    Kvinnors upplevelse av att leva med diabetes typ 22013Report (Other academic)
    Abstract [sv]

    Bakgrund: Diabetes typ 2 är en kronisksjukdom som ökar på grund av förändrade kostvanor och minskad fysisk aktivitet. Det är en sjukdom som ställer stora krav på individen eftersom det är viktigt med välfungerande egenvård för att må bra. Att drabbas av en kronisk sjukdom upplevs oftast negativt samtidigt som det är viktigt att få ett namn på sjukdo- men. Det finns studier som tyder på att kvinnor verkar må psykiskt sämre och har sämre reglerad diabetes än män. För att kunna stödja kvinnorna i egenvården behövs kunskap om hur de upplever det att leva med diabetes. Syfte: Undersöka kvinnors upplevese av att leva med typ 2 diabetes. Metod: Litteraturstudie som bygger på nio kvalitativa artiklar. Resultat: Fyra teman hittades; besvikelse och svek, kluvenhet, skuld och skam samt oro för framtiden. Diskussion: Det är svårt att vara kvinna och leva med diabetes typ 2. Sjukdomen kräver daglig egenvård och kombinerat med andra roller som kvinnan har i sitt dagliga liv kan kraven upplevas som stressande för en del av kvinnorna. Många kvinnor med diabetes typ två upplevde att de var i behov av bättre stöd och information för att kunna leva med sin sjukdom. Slutsats: Vårdpersonal behöver mer utbildning och ha med ett genusperspektiv för att förstå kvinnornas behov. Diabetesutbildningen behöver utveckas till att omfatta hela familjen, men samtidigt måste den vara individanpassad.

    Abstract [en]

    The aim of this study was to demonstrate how people with incurable cancer experienced soft massage in a palliative care setting in which massage was used as an established and integrated part of the nursing care. To reach a deep understanding of the experiences of receiving soft massage a qualitative method with a phenomenological approach was chosen. The study was based on interviews with eight patients in an advanced home care setting who had all received soft massage as part of their daily care. Soft massage was experienced by the informants as a way to find inner peace. During the massage the patients felt dignified, while memories from past massage sessions were about becoming free. These experiences of dignity and freedom brought hopes for the future. The conclusion is that soft massage ought to be offered in the ordinary palliative care. More research is needed to understand what is needed to integrate and establish methods such as soft massage in the palliative care.

    Andelen äldre personer ökar i det svenska samhället. Med stigande ålder ökar risken för olika sjukdomar och besvär. För att kunna tillgodose en trygg och god vård till denna patientkategori krävs samordnade och välplanerade insatser. Distriktsnämnden för Nordöstra Skånes sjukvårdsdistrikt har beslutat att äldrevårdsteam ska tillskapas för att kunna erbjuda hembesök till äldre vid akuta/halvakuta sjukvårdsbehov. Syftet med detta arbete var att beskriva processen hur projektet äldrevårdsteam enligt "Hässleholmsmodellen" initierades, planerades och startades upp samt som en del i utvärderingen av projektet undersöka vad olika berörda intressenter ansåg var nyttan med äldrevårdsteam utifrån sitt perspektiv. Metoden som användes var intervjuer med yrkesverksamma och intervjuerna har analyserats med kvalitativ innehållsanalys. I intervjuerna framkom att äldrevårdsteamet kan vara en resurs och ett stöd vid bedömning samt utgöra en möjlighet för förbättrad samverkan och delaktighet kring patienten. Genomgående bedömde informanterna att det innebär en stor nytta för den äldre personen att kunna få vård i hemmet och slippa åka iväg till sjukhuset eller vårdcentralen. I några intervjuer betonades värdet av att svårt sjuka och döende människor bereds möjlighet att få vara kvar i sitt hem. Det är av största betydelse att utvärdera projekt på olika sätt och en viktig del är att ta reda på vilken nytta äldrevårdsteamet innebär för den enskilde patienten.

    När visioner ställs mot principer: en rapport från arbetet med att utforma en ämnes- och lärosätesövergripande nätutbildning2006Report (Other academic)
    Abstract [en]

    The Masters course ”Teamwork in Home Care” was originally developed as a project supported by the Agency for the Swedish Netuniversity (In 2006 this agency was incorporated into the newly formed Swedish Agency of Networks and Cooperation in Higher Education). This programme aims to increase the ability of personnel employed in care and rehabilitation programs for the elderly and disabled to work in interdisciplinary teams. An earlier program review of Swedish comprehensive home care suggested that training programs should include these skills in their curriculum.

    Up to now, educational programmes in the area have concentrated on making students more aware of a variety of perspectives by comparing and contrasting the views of elderly and disabled individuals, with those of their relatives and health care professionals with different backgrounds that can be found in the national healthcare system.

