficient for changing the organizational prerequisites for working in line with the palliative care approach. When implementing a palliative care approach in RCFs, more focus on support to the leaders in needed in order to maintain sustainable changes.

    BACKGROUND: Palliative care should be provided, irrespective of setting to all patients facing a life-threatening illness and to their families. The situation and needs of older people differ from those of younger people since they often have several co-existing diseases and health complaints. This implies an extensive need for care and for longer periods of palliative care. The main providers of palliative care for older people are nurse assistants, who are also those with the shortest education. AIM: The aim of this study was to illuminate nurse assistants' experience of palliative care for older people in residential care. DESIGN: The study had an explorative, descriptive design. SETTINGS: Thirteen residential care units in three different districts in a large city in southern Sweden. PARTICIPANTS: Twenty-five nurse assistants selected to represent variations in age, gender workplace and work experience. METHODS: Data were collected from six focus-group interviews and subjected to content analysis to gain an understanding of the phenomenon. RESULTS: The nurse assistants described palliative care as a contrast to the everyday care they performed in that they had a legitimate possibility to provide the care needed and a clear assignment in relation to relatives. Palliative care also meant having to face death and dying while feeling simultaneous that it was unnatural to talk about death and having to deal with their own emotions. They emphasised that they were in need of support and experienced leadership as invisible and opaque, but gained strength from being recognized. CONCLUSION: In order to support nurse assistants in providing high quality end-of-life care, more focus is needed on the trajectory of older peoples' dying, on the importance of involving relatives throughout the period of care provision, and on support when encountering death and dying. There is also a need for engaged care leaders, both registered nurses and managers, to recognize the work of nurse assistants and to support care provision for older people within the framework of palliative care philosophy.

    This exploratory study compares orthopaedic nurses’ perceptions of individualised nursing care with previously published orthopaedic patients’ perceptions. Orthopaedic nurses (N = 243) from one university, two central and two county hospitals working within in-patient care were surveyed using the Individualised Care Scale-Nurse (ICS-Nurse) in 2009 (response rate 74%, n = 180). The data were analysed using both descriptive and inferential statistics. About 60% of the nurses stated that it was very important that the care provided is individualised in comparison with 86% of the patients as previously reported (p-value <0.001). The highest rated assessment of individualised care was the clinical situation and the lowest the personal life situation which is in line with the patients’ experiences. This result demonstrates the need of managers in healthcare organisations to redouble their efforts in the implementation of individualised care by investigating nurses’ contemporary beliefs about, and forces that hinder the provision of individualised nursing care.

    The purpose of the study was to explore the encounters with the health care system in Sweden of women from Somalia, Eritrea, and Sudan who have been genitally cut. A qualitative study was performed through interviews with 22 women originally from Somalia, Sudan, and Eritrea who were living in Sweden. The women experienced being different and vulnerable, suffering from being abandoned and mutilated, and they felt exposed in the encounter with the Swedish health care personnel and tried to adapt to a new cultural context. The results of this study indicate a need for more individualized, culturally adjusted care and support and a need for systematic education about female genital cutting for Swedish health care workers.

    Female Genital Mutilation (FGM) or the more value neutral term, Female Genital Cutting (FGC) is widely practised in northern Sudan, where around 90% of women undergo the most extensive form of FGC, infibulation. One new approach to combating FGC in Sudan is to acknowledge the previously hidden form of FGC, reinfibulation (RI) after delivery, when the woman is sewn back so much as to mimic virginity. Based on a qualitative study in Khartoum State, this article explores Sudanese women's and men's perceptions and experiences of FGC with emphasis on RI after delivery. The results showed that both genders blame each other for the continuation of the practices, and the comprehensive understanding of the perceptions and experiences was that both the women and the men in this study were victims of th e consequences of FGC and RI. The female narratives could be understood in the three categories: viewing oneself as being "normal" in having undergone FGC and RI; being caught between different perspectives; and having limited influence on the practices of FGC and RI. The male narratives could be understood in the three categories: suffering from the consequences of FGC and RI, trying to counterbalance the negative sexual effects of FGC and striving in vain to change female traditions. The results indicate that the complexity of the persistence of FGC and RI goes far beyond being explained by subconscious patriarchal and maternalistic actions, related to socially constructed concepts of normality, female identity,tradition and religion a"silent" culture betweenmen and women.

