    Background:

    Every woman giving birth has expectations. Identifying women’s expectations, wishes, needs and fears enable the health care provider(s) to work toward a common goal of safe and positive childbirth experience.

    Aim:

    The aim was to determine women’s preferences and needs during labor.

    Design:

    Analytic- cross sectional study.

    Setting:

    El Kasr- Aini, - Cairo- University Maternity Hospitals.

    Sample:

    A random sample of 400 women were recruited in the present study with the following criteria; age ranged between 20-30 years old, can read and write, primigravida, nulliparous woman with singleton low risk pregnancies, in the third trimester, up to 37 weeks of gestational age, no previous abortion and free from any medical complains.

    Procedure:

    Data collection took place in the antenatal clinic, each interview administrated questionnaire took a time between 10-15 minutes. Mothers completed the questionnaire in the antenatal clinics during their waiting for medical examination.

    Results:

    Twenty two need requirements emerged from the women's perspective. The highest ranked needs for parturient women during labor are; maintaining privacy through all procedures of 86.5%, accessibility of nurses demonstrate empathy of 67.5%, availability to ventilate and expressing fear and anxiety of 57.5%, quick response to request of 67.5%, frequent monitoring of 52.8%, accessibility of caring medical staff of 47.2% and short delivery of 52.8%.

    Conclusion:

    Despite good general coverage of labor care among women, there were clear variations in the type of management given to them or needed by them. This study confirms that different factors predict the multidimensionality of childbirth satisfaction.

    Abstract [en]

    Background: Recent reports from the developing world have documented rheumatic fever (RE) incidence rates as high as 206/100 000 and Rheumatic Heart Disease (RHD) prevalence rates as high as 18.6/1000. The high frequency of RHD in the developing world necessitates aggressive prevention and control measures. Aim: The aim of the study was to examine the effect of intensive antenatal follow up program on decreasing cardiovascular complications among rheumatic pregnant woman. Design: Randomized controlled trial. Setting: This study was conducted at El Kasr-EL Aini, maternity hospital- Cairo- University Hospitals. Sample: A total of 100 rheumatic pregnant women that attended the antenatal clinic at El Kasr-EL Aini, maternity hospital who could read and write, had functional class I and II cardiac disease, primigravida, nullipara with single fetus were recruited to this study and assigned randomly into two groups. Procedure: Women who met the inclusion criteria were interviewed and baseline assessment was done. The intervention was an intensive antenatal follow up program for decreasing cardiovascular complications among rheumatic pregnant woman. Patients in the study group (SG) were offered an intervention at three levels, basic, intermediate and comprehensive. Evaluation of the program was carried out in term of assessment of maternal and neonatal outcome. Results: There were several significant differences between the study group (SG) and control group (CG). In the SG, antepartum complications was less common, gestational age at birth was higher, postnatal hospital stay shorter, birth weight higher, more had vaginal deliveries with episiotomy, fewer had caesarean section, forceps deliveries and termination of pregnancy, and cardiac class was better at follow up as compared to the CG. Conclusion: The difficult issues in pregnancy complications by cardiac disease are best managed through a team approach. Establishing a pre-pregnancy plan will foster a better therapeutic relationship between all parties and allow for better understanding of the patient’s beliefs regarding pregnancy, her disease and attitude towards medications.

    The H295R in vitro cell line produces the majority of the steroidogenesis, for which reason it is commonly used as a screening tool for endocrine disrupting chemicals. Simultaneous determination of the precursor cholesterol and key steroid hormones could give a broad insight into the mechanistic disruption of the steroidogenesis. Steroid hormones have primarily been extracted from H295R incubation medium by means of liquid-liquid extraction (LLE) and the obtained recoveries and matrix effects have typically not been stated or assessed. In the present study a solid-phase extraction (SPE) method was developed and validated for the simultaneous extraction of cholesterol and five key steroid hormones pregnenolone, 17-hydroxyprogesterone, testosterone, cortisol and aldosterone from H295R incubation medium, and finally detected by LC-MS/MS. Cholesterol was recovered at a level of 55.7%, while steroid hormone recoveries ranged from 98.2 to 109.4%. Matrix effects varied between -0.6% and 62.8%. Intra-day precision was deemed acceptable, but the inter-day precision for pregnenolone and aldosterone exceeded the precision limit of 15% RSD. Although LLE has been the most frequently used extraction method in H295R studies, however, our investigation has shown that SPE may relatively easily extract and recover steroid hormones, potentially replacing LLE.