    The ”Teamwork in Home Care” course was developed as a collaboration between University College of Kalmar, Kristianstad University College, Jönköping University, Blekinge Institute of Technology, and Växjö University. This project was driven by the idea that the number of course offerings and teacher competence would increase if the institutes of higher educations were to collaborate instead of competing with one another. Modern information and communication technology would enable modes of collaboration and flexible learning that were deemed necessary for success.

    Collaboration between independent higher education institutions entails many unforeseen obstacles and difficulties. Different institutions each have their own routines, their own ways of interpreting rules and regulations, and their own epistemologies and learning philosophies. In addition they have adopted different learning platforms. This report gives a closer description of these obstacles and how the project team handled them.

    Case-based methods were employed in order to foster an awareness of the relevant perspectives and promote a heightened understanding of the end user. By building a Masters program around the systematic approach to human narrative students are provided with the tools they need to supervise teams in their future careers in health services.

    Although the obstacles have been many, collaboration between the different disciplines and institutions has also brought some obvious advantages. When people with different background come together there is often a lot to be learned. This was also the case regarding not only the collaboration that took place between different care and rehabilitation teams, but also between teachers from different universities and professional backgrounds.

    I forskningsplattformens uppdrag ingick att följa och dokumentera närsjukvårdsutvecklingen i regionen. Som ett första steg i detta arbete intervjuades 97 medborgare, medarbetare och beslutsfattare om vad de lade in i begreppet närsjukvård. Avsikten var att ta reda på hur olika grupper såg på vad närsjukvård var eller borde vara. Ett vanligt svar bland framför allt medborgare och medarbetare var att de hade hört talas om närsjukvård men att termen var tvetydig, oklar och kunde förstås på olika sätt. I avsaknad av en klar definition försökte de förstå genom att associera till orden nära och sjukvård.

    BACKGROUND: Persistent pain is a common problem for older people. Knowledge about how nursing and paramedical staff perceive these people and what they do to relieve the pain seems scarce. AIM: To explore nursing and paramedical staff perceptions of older people in persistent pain and their day-to-day management of pain. METHODS: Interviews in Swedish with 52 nursing auxiliaries, Registered Nurses, physiotherapists and occupational therapists were collected from February to May 2000. The analysis was based on their stories (n = 150) about older people in persistent pain who received help in their own homes or in special accommodation. A typology of staff perceptions of pain in older people was developed. Activities to manage pain were examined using content analysis. RESULTS: Respondents perceived the pain as real, exaggerated, trivial, care-related, endured, concealed, self-caused or inarticulate. Older people perceived as exaggerating the pain, those with care-related and self-caused pain evoked frustration in the staff, while those perceived as enduring their pain evoked satisfaction. Various strategies to manage pain were used: no activity, medication, mediating contacts, distracting activities, physical therapies, mobility, work in a gentle way, rest or relieving pressure on body part, and communication concerning pain. The activities differed between the types, as well as between staff with different professional backgrounds. CONCLUSION: Care and treatment provided by staff should be based on older people's needs rather than on staff attitudes and preferences. The typology revealed that staff perceived older people in pain as a heterogeneous group and that their perceptions affected the pain-relieving activities that were offered. It seems urgent to address how to handle pain in older people who never complain and those who complain a great deal, as well as how to handle pain in people with impaired communicative ability. Reflective discussions on feelings related to different individuals are needed.

    Abstract [en]

    Background

    This report is the result of a limited project with the aim of examining priority setting in care for the elderly in which multiple care providers are involved. The main issues dealt with the kind of prioritizing that is done daily and the motivations or considerations that the priorities are based on. The health care services provided to the elderly are based on an organization’s or society’s motivations and values but can also be based on the individual care provider’s own comprehension. Before the relevance of selected services can be assessed we need insight into how priorities are set and on which values they are based, in other words, the priority setting process requires transparency. This pilot study is an attempt to penetrate the priority setting process and thereby function as a point of departure for continued studies of how elderly care can be provided –based on elderly people’s experience and wishes.

    Aim

    The aim of this pilot study is to obtain knowledge of what characterizes an elderly person’s path through the flow of care services, which decisions are made and on what grounds. Based on these questions, new areas are identified that require in-depth study and analysis in order to develop issues and methodologies for coming projects dealing with priority setting in the flow of care for the elderly from the user perspective.

    Method

    A strategic selection of eleven people, employed in both municipal and county council elderly care have been interviewed with the aid of hypothetical case descriptions. The interviews have been supplemented with questions such as if their own organization has any policy documents, guidelines or informal regulations (known by everyone but not in writing) for care and treatment of particular health conditions. Existing policy documents/guidelines have been collected and comprise part of the empirical material for the pilot study. Each interview has been analyzed using the method content analysis.