    Denna bok och webb är den efterfrågade och fristående fortsättningen på Omvårdnadens grunder. Redan före sin publicering har Omvårdnad på avancerad nivå varit känd för många under det lekfulla arbetsnamnet Omvårdnadens höjder. Innehållet är två sammanflätade delar: en tryckt bokdel och en digital webbdel. Den senare är helt unik i sitt slag. Ett åttiotal forskande och/eller kliniskt verksamma sjuksköterskor har skapat en digital artikelsamling som beskriver tillämpningen av avancerad omvårdnad inom respektive specialistområde. Webbmaterialet innehåller även sammanfattande ljudklipp, frågor samt den tryckta boken i sin helhet så att den kan läsas på exempelvis iPad. Detta är det första bokverket i Sverige som beskriver huvudområdet omvårdnad på avancerad nivå. Utgångspunkten är de sex kärnkompetenser som är gemensamma för alla professioner inom hälso- och sjukvården och som Svensk sjuksköterskeförening rekommenderar bör utgöra en röd tråd med progression genom utbildningarnas nivåer:

    • Samverkan i team
    • Evidensbaserad vård
    • Förbättringskunskap för kvalitetsutveckling
    • Säker vård 
    • Informations- och kommunikationsteknologi

    Tillsammans bildar bok och webb ett gemensamt och mångfaldigt innehåll för specialistsjuksköterskeutbildningarna. Avsikten är att skapa en grund för dessa utbildningar och deras motsvarigheter där omvårdnad på avancerad nivå kan beskrivas och problematiseras.     Personer som bor och vårdas på särskilt boende (säbo) har omfattande omvårdnadsbehov och ofta nedsatt kognitiv funktion, de är ofta multisjuka och har många olika läkemedel samtidigt.

    Om en god vårdkvalitet ska kunna garanteras, måste kompetensen bland vårdpersonalen höjas, framför allt avseende specifik geriatrisk kompetens.

    En betydande andel av de äldre som bor och vårdas på säbo avlider också där, trots att förutsättningarna för att ge en god palliativ vård vid livets slut är begränsade.

    I sjuksköterskeutbildningen vid Högskolan Kristianstad har sedan 2008 använts en modell för handledning och examination av examensarbetet med fokus på studenternas lärande. Trots att handledningsmodellen varit i bruk under några år visade genomförda kursutvärderingar att studenter uppfattade att det fanns skilda uppfattningar hos handledare och examinatorer om hur examensarbetet ska genomföras. På grund av detta ansöktes medel från Lärande Resurs Centrum (LRC) och Sektionen för Hälsa och Samhälle för att i kollegiet skapa möjligheter att diskutera vilka pedagogiska möjligheter och svårigheter som finns samt hur likheter och olikheter i syn på kunskap kan främja såväl som hindra studenternas lärandeprocess under examensarbetet. I denna rapport beskrivs projektets upplägg och genomförande samt centrala aspekter från den dialog som fördes i kollegiet under projektets gång.

    Abstract [en]

    Purpose: Collaboration has become an increasingly more common strategy when developing care sectors while, coincidentally, experience and research show that collaboration development may be problematic in itself. This study aims to achieve better understanding of collaboration processes.

    Method: A local project that aimed at improving the quality of healthcare and social care by developing the co-operation between organizations took place in Sweden, and by using participatory action research, PAR, this process was followed between 2004 and 2008. Material was gathered through participant observation from the perspective of patients, relatives, staff, managers and politicians. A descriptive narrative was compiled and a hermeneutic interpretation was performed.

    Results: Initially, the development of collaboration was impeded due to lack of communication between the participants from various levels within the organizations. With the support of PAR, communication arenas were arranged to handle social interaction as well as different perspectives and conflicts, which led to improved collaboration within the organizations as well as between the care organizations.

    Conclusion: Development of collaboration between organizations reflects how collaboration within one organization works. Collaboration is a social and interpersonal phenomenon, and readily available communication arenas are crucial for its development.

    Abstract [en]

    Purpose: To examine independence in activities of daily living (ADL) at the 1- and 2-year follow-ups of the health-promoting study Elderly Persons in the Risk Zone.