    Analytical methodologies based on liquid or gas chromatography coupled to tandem mass spectrometry for the simultaneous determination of two or more endogenous steroid hormones in human and animal plasma and serum has received increased attention the last few years. Especially in the clinical setting steroid profiling is of major importance in disease diagnostics. This paper discusses recent findings in such multi-steroid hormone procedures published from 2001 to 2012. The aim was to elucidate possible relationships between chosen analytical technique and the obtained analyte sensitivity for endogenous steroid hormones. By evaluating the success, at which the currently applied techniques have been utilized, more general knowledge on the field is provided. Furthermore the evaluation provides directions in which future studies may be interesting to conduct.

    This study demonstrates the addition of (14)C-cholesterol to the human cell line H295R will in-situ form radiolabeled steroid hormones allowing for new mechanistic and metabolic insights. The aim of the present study was to in-situ radiolabel steroid hormones from cell line-incorporated (14)C-cholesterol using the OECD guideline 456, H295R steroidogenesis in-vitro assay. Radiodetection of the steroid metabolites of the steroidogenic pathway allows for an improved understanding of the various enzymatic mechanisms involved without necessarily being dependent on quantification. Generated radiolabeled steroids were analyzed using HPLC hyphenated with a Flow Scintillation Analyzer (FSA). H295R cells were incubated with radiolabeled cholesterol and cell media were collected and prepared by solid phase extraction and analyzed with HPLC-FSA. For successful radiolabeling of the steroids in the steroidogenesis of H295R cells, radioactive cholesterol may potentially only need to be added just before the cells are incubated for 72h in well plates. Based on the obtained HPLC-FSA chromatograms, and confirmation of the observations by studies in the literature, a qualitative time profile for the production of steroid hormones was estimated. Multiple radiolabeled steroid hormones were identified by means of analytical standards and UV (ultraviolet) co-chromatography, though the elucidation of multiple metabolites remains unresolved. Although online radiodetection proved to suffer from suboptimal sensitivity, the concept of radiolabeling the steroidogenesis in H295R cells with (14)C-cholesterol and detecting the radiolabeled steroid hormones online was proved and may assist in further toxicological studies.

    Samarbete och samverkan tas ofta för givet som något gott. Men trots höga förväntningar på positiva effekter är det svårare än vad många förväntar sig. Familjecentralen som den här artikeln handlar om är ett exempel på hur tvärsektoriell samverkan kan se ut i praktiken. Personalen har tillsammans med en forskare studerat samarbetets grundvalar och i artikeln diskuteras det spänningsfält som personalen på grund av samlokalisering befinner sig i och som innebär såväl hinder som möjligheter för samarbete. För att skapa gynnsamma förutsättningar för tvärsektoriellt samarbete i en samlokaliserad verksamhet behöver följande aspekter diskuteras och övervägas. - Moderorganisationernas ledning behöver se över ekonomiska och administrativa rutiner för familjecentralen och skapa yttre förutsättningar i form av handledning och tid för lärande och utveckling inom gruppen. - Personalen behöver tänka över betydelsen av sin egen och professionens utveckling samt aspekter av den organisationskultur som de representerar och bär upp. Olikheter kan bli bränsle för lärande och utveckling i gruppen, som i sin tur kan leda till en utveckling och nytänkande i verksamheten och man kan undvika att olikheter blir grogrund för samarbetsproblem.

    In this article we use two immigrant projects, operating in a Swedish context, as a basis for discussing and analysing the potential of joint projects as a tool for change in human welfare service organisations that target the general public. The extensive use of projects in the development of a slimmed-down public sector can be seen as originating from the contradictory promises of projects regarding flexibility and rationality in the new public management context. Projects are also used as a way to obtain more resources for specific long-term needs, but they are seldom implemented as intended in the regular 'mainstream' organisation. Therefore the long-term development of social services and healthcare for immigrants remains inadequate. It appears that politicians, managers and civil servants prefer to focus on the positive aspect of projects to legitimate the organisations and to bring about change by targeting and steering activities. However, they tend to overlook the negative consequences of using projects, and there is a risk that a backlash will undermine the legitimacy of organisations. Citizens and participants in general, and immigrants in particular, experience frustration and distrust as a result of the short-term nature of project-based action. The problems that the projects were expected to address may remain, or even increase.

    In this article we use two immigrant projects to identify similarities and consequences in order to discuss and analyse outwarding collaborative projects in human service organizations as a tool for change. The heavy use of projects in a slimmed public sector originates from projects’ contradictory promises of flexibility and effectiveness in the New Public Management context. Usually projects are used as a way to get more resources to long-term needs but are seldom implemented as planned in the regular organisation. Seemingly politicians, management and civil servants preferred are focusing on the positive side of projects as means to legitimate the organizations and to bring change by targeting and steering activities. However, they tend to overlook the negative consequences from projects. Citizens and participants experience frustration and distrust. The long-term development in organizations is scarce. Projects are seldom implemented meanwhile as the problems the projects were expected to target still are remaining or even are increasing.