    Results

    The study showed that the priority setting described was seldom about choices between patient categories. Instead choices were described as being made between different forms of health care and between different rehabilitation efforts. The choices that were most often described were if the patient should be offered hospital care or care at home and if the patient should be offered rehabilitation in the form of training or rehabilitation, primarily, in the form of aids. The analysis showed that the decision was based on considerations related to the patient, personnel and/or the organization. In a normal case, the decision was based on a combination of these considerations. The pilot study provides a complex picture of the considerations personnel based their prioritizing on. One finding is that decisions on care and rehabilitation are not only based on factors related to the individual patient but also on factors related to personnel and the organization where the decision is made. The report recommends a general discussion about priority setting among personnel who work with elderly persons by examining how to apply the human dignity principle when, e.g. the elderly person’s social network is considered as a factor when prioritizing between training or not. The report recommends a discussion on the choice of gauge for assessing the effect of different treatments.

    BACKGROUND: Although the topic of pain among older people has received increasing interest, little is still known about how pain is experienced or handled by those who no longer manage independently but depend on professionals for help with daily living. Developing pain management for older people requires such knowledge. AIM: To explore sense of self, sense of pain, daily living with pain, sense of others and ways of handling pain in older people with persistent pain. METHODS: Interviews with 90 older people receiving home care from nursing auxiliaries in their own homes or in sheltered accommodation were collected from January to June 2000. A typology of older people in persistent pain was developed. Activities for handling pain were examined using content analysis. FINDINGS: Respondents' experiences of themselves and their pain varied. Two groups of older people, considered as 'competent and proud' and 'confident and serene', expressed satisfaction in spite of pain, while the groups 'misunderstood and disappointed' and 'resigned and sad' expressed dissatisfaction. The most common strategies used were medication, rest, mobility, distracting activities and talking about pain. Respondents chose strategies by balancing the advantages of the activities against the disadvantages these brought for their daily living. CONCLUSION: This study indicates that characteristics of the older people, such as their way of experiencing themselves, how pain affects their daily life and how they perceive effects and side-effects of pain management are areas that need to be identified when staff assess pain and plan pain management. Caring for older people in pain could be improved by listening to and believing their complaints, evaluating effects and side-effects from medications and nonpharmacological pain management and by emphasising the importance of common everyday activities such as mobility and distraction to relieve pain.

    Boken belyser äldre personers hälsa och ohälsa och bygger på sammanlagt 27 svenska forskares beskrivningar av det aktuella kunskapsläget. De flesta författare har sin utgångspunkt i omvårdnad, men bland författarna återfinns även företrädare för medicin, oral hälsa, kulturgeografi, filosofi, socialt arbete, gerontologi, medicinsk etik, arbetsterapi och fysioterapi.

    Pain is a common problem for older persons who need professional help for their daily living. In this study 94 older persons (75+) in persistent pain were compared with 52 care providers concerning the pain management methods they had used/administered during the previous week and how helpful they perceived these methods to be. Interviews were based on 16 items from the original version of the pain management inventory (PMI). Both groups perceived prescribed medication, rest and distraction as the most frequently utilized methods. Specific methods such as transcutaneous electrical nerve stimulation (TENS) or bracing/splinting or bandaging the affected body part were seldom employed, although most users perceived these methods as helpful. Care providers perceived most methods for managing pain as more effective than older persons did. The results imply that care providers need skills in a variety of pharmacological and nonpharmacological methods to manage pain and a need to evaluate effectiveness of the methods in a systematic way.

    This study aimed to investigate the presence of pain, pain duration, localization(s), intensity, type and pharmacological treatment among older adults living in sheltered accommodation or receiving rehabilitation, as well as the agreement between pain assessments performed by staff and the older adults. Twenty-nine randomly selected older adults (65+ years) and the staff who looked after them participated in a structured interview based on standardized measures for pain assessment and physical, intellectual and communicative functions. Pain was found to be common, with a majority of participants experiencing it every day or all of the time. Nine out of 22 of the older adults in pain had no pain relief drugs at all. Agreement between assessments by the older adults and the staff was no higher than moderate and in general pain levels were underestimated. The findings indicate that older adults were at risk of undetected and untreated pain and the risk was even higher for those with speech difficulties. The provision of good nursing care for older adults in sheltered accommodation requires systematic routines for frequent pain assessments.

