    Abstract [en]

    Objective

    The purpose of this study was to explore licensed practical nurses’ (LPNs’) views of being exposed to secondhand smoke in municipal home nursing care.

    Design and sample

    Using a qualitative approach, data were collected through three focus group interviews with 15 LPNs, smokers, and nonsmokers. Qualitative content analysis was used to interpret the interview text.

    Results

    Exposure to secondhand smoke during working hours, which presents a risk to the health and wellbeing of the LPNs, is a consequence of placing the interests of the smoking care recipients first and the employers’ reluctance to take action on behalf of the LPNs. These factors prompted LPNs to propose solutions aimed at resolving a major dilemma in routine home nursing care.

    Conclusions

    A combination of actions, particularly by managers, is required to improve the working environment for LPNs while concurrently respecting the care recipients’ right to take decisions in their own homes.

    Abstract [en]

    Students with neurodevelopmental disorders (NDDs) are present in every school, and most likely, there are a few students in every class. School health care is responsible for providing support to all students, especially those with special needs. The aim of the study was to describe school nurses' experiences of supporting students with NDDs. A qualitative method consisting of seven focus group interviews (that included a total of 35 school nurses) in Southern Sweden was conducted. Three themes were identified in the findings: helping students with NDDs to interpret sensations, detecting early signs of distress among students with NDDs, and using an inclusive design for health education. This study highlights the importance of school nurses in identifying the needs of students with NDDs and promotes a person-centered approach to achieve a healthy and safe learning environment for all students.

    Abstract [en]

    Pain and depressive symptoms are common reasons for adolescents to contact the school nurse. The aim was to describe the prevalence of pain (headache, abdominal pain, and back pain) and depressive symptoms among adolescents and to examine whether there is an association between pain and depressive symptoms. This cross-sectional survey included students ( N = 639) in Sweden (median age: 16 years). Over half of the female participants (56%) and one third of male participants (33%) had weekly headaches, abdominal pain, or back pain. Almost every second girl (48%) and one in four boys (25%) had depressive symptoms (as measured by the Center for Epidemiological Studies Depression Scale, scores ≥ 16). There was a significant association between having pain (headache, abdominal pain, or back pain) and having depressive symptoms. It is of great importance for school nurses to adequately identify and treat the cause of pain and other factors contributing to depression.

    I rapporten sammanfattas det vetenskapliga underlaget för två typer av program: dels de som främst syftar till att förebygga utagerande beteenden hos barn och ungdomar, dels de som i första hand syftar till att förebygga inåtvända problem som ångest, depression och självskadebeteende. Program som har en allmänt hälsobefrämjande effekt, t ex för att förebygga drogmissbruk och våldshandlingar ingår följaktligen inte. Programmen är avsedda att ha effekt, inte bara direkt efter att programmet har avslutats utan även i framtiden. Rapporten har tagits fram på förfrågan av Kungliga Vetenskapsakademien och UPP-centrum (Utvecklingscentrum för barns psykiska hälsa) vid Socialstyrelsen. Båda har efterfrågat en systematisk litteraturöversikt för att klarlägga nyttan med att använda program för att förebygga psykisk ohälsa hos barn.

    Att arbeta som skolsköterska ställer höga krav på att dokumentationen hanteras professionellt och enligt gällande regelverk. Bristande dokumentation i journalen gör att sammanställningen av skolbarns hälsoutveckling inte blir tillförlitlig. Då blir skolsköterskans arbete mindre synligt, vilket innebär att det kan vara svårt att göra uppföljningar över tid. En annan viktig aspekt är att i lagens mening är det som inte är dokumenterat inte heller utfört.