    A current challenge for many European countries is to enable forced migrants to access health and social care that meets their needs. One solution is to use paraprofessionals - that is, trained individuals who are not professionals - to act as bridge-builders between minority communities and the health and social care sectors. This paper explores the development of a quality improvement project in Sweden. The project aimed to recruit and train forced migrants who had lived in Sweden for more than five years to act as bridge-builders. The aim was to use their unique experiences and knowledge in two ways - to work with service staff in developing new ways of working, and to become culturally competent paraprofessionals working with their own communities. This paper focuses on how an understanding of the role evolved as the participants reflected on their experiences and undertook an inquiry process culminating in the development of three possible models of what the task of a bridge-builder should be in developing culturally appropriate health and social services that are responsive to the needs of forced migrants. The conclusion reflects on what was learned and how the experiences from this project may be useful for others who are struggling with the same kind of problem in Sweden and in other countries.

    Familjecentralens verksamhetsidé för öppna förskolan på familjecentraler – formulerat som en hypotes - har utvärderats steg för steg genom samla in data på sammanlagd sex familjecentraler för att pröva hållbarheten i hypotesen. Utvärderingen visar på ett föräldraskapande bland småbarnsföräldrar genom att de lär sig om barn och föräldraskap, får möjlighet att utöka sitt sociala kontaktnät och att bara vara i en trygg omgivning där socialt stöd finns till hands. De får tillsammans med andra känna sig ’good enough’ som en vanlig förälder med ett normalt barn. Professionell följsamhet utmärker personalens arbetssätt. De bedömer den ”mognadsgrad” som föräldern befinner sig i för att söka stöd och anpassar sitt förhållningssätt till föräldern genom fysiskt avstånd och djup i samtalet. På så vis kan pedagogiska, sociala och psykologiska mekanismer triggas igång och förklara de resultat som föräldrar upplever för dem själva och deras barn. Artikeln avslutas med en omformulerad hypotes om öppna förskolors verksamhet på familjecentraler.

    OBJECTIVE: to use empirical data to assess the theoretical relevance of using a salutogenic, instead of a pathogenic, perspective to prevent smoking during pregnancy. DESIGN: quantitative study, a questionnaire was completed during the first trimester of pregnancy and an interview was conducted after the baby was born. SETTING: a geographically defined area in the south-east of Sweden. PARTICIPANTS: all 395 women in the study area who were pregnant during the study period 1994-1995. FINDINGS: the women were categorised according to their smoking habits. A significant difference in the sense of coherence (SOC) score was shown between smoking and non-smoking women in indicators of bad health. Women who relapsed to smoking showed a lower level of SOC, particularly in the manageability component, than others. The SOC score was higher in the whole study group than in other comparable, non-pregnant populations. KEY CONCLUSIONS AND IMPLICATIONS FOR PRACTICE: a salutogenic perspective could be used in antenatal care as a basis for encouraging pregnant women to stop smoking. This could enhance the SOC by making smoking more understandable for the woman, by discussing smoking as a way of coping, and by encouraging the woman's own capacity and motivation to stop smoking. Starting a dialogue about smoking from the woman's point of view could do this, with the midwife and the woman exploring together the woman's thoughts about the smoking problem.

    Background: The objective of this study was to assess different smoking patterns during and after pregnancy and relate these patterns to socioeconomic conditions and different health issues such as symptoms, drug consumption and health care use. Methods: The study group included 337 pregnant women from a district in south Sweden. A questionnaire was filled in during the first part of pregnancy and an interview was conducted after the baby was born. The women were categorised according to their smoking habits. Results: The categories defined were continuers, relapsers, decreasers, quitters and non-smokers. Among relapsers three symptoms, difficulty in relaxing (OR 4.48), restlessness (OR 9.59) and dysphoria (OR 3.98), were more common than among non-smokers. All three musculoskeletal symptoms were most common among continuers. Among quitters the OR for backache was 2.05, for numbness In the arms and legs 2.76 and for tearfulness 2.92. Educational level was relatively high among quitters and few had a smoking partner. Among smokers (decreasers, relapsers and continuers) 24% used drugs regularly, compared to 5% among non-smokers. Conclusions: To prevent smoking during pregnancy awareness of the relations between different smoking categories and symptoms could make it easier for hearth personnel to individualise support. A possible starting point is to discuss how to cope with different symptoms occurring during smoking cessation. Women with predictors for continued smoking during pregnancy, such as low education, unemployment, a smoking partner and multiparous, should be focused on more intensively and preventive strategies suitable for their special needs should be developed.