    Sixty-six randomly selected older adults and their contact nurses participated in interviews based on standardised assessments of pain and open-ended questions focusing how pain was expressed and recognised. The sample included older adults with normal as well as cognitively impaired function. Seventy-nine percent of older adults with normal cognition were often in pain. Contact nurses assessed pain in 57% of cognitively impaired older adults. The content in the statements showed that pain recognition was a communicative interactive process based on verbal and non-verbal expressions. The process comprised attempts to understand the cause and intention of the expression and to verify the presence of pain. Changes in mood, facial expressions and physiological responses were described less often by older adults than by their nurses. Contact nurses of cognitively impaired older adults judged immobility as the source of pain, that it was possible to see when the person was in pain and that pain was expressed by paralinguistic and body language more often than contact nurses of cognitively healthy older adults. Characteristics of nurses and older adults could facilitate or hinder pain recognition. The findings indicate a need for reflective discussions in the staff group focusing on how to perform systematic assessments of verbal and non-verbal expressions and of hindrances and facilitators for recognising pain in older adults.

    Although there is a trend towards developing health care in a patient-centred direction, changes are usually planned by the professionals without involving the patients. This paper presents an ongoing participatory action research project where patients with chronic renal failure, nurses at a specialist renal failure unit, a hospital manager and a researcher worked together to develop patient-centred care. The project combined the expertise of patients in their own experiences of living with a chronic condition with the professional expertise of nurses, the manager and the researcher. As the workload on the unit was uneven, the development work needed to be low in intensity but long-term. Based on a number of dialogues in focus groups, four main development areas were identified; access to test results, prerequisites for postponing the progress of the illness, general awareness and understanding of living with chronic renal failure, and family-focused care. A number of changes have been planned or implemented, such as developing a prototype for a web-based feed-back system, expanding patient education to newly diagnosed patients, steering the nurses' role towards a guiding and family-focused function, and planning a digital story-telling workshop. Involving committed people who have the mandate to change practices were prerequisites for success.

    AIMS AND OBJECTIVES: This study aimed at investigating life satisfaction and its relation to living conditions, overall health, self-care capacity, feeling lonely, physical activities and financial resources among people (65+) with reduced self-care capacity. BACKGROUND: Knowledge about factors related to low life satisfaction among older people with reduced self-care capacity is sparse, although this is important in health care and nursing so that the care is adapted to their needs and perspective. Previous research has mainly focused on isolated aspects such as pain in relation to life satisfaction among older people in general and less among so those with reduced self-care capacity in general. DESIGN AND METHOD: A subsample of 522 persons was selected from a randomly selected cross-sectional survey using a modified form of the Older Americans' Resources Schedule and Life Satisfaction Index Z. RESULTS: The mean age in the total sample was 77.9; women (79.5) were significantly older than men (77.0). Low life satisfaction was found among women, as well as those living in special accommodations. Life Satisfaction Index Z was 15.3 (SD 5.6) in the total sample. Gender and living conditions did not explain life satisfaction whilst poor overall self-reported health and poor financial resources in relation to needs had the strongest explanatory value. Also of significant importance were loneliness, the degree of reduced self-care capacity and feeling worried. CONCLUSION: Life satisfaction in older people with reduced self-care capacity is determined by several factors, with social, physical, mental and financial aspects probably interacting with each other; especially feeling lonely, degree of self-care capacity, poor overall health, feeling worried and poor financial resources in relation to needs. These factors need to be considered in the care of these people to preserve or improve their life satisfaction. RELEVANCE TO CLINICAL PRACTICE: Nursing interventions in terms of preventive home visits, rehabilitation, health education directed towards physical, psychological, social and economic aspects of importance may help to preserve or improve life satisfaction for those with reduced self-care capacity.

    I sjuksköterskeutbildningen vid Högskolan Kristianstad har sedan 2008 använts en modell för handledning och examination av examensarbetet med fokus på studenternas lärande. Trots att handledningsmodellen varit i bruk under några år visade genomförda kursutvärderingar att studenter uppfattade att det fanns skilda uppfattningar hos handledare och examinatorer om hur examensarbetet ska genomföras. På grund av detta ansöktes medel från Lärande Resurs Centrum (LRC) och Sektionen för Hälsa och Samhälle för att i kollegiet skapa möjligheter att diskutera vilka pedagogiska möjligheter och svårigheter som finns samt hur likheter och olikheter i syn på kunskap kan främja såväl som hindra studenternas lärandeprocess under examensarbetet. I denna rapport beskrivs projektets upplägg och genomförande samt centrala aspekter från den dialog som fördes i kollegiet under projektets gång.