    Design and Method: A randomized, three-armed, single-blind, and controlled study. A representative sample of 459 independent and community-dwelling older adults, 80 years and older, were included. A preventive home visit was compared with four weekly multiprofessional senior group meetings including a follow-up home visit.

    Results: Analysis showed a significant difference in favor of the senior meetings in postponing dependence in ADL at the 1-year follow-up (odds ratio [OR] = 1.92, 95% confidence interval [CI] = 1.19–3.10) and also in reducing dependence in three (OR = 0.52, 95% CI = 0.31–0.86) and four or more ADL (OR = 0.40, 95% CI = 0.22–0.72) at the 2-year follow-up. A preventive home visit reduced dependence in two (OR = 0.40, 95% CI = 0.24–0.68) and three or more ADL (OR = 0.37, 95% CI = 0.17–0.80) after 1 year.

    Implications: A long-term evaluation of Elderly Persons in the Risk Zone showed that both senior meetings and a preventive home visit reduced the extent of dependence in ADL after 1 year. The senior meetings were superior to a preventive home visit since additional significant effects were seen after 2 years. To further enhance the long-term effects of the senior meetings and support the process of self-change in health behavior, it is suggested that booster sessions might be a good way of reinforcing the intervention.

    Aktionsforskning innebär ett möte mellan forskarens och praktikerns kunskapsfält. Det är genom deras samverkan som aktionsforskning sker. Praktiker kan sakna möjligheter och metoder för att synliggöra vardagsarbetet, medan forskare kan sakna att ingå i ett arbetslag och vara en del av en verksamhet. Aktionsforskning förenar praktik med teori och är ett användbart redskap i arbetet med att förändra och förbättra en verksamhet. 

    I den här boken ges flera exempel på hur aktionsforskning har använts inom områden som vård och omsorg, t.ex. på en familjecentral eller vid matdistribution till äldre. Författarna ger en teoretisk introduktion till aktionsforskning och visar hur forskningsresultat kan implementeras i den dagliga verksamheten. Boken innehåller också reflektionsfrågor som utmanar läsaren att uppmärksamma både sitt eget arbete och verksamheten i stort och på så sätt hitta möjligheter till förbättring.

    Aktionsforskning i vård och omsorg. Tillämpning och teori vänder sig till studerande på avancerad nivå och till yrkesverksamma inom vård, omvårdnad, socialt arbete och social omsorg.

    Prostate cancer is the most common cancer among men in Sweden, and treatment is negatively affecting the patients' quality of life. Even so, long term experiences are sparse and implications for nursing practice are little known. The aim of this study was to determine areas of functioning and factors impacting quality of life, QOL, during and five years after radical prostatectomy (RP) using a quality of life questionnaire and a specific module for prostate cancer. A longitudinal study was performed with consecutively included Swedish men from baseline and after RP treatment (n=222) from 2003 to 2011 to obtain their opinions on quality of life. Data was gathered through a mail out - mail in procedure at baseline, 3 months, 1-3 and 5 years after treatment with a response rate of 94.14% - 75.2%. One reminder was sent on each occasion. Identified areas with increased functioning after five years were emotional and social functioning. QOL ratings did not change over the years. Sexual activity and functioning decreased and hormonal treatment-related symptoms increased. Impact on QOL was found regarding emotional and social functioning, nausea/vomiting, pain and hormone-related symptoms. Increasing age, living with a partner and educational level had no significant impact on QOL. Implications for nursing are to initially focus on physical problems and at times for follow-up visits pay attention also to emotional and social aspects of life. To be able to make a difference in the patient's life, nurses need to bridge the gap between in-hospital treatment and everyday life outside hospital.

    Abstract [en]

    Rationale, aims and objectives  Studies have challenged the validity and underlying measurement model of the physical and mental component summary scores of the 36-item Short-Form Health Survey in, for example the elderly and people with neurological disorders. However, it is unclear to what extent these observations translate to physical and mental component summary scores derived from the 12-item short form (SF-12) of the 36-item Short-Form Health Survey. This study evaluated the construct validity of the SF-12 in elderly people and people with Parkinson's disease (PD) and stroke.

    Methods  SF-12 data from a general elderly (aged 75+) population (n = 4278), people with PD (n = 159) and stroke survivors (n = 89) were analysed regarding data quality, reliability (coefficient alpha) and internal construct validity. The latter was assessed through item-total correlations, exploratory and confirmatory factor analyses.