    I boken Skolsköterskans dokumentation får du en sammanfattning av de lagar och styrdokument som du som skolsköterska måste följa. Vi beskriver också hur e-hälsa och digitaliseringsutvecklingen påverkar dig som skolsköterska. Du får förslag på hur hälsobesök i olika årskurser, från förskoleklass till och med årskurs 1 i gymnasiet, kan dokumenteras och exempel på hur du kan upprätta hälsoplaner. Ett kapitel behandlar etiska överväganden och praktiska dilemman som man kan mötas av vid dokumentationen. I bokens avslutande kapitel presenteras kvalitetsregister för EMI med fokus på EMQ.

    Resultatet visar på betydelsen av att utveckla dokumentationen i skolhälsovårdsjournalen till såväl struktur som innehåll och med särskild betoning på den psykosociala hälsan. Resultatet visar även på betydelsen av att utveckla metoder och pröva modeller för att förbättra organisation och samverkan.

    Abstract [en]

    This study examines the effectiveness of therapeutic conversations with families (through family sessions) in alleviating health complaints among adolescent girls in a school setting. Four girls with recurrent, subjective health complaints and their families were included in the study. Three sessions were held with each family, using genograms, ecomaps, interventive questions, and other family nursing interventions; practicing school nurses were also present. A therapeutic letter was sent to each family at the end of the sessions. The Strengths and Difficulties Questionnaire was used as a pre- and posttest measure. Evaluative interviews were carried out with the families and with school nurses. The families reported feeling relief and described positive affective, behavioral, and cognitive changes as a consequence of the interventions. The school nurses experienced the family sessions as time-saving and easy-to-use tools in their work. Involving the family when schoolchildren's recurrent mental health problems are addressed may reduce future suffering.

    I denna studie beskrivs skolsköterskors erfarenheter av att stödja elever med neuropsykiatriska funktionsnedsättningar, NPF. Studien belyser skolsköterskors betydelse för att identifiera särskilda behov hos elever med NPF vilket kan främja en hälsosam inlärningsmiljö för alla elever.

    Studien genomfördes i södra Sverige i form av fokusgruppsintervjuer med skolsköterskor. Tre teman identifierades: att hjälpa elever med NPF att tolka sinnesintryck, att upptäcka tidiga tecken på ångest eller andra besvär bland elever med NPF, och att använda ett inkluderande förhållningssätt i sina hälsopedagogiska insatser.

    Evaluation of models for health supportive family conversation intended to turn a negative weight development in preschool children2015Conference paper (Other academic)
    Abstract [en]

    Introduction: Obesity in children is a growing health problem in the world. In Sweden, 14-15 percent of 4-year-old children are overweight and 3 percent are obese. Prevention of overweight and obesity is most effective if started as early as possible. Studies have shown the need for involving the family since parents’/guardians’ attitudes and beliefs may be crucial for the development of the child's eating habits and have thus a key role in the preventive work regarding a healthy lifestyle for the children. In Sweden, the Child Health Service (CHS) comprises a comprehensive universal nurse-led CHS program with health surveillance directed to the children and their parents, including parental support. The CHS have the opportunity to pay attention to early development of overweight and implement measures that can contribute to a healthy lifestyle during childhood as well as adulthood. The need for evidence based methods/models to be used in combating child overweight and obesity is obvious. However, there is a lack of evidence based models to prevent the prevailing childhood obesity used by clinical active nurses.

    Purpose: The overall purpose is to evaluate models for health supportive family conversations to families with preschool children with the intent to turn a negative weight development.

    Method: The study has a longitudinal design with health supportive family conversations being evaluated both qualitatively and quantitatively. A strategic sample will be applied on CHS centers from different socioeconomic areas in the southern part of Sweden. The models that will be used are the Calgary Family Assessment Model (CFAM) and The Calgary Family Intervention Model (CFIM). The models have its their theoretical foundation in e.g. theories of systems, communication, and change. The study will have a naturalistic approach with a pre-posttest design in which nurses in CHS are trained in the model for health supportive family conversations. Supervision will be provided during the intervention. Children’s IsoBMI and The Lifestyle Behavior Checklist will be used as pre- and post-test and at the 12 month follow-up. Evaluation interviews will be held with the families and the nurses separately.