    BACKGROUND: Knowledge about how elderly persons perceive the impact of chronic heart failure (CHF) on daily life is important when planning nursing care. For this purpose, disease specific instruments are needed. However, few instruments have been developed or tested specifically on elderly persons. AIM: To validate a Swedish version of the Minnesota Living with Heart Failure Questionnaire (LHFQ) on elderly persons with CHF, and use it to describe the impact of CHF on daily life in the same population. METHODS: The sample comprised of 357 persons, aged between 65 and 99, diagnosed with CHF. A questionnaire including background data, the LHFQ and the SF-12 was used. RESULTS: A factor analysis resulted in four dimensions: physical, emotional, treatment and pleasure. LHFQ showed convergent validity and ability to discriminate between known groups. Cronbach's alpha for the total scale was 0.94. Impairments in the physical dimension were most common, especially fatigue (88%) and shortness of breath (87%). CONCLUSIONS: The LHFQ showed satisfying psychometric properties in an elderly Swedish population with CHF and can, with minor alterations, be recommended for research and clinical use. The impact of chronic heart failure on daily life was mostly physical, but other impairments were also common.

    Background

    A main goal in nursing care of persons with chronic heart failure (HF) is to strengthen their health related quality of life (HRQoL). This presupposes knowledge about the relation between heart failure and HRQoL. Existing studies have shown incongruent results about whether HRQoL is affected differently depending on age or sex of elderly persons with chronic HF.

    Aim

    This study aimed to investigate if age, sex, disease severity, comorbidity and living conditions predict health related quality of life among persons 65 years or older with chronic HF.

    Methods

    The study included a sample of 357 persons. HRQoL was measured by the Minnesota Living with Heart Failure Questionnaire and the Short Form-12 Health Survey Questionnaire. Multiple regression analyses were performed to analyse the relation between the predictors and HRQoL.

    Results

    The main finding was that self-rated disease severity was strongly associated with HRQoL, but also age, sex, diabetes and respiratory diseases was associated with some of the dimensions of HRQoL.

    Conclusions

    Interventions aimed at delaying the progress of the disease, assist persons' to cope with the disease and maintain the domains of HRQoL that are still feasible could be important to improve HRQoL in elderly persons with chronic HF.

    Kurator på akuten?: personalens syn på behovet av en kurator på akutmottagningen2012Report (Other academic)

    Bakgrund: Dagligen sker akuta besök i hälso- och sjukvården där de medicinska behoven vanligen har högst prioritet. Samtidigt inrymmer dessa situationer även praktiska, sociala, emotionella och existentiella aspekter som inte bara rör personen som är akut sjuk utan också deras anhöriga och personalen. Patient och anhöriga kan komma i kläm då det kan saknas tid, resurs och kunskap för att möta deras behov. Även personalen kan hamna i svåra situationer då de måste tackla svåra händelser som inte alltid bearbetas. På vissa akutmottagningar används en kurator som har till uppgift att stödja patienter, anhöriga och personal.

    Syfte: Syftet med den aktuella studien var att undersöka vårdpersonalens syn på behovet av en kurator på en akutmottagning.

    Metod: En intervjustudie genomfördes där datainsamlingen skedde i tre fokusgrupper. Materialet analy- serades med en kvalitativ innehållsanalys.

    Resultat: Personalen uttryckte att det fanns behov av en kurator inom fyra skilda områden: Som stöd och resurs för anhöriga och patienter i kris; Som stöd och resurs för personalen i anslutning till krävandesituationer; Som ansvarig för debriefing samt Som ansvarig för att utbilda personal i samtalsmetodik ochkrishantering. Vikten av att kuratorn är känd av personalen och känner akutmottagningens verksamhet påtalades genomgående i intervjuerna.

    Diskussion: I verksamheten på en akutmottagning finns behov av att ge stöd inte bara till patienter och anhöriga utan också till personalgruppen som ska tackla svåra och krävande situationer i sitt arbete. Om inte ett sådant stöd finns är risken stor att omvårdnaden blir uppgiftsinriktad istället för omsorgsinriktad och att personalens hälsa i förlängningen påverkar negativt. En kurator med koppling till akutmottagningen skulle kunna fylla en sådan funktion.

    Diabetestyp 2 är en vanlig kronisk sjukdom med stort fokus på egenvård. Sjukdomen kräver livslång och regelbunden kontakt med hälso- och sjukvården för rådgivning och uppföljning. För att kunna erbjuda en tjänst som svarar upp till behoven vid egenvård behövs insikter om hur personer med typ 2-diabetes uppfattar sin situation. Det finns därför behov av att utveckla hälsovård ur ett användarcentrerat perspektiv. Deltagarbaserad design är en forskningsstrategi som har för avsikt att involvera människor för att ge dem inflytande i designprocessen.

    AIM AND OBJECTIVES: The aim of this study was to identify perceived challenges related to self-management among recently diagnosed adults and those with longer experience of type 2 diabetes as a foundation for the future development of a person-centred information and communication technology service.

    BACKGROUND: Learning self-management of type 2 diabetes includes mastering the skills required to complete complex emotional and physical tasks. A service developed with the participation of stakeholders may be an alternative way to meet rising needs for self-management.

    DESIGN: Qualitative descriptive design influenced by a participatory approach.