    Results  Completeness of data was high (93–98.8%) and reliability was acceptable (0.78–0.85). Item-total correlations argued against the suggested items-to-summary scores structure in all three samples. Exploratory factor analyses failed to support a two-dimensional item structure among elderly and stroke survivors, and cross-loadings of items were seen in all three samples. Confirmatory factor analyses showed lack of fit between empirical data and the proposed items-to-summary measures structure in all samples.

    Conclusions  These observations challenge the validity and interpretability of SF-12 scores among the elderly, people with PD and stroke survivors. The standard orthogonally weighted SF-12 scoring algorithm is cautioned against. Instead, when the assumed two-dimensional structure is supported in the data, oblique scoring algorithms appear preferable. Failure to consider basic scoring assumptions may yield misleading results.

    This study explores the perceptions of a small group of nurses working at a newly established 24-hour community-based service enter (SC) for users with psychiatric disability using a qualitative approach. Since the mental health reform in Sweden in 1995 where the communities (in Sweden called municipalities) were given the responsibility to establish service and support to people with severe psychiatric disabilities, they have struggled in finding suitable forms of these kinds of areas. In 2010, this led to the creation and development of a new center aiming to provide services and support based on the expressed needs of people with physchiatric disability in a community located in southern Sweden. During 2011, a total of three group interviews were performed to capture the employed nurses’ perceptions of this newly established SC. The interview texts were analyzed by way of qualitative content analysis. A first reading of the interview texts revealed that the nurses’ perceptions of the service center were unwaveringly positive but that their beliefs about who the specific target group were differed. The main finding was summarized by the theme: Making a difference—on an individual, professional, and organizational level. The sub themes were: 24-hour availability, unclear assignment, and preventing mental illness. The findings indicate a need for a community round-the-clock service center in this Swedish community and a more clear definition of the target group.

    Background: Older persons with multimorbidity being cared for at home often have complex needs which can´t be met by one player. Interprofessional collaboration is therefore considered necessary if care is to be organised according to the needs of the elderly. To achieve coherent healthcare, municipalities and regions need to develop this area.

    Aim: The aim of the study was to illustrate how various professionals belonging to homemaker services, home care services in municipality and Hospital-Based Home Care Services experience their collaboration in caring for older persons with multimorbidity.

    Method: Eleven informants took part in the study and individual interviewed. The material was analysed using a hermeneutic data analysis.

    Result: The result shows that collaboration between players contains various types of experiences which influence not only the staff who are involved in collaboration but also the result of the collaboration itself. The informants´ experience of collaboration was defined by trust and distrust and by security and insecurity. These influenced both the staff who were involved in collaboration and the result of collaboration itself.

    Conlusion: Complex situations could not be solved with simple models. Instead a flexible approach appears necessary with focus shifted from structures to interpersonal relations and interactions. Therefor the different professions have to work as a transprofessional team where close interactions, flexibility and improvisation is a key to success.   

    Relevance to clinical practice: A transprofessional approach to teamwork collaboration can blur professional boundaries and take the competence of all staff into account when home health care to older people with multimorbidity is to be provided by multiple caregivers. This is suggested as an approach with potential to provide high quality and safe care to a vulnerable population.

    The aim of this study was to describe the experiences of undergoing a presymptomatic genetic test for the hereditary and fatal Huntington’s disease, using a case study approach. The study was based on 18 interviews with a young woman and her husband from the decision to undergo the test, to receiving the results and trying to adapt to them, which were analysed using a life history approach. The findings show that the process of undergoing a presymptomatic test involves several closely connected ethical and medical questions, such as the reason for the test, the consequences of the test results and how health-care services can be developed to support people in this situation.

    Aim: The aim of the study was to illuminate older immigrants’ experiences of moving to a residential care facility.

     

    Background: Increased immigration will lead to a future increase in the number of older immigrants in residential care. In order to adjust nursing care to their needs it is important to acquire knowledge of their situation.

     

    Methods: Semi-structured interviews with seven immigrants living in residential care in Sweden were analysed using a qualitative manifest content analysis.

     

    Findings: Older immigrants’ experience of everyday life in residential care facilities was: Trying to adjust to relocation to residential care, Feeling alienated by communication difficulties, and Trying to stay connected by keeping ties to the origin and past, with internal variation seen as subcategories.