    BACKGROUND: Samis are indigenous people in north Europe. In the territory called Sápmi (Lapland), reindeer herding is the traditional base for the Sami economy. The relation between living conditions and positive health of the Swedish Samis has been sparsely studied. As health is closely linked to sense of coherence (SOC), an understanding of the background factors to SOC may contribute knowledge that might be useful in promoting living conditions and health.

    METHODS: The study examines relations between the level of SOC and background factors from surveys in a Sami population (n=613) in comparison to a non-Sami population (n=525) in Sweden, and in comparison between 2 subsamples of Samis, that is, herders and non-herders.

    RESULTS: There are more similarities than differences between the Sami and non-Sami populations. However, dividing the Sami population, reindeer herders had significantly lower SOC, and in specific the subcomponent manageability, that is, less ability to use available resources to meet different demands in life, compared to non-herders.

    CONCLUSIONS: In addition to age and health, predictors of SOC are related to the life form of reindeer husbandry and the belonging to the herding community.

    Introduction: Family centres in Sweden are integrated services that reach all prospective parents and parents with children up to their sixth year, because of the co-location of the health service with the social service and the open pre-school. The personnel on the multi-professional site work together to meet the needs of the target group. The article explores a program theory focused on the open pre-schools at family centres.

    Method: A multi-case design is used and the sample consists of open pre-schools at six family centres. The hypothesis is based on previous research and evaluation data. It guides the data collection which is collected and analysed stepwise. Both parents and personnel are interviewed individually and in groups at each centre.

    Findings: The hypothesis was expanded to a program theory. The compliance of the professionals was the most significant element that explained why the open access service facilitated positive parenting. The professionals act in a compliant manner to meet the needs of the children and parents as well as in creating good conditions for social networking and learning amongst the parents.

    Conclusion: The compliance of the professionals in this program theory of open pre-schools at family centres can be a standard in integrated and open access services, whereas the organisation form can vary. The best way of increasing the number of integrative services is to support and encourage professionals that prefer to work in a compliant manner.

    OBJECTIVE: to describe the qualitatively different ways in which midwives make sense of how to approach women smokers. DESIGN, SETTING AND PARTICIPANTS: a more person-centred national project 'Smoke-free pregnancy' has been in progress in Sweden since 1992. Using a phenomenographic approach, 24 midwives who have been regularly working in antenatal care were interviewed about addressing smoking during pregnancy. FINDINGS: four different story types of how the midwives made sense of their experiences in addressing smoking in pregnancy were identified: 'avoiding', 'informing', 'friend-making', 'co-operating'. KEY CONCLUSION: the midwives' story types about how they approached women who smoke illustrated the difficulties of changing from being an expert who gives information and advice to being an expert on how to enable a woman in finding out why she smoked and how to stop smoking. IMPLICATIONS FOR PRACTICE: health education about smoking that is built on co-operation and dialogue was seen by the midwives as a productive way of working. The starting point should be the lay perspective of a woman, which means that her thoughts about smoking cessation are given the space to grow while she talks.

    In general, most women are familiar with the need to stop smoking when they are pregnant. In spite of this, many women find it difficult to stop. Using a phenomenographic approach, this study explored Swedish pregnant and post-pregnant women's ways of making sense of smoking during pregnancy. A total of 17 women who either smoked throughout pregnancy or stopped smoking during pregnancy were interviewed. Five different story types of how they are making sense of smoking during pregnancy were identified: smoking can be justified; will stop later; my smoking might hurt the baby; smoking is just given up; smoking must be taken charge of. Based on the study it is argued that the approach used in health education in relation to smoking cessation in antenatal care needs to move from information transfer and advice-giving to the creation of a dialogue. The starting point should be the woman's knowledge, concerns, rationalizations and prejudices. A model is suggested in which a woman may move in a space on three axes depending on life encounters, dialogue and reflections on meaning. The goal in health education would be to encourage movement along three axes: 'increase of self-efficacy towards control', 'increase awareness by reflection on meaning of the smoking issue' and 'avoidance of defense of the smoking behavior'.

    Objective:  To evaluate the Dental Hygienist Beliefs Survey (DHBS) and the test–retest reliability of DHBS in a group of general dental patients.

    Material and methods:  The DHBS, which is a questionnaire constructed to assess patients’ specific attitudes towards dental hygienists (DHs), was distributed together with the Dental Anxiety Scale adapted to specifically assess fear of DH treatment (DHAS). It was hypothesized that DHBS would correlate with DHAS and gender. The questionnaires were consecutively distributed to 80 patients at their first visit and after a clinical examination performed by a DH student. Retest assessments of DHBS were conducted approximately two weeks later in conjunction with the next visit at the DH student and before treatment (scaling session). The final study sample included 77 adult general dental patients in treatment at an education clinic for DH students.