    Preliminary results: A pilot interview showed that the health supportive family conversations led to increased knowledge and understanding about the family lifestyles related to the child obesity. Involvement of the family lead to a consciousness within the family about ingredients in sweet drinks and food. The fact that the CHS nurse was the one who conducted the conversations were of great importance, since the nurse was well known to the family.

    Skolsköterskors utmaningar med att dokumentera skolbarns psykosociala hälsa – en kvalitativ studie

    Syfte: Syftet med studien var att undersöka skolsköterskors utmaningar relaterat till dokumentation av skolbarns psykosociala hälsa

    Metod: Sex fokusgruppsdiskussioner genomfördes. I diskussionerna ingick frågor om situationer som var särskilt utmanande att dokumentera samt vad som begränsar och/eller underlättar dokumentationen av den psykosociala hälsan. Kvalitativ innehållsanalys användes för att tolka data.  

    Resultat: Analysen resulterade i ett övergripande tema: Att göra sin plikt och rädsla för att göra fel samt tre underteman: osäkerhet relaterat till den egna förmågan; rädsla för framtida konsekvenser och strategier för att hantera dokumentationen. 

    Slutsats: Skolsköterskor som förlitar sig på sin intuition och som använder en strukturerad dokumentationsmodell ökar möjligheterna till en tillförlitlig dokumentation. För att ytterligare utveckla sina yrkeskunskaper kan regelbunden yrkesmässig handledning vara av stor betydelse. Detta kan i sin tur bidra till ökad forskning och utveckling till förmån för skolbarns psykosociala hälsa.

    Abstract [en]

    Aim: The overall aim of this thesis is to explore School health nursing through school nurses’ descriptions of school children’s health and to analyse factors influencing the recording of schoolchildren’s health in the School Health Record (SHR). An additional aim is to evaluate family nursing interventions as a tool for the school nurses in the School Health Service (SHS).Methods: The thesis comprises four papers. A combination of qualitative and quantitative methods was used through individual interviews with a strategic sample of school nurses (n=12) (PI), a national survey to a representative sample of school nurses (n=129) (PII, III) and the implementation of family nursing models developed in Canada with girls in their early adolescence with recurrent health complaints and their families (n=4) in co-operation with their school nurses (n=2) (PIV). The Strengths and Difficulties Questionnaire (SDQ) was used as pre and post test. Evaluation interviews were conducted with the families and the nurses separately. Qualitative content analyses were used to analyze the interview text with the school nurses and the families. Manifest content analysis was used to analyze the free text answers of the survey and the evaluation interview with the school nurses. Descriptive statistical analyses were used to describe demographic data in all four papers. The SDQ was hand-scored statistically.Findings: The findings showed that nurses judged the schoolchildren’s mental health as deteriorated, especially in socially disadvantaged areas and more generally among girls expressed as psychosomatic symptoms. Individual factors related to lifestyle affected the schoolchildren’s physical health, and the mental health was, to a large extent, affected by the school environment and family relations. The latter seemed to be the most important factor affecting schoolchildren’s mental health. The basis for the school nurses judgement of the physical health was health check-ups and the health dialogues. Spontaneous visits were more commonly used to judge the mental health. Recording schoolchildren’s mental health was a challenge for school nurses. Difficulties were related to ethical considerations, tradition, lack of time and the improper structure of the SHR. Fears of marking the schoolchild for life related to the schoolchild itself, the parents or to other authorities/successive caregivers were brought up as hinders for recording mental and social health. Family sessions may be useful within the profession when handling recurrent health complaints among adolescence girls. The girls and their families experienced relief, they felt confirmed and that their feelings and reactions were normal in that situation. The families became aware of their own strengths and possibilities and this was supported by the SDQ which showed an increased well-being. The school nurses valued this way of working and meant that the sessions seemed to start a changing process within the families.Conclusions: The results indicate that school nurses have a deep knowledge about schoolchildren’s health which is not used to its full potential in a public health perspective. However, the experienced difficulties recording schoolchildren’s mental health seem obvious, which would demand developing the SHR for the needs of today. Family sessions in SHS with the school nurse as a collaborator with the family seemed useful and may be transferable to other health problems expressed by the schoolchildren. Bronfenbrenner’s ecological systems theory and other models for health determinants are used to illustrate the school nurse as a mediator working on the bridge over different health streams with schoolchildren’s health on an individual and a population level.