     

    Conclusion: Older immigrants, who move into residential care are vulnerable and experience a major transition as they move from a familiar social and cultural environment into a place mainly designed for older Swedes. There is a great risk that they will become alienated and highly dependent on their relatives if not nursing care interventions with specific focus on their needs are developed.

    Abstract [en]

    Objective. The aim of this study was to investigate expectations on and satisfaction with treatment among patients referred for comprehensive treatment to specialist clinics in periodontology and to explore factors associated with satisfaction in regression analysis.

    Materials and methods. Patients referred for comprehensive periodontal treatment were sampled for the study. The study was based on a questionnaire in a before-and-after design. The first questionnaire was sent to the patients before their first appointment at the specialist clinic. The second questionnaire was sent after  6 months. Three questions were used to measure expectations and four questions to measure satisfaction. The first questionnaire was sent to 273 patients with a response rate of 31% and the second questionnaire was sent to 85 patients with a response rate of 73%. In non-response analysis, no difference between respondents and non-respondents were detected as to age and gender.

    Results. Many of the patients viewed it as important or very important to have healthy teeth (98%) and improved well-being (93%) after periodontal treatment. More than 50% of the patients were satisfied with the relation to the caregiver. When measuring the satisfaction in general, 42% indicated the highest score on the summarized Dental Visit Satisfaction Scale. Having confidence (p ≤ 0.001) and a good relation (p = 0.001) to the caregiver indicated higher satisfaction.

    Conclusion. Having a good relation to the caregiver and having confidence in the caregiver seems to indicate satisfied patients receiving periodontal treatment at periodontal clinics.

    Abstract [en]

    The aim of this study was to investigate changes in knowledge of periodontal disease among patients referred to periodontal specialist clinics. A further aim was to investigate the patients' self- perceived oral health before the treatment. Patients referred to five specialist clinics in periodontology for comprehensive periodontal treatment were consecutive sampled. The study was based on a questionnaire in a before and after design. The first questionnaire was sent to the patients before visiting the specialist clinic and the second was sent after six months. Four questions were analysed, two to measure knowledge about periodontitis and two to measure the patients self- perceived oral health. The first questionnaire was sent by post to 273 patients with a response rate of 31%. The second questionnaire was sent to 85 patients with a response rate of 73%. The results of the study showed a statistically significant improvement of correct answers on the knowledge questions after six months was found for scaling (p = 0.006), X-ray examination (p = 0.001) and increased space between the teeth (p = 0.001). The most frequent self-perceived trouble from the mouth was bleeding gum (70%) and sensitive teeth (51%). In conclusion knowledge of periodontitis improved after visiting the specialist clinic of periodontology. Many of the patients experienced some problems of the mouth.

    Abstract [en]

    This inquiry explored how a group of teachers experienced resources for their well-being, both at work and in their private lives. The findings indicate that caring, for others and for oneself, is central for teachers' well-being. Caring is manifested in being present in the moment, and in actions which promote the well-being of oneself and others. Implications from the findings suggest that both school administration and teacher education should pay special attention to the caring aspects of teaching, as they influence teachers' well-being and retention, as well as the pupils' learning. Health promotion interventions could benefit from these findings.

    Background  In workplace health promotion, a questionnaire could be of great use. Unfortunately, fatigue regarding answering questionnaires has recently become greater than before. An action research approach could be a possible way of increasing employee participation.

    Aim  This study reports an attempt to explore key aspects for participation in, and commitment to, a workplace health promotion questionnaire process.

    Method  The study was conducted at two wards in a Swedish hospital. Data was collected during an action research process. Data were analysed with regard to a framework of questions.

    Findings  The three key aspects for participation in, and commitment to, a workplace health promotion questionnaire process were: an applicable questionnaire, a meaningful questionnaire process and a continuous and sustainable questionnaire process. A structure is presented as practical advice to managers, describing how such a process could be established to be applicable, meaningful and sustainable.

    Conclusion  This study has identified key aspects and prerequisites for questionnaire processes. The prerequisites – share decision-making, involve a core group and follow a structure – are discussed and proposed for managers and workgroups to consider in further workplace health promotion questionnaire processes.

    Implications for nursing management  The key aspects and prerequisites presented could provide a stimulating standpoint or advice, useful for planning and accomplishing workplace questionnaire processes.