    Results:  The results verified a statistically significant correlation between DHBS and DHAS. The DHBS sum of scores showed high internal consistency with Cronbach’s a coefficient of 0.88 and 0.91 at the first and second assessments, respectively, and the test–retest reliability of the DHBS was acceptable with intraclass correlation coefficient of 0.76. No statistically significant association was found between DHBS and gender.

    Conclusion:  The results suggest that the DHBS is a reliable and stable scale to use to assess patients’ specific attitudes towards DHs. Moreover, DH beliefs are associated with fear of DH treatment.

    AIMS: This study was designed to evaluate the effect of gap width and graft placement on bone healing around implants placed into simulated extraction sockets in the mandibles of four beagle dogs. MATERIALS AND METHODS: Four Ti-Unite implants (13 mm x 3.3 mm) were placed on each side of the mandible. Three implants were surrounded by a 1.35 mm circumferential and a 5 mm deep gap around the coronal portion of the implants. A fourth implant was inserted conventionally into both sides of the mandibles as a positive control. The gaps were filled with either Bio-Oss, autogenous bone or with a blood clot alone. The study design was balanced for animal, side and modality. Ground sections were prepared from biopsies taken at 3 months, and computer-aided histometric measurements of bone/implant contact and area of bone within threads were made for the coronal 5 mm. Data were analysed using analysis of variance. RESULTS: The mean bone/implant contact was 9.8 mm for the control and ranged from 9.3 to 11.3 mm for the three test modalities. The corresponding values for area within threads were 1 mm(2) and 1-1.2 mm(2). Modality had a significant effect on both bone/implant contact (F=16.9; P<0.0001) and area within threads (F=16.7; P<0.0001). CONCLUSION: The results of this study suggest that both autogenous bone graft and Bio-Oss played an important role in the amount of hard tissue fill and osseointegration occurring within marginal bone defects around implants.

    Purpose: To explore and compare cancer patients' perceptions on the quality of nursing care in four European countries. Methods: Data were collected in Cyprus, Finland, Greece and Sweden. The sample comprised 596 hospitalized cancer patients. The quality of nursing care was measured using the "Oncology Patients' Perceptions of the Quality of Nursing Care Scale" (OPPQNCS). Patient characteristics were also collected. Analysis of variance was used to examine the effects of country on the perceptions of the quality of nursing care. Results: Patients' age ranged from 18 to 86 years, and 58% were male. The comparison of cancer patients' perceptions regarding the quality of nursing care between the four countries showed a statistically significant difference in the total OPPQNCS scores (p <0.001) as well as in the subscales responsiveness (p <0.001), individualization (p<0.001), co ordination (p<0.001) and proficiency (p<0.001). The Cronbach's alpha coefficient for the OPPQNCS ranged between 0.89 and 0.95. A multivariate analysis of variance for the OPPQNCS controlled by respondents' demographic characteristics revealed that only the patient's country was significantly related with the patients' perceptions of quality care. Conclusion: Quality of nursing care as perceived by cancer patients was high, but differed between the four countries. The impact of the clinical status of cancer patients on the quality of nursing care and managerial factors such as staffing/nursing care delivery models that influence the ability of nurses to offer high quality care should also be explored by more focused studies.

    Aim: The aim of this study was to evaluate the effects of non-surgical periodontal therapy on intra-oral halitosis 3months after therapy. Material and methods: Sixty-eight adults with intra-oral halitosis were included in a case series. Intra-oral halitosis was evaluated at baseline, and at 3months after treatment using the organoleptic scores (OLS), Halimeter (R), and a gas chromatograph. Results: Significant reductions for OLS (p<0.01), total sum of volatile sulphur compounds (T-VSC) (p<0.01) and methyl mercaptan (MM) (p<0.05) values were found after treatment. Hydrogen sulphide (H2S) levels were not significantly reduced. The numbers of probing pockets 4mm, 5mm and 6mm were significantly reduced as a result of therapy (p<0.001). Bleeding on probing (BOP) and plaque indices were also significantly reduced (p<0.001). For the 34 individuals with successful periodontal treatment (BOP<20% and a 50% reduction of total pocket depth) reductions in OLS (p<0.01) and T-VSC scores (p<0.01) were found. Eleven individuals were considered effectively treated for intra-oral halitosis presenting with a T-VSC value <160ppb, a H2S value <112ppb and a MM value <26ppb. Conclusion: Non-surgical periodontal therapy resulted in reduction of OLS, MM and T-VSC values 3months after therapy. Few individuals were considered as effectively treated for intra-oral halitosis.

