    Abstract [en]

    Intersectorial cooperation is commonly regarded as a desirable development in order to achieve increased efficiency and quality, and to reduce redundancies. In this report the experiences are presented from a project where nurses, midwives, preschool teacher and social worker integrated their activities around families and children. The results show that although all involved personnel were confident that the integration was favorable, they all experienced tensions, due to flaws in administrative and managerial procedures, perverse economic incentives, differences in professional background and organizational culture and personal preferences. This study indicates that integrative policies need to be followed by determined implementation.

    Abstract [en]

    In this article we use two immigrant projects, operating in a Swedish context, as a basis for discussing and analysing the potential of joint projects as a tool for change in human welfare service organisations that target the general public. The extensive use of projects in the development of a slimmed-down public sector can be seen as originating from the contradictory promises of projects regarding flexibility and rationality in the new public management context. Projects are also used as a way to obtain more resources for specific long-term needs, but they are seldom implemented as intended in the regular 'mainstream' organisation. Therefore the long-term development of social services and healthcare for immigrants remains inadequate. It appears that politicians, managers and civil servants prefer to focus on the positive aspect of projects to legitimate the organisations and to bring about change by targeting and steering activities. However, they tend to overlook the negative consequences of using projects, and there is a risk that a backlash will undermine the legitimacy of organisations. Citizens and participants in general, and immigrants in particular, experience frustration and distrust as a result of the short-term nature of project-based action. The problems that the projects were expected to address may remain, or even increase.

    In this article we use two immigrant projects to identify similarities and consequences in order to discuss and analyse outwarding collaborative projects in human service organizations as a tool for change. However, they tend to overlook the negative consequences from projects. Citizens and participants experience frustration and distrust. The long-term development in organizations is scarce. Projects are seldom implemented meanwhile as the problems the projects were expected to target still are remaining or even are increasing.

    A current challenge for many European countries is to enable forced migrants to access health and social care that meets their needs. One solution is to use paraprofessionals - that is, trained individuals who are not professionals - to act as bridge-builders between minority communities and the health and social care sectors. This paper explores the development of a quality improvement project in Sweden. The project aimed to recruit and train forced migrants who had lived in Sweden for more than five years to act as bridge-builders. The aim was to use their unique experiences and knowledge in two ways - to work with service staff in developing new ways of working, and to become culturally competent paraprofessionals working with their own communities. This paper focuses on how an understanding of the role evolved as the participants reflected on their experiences and undertook an inquiry process culminating in the development of three possible models of what the task of a bridge-builder should be in developing culturally appropriate health and social services that are responsive to the needs of forced migrants. The conclusion reflects on what was learned and how the experiences from this project may be useful for others who are struggling with the same kind of problem in Sweden and in other countries.

    Barnens hälsa påverkas redan tidigt i livet av föräldrarnas livsvillkor. Låg social status, brist på vänner och stress tidigt i livet leder till hälsoskillnader. Familjecentraler med en öppen förskola och förebyggande socialt arbete är en tidig insats och en lågriskstrategi som riktar sig till alla. Den har goda förutsättningar för att stärka föräldrarnas tillit och gemenskap med andra och kan bidra till att minska hälsoskillnader genom att främja barnens hälsoutveckling. Syftena med utvärderingen av de 16 familjecentralerna var att undersöka i vilken mån besökarna är representativa för befolkningen i upptagningsområdet och av vilka anledningar föräldrarna besöker öppna förskolan på familjecentralen. Jämförelse mellan befintlig befolkningsdata i upptagningsområdena och föräldrabesökares bakgrundsdata visar att familjecentralen i huvudsak når en representativ del av befolkningen. Föräldrar använder sig av familjecentralen för att få råd, utbyte av erfarenheter, barnens skull och att umgås med andra. I synnerhet för invandrare har familjecentralen stor betydelse för att komma in i det svenska samhället.