    Objective and methods: Time-use methodologies have been proposed to be established research techniques when exploring aspects of daily occupations. In this study, two graphs illustrating the time arrangement of occupations as they appear in a continuous sequence were used in order to encourage individuals to reflect on their everyday life. The aim was to investigate the usefulness of a time-geographical diary method (using illustrative graphs) in combination with stimulated-recall interviews, to facilitate reflections on how patterns of daily occupations change over time and the causes that lie behind these changes. The study had a qualitative design. The participants were two working, married mothers, i.e. individuals considered to have highly complex patterns of daily occupations. The data analysis was performed by using thematic content analysis. Results: The results showed that the stimulated-recall interviews, based on the graphs, facilitated new insights that came to light concerning the scope of the participants' daily life. The method enabled the participants to reflect on their patterns of daily occupations and become aware of changes relevant to explain the causes for engaging in occupations the way they did. Conclusions: The method thus seems useful in research and practice for occupational therapists working with individuals with a need to change lifestyle.

    There are many instruments assessing the wellbeing of staff, but far from all have been psychometrically investigated. When evaluating supportive interventions directed toward nurse assistants in residential care, valid and reliable instruments are needed in order to detect possible changes. The aim of the study was to investigate validity in terms of data quality, construct validity, convergent and divergent validity and reliability in terms of the internal consistency and stability of the Job Satisfaction Questionnaire, the Psychosocial Aspects of Job Satisfaction, the Strain in Dementia Care Scale (SDCS), and the Stress of Conscience Questionnaire (SCQ) in a residential care context. The psychometric properties of the instruments were investigated in terms of data quality, construct validity, convergent and divergent validity and reliability, including test-retest reliability, in a residential care context with a sample consisting of nurse assistants (n=114). The four instruments responded with different psychometric-related problems such as internal missing data, floor and ceiling effects, problems with construct validity and low test-retest reliability, especially when assessed on the item level. These problems were however reduced or disappeared completely when assessed for total and factor scores. From a psychometric perspective, the SDCS seemed to stand out as the best instrument. However, it should be modified in order to reduce floor effects on item level and thereby gain sensitivity. The Job Satisfaction Questionnaire seemed to have problems both with the construct validity and test-retest reliability. The final choice of instrument must, however, be made dependent on what one intends to measure.

    Background:While the work situation for nurse assistants in residential care is strenuous, they themselves often state that they are satisfied with their job. More knowledge is clearly needed of the interrelationship of variables associated with job satisfaction. This study aims to investigate job satisfaction and explore associated variables among nurse assistants working in residential care.

    Methods: A total of 225 respondents completed a questionnaire measuring general job satisfaction, satisfaction with nursing-care provision and measures concerning person-centered care, work climate, leadership, and health complaints. Job satisfaction was the outcome measure and comparisons were made among those reporting low, moderate, and high levels of job satisfaction; multiple regression analyses were used to explore associated variables.

    Results: The caring climate and personalized care provision were associated with general job satisfaction. High levels of satisfaction with nursing-care provision were also associated with the general work climate, organizational and environmental support, and leadership. Low job satisfaction was mainly associated with health complaints.

    Conclusions: Nurse assistants working in a positive work climate, caring climate, with a positive attitude to their leaders, who receive organizational and environmental support, provide person-centered care and experience a higher degree of job satisfaction. It seems essential, however, to include both general and context-specific measures when investigating job satisfaction in this field as they reveal different aspects of the nurse assistant's work situation.

    The purpose of the present study was to describe how one of 290 Swedish municipalities improved its FS service by terminating their earlier service of food distribution (FD) which was the delivery of ready cooked meals produced at a central kitchen in the community and introducing food preparation at the client’s home. The revised system is referred to as the new FS. This study was performed using an action research approach. The data was collected by individual- and group interviews and through participatory observations. The transcribed interview material was analysed using qualitative content analysis. The (n=30) subjects were recruited The findings of this study revealed that the new FS was experienced as being a good service, that the new municipal FS met individual needs in a better way and that the elderly recipients could participate more actively in planning and preparing their meals. In conclusion, it was found that having their food prepared at home was considered by many of the recipients to be synonymous with individually adjusted help. The results of the study could have implications for nursing, public administration and gerontology.