    This paper explores whether and in what way “dual learning” can develop understanding of the relationship between structure/judgement and explores audit student’s perceptions of the audit profession. Work Integrated Learning (WIL) module, serving as a tool of enabling dual learning, represents the context for this exploration. The study is based on a focus group and individual interviews conducted with students performing their WIL. Our data and its analysis indicates that when in a WIL context, students develop awareness of the use of standards and checklists on the one hand and the importance of discretional judgement on the other. Based on these results, we theorise as to how dual learning manifests itself in students’ experiences and understanding of the relationship between structure and judgement.

    Background Limited evidence exists on the efficacy of regenerative treatment of peri-implantitis Materials and methods Subjects receiving antibiotics and surgical debridement were randomly assigned to placement of autogenous bone (AB) or bovine derived xenograft (BDX) with placement of a collagen membrane. The primary outcome was: evidence of radiographic bone fill and the secondary outcomes included reductions of probing depth (PD) bleeding on probing (BOP) and suppuration. Results 22 subjects were included in the AB and 23 subjects in the BDX group. Statistical analysis failed to demonstrate differences for 38/39 variables assessed at baseline. At 12 months, significant better results were obtained in the BDX group for bone levels (p < 0.001), BOP (p = 0.004), PI (p = 0.003), and suppuration (p < 0.01). When adjusting for number of implants treated per subject, a successful treatment outcome PD≤ 5.0 mm, no pus, no bone loss and BOP at 1/4 sites the likelihood of defect fill was higher in the BDX group (LR: 3.2, 95 % CI: 1.0 to 10.6, p < 0.05). Conclusions Bovine xenograft provided more radiographic bone fill than autogenous bone. The success for both surgical regenerative procedures was limited. Decreases in PD, BOP, and suppuration were observed.

    This article considers Julie Otsuka's representations of the World-War-II internment of Japanese Americans in When the Emperor Was Divine (2002) and The Buddha in the Attic (2011) from the perspective of collective remembrance, thus highlighting the interconnectedness of remembrance, forgetting, silence and race. Remembering and forgetting are understood as contingent on one another, and on the ideological currents and countercurrents that affect the construction of collective remembrance. The article argues that the content and form of Otsuka's novels mediate the cultural silence of the internment. In addition, they illustrate the changing nature of the narrativized remembrance of the internment as accounts of the lived experience of the Japanese Americans who went to camp are being replaced by trans-generationally transmitted, imaginatively recreated memories. The historical silence of the incarceration and its aftermath is sometimes explained in terms of "Japanese culture," but such a description risks reducing the impact of the racialization of Japanese Americans, and obscuring its effect on resistance. Finally, the analysis demonstrates that in Otsuka's texts, remembrance of the internment is characterized by a negotiation between repressive erasure and restorative forgetting.

    The Storyline approach is little known in language teaching contexts although it has much in common with task-based education. Learners play the parts of characters in an unfolding narrative, collaborating on tasks in small groups, a method which combines the use of language skills with practical work. A word often used by participants in a Storyline topic is `fun′. This article reports on a study in which I attempted to identify the features that particularly appealed to a class of Swedish 11–13 year olds, and how working in this way impacted on their learning of English. The data show that learners were strongly motivated by particular tasks and that the experience of taking part in a Storyline brought specific language benefits.

    In the Storyline approach, a fictive world is created in the classroom, with learners working in small groups, taking on the role of characters in a story. The story develops as they work on a range of tasks which integrate the practical and theoretical content of the curriculum. This article reports on a study based on the syllabus for English, in which a class of Swedish 11-13 year olds took on the roles of families who had moved into a new sreet in England, and highlights the role played by cooperative group work in the second language learning process.

    Abstract [en]

    Teachers' work in tuition-free (non-classroom) time was investigated to see to what degree teachers do work that could be considered as qualifying for the status of professional autonomy. The question arises in Sweden and elsewhere as both teachers and the state actively, and in tandem, strive to professionalise the work of the teacher. Abbott's work on the division of expert labour is taken as a point of departure. Based on data collected by an organisation sampling method, the article describes what teachers do in their tuition-free time. The study data consist of 1166 reports from 59 Swedish teachers' daily work situations in school years 1-12. Qualifying teacher work is related to teachers' ways of handling divergent cases, discretionary work and problem solving. Situations where teachers can use specific professional knowledge are described. One of the main findings is that in 22.2% of the situations studied, such professional knowledge can be applied. This corresponds to at least 7.5 hours of the Swedish teachers' weekly working hours; with the reservation that 10 hours of teachers' weekly 45 hours working time is not examined. This time corresponds roughly with the non-regulated working time, that is, time when the teachers do not have to be at school.