    Familjecentralens verksamhetsidé för öppna förskolan på familjecentraler – formulerat som en hypotes - har utvärderats steg för steg genom samla in data på sammanlagd sex familjecentraler för att pröva hållbarheten i hypotesen. Utvärderingen visar på ett föräldraskapande bland småbarnsföräldrar genom att de lär sig om barn och föräldraskap, får möjlighet att utöka sitt sociala kontaktnät och att bara vara i en trygg omgivning där socialt stöd finns till hands. De får tillsammans med andra känna sig ’good enough’ som en vanlig förälder med ett normalt barn. Professionell följsamhet utmärker personalens arbetssätt. De bedömer den ”mognadsgrad” som föräldern befinner sig i för att söka stöd och anpassar sitt förhållningssätt till föräldern genom fysiskt avstånd och djup i samtalet. På så vis kan pedagogiska, sociala och psykologiska mekanismer triggas igång och förklara de resultat som föräldrar upplever för dem själva och deras barn. Artikeln avslutas med en omformulerad hypotes om öppna förskolors verksamhet på familjecentraler.

    BACKGROUND: Samis are indigenous people in north Europe. In the territory called Sápmi (Lapland), reindeer herding is the traditional base for the Sami economy. The relation between living conditions and positive health of the Swedish Samis has been sparsely studied. As health is closely linked to sense of coherence (SOC), an understanding of the background factors to SOC may contribute knowledge that might be useful in promoting living conditions and health.

    METHODS: The study examines relations between the level of SOC and background factors from surveys in a Sami population (n=613) in comparison to a non-Sami population (n=525) in Sweden, and in comparison between 2 subsamples of Samis, that is, herders and non-herders.

    RESULTS: There are more similarities than differences between the Sami and non-Sami populations. However, dividing the Sami population, reindeer herders had significantly lower SOC, and in specific the subcomponent manageability, that is, less ability to use available resources to meet different demands in life, compared to non-herders.

    CONCLUSIONS: In addition to age and health, predictors of SOC are related to the life form of reindeer husbandry and the belonging to the herding community.

    Introduction: Family centres in Sweden are integrated services that reach all prospective parents and parents with children up to their sixth year, because of the co-location of the health service with the social service and the open pre-school. The personnel on the multi-professional site work together to meet the needs of the target group. The article explores a program theory focused on the open pre-schools at family centres.

    Method: A multi-case design is used and the sample consists of open pre-schools at six family centres. The hypothesis is based on previous research and evaluation data. It guides the data collection which is collected and analysed stepwise. Both parents and personnel are interviewed individually and in groups at each centre.

    Findings: The hypothesis was expanded to a program theory. The compliance of the professionals was the most significant element that explained why the open access service facilitated positive parenting. The professionals act in a compliant manner to meet the needs of the children and parents as well as in creating good conditions for social networking and learning amongst the parents.

    Conclusion: The compliance of the professionals in this program theory of open pre-schools at family centres can be a standard in integrated and open access services, whereas the organisation form can vary. The best way of increasing the number of integrative services is to support and encourage professionals that prefer to work in a compliant manner.

    OBJECTIVE: to describe the qualitatively different ways in which midwives make sense of how to approach women smokers. DESIGN, SETTING AND PARTICIPANTS: a more person-centred national project 'Smoke-free pregnancy' has been in progress in Sweden since 1992. Using a phenomenographic approach, 24 midwives who have been regularly working in antenatal care were interviewed about addressing smoking during pregnancy. FINDINGS: four different story types of how the midwives made sense of their experiences in addressing smoking in pregnancy were identified: 'avoiding', 'informing', 'friend-making', 'co-operating'. KEY CONCLUSION: the midwives' story types about how they approached women who smoke illustrated the difficulties of changing from being an expert who gives information and advice to being an expert on how to enable a woman in finding out why she smoked and how to stop smoking. IMPLICATIONS FOR PRACTICE: health education about smoking that is built on co-operation and dialogue was seen by the midwives as a productive way of working. The starting point should be the lay perspective of a woman, which means that her thoughts about smoking cessation are given the space to grow while she talks.

    In general, most women are familiar with the need to stop smoking when they are pregnant. In spite of this, many women find it difficult to stop. Using a phenomenographic approach, this study explored Swedish pregnant and post-pregnant women's ways of making sense of smoking during pregnancy. A total of 17 women who either smoked throughout pregnancy or stopped smoking during pregnancy were interviewed. Five different story types of how they are making sense of smoking during pregnancy were identified: smoking can be justified; will stop later; my smoking might hurt the baby; smoking is just given up; smoking must be taken charge of. Based on the study it is argued that the approach used in health education in relation to smoking cessation in antenatal care needs to move from information transfer and advice-giving to the creation of a dialogue. The starting point should be the woman's knowledge, concerns, rationalizations and prejudices. A model is suggested in which a woman may move in a space on three axes depending on life encounters, dialogue and reflections on meaning. The goal in health education would be to encourage movement along three axes: 'increase of self-efficacy towards control', 'increase awareness by reflection on meaning of the smoking issue' and 'avoidance of defense of the smoking behavior'.