    The aim of this study was to explore school nurses’ experience of challenges related to documenting schoolchildren’s psychosocial health in Sweden. Six focus group discussions were carried out. Areas for discussions included questions about situations, especially challenging to document as well as what constrains and/or facilitates documenting psychosocial health problem issues. Qualitative content analysis was used for interpreting the data. The analysis resulted in one overarching theme: having to do one’s duty and being afraid of doing wrong; and three subthemes: uncertainty related to one’s own ability, concerns related to future consequences, and strategies to handle the documentation. School nurses relying on their intuition and using a structured documentation model may increase the opportunities for a reliable documentation. To further develop their professional skills with regular, clinical supervision can be of great importance. This in turn may increase contributions to research and development for the benefit of schoolchildren’s psychosocial health.

    I forskningsmiljön Children’s and Young People’s Health in Social Context (CYPHiSCO) förenas forskare knutna till Högskolan Kristianstad i sitt gemensamma intresse för barns och ungdomars hälsa och välmående. När vi nu ger ut vår tredje antologi, de tidigare utkom 2017 respektive 2019, tar vi bland annat utgångspunkt i Agenda 2030 och de globala målen för hållbar utveckling där fera av målen kan kopplas till den forskning som bedrivs inom miljön. Samtliga 17 mål är av betydelse för barn och ungdomar, och i forskningssammanhang vill vi lyfta fram delmål 16.7 i Agenda 2030 om lyhördhet och deltagandebaserat och representativt beslutsfattande på alla nivåer. Barns och ungdomars deltagande i forskning möjliggör att de får komma till tals i frågor och beslut som berör dem. I denna antologi gör vi några nedslag, främst utifrån barns och ungdomars perspektiv, men även utifrån betydelsefulla vuxna i deras närhet.

    This study explored Swedish school nurses' experiences of school health record documentation. Fifty per cent of a representative sample of Swedish school nurses (n = 129) reported difficulties with documenting mental and social health problems in family relationships, schoolchildren's behaviour, and school situations. Ethical considerations concerning fears of misinterpretation and practical barriers to documentation were expressed as reasons for their worries. Mental and social ill health is an increasing and often dominating problem among schoolchildren, thus proper documentation is a basic issue, both for individuals and for the population as a whole. School nurses obviously need professional guidance regarding documentation and ethical challenges. Systematic effort should be directed towards recognition and support of these nurses' unique opportunities to consider, follow and promote all aspects of schoolchildren's health.

    AIMS: To use school nurses' knowledge and experience for a better understanding of schoolchildren's health problems and their association to socioeconomic background and gender. METHODS: Mail questionnaires were sent to a nationally representative, random sample of Swedish school nurses (n=129). The questionnaire included structured and open-ended questions asking for school nurses' judgement of schoolchildren's health status; changes over the previous two years; estimation of schoolchildren's most common reasons for consulting the school nurse; and estimation of factors influencing schoolchildren's health. RESULTS: Swedish school nurses judged schoolchildren's mental health to have deteriorated during the previous two years with increasing health complaints, especially among girls and in disadvantaged housing areas. Disturbed family relations were considered as one important explanatory factor. Girls were more inclined to consult school nurses with subjective health complaints. Boys more often consulted the nurses with physical injuries. CONCLUSIONS: School nurses work closely with the children and meet them continuously during the school age period. They have a genuine knowledge of schoolchildren's health, which should be used even more, both in research and practice. The results may be applicable in other countries with similarly organized school health systems.