    METHODS: Multistage focus group interviews among one group of recently diagnosed (≤3yrs, n=4) adults and one group with longer experience (≥5yrs, n=7) of type 2 diabetes.

    RESULTS: Challenges in self-management in everyday life with type 2 diabetes were identified: understanding; developing skills and abilities; and mobilizing personal strengths. Both groups described challenges in understanding the causes of fluctuating blood glucose and in, developing and mobilizing skills for choosing healthful food and eating regularly. The recently diagnosed group was more challenged by learning to accept the diagnosis and becoming motivated to change habits while the experienced group was mainly challenged by issues about complications and medications.

    CONCLUSION: Adults with diabetes have different needs for support during different phases of the disease. From a person-centred perspective, it would be desirable to meet individual needs for self-management on peoples' own terms through a technological service that could reach and connect to a large number of people.     This paper reports the wishes and needs of people with type 2 diabetes (T2DM) for a future information and communication technology (ICT) self-management service to help manage their condition and their everyday life. Diabetes is a chronic disease affecting more and more people and placing increasing demands on health care. The self-management of diabetes includes instrumental and, decision-making skills and skills in managing daily activities, which may be supported by an ICT service. In this study we used a participatory design including two sessions of Future Workshop (FW) as part of a larger research project on the self-management of diabetes. Adults with type 2 diabetes participated in two FW sessions in which their expressed wishes and needs for an ICT service all fell under the broad category of Acceptance of the diagnosis, with three other suggestions; Trust in partnerships, Communication, and Individualized information. The participants’ experience of the FW as a democratic process and their appreciation of mutual learning contributed to these results, which are consistent with the aims of person-centred care.

    Methods: A retrospective, quantitative study design was obtained. Librarians (n = 2) andteachers (n = 4) randomly examined selected candidate theses (every third thesis, n = 89)from the years 2012, 2014, and 2016.

    Results: The result showed a significant improvement over the years (from 2012 and 2014to 2016) regarding the use of a sufficient number of synonyms, matching search terms to therespective database, use of the Boolean operator OR, and the use of subject headings and freetext searches. Use of the title/abstract search largely disappeared. There was a significantchange in the types of searches being done. The searches have become more structured inlater years as the use of block searches increased significantly; in other words, moresystematic and relevant searches have been done in recent years.

    Conclusion: The result of this study shows that the quality of the students’ search strategiesimproved significantly during the studied years. It is recommended that search documents areused in both formative and summative assessments to evaluate students’ search strategies.Educational development in the form of enhanced collaboration between librarians andteachers in nursing programs is recommended because it might help to develop studentsearch strategies in literature-based candidate theses.

    Abstract [en]

    Aims and objectives

    To investigate changes over time in an interdisciplinary group that was engaged in development work regarding pain and pain assessment.

    Background

    The outcomes of nursing research do not always find their way to the daily care of patients. This is evident within, for instance, physical rehabilitation, where relieving patients' pain is a major challenge. To gain new understanding, develop and change, registered nurses have a great part in engaging their staff.

    Design

    A qualitative study using a participatory action research approach was used, and a hermeneutic analysis was conducted.

    Methods

    A group of three registered nurses, two assistant nurses and a physiotherapist took part in focus group sessions. This group was followed with seven repeated sessions during a period of five months from January 2010 until June 2010. A hermeneutic analysis was used.

    Results

    The participants changed their attitudes towards the patient in pain, their own caring role and the team's role towards a more person-centred care.

    Conclusions

    Participation and reflection were key aspects as means to transfer knowledge into action and establish change. The participants came to the sessions, shared actively their experiences and expressed pride in the work they accomplished. This can be seen as an expression of a need to share and a joy to be working together in a person-oriented development area. In addition, the approach seemed to contribute to increased job satisfaction.

    Relevance to clinical practice

    The participants expressed that their work resulted in a changed approach to pain and pain assessment in their daily practice at the ward. The participants were actively engaged in enhancing their work with pain and pain assessment in their own daily practice and in implementing research-based knowledge.

    Abstract [en]

    The number of older people needing care is increasing, and care is often provided by informal caregivers. The mission of family care advisors (FCAs) is to provide them with support; however, whether and how support in existential matters such as existential loneliness is provided is unclear. Therefore, the aim of this study was to describe FCAs’ views on existential loneliness, and existential support provided to relatives who act as informal caregivers to older people. A national survey was distributed to 349 FCAs in Sweden, response rate n = 120 (36%). The STROBE checklist was followed when presenting the study. Existential loneliness was viewed as thoughts about life and meaning (78%). Existential support was provided by dialogues (87%), visits (75%) and support groups (73%); 45% of FCAs stated that they had time to provide existential support and 27% reported having knowledge of how to encounter existential loneliness. FCAs provide existential support, but often lack experience, knowledge and time. Time and knowledge are important prerequisites for acknowledging existential needs.