    This study explores adolescent reasoning behind the use of alcohol at different types of parties, often house parties, and about the strategies to achieve maturity and prevent losing control. The data consist of semi-structured interviews with 23 adolescents aged 16-18 years (16 males and seven females). The interview transcripts were analysed using an inductive, thematic approach. All informants had personal experience with drinking at parties in different social settings. Our results suggest that the process of learning how to drink, often through failure in terms of being intoxicated, is important for adolescents' who strive to control their alcohol intake resulted in a good time and a break from everyday life. Furthermore, the results indicate that different social settings and party types engender different drinking patterns. Maturity and controlled conduct come across as desired ideals that provide a person with symbolic capital and thus, social status.

    Att vara rektor innebär att ständigt bli inkastad i olika händelser. Rektor förväntas ta ansvar för och lösa alla problem som uppstår i skolan oavsett om det handlar om ett läckande rör, en elev som inte uppnår målen eller ett ekonomiskt underskott som ska vändas till plus.

    Författarna erbjuder i denna bok verktyg för att bättre kunna förstå och hantera den faktiska vardagen som rektor eller skolledare. Dessa verktyg gör det möjligt för skolledare att utveckla förmågan att manövrera i en komplex och föränderlig värld, där paradoxer som kontroll och icke-kontroll är ständigt närvarande. Genom att skapa strukturer där människor tillåts hantera komplexiteten på egna villkor utvecklas mening och passion för verksamheten. Det innebär att rektor har ett medansvar snarare än ett odelat ansvar som ledare.

    Med verkligheten som utgångspunkt erbjuder författarna rektorer och skolledare på olika nivåer verktyg för att bättre kunna förstå och hantera den faktiska vardagen. Att som ledare utveckla sin förmåga att manövrera i en värld som är komplex och föränderlig, där paradoxer som kontroll och icke-kontroll ständigt finns närvarande. Författarna belyser och diskuterar hur den komplexa vardagen kan förstås och hur människor konstruerar mening. Genom att skapa strukturer där människor tillåts hantera komplexiteten på dess egna villkor utvecklas mening och passion för verksamheten. Detta innebär att rektorn har ett medansvar som ledare i nära samverkan med all personal.

    Boken vänder sig i första hand till blivande och verksamma ledare inom skolans verksamheter, rektorer och skolledare inom grund- och gymnasieskola samt förskolechefer, arbetslagsledare och andra som är intresserade av skolans organi-sation och styrning.

    Is it possible to design for sustainable work systems?: exploring the conjunction between regenerative work and complexity2008Conference paper (Other academic)
    Abstract [sv]

    The aim of this paper is to explore the social challenge of sustainable work systems and regenerative work. This is done by using complexity theory to grasp and understand the vision of sustainable work systems. A comprehensive literature review generated a theoretical framework for both sustainable work systems and understanding of complexity and in addition these two phenomena’s point of contact. Two case studies provide an empirical example and illustrate the potential in the conjunction of sustainability and complexity.A sustainable work system can be described as a simultaneous development and convergence between different levels and interests interrelated to the work organisation; from the individual perspective to the surrounding world of the organisation. A sustainable work system is characterised by having the capacity to regenerate and support the growth of the individual’s (employees) cognitive and emotional resources and at the same time maintain or increase competitiveness of the organisation. From the employee’s perspective this paper suggests that in order to reinforce and strengthen the individual, these same resources must be both recognised, adopted and have satisfactory support in the work organisation.This paper suggests that work organisations ought to create a work environment condition that supports self organisation, and acknowledge complexity and uncertainty as the normal state of reality.

