Abstract [en]

Antiretroviral therapy (ART) is the cornerstone of HIV clinical care and recognized as a key component of HIV prevention. However, the benefits of ART can be realized only if HIV-infected persons maintain high levels of adherence. Maintaining adherence to ART over time is challenging and little is known about patients’ perceptions regarding ART in Zambia.The aim of the study was to explore patient’s perceptions and understanding of adherence to ART usinga qualitative exploratory research approach. The sample was chosen by purposive sampling with a maximum variation by age, gender, educational level, marital status and duration on ART. Twelve HIV positive men and women aged 25 to 55 years on ART at Livingstone Central Hospital participated in semistructured in-depth interviews that were transcribed verbatim and analysed using conventional content analysis. The results showed that the perceptions and understanding about adherence to ART were that the therapy was a means to prolong life. However, several perceptions and understandings concerning non adherence to the therapy was also found; wanting to avoid side effects, perceiving ART as a threat to integrity, being afraid of stigmatization, relying on alternative methods, and being influenced by family and the church. There seems to be great misconceptions, myths, and incorrect information about HIV and ART among people in Zambia. For adherence programs to succeed, policies targeted at improving adherence should be in place and be implemented. Interventions that encourage recommended levels of adherence should be accompanied by accessible and consistent information about ART.