Skolsköterskan är en uppskattad person och en värdefull tillgång för såväl elever som deras familjer och skolpersonal. Avsikten med boken är att belysa skolsköterskans arbete och visa på fördjupad kunskap om användbara metoder och modeller i arbetet utifrån aktuell forskning.

    Boken vänder sig i första hand till studenter i specialistsjuksköterskeutbildningarna samt till redan yrkesverksamma skolsköterskor. Den kan också vara en källa till kunskap om skolsköterskans arbete för övrig personal inom Elevhälsan samt för skolledare, ansvariga chefer och politiker.

    I samband med regelbundna hälsobesök möter skolsköterskan alla elever i skolan och får kunskap om deras hälsa och livssituation. Tillsammans med övriga yrkeskategorier inom elevhälsan och i skolan har skolsköterskan en viktig uppgift att främja elevernas kunskap om hälsosamma levnadsvanor och om faktorer som bidrar till hälsa eller ohälsa samt stödja elevernas utveckling mot utbildningens mål.

    Denna omarbetade och utökade upplaga belyser skolsköterskans insatser för att möta de behov som barn och unga har i dag. Dessutom visar den på skolsköterskans roll i skolan, inspirerar till utveckling av yrket och lyfter fram forskning och metoder som kan användas i arbetet med skolbarns hälsa.

    Abstract [en]

    This study highlights school nurses' view of schoolchildren's health and their attitude to document it in the school health records. A strategic sample of 12 school nurses was interviewed. The interviews were semistructured and analysed with qualitative content analysis. The findings showed that the school nurses' viewed schoolchildren as physical healthy although they called attention to growing problems related to a changed lifestyle. Psychosocial ill-health was however increasing and the most common reason for visiting the school nurse was psychosomatic expressions. According to the nurses' descriptions, health was related to the individual, the school and the family situation. The family situation was mentioned as one of the most important factors of schoolchildren's health. The nurses described no problem to document schoolchildren's physical health. Ethical consideration, tradition, lack of time and the structure of the record were however factors that were said to hinder the documentation of the psychosocial health. In order to promote, protect and recover schoolchildren's health, more research is needed about how beliefs, experience, ethical consideration and resources influence the school nurse's daily work with schoolchildren's health.

    BACKGROUND: Physical activity is important to reduce mortality, morbidity and risk factors in patients with coronary heart disease. This report evaluates to what extent patients are still physically active following an exercise-based cardiac rehabilitation programme 12-14 months post-myocardial infarction and factors predicting why not.

    METHODS: Data from the National Quality Registry Swedeheart with post-myocardial infarction patients ( n=368) admitted from July 2012 to November 2014 were collected with outcomes of physical activity after 12-14 months. Baseline data included demographics, clinical variables, participation in exercise programmes, prescribed physical activity, health-related quality of life and self-reported health (EQ-5D-3L/EQ-VAS). A direct binary logistic regression analysis was used to identify indicators of low physical activity.

    RESULTS: Physical activity frequency per week (PA/week) was low, i.e. zero to three times, in older patients over 64 years ( P=0.00) and in those having problems with pain/discomfort (138 PA/week vs. 195) ( P=0.01), problems with mobility (60 PA/week vs.273) ( P=0.04) and anxiety/depression (128 PA/week vs. 205) ( P=0.04).

    CONCLUSION: Indicators predicting low physical activity can be used targeting improved post-myocardial infarction care outlining person-centred rehabilitation programmes and specialist nursing-led programmes.