    Abstract [en]

    The effects that work has on individuals are not unknown. The importance of work in terms of the subjective well-being and growth of individuals through their work, however, is not as extensively discussed and documented. Risk factors and unhealthy factors have long been discussed at the expense of positive conditions for well-being and personal growth. We like to call work with that type of potential regenerative work. Hence, that is the type of work that we have chosen to focus on in this study. The following research objectives have guided us through the study: Which everyday work processes may contribute to sustainable prerequisites for the subjective well-being and growth of individuals? The study takes an explorative approach to two workplaces - a care unit at a hospital and a manufacturing company. Our aim is to increase the understanding of prerequisites for the well-being and growth of individuals through their work. Three themes emerged in this type of regenerative work: performance, general overview and participation, and dialogue. These themes are problemised by looking at their potential in relation to the organisation of the work. Prerequisites for performance, general overview and participation, and finally dialogue prove to be more efficient in contexts where the complexity of the work and organisation is acknowledged and where there is room for inter-subjective sense-making and self-organising.

    Abstract [en]

    How can discharge planning (DP) for patients who require care in the home following a period in hospital be understood and developed through the lenses of complexity theory? With the help of complexity theory and practice-based narrative research this study discloses the formal routines and complex dynamic practices that are associated with DP. A study of the literature established that there was an almost total absence of complexity-theoretical perspectives on interpreting and developing DP.                 

    The researchers collected narratives about the DP processes using qualitative interviews with the nurses responsible for this in a hospital ward: these were audio-recorded and transcribed verbatim. They also participated in and documented meetings where these nurses, as a group, discussed DP.                 

    The findings show that nurses have to continuously deal with uncertainty, surprises and the unknown. They have to make sense and take charge of dynamic complex events and new knowledge, and manage complex relations and information. The researchers argue that looking upon practice from the lenses of complexity theory, and therefore accepting the complexity of practice, could facilitate the development of nurses' skills in order to guarantee good quality in DP.

    Since the closure of large psychiatric institutions, various types of community-based supportive housing for people with serious mental illness (SMI) have been developed. There is currently limited knowledge about users' experiences of living in supportive housing. The aim of the present study was to describe user experiences of living in supportive housing for people with SMI. Twenty-nine people living in such facilities participated in open, qualitative interviews. Data were subjected to latent content analysis. Three main themes emerged from this analysis: (i) having a nest, which included the subthemes of a place to rest and having someone to attach to; (ii) being part of a group, with the subthemes of being brought together and a community spirit; and (iii) leading an oppressive life, including the subthemes of questioning one's identity, sense of inequality, and a life of gloom. It could be concluded that user experiences of living in supportive housing are complex and paradoxical. In order to provide supportive housing, staff need to recognize and work within social group processes, and perform continual and structural evaluations of users' social and emotional needs.

    Family centres strengthen the preventive mechanisms of the social services. In order to ascertain if the work involved at the family centres gives an early preventive capability, a group of social workers from ten family centres did research on the assumption that the general measures taken for the benefit of all families even helped families with young children having special needs. Dialogue, reflection and questioning were the main methods used in the research circles. The research showed that the above assumption was substantiated. It showed that the family centres’ work in reaching all families, even helped in the identification and the early detection of families needing extra social support. Since the social workers were easily accessible, they could identify the families with special needs at an early stage, thereby preventing later complications with resultant destructive processes.

    This article takes off from the perspective of the personnel. Firstly, if and what changes occur in the style of working in personnel after they had started to work at a family centre in Sweden was assessed. They were detecting the extra needs of families more easily because of the presence of other professionals. Secondly, in making a deeper assessment, other personnel were asked to write their story about working at a family centre. The family centres can earlier than the traditional healthcare detect more complex needs of families with young children. However, more specialised support needs to be available when the family centres resources are not enough.

    Background: Current health measurement instruments tend to measureaspects of ill-health rather than health in general. There isa need for a salutogenic perspective when describing healthand developing a health measurement instrument. The aim of thisarticle is to present the development process and quality assessmentof a salutogenic health indicator scale. Methods: A descriptionof health, emanating from the concept of health, positive healthand well-being, was used as a basis for the construction ofthe scale. The scale is a semantic differential consisting of12 indicator items covering nine heath-related dimensions. Aprincipal component analysis was performed, and three healthindicator indexes were constructed. Correlation with self-ratedhealth questions was investigated, weighted kappa values werecalculated, and Cronbach's alpha (CA) was used to check internalconsistency. Results: The analysis resulted in a two-factormodel, and the indexes were named intrapersonal characteristics(CA= 0.90) and interactive function (CA= 0.84), summarised intohealth complete (CA= 0.92). Kappa values ranged from 0.44 to0.67, and correlations with self-rated health status were strongerthan those with self-rated sick-leave. Conclusions: Our healthdescription was characterized by complexity, but the instrumentis a short salutogenic health indicator scale. The shortnessincreases the usability. The instrument seems to be able tooffset the current problem of there being a lack of salutogenichealth measurement instruments. The results indicate that furthertesting is justified.