    Purpose: Spouses are in a vulnerable situation when caring for a frail partner late in life. Exploring their existential loneliness can be a way to understand more about their existential needs.Method: A hermeneutic approach was used. Multistage focus group interviews were conducted with two groups consisting of five spouses, respectively, who met three times each. To work with the text, an approach was adapted where quotations are converted into poems in a linguistic manner.Results: Existential loneliness can be understood as the following: 1) being in a transition from us to merely me, 2) being forced to make decisions and feeling excluded, 3) navigating in an unfamiliar situation and questioning oneself, and 4) longing for togetherness but lacking the energy to encounter other people. The main interpretation is that existential loneliness emerges when one is in moments of inner struggle, when one is forced to make impossible choices, when one is approaching and is in limit situations, and when one is experiencing the endless loss of the other.Conclusion: For health care professionals to achieve a holistic picture, person-centeredness can be a way to make the spouses' existential needs visible and to provide support based on their needs.

    Introduktion och rapportens uppläggning

    Det finns ett fåtal studier som har undersökt situationen för närstående till yngre personer med demenssjukdom. Dessa studier talar för att närstående som lever tillsammans med en person med demenssjukdom kommer att belastas allt tyngre och de kommer att behöva individuellt anpassat stöd för att orka med sin situation eftersom varje närståendevårdares situation är unik (Luscombe, Brodaty & Freeth, 1998, Jubb, Pollard & Chaston, 2003, Armstrong, 2003). Om närstående ska orka leva tillsammans med en partner som är demenssjuk behöver de erbjudas ett stöd som är utformat utifrån deras situation och behov. I Hälso- och sjukvårdprogrammet för Region Skåne betonas vikten av att lyfta fram yngre personer med demenssjukdom och deras närståendes situation ur ett regionperspektiv (Hälso- och sjukvårdsprogram, region Skåne, 2005). En studie där närstående själva får delta i att utforska hur ett närståendestöd bör utformas kan ge värdefull kunskap som kan bilda en bas för regionens planering av olika stödjande insatser. Syftet med denna studie var att i samarbete med närstående till personer vars demenssjukdom debuterat före 65 års ålder planera ett närståendestöd.

    Bakgrund till projektet

    Att få en demenssjukdom och att vara närstående till en person som utvecklar en demenssjukdom är en ny situation för alla inblandade. Vi vet ganska lite om hur närstående till yngre personer med en demenssjukdom upplever sin situation. För att undvika att bördan blir för stor för de närstående behövs olika samhälleliga insatser. För att öka kunskapen kring närståendes situation genomfördes fokusgruppsintervjuer med närstående till yngre personer som drabbats av demenssjukdom.

    Studiens genomförande och etiska överväganden

    Studien pågick mellan januari-april 2008 och genomfördes som ett deltagarbaserat aktionsforskningsprojekt där närstående till yngre personer med demens tillsammans med forskare samtalade om vilka problem närstående har att tackla samt gav förslag på hur ett närståendestöd skulle kunna utformas. Samtliga närstående var maka eller make till en person med demenssjukdom.

    Vad fokusgrupperna kom fram till

    Diskussionerna i fokusgrupperna omfattade tre faser; Tiden före diagnos – när man anar att allt inte står rätt till, Tiden i samband med och efter diagnos – bekräftelse på att det man anat var sant, Den sena fasen – när det blev ohållbart. Dessutom framkom att det fanns behov av en funktion som benämndes Lotsen. Namnet tillkom då fokusgrupperna signalerade ett behov av en lotsfunktion, en person som skulle kunna leda och lotsa närstående i vård- och omsorgsdjungeln.

    Vad hände sedan?

    Studien har lett till att en dagsjukvårdsenhet har startat ett projekt med avsikt att stödja relationen mellan yngre personer med demenssjukdom och deras närstående.

    Utvärderingsplaneringen och utvärderingen har utförts av studenter i sjuksköterskeprogrammet vid högskolan Kristianstad under handledning av forskare vid forskningsplattformen för Närsjukvård.

    Slutord

    Inom professionen tror man att man vet vilket stöd närstående behöver och min bestämda uppfattning var att närstående till yngre personer med demens framförallt behöver emotionellt stöd. Under studiens gång blev det dock tydligt för mig att det är omöjligt att ge stöd till närstående utan att samtidigt ta hänsyn till den sjukes behov och situation. Redan tidigt i sjukdomsförloppet behövs individuellt anpassat stöd som bör förändras i takt med sjukdomens fortskridande. Dessutom behövs en sammanhållande länk som kan lotsa vidare i processen och vara ett stöd både för den sjuke och närstående. Demenssjuksköterskan i kommunens hälso- och sjukvård skulle kunna fungera som den lots som efterfrågas.