    Abstract [en]

    Teachers’ work has during the last ten years gone through great changes. The effects of postmodern society has made the work more and morecomplex and difficult to handle and understand for those involved as well as outsiders. Researchers are trying to keep up with things. The developmentof knowledge of teachers’ work takes place on different levels, in separate disciplines, from various starting-points and with different foci.To be able to describe, understand and explain the »new« work of teachers in a vigorous way there is need of getting these research initiativestogether. Arenas should be established where interchanges and coordination between researchers could take place. In order to make this happen wehave the intention of building a Swedish (our aim is to expand the network to the Nordic countries after the establishment in Sweden) network ofresearch on teachers’ professional lives.Besides presenting the intention and design of our network we also bring along some examples of research projects in line with the network ideas.

    OCTOPUS in EDUCATION: new models of welfare – new demands on the teachers' profession2009Konferansepaper (Fagfellevurdert)
    Abstract [en]

    In the Nordic countries the Welfare State has a broad political base, but the meaning given the concept of welfare appears to be changing. The welfare state is not only talked about in terms of common responsibilities towards citizens in need of help or as a duty to protect citizenship and civic rights. Welfare is also talked about as a “service” directed to the individual and as an investment to insure the best possible development of national competitiveness in a global world. This transformation or displacement brings new demands on the knowledge and competence professionals need to be able to manage their daily work.

    Drawing on empirical examples from research projects looking into everyday school work in Pre-school, Elementary school and Secondary school in Denmark and Sweden this study claims that the welfare state transformation introduces fields of tensions that teachers have to cope with. In this context raise new questions of professionalization understood as building up new bases of knowledge, developing new strategies for work organisation and time management and creating new spaces for ethical or democratic reflections.

    The paper draws on empirical examples from 4 research projects studying teachers work in Pre-school, Primary School and Secondary school in Denmark and Sweden.

    Abstract [en]

    In many countries, practice-oriented programmes have been recast as university programmes, fuelling debate on the purpose of higher education. We highlight two ways of talking about the challenges for higher education we think are already familiar to readers. We label them ?political-worry discourse? and ?academic-worry discourse?. We argue for a third position that gives due recognition to the turn in governance. Professional work today is performed in a new landscape where maintaining the conditions for autonomy and discretionary work is more important than ever. This has implications for the pedagogic focus of higher education institutions. The proposed ?dual learning? concept addresses this challenge through educating students about governance and allowing students to position themselves in relation to the governance of their professions. By discussing governance, we contribute to a previously neglected field and support the position that higher education may indeed be the best place to provide professional education.

    Studies have demonstrated that neoliberal governance dismantles professionals' will to critique, counteracting efforts to improve quality by preventing professionals in all sectors of the labour market from grounding work in their professional convictions. Managing attempts at governance has therefore become an important professional competence. This paper investigates how higher education prepares students for work under neoliberal forms of control. How are they trained to analyse and describe the effects of governance and prepared to work with the standards, manuals and documentation that influence quality in their lines of work? 'Interruptive focus group conversations' were performed with students from various professional programmes at a Swedish university. Student reasoning about governance is explored using theories of governance and subjectification. The results demonstrate that students are aware of the ways in which their professions are governed, the need to conduct discretionary work, but do not have skills to deal with governance critically. This latitude for pedagogical intervention is underutilized.

    Rehearsing accountability: how pre-school teachers go about appearing professional2009Konferansepaper (Fagfellevurdert)
    Abstract [en]

    Today’s professionals appear to be under constant pressure to be able to account for their actions and to do boundary work because there are not any evidently borders for what can be a work task. We want to contribute to a discussion about what demands pre-school teachers has to handle to perform as good professionals in these specific circumstances. From our example I pre-school settings we want to answer the question: what do professionals need to rehearse? And what new forms for this rehearsing accountability are growing up. In a seminal article Lyman and Scott argued that accounting for ones actions is a basic human activity and we use Goffman’s theory about team performance to understand these types of accounting in professional settings.

    Standardization meets practice: the case of integrating research-based work placement2015Konferansepaper (Fagfellevurdert)
    Abstract [en]

    A local reform, with the vision to educate Sweden’s most employable students was adopted by the board at a Swedish university. An important means for reaching the vision was a work placement. Students in all 28 programs at the university should be guaranteed at least five weeks supervised research based practice and learning in companies and organizations. Following the decision, activities were initiated in all Higher education programs to implement the placement reform. To document strategy work, yearly interviews were conducted with the head of the programs, course heads and teachers. Syllabi were collected and changes kept track of. Program dialogues were followed using ethnographic field work methods. University and program strategizing is explored from three questions: 1/ how did staff make sense of and turn strategy into practice? 2) what resources became important to strategy work? and 3) what kind of placement, what kind of examinations, what kind of supervision etcetera were the result. A strategy as practice-perspective is used to explore what became of the standardized model for placement envisioned in the university boards strategy. The result show that standardized placement was not possible for the programs. Instead different understandings of the reform based on the programs different conditions, led to a variation in what kind of placement they introduced.