    Abstract [en]

    Background

    An action research project was initiated in order to explore young teenage girls’ opinions of leisure time and activities and initiate health promoting activity groups for girls age 13-16 in two municipalities in the south of Swed

    Study design

    The project started with a focus group study that explored girls’ opinions about leisure time and activities.

    Sample size and composition

    A total of fifteen semi structured focusgroup interviews were conducted in February-April 2011. A total of 110 girls, age 13-16, were participating in the study. A combination of a purpose fold selection strategy and randomisation was used for facilitating transferability of the results.

    Analysis

    The results were analysed through a tape based qualitative content analysis that focused on the manifest content of the interviews.

    Results

    The findings showed that the girls wanted their leisure time to be characterised by a variety of different activities. Activities were often related to a combination of the social, physical and psychological dimension of health. The psychological dimension was associated with activity related positive experiences and emotions.Conclusions

    Health promoting activity groups needs to be implemented with support from all the three dimensions of health and characterised by a variety of different activities.

    Success factors for visual artists functioning as health promoters at a workplace: results from a Swedish case study2009In: / [ed] American Psychological Association, 2009Conference paper (Refereed)

    Introduction

    The interest in relations between arts and workplace related health has increased. The focus has mostly been on the work of arts and health from a disease perspective and to the best of our knowledge the research focusing on relation between the artists and workplace related health from a salutogenic perspective is rare. In 2007 a project called the Contemporary Artists in Contemporary Society (CACS) Scania project was implemented and evaluated. The project consisted of twelve visual artists being positioned at eight workplaces on half time for a period of six months. The idea of the project was that unprejudiced meetings between the artists and the staff could result in workplace related health promoting processes. This study will try to unravel some of the mystery of how artists’ presence can result in workplace related promotion of health. 

     

    Aim

    The aim was to identify success factors for visual artists functioning as health promoters at a workplace.

     

    Procedure

    The search for success criterion started with going through the project descriptions and the evaluation reports from the CACS Scania project. This exposition led to the selection of one project that was considered a particularly successful case. Two artists had been placed at the office for management of regional development in Scania. The employees consisted mainly of civil servants and administrators. The evaluation material belonging to this particular project was studied, searching for possible explanations to the success of the project. The material consisted of digital recordings from a focus group interview with five of the participating staff, an interview with the manager, an interview with the two artists as well as stories written by the two artists throughout the project and finally the project description as well as the folder that the two artists produced as a summary of the project. The analysis of the material was influenced by qualitative content analysis and three categories of success factors were found.

     

    Results

    The experience of the participating staff

    The result showed that the staff mainly had had positive project related experiences. The staffs’ experiences could be linked to the salutogenic factors comprehensibility, manageability and meaningfulness. The various project related activities were found to be meaningful and the different activities were considered a pleasant reflective break from an everyday routine based and hectic practice. Some of the staff reported having problems managing the openness and indistinctiveness of the project, but the frequent communication with the artists, as well as support from the manager made the indistinctiveness manageable. The presence of the artists and the different project related activities were often found to be amusing, with adherent facilitation of wellbeing among the staff. At other occasions the presence of the artists could be considered disturbing. The artists brought new perspectives into the workplace that sometimes challenged the staffs’ customary way of thinking and acting, opening up possibilities for creativity and reflective processes of work related learning. It seemed as if the positive health related experiences of the staff relied on communication and mutual construction of intellectual fellowship and project related meaning (intersubjectivity).

     

    A framework for the work of the artists

    Four criteria were considered a useful framework for a description of the artists successful work at the workplace. 1. Presence - The artists were often present at the workplace. 2. Inspiration – The artists were inspired by the workplace. 3. Activity - The artists were constantly presenting things and activated the staff through out the project 4. Communication – The artists communicated with the staff during the development, implementation and completion process of the project.

     

    Organisational climate

    It seemed as if the organisational climate was suitable for using artists as health promoters. The staff and the manager were for instance describing them as willing to try new and innovative strategies for the development of their work in general and their work related health in particular.