    Bakgrund: Kunskapen om att vissa barn far illa finns och Barnhälsovårdssjuksköterskan har en skyldighet att anmäla vid oro eller vetskap om detta. Barn kan fara illa genomföräldrars bristande omsorg, och/eller genom fysiska och psykiska kränkningar. Detta kan ge upphov till livslångt lidande. BHVsjuksköterskans roll är att identifiera och uppmärksammabarn som riskerar att eller far illa, för att dessa barn ska få den hjälp dehar rätt till av socialtjänsten. Syftet med studien var att undersöka BHVsjuksköterskan serfarenhet av att överväga anmälan till socialtjänsten vid misstanke om att barn far illa. Metod: Tio BHVsjuksköterskor intervjuades och kvalitativ innehållsanalys med induktiv ansats användes för att analysera data. Resultat: Studien resulterade i ett övergripandetema, tre huvudkategorier och åtta underkategorier. Det övergripande tematblev "En skyldighet men inte en självklarhet", huvudkategorierna är, Perspektiv utifrånyrkesrollen, Perspektiv utifrån personliga erfarenheter och Mötet med barn och föräldrar. Slutsats: Samtliga BHVsjuksköterskor ansåg att det inte alltid var lätt att se vilka barn som for illa. De upplevde det tråkigt och sorgligt att behöva göra en anmälan. De satte alltid barnet främst och menade att det inte kunde bli sämre för barnet om en anmälan gjordes. En återkommande beskrivning hos samtliga BHVsjuksköterskor som medverkade i intervjun var att de fick en känsla av att något inte stämde.

    Självskattad psykisk ohälsa ökar bland ungdomar och det finns tydliga skillnader mellan flickor och pojkar. Fler flickor rapporterar symtom på ohälsa och dessa skillnader har ökat över tid. Det är fler flickor som rap- porterar att de känner sig stressade och färre flickor uppger att de trivs med livet jämfört med pojkar. Flickor är medvetna om kraven på deras utseende och kravet på att lyckas i livet. De är också medvetna om de könsskillnader som finns både i skolan och media och som påverkar dem. Samhörighet med kamrater och familj är betydelsefullt liksom trygghet i form av tillit till kamrater och att känna sig trygg där de vistas enligt tonårsflickorna själva.

    Detta kapitel kommer att handla om tonårsflickors hälsa och hur tonårs- flickor själva upplever sin vardag, vilket kommer att belysas med resultat från en forskningsstudie, samt diskuteras utifrån ett genusperspektiv och Känsla av sammanhang (KASAM).

    Barn och ungas hälsa och välbefinnande är en global angelägenhet. Hälsobesvär kan vara både fysiska och psykiska, och kan komma till uttryck genom exempelvis sömnsvårigheter, smärta och ledsenhet. Eftersom hälsobesvär uttrycks på olika sätt kan de vara svåra att tolka och det kan därför vara utmanande att ge effektivt stöd och behandling. Det finns dessutom en risk för att vardagliga känslouttryck betraktas som sjukliga och att de medicineras (Horwitz & Wakefield, 2007). Ledsenhet är å ena sidan en känsla som människor återkommande erfar under livet, men ledsenhet är också ett av symtomen vid depression. I föreliggande kapitel presenterar vi populärvetenskapliga sammanfattningar av flera studier som genomförts inom ramen för forskningsprojektet ”Stress, smärta och läkemedelsanvändning hos barn och ungdomar” vid Forskningsplattformen Hälsa i Samverkan, Högskolan Kristianstad. Den första undersökningen är en tvärsnitts-studie som handlar om ledsenhet och hälsobesvär hos ungdomar. Därefter presenteras två intervjustudier om skolbarns erfarenheter av stress respektive smärta. Slutligen presenterar vi två intervjustudier som belyser skolsköterskors perspektiv. Den ena fokuserar på skolsköterskors stöd till elever som upplever stress och smärta och samtidigt har en neuropsykiatrisk funktionsnedsättning. Den andra studien beskriver skolsköterskors erfarenheter av att arbeta under covid-19-pandemin.