    Abstract [en]

    The overall aim of this study was to gain insight into municipal food distribution from several perspectives: that of the clients, mainly elderly people, and also from the perspectives of different professionals and decision makers. An action research approach was chosen, as the characteristics of this method were deemed to be correct in relation to the basic aim of the study. This thesis is based on four empirical studies. Study I aimed to explore various professionals’ experiences of involvement in food distribution (FD) in order to get a comprehensive understanding of the organization, responsibilities and roles. The results showed that FD is a fragmentary intervention where a comprehensive perspective and clear roles of responsibility are lacking. Study II aimed to gain insight and to describe the experiences of home-living elderly people who receive hot meals distributed by their municipality. The results showed that there were feelings of dependency, and loneliness among the elderly participants, however they also expressed gratitude for the opportunity to have their meals delivered to their home.  Study III aimed to describe how the professionals can identify needs for improvement and improve the municipal FD service to home living elderly people. The need to update and increase the FD recipient’s knowledge in nutrition by sending information letters was found to be an important area to focus on. Evaluation of letters resulted in the decision to continue preparing and distributing information letters to all FD recipients twice a year and to make the information letters assessable on the websites of the six municipalities and county councils involved. Study IV aimed to describe the decision maker’s views on FD and to report their suggestions for which areas reported from studies I-III should be taken into consideration as starting points to improve the municipal FD. The findings resulted in the decisions to monitor the elderly’s health and wellbeing while providing FD and to increase the professionals’ competence and to create a forum for inter-professional communication. In conclusion, this thesis illustrates that there is a necessity to identify the needs for general improvement of FD by involving the participants and to start out from their personal experiences and in their own context.

    The municipal Food distribution service (FD) to the elderly living at home is a part of the public social and care service in Sweden, The objective of this service is to ensure proper food intake for persons who are unable to do their own shopping, and prepare their own meals. The foremost reasons for the need of the FD service are in situations where there are illness related physical or psychological limitations.This means that the Swedish welfare system takes on the responsibility for its citizens when they have a legal social related need of care. Further, according to the Swedish social legislation, children or other relatives have no legal obligations to take care of their parents or elderly disabled relatives. This also means that the children or relatives of elderly people requiring social support have no legal right to be involved in the evaluation procedure of need assessment or the outcome of any social and care services granted by the Swedish social welfare system.

    The aim of the present study was to gain insight into how the relatives of elderly people living at home in Sweden experience the municipal service of ready-made meals distributed daily.

    The data was collected using in-depth interviews with relatives of elderly persons who use the municipal food distribution (FD) service (n=8). The transcribed interview material was analysed using the grounded theory method.

    The findings of this study revealed that the relatives of the municipal FD service recipients advocate for a food preparation service in the home of the recipient rather than the distribution of ready-made meals from a central kitchen. The results also revealed that the participating relatives felt frustrated by the legal limitations that make it impossible for them to influence the municipal FD service. The findings in this study also indicate that relatives should be considered as a resource in this matter and could actively participate, and have a positive influence on the quality of the municipal FD service for the elderly. The results of the study could have implications for nursing, public administration and gerontology.

    Utvärdering av den nya servicen matlagning i hemmet2013Rapport (Annet vitenskapelig)
    Abstract [en]

    Den nya matservicen, matlagning i hemmet, är en insats som riktar sig till en heterogen grupp äldre och som involverar ett stort antal personalgrupper. Vård- och omsorgsförvaltningen i Lunds kommun genomförde under år 2012 en förändring när det gäller matservice. Förändringen inriktades på att upphöra med matdistribution den 31 mars 2012 och införa insatsen matlagning i hemmet den 1 april 2012. Matlagning i hemmet definierades som en enklare form av matlagning med beräknad schablontid på ca 30 minuter. Intentionen med att införa matlagning i hemmet var att skapa en fungerande helhetslösning för brukare med stora vård- och omsorgsbehov, att öka valfriheten, öka kvaliteten på äldres måltider och att använda ekonomiska resurser på ett bättre sätt.

    Denna utvärdering är ett sätt att blicka tillbaka och kritiskt granska hur den nya matservicen har fallit ut och mvilka förbättringsmöjligheter som finns, i syfte att erbjuda matservice av hög kvalitet till kommunens invånare.