     

    Conclusion

    To conclude there is a health promoting potential when involving artists as health promoters at a workplace. For this potential to be realised we found three categories of success factors. The experience of the participating staff were considered important since positive experiences, with adherent positive feelings, form the base for psychological and biological processes that generally have a positive impact on health. These experiences are on the other hand dependent on other facilitating factors, that here can be linked to for instance the artists as well as the organisational climate.

    Abstract [en]

    Background:

    Despite the low prevalence of daily smokers in Sweden, children are still being exposed to nvironmental tobacco smoke (ETS), primarily by their smoking parents. A prospective intervention study using ethods from Quality Improvement was performed in Child Health Care (CHC). The aim was to provide nurses with ew methods for motivating and supporting parents in their efforts to protect children from ETS exposure.

    Method:

    Collaborative learning was used to implement and test an intervention bundle. Twenty-two CHC nurses ecruited 86 families with small children which had at least one smoking parent. Using a bundle of interventions, urses met and had dialogues with the parents over a one-year period. A detailed questionnaire on cigarette onsumption and smoking policies in the home was answered by the parents at the beginning and at the end of he intervention, when children also took urine tests to determine cotinine levels.

    Results:

    Seventy-two families completed the study. Ten parents (11%) quit smoking. Thirty-two families (44%) ecreased their cigarette consumption. Forty-five families (63%) were outdoor smokers at follow up. The proportion f children with urinary cotinine values of >6 ng/ml had decreased.

    Conclusion:

    The intensified tobacco prevention in CHC improved smoking parents’ ability to protect their children rom ETS exposure.

    Based on one-to-one interviews and focus group interviews with management representatives, trade union representatives, and blue- and white-collar workers this article investigates how the global financial crisis was handled in Swedish industry. In the light of the crisis agreement signed between trade union IF Metall and company representatives how can the implementation of the agreement be described? How were these new conditions interpreted and how were they experienced? Three conceptual themes emerged, trust, loyalty, and principle of fair-play. Large differences were shown that were related to attitudes towards blue- and white-collar workers, recognition of work, and the size of the town.

    Community-based psychiatry as an arena for studying work efforts is relatively unexplored. The professional role of a housing support worker (HSW) in congregated supported housings for people with severe mental illness (SMI) is fairly new. The role has emerged due to major changes in Swedish mental care and services. Our aim was to describe the work experiences of HSWs and to understand the conditions provided when work assignments are being constructed. By doing this we provide an important piece of the puzzle in understanding the processes in contemporary congregated supported housings for people with SMI. Four focus group interviews were conducted and included 25 participants. Four different facilities of congregated supported housings for people with SMI were represented in the material. The interpretation of the material was inspired by a constructionist approach. The work of an HSW is experienced as complex and ambiguous. Together with different significant actors (including objects), the HSWs are negotiating the content of the responsibilities. Thus, different actors help co-construct the work content. Furthermore, the HSWs are caught between two different logics, one that is related to a complex practice and one that is related to instrumental and routine-based ideas. The complex emotional work of an HSW is often not supported by the principles of the organization. The findings can be helpful when organizing the work in congregated supported housings for people with SMI. This paper highlights the importance of bridging the gap between organizational demands and expectations and the individual preconditions and basic needs.

    BACKGROUND: Work and employees are often marginalized in studies on community-based psychiatric care and support systems. This paper highlights the role of the worker at congregated supported housing for people with severe mental illness (SMI). Housing support workers (HSW) are a fairly new professional role and have developed as a result of major changes in Swedish mental health care and services. The development of new roles is not unproblematic and raises intriguing questions.

    OBJECTIVE: The purpose of this article is to describe housing support workers’ experiences of meaning in their work, and in addition illuminate how the work identity of HSWs can be negotiated and constructed.

    METHODS: Four focus group interviews were conducted with a total of 25 participants. Additionally, three follow up sessions were conducted with the same participants. The material consists of employees from four different sites. The intepretation of the material was inspired by a constructionist approach.

    RESULTS: The analysis generated three themes: to do a good job, everyday needs and the formal role. The work identity for HSWs is complex and not easily interpreted.

    CONCLUSIONS: The experience of an unclear assignment affects the description of what is meaningful and important—the work identity and significant affiliations for HSWs. In the long run, this fragmented world can have negative implications for the HSW.