    How do teachers think quality? This question is important since the teaching profession seems to be characterized by constant work intensification (Sarfatti-Larson, 1980). Repeated research show a steady increase in the number of tasks teachers are expected to perform (Apple 1989, Faber 1991, Hargreaves 1994, Day 2000, Grundy & Bonser 2000, Sutton 2007, Day & Gu 2007, Aili & Brante 2007, Chang 2009, Steen-Olsen & Eikseth, 2010). Teachers must therefore constantly prioritize between competing tasks and (to the extent they are pursuing quality of teaching) the question that arises is - how do they construct  realtions between tasks that are performed before the lesson and the teaching quality.

    In this paper we present an explorative study of how teacher formulate themselves about their daily work tasks in relation to the quality of their teaching. 60 teachers were invited to answer a semi structured web-questionnaire about work tasks they had perform during the day, once a day during two weeks. They were asked to choose two tasks that, from their perspective, had impact on the quality of their teaching in the nearest future. In addition to this they were asked to give each task a name, describe its content and under which circumstances it had been performed, and to specify in which way the task had impact on the teaching nearby. The material includes total 325 respectively 12 received questionnaires.

    The analysis focus is on teachers’ meaning-making of what quality is, and the relation between the work before teaching and the outcome of the teaching. A grounded content analysis was performed. The seven tasks identified by the teacher teams in the second questionnaire were analyzed in conjunction with a random sample of answers from the individual questionnaire regarding the same tasks. These tasks are labeled by the teachers name for the work task: Work with individual development plans, Planning of next day’s teaching, Planning of thematic work, Planning a school cinema visit, Production of material and Pupils’ choice.

    Some categories for the type of quality impact the teachers report have been construed such as for example “Absolute conditions” for links to a work task to the teaching such as the necessity to have ordered the groceries  before a lesson in cooking or ordered a bus for the transport to a school cinema visit. Another example is “Quality in a presentation” for links between high concentration works such as putting pedagogical considerations into a specific content. The way the teachers construe the reasons for tasks being important to quality are discussed in relation to earlier research about teachers’ professional language, characterization of teacher work, and teacher thinking.

    One of the central characteristics of disciplines is that they create their own particular ways of knowing the world. This process is facilitated by the specialization and refinement of disciplinary-specific semiotic resources over time. Nowhere is this truer than in the sciences, where it is the norm that disciplinary-specific representations have been introduced and then refined by a number of different actors. As a consequence, many of the semiotic resources used in the sciences today still retain some traces of their historical roots.

    In this paper we analyse one such disciplinary-specific semiotic resource from the field of Astronomy—the Hertzsprung-Russell diagram. We audit the potential of this semiotic resource to provide access to disciplinary knowledge—what Fredlund et al (2012) have termed its disciplinary affordances. Our analysis includes consideration of the use of scales, labels, symbols, sizes and colour. We show how, for historical reasons, the use of these aspects in the resource may differ from what might be expected by a newcomer to the discipline.

    We suggest that some of the issues we highlight in our analysis may, in fact, be contributors to alternative conceptions and therefore propose that lecturers pay particular attention to the disambiguation of these features for their students.

    Becoming part of a discipline involves learning to interpret and use a range of disciplinary-specific semiotic resources (Airey, 2009). These resources have been developed and assigned particular specialist meanings over time. Nowhere is this truer than in the sciences, where it is the norm that disciplinary-specific representations have been introduced and then refined by a number of different actors in order to reconcile them with subsequent empirical and theoretical advances. As a consequence, many of the semiotic resources used in the sciences today still retain some (potentially confusing) traces of their historical roots. However, it has been repeatedly shown that university lecturers underestimate the challenges such disciplinary specific semiotic resources may present to undergraduates (Northedge, 2002; Tobias, 1986).

    In this paper we analyse one such disciplinary-specific semiotic resource from the field of Astronomy—the Hertzsprung-Russell diagram. First, we audit the potential of this semiotic resource to provide access to disciplinary knowledge—what Fredlund et al (2012) have termed its disciplinary affordances. Our analysis includes consideration of the use of scales, labels, symbols, sizes and colour. We show how, for historical reasons, the use of these aspects in the resource may differ from what might be expected by a newcomer to the discipline. Using the results of our analysis we then created an online questionnaire to probe what is discerned (Eriksson, Linder, Airey, & Redfors, in press) with respect to each of these aspects by astronomers and physicists ranging from first year undergraduates to university professors.

    Our findings suggest that some of the issues we highlight in our analysis may, in fact, be contributors to the alternative conceptions of undergraduate students and we therefore propose that lecturers pay particular attention to the disambiguation of these features for their students.