    Abstract [en]

    Objectives

    To derive improved understanding of the implicit meanings of challenges in daily life from the perspective of persons with type 2 diabetes.

    Methods

    A meta-synthesis was conducted with an interpretive and constructivist approach. Four databases were searched for articles published between 2007 and 2011, producing 37 articles for analysis. Van Deurzen's life world theory was applied as an analytic grid.

    Results

    Challenges in daily life with type 2 diabetes could be understood as living in a tension between opposing forces, implying a struggle with inevitable paradoxes: living in the present and for the future, trusting oneself while relying on others, and being normal while feeling changed and different.

    Discussion

    This synthesis adds knowledge to previous understanding of living with type 2 diabetes, revealing the complexity of daily life when struggling with a lifelong illness. Person-centred care could be used to understand what challenges diabetes may cause in family and working life and the ambivalent feelings the illness can lead to. Future research is needed to implement and evaluate a person-centred care in practice. Since new qualitative research is continuously added to this topic, metasyntheses should be undertaken regularly.

    Abstract [sv]

    Bakgrund: Smärta är vanligt hos Sveriges befolkning. I en undersökning av levnadsförhållanden i Sverige från 2008 framgår att 19 % i åldrarna 16-84 lever med svår värk. Forskning visar att sjukvården brister i smärtdokumentation vilket kan innebära att vårdens smärtlindring blir lidande. Syfte: Syftet var att, hos en grupp sjukhusvårdade patienter med smärta, beskriva smärta och i vilken utsträckning smärtan dokumenterats samt belysa hinder för smärtdokumentation. Metod: Empirisk studie med kvantitativ, beskrivande statistik samt ett avsnitt med kvalitativ innehållsanalys. Resultat: Smärta hos en grupp patienter visar att en stor andel alltid har ont vid aktivitet men även vid vila. Dokumentationen brister främst gällande duration och är mest tillfredsställande angående lokalisation. Ett hinder för god dokumentation är patienter som väljer att inte visa eller berätta om sin smärta. Diskussion: En god relation mellan sjuksköterska och patient underlättar smärthantering. Sjukvårdens smärtlindring är otillräcklig och alltför många lider av att ha ont. Nuvarande smärtbehandlingsmetoder har god möjlighet att, om inte eliminera, så ge en betydande smärtlindring. Dokumentation är viktig för att kunna lindra smärta och därmed minska patienters lidande. Förslag till vidare forskning inriktar sig på hur tillfredställande dokumentation ser ut. Även forskning kring alternativa behandlingsmetoder behövs för att kunna sänka andelen som lever med smärta och värk. Detta skulle öka livskvaliteten hos Sveriges befolkning.

    This paper addresses a feasibility study of a method for recurrent collegial reflection. A qualitative approach, using a participatory research design, was adopted. The collegial reflection was implemented in a school, in a middle-sized municipality in southern Sweden, with 21 teachers participating in the intervention. Data collection included digital recordings of collegial reflection, open questions by mail, and individual interviews. Findings indicated one major theme, paradoxes in the design of the collegial reflection, and three categories: wanting to decide and wanting to be guided; meeting each other as teachers and/or as persons; and looking for the safe and/or looking for the new. Before implementing the method in another context, management needs to appreciate these contradictory experiences, allow for voluntary participation, address participants’ expectations, and allocate time and tasks. This study implicates that collegial reflection may contribute to teachers’ professional development, and it is thereby relevant to teachers’ classroom practice and pupils’ learning. We conclude that, by creating a structure which supports teachers’ collegial reflection, the school may function as a supportive environment, which may contribute to teacher retention.

    BACKGROUND: Experiencing work-life balance is considered a health promoting resource. To counter-balance the negative development of teachers' work situation, salutogenic resources need to be examined among teachers.

    OBJECTIVE: To examine resources related to teachers' experience of their work-life balance.

    METHODS: Using a cross-sectional design, a questionnaire was distributed to 455 teachers in compulsory schools in a Swedish community. A total of 338 teachers participated (74%). A multiple linear regression method was used for the analysis.

    RESULTS: Four variables in the regression model significantly explained work-life balance and were thereby possible resources: time experience at work; satisfaction with everyday life; self-rated health; and recovery. The strongest association with work-life balance was time experience at work. Except time experience at work, all were individual-related.

    CONCLUSIONS: This study highlights the importance of school management's support in reducing teachers' time pressure. It also emphasizes the need to address teachers' individual resources in relation to work-life balance. In order to support teachers' work-life balance, promote their well-being, and preventing teachers' attrition, we suggest that the school management would benefit from creating a work environment with strengthened resources.
